Herein, we report a rare instance of paraplegia following percutaneous nephrolithotomy under thoracic epidural anesthesia in a conscious patient. The possible factors include low body mass index, chronic renal failure, and multiple "in and out" needle passes during the procedure. Direct cord trauma with epidural bloody collection resulted in the neurological damage. However, the patient did not show signs of spinal cord trauma during the needle injury, possibly due to a prior accidental subarachnoid block. This not only delayed the recognition of the disorder, but also delayed treatment, conse

OBJECTIVE: To identify the risk factors predicting septic shock and severe hemorrhage in percutaneous nephrolithotomy (PCNL).
METHODS: We retrospectively analyzed 420 renal calculi patients who underwent ultrasound-guided PCNL from March 2005 to May 2011. Data on patients who experienced infectious shock requiring anti-shock therapy and severe renal bleeding requiring angiographic renal embolization or nephrectomy were compared with other patients using univariate analyses.
RESULTS: Of 420 patients, 10 (2.4%) suffered septic shock and 4 (1%) had severe hemorrhage. The two significant risk fa

PURPOSE: Percutaneous nephrolithotomy (PCNL) requires perforating the kidney, which may damage part of the patient's nephron. Further, compared with single-channel PCNL (S-PCNL), the safety of multi-channel PCNL (M-PCNL) and whether it affects the renal function of patients has been debated. The meta-analysis aimed to comprehensively evaluate the safety of M-PCNL.
METHODS: We carefully searched the Pubmed, Embass, and Web of Science databases for relevant research reported before October 30, 2021, and analyzed the included literature using the Stata software. Changes in the serum creatinine l

BACKGROUND: Renal stones in infants requiring Percutaneous nephrolithotomy (PCNL) is rare. There is insufficient literature on the long-term implications of PCNL in growing kidneys of these children.
OBJECTIVES: To review our experience of PCNL amongst infants i.e., < 1year of age and to analyse the safety and efficacy of this procedure and assess its long-term renal outcomes.
STUDY DESIGN: This was a retrospective analysis of a prospectively maintained data base between 2005 and 2020. All infants with unilateral renal stones >12 mm underwent PCNL. Changes in the serum creatinine, estimated

Spinal cord injury patients are at increased risk of developing nephrolithiasis and may require percutaneous nephrolithotomy for treatment of large stone burdens. Our objective was to compare outcomes of PCNL in SCI patients as compared to a matched cohort of non-SCI patients. Data from the Healthcare Cost and Utilization Project State Inpatient Database for Florida and California were used to identify patients by ICD-9 codes who underwent PCNL between 2007 and 2011. SCI was identified by having a paralysis diagnosis on the chronic comorbidity indicator. One-to-one matching was performed based

A 68-year-old man with serious cervical kyphosis and dorsolumbar scoliosis due to ankylosing spondylitis was admitted with a stone 17 mm in size in left kidney lower calyx. A percutaneous nephrolithotomy operation was decided considering the size and location of stone and the anatomical deformities of patient. The kidney was accessed through monoplaner triangulation method by giving a special position of the patient's spinal deformity and stone was successfully removed. Percutaneous nephrolithotomy is a feasible method in ankylosing spondylitis patients in case that the right position is achie

PURPOSE: To determine whether a delayed percutaneous nephrolithotomy (PCNL) reduces the rate of bacteremia/sepsis in patients with neuromuscular disorders. Patients with neuromuscular disorders are at higher risk of developing complications after PCNL. One strategy to reduce the risk of infectious complications is to place a percutaneous nephrostomy tube at least 24 h prior to performing PCNL. We analyzed the rates of bacteremia/sepsis in patients with neuromuscular disorders who had access on the day of PCNL (same-day) versus more than 24 h prior to the treatment for the stone (delayed).

PURPOSE: To develop a training model for simulated percutaneous nephrolithotomy (PCNL) under ultrasound and fluoroscopy-guided access.
MATERIALS AND METHODS: The laboratory model for PCNL described by Hammond and associates (J Urol 2004;172:1950-1952) was modified. We used an ex-vivo perfused porcine kidney (freshly removed after commercial slaughtering), a chicken carcass (supermarket), as well as the standard equipment for PCNL. For imaging, ultrasound (7.5 MHz) and a fluoroscopy unit are necessary. Artificial stone material is implanted in the renal pelvis. The ureter is cannulated for ret

Percutaneous nephrolithotomy (PCNL) is the gold-standard treatment for large renal and upper ureteral stones. Continuous irrigation is essential for visualization, thermal control, and fragment clearance but carries risks such as hypothermia, electrolyte imbalance, elevated intrarenal pressure, and infection. This review summarizes current evidence on irrigation strategies during PCNL and provides recommendations to mitigate these risks. A systematic literature search was conducted for English-language full-text articles published up to January 2025, using terms related to PCNL and irrigation.
