Preplacement nerve testing for carpal tunnel syndrome: is it cost effective?

UNLABELLED: Is not hiring otherwise-qualified workers who have an abnormal post-offer preplacement (POPP) median nerve test a cost-effective strategy to reduce workers' compensation expenses related to carpal tunnel syndrome (CTS)? We performed a retrospective dynamic cohort study based on 2150 workers hired at a company between January 1996 and December 2001 and who underwent POPP median nerve testing. Workers were followed until they left the company or until follow-up ended in May 2003. RESULTS: Thirty-five cases of work-related CTS occurred during follow-up, and 9.13 cases could have been

The effects of 4-aminopyridine and methylprednisolone on recovery of the facial nerve crush injury.

OBJECTIVE: 4-Aminopyridine (4-AP) is a potassium channel blocker that enhances nerve excitability. In this study, rat models that have facial nerve crush injury (FNCI) were grouped and treated with methylprednisolone (MP), 4-AP, and a combination of these two drugs. Electrophysiologic and histopathologic outcomes of these groups will be compared with a control group. MATERIALS AND METHODS: Thirty healthy male Wistar rats (mean weight of 265 g) were used in this study. The rats were randomly divided into five groups with six subjects in each: Group 1 (sham group), Group 2 (control group), Grou

Aged skeletal muscle retains the ability to fully regenerate functional architecture.

While the general understanding of muscle regenerative capacity is that it declines with increasing age due to impairments in the number of muscle progenitor cells and interaction with their niche, studies vary in their model of choice, indices of myogenic repair, muscle of interest and duration of studies. We focused on the net outcome of regeneration, functional architecture, compared across three models of acute muscle injury to test the hypothesis that satellite cells maintain their capacity for effective myogenic regeneration with age. Muscle regeneration in extensor digitorum longus musc

Efficient and robust control with spikes that constrain free energy.

Animal brains exhibit remarkable efficiency in perception and action, while being robust to both external and internal perturbations. The means by which brains accomplish this remains, for now, poorly understood, hindering our understanding of animal and human cognition, as well as our own implementation of efficient algorithms for control of dynamical systems. A potential candidate for a robust mechanism of state estimation and action computation is the free energy principle, but existing implementations of this principle have largely relied on conventional, biologically implausible approache

Network-oriented neurorehabilitation after stroke: a paradigmatic approach.

Stroke recovery increasingly demands a reconceptualization of the brain as a dynamic network system. This opinion paper advocates for a network-oriented neurorehabilitation paradigm, grounded in the understanding that functional recovery relies on the reorganization of distributed, interconnected neural systems across the central nervous system. While traditional rehabilitation approaches often target isolated impairments, a broader challenge is in the insufficient specification and documentation of therapeutic processes and their underlying mechanisms, which are often incompletely described a

Large-Scale Neural Recording Technologies for Investigating Circuit Dysfunction and Functional Biomarker Discovery in Animal Models of Alzheimer's Disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder associated with amyloid beta accumulation, tau pathology, and neuronal loss. Increasing evidence suggests that AD is associated with disruptions in large-scale circuit activity across multiple disease animal models, even before clear cognitive decline. These findings highlight the need for functional biomarkers that capture dynamic changes in neuronal network activity, rather than relying solely on molecular or anatomical measures. Recent advances in neural recording technologies now allow AD-related network dysfunction to be

Viral Tracing Gone Viral: Expanded Possibilities Call for Increased Caution in Describing Cerebellar Network Connections.

This review discusses the huge expansion of using cleverly constructed neurotropic viruses and viral transgenes in neuroanatomical research, with a special focus on the cerebellar field. Thanks to the near-limitless possibilities these new tracing techniques offer, vital new information on connections and functions within the brain are rapidly becoming available. However, tracing techniques in general and viral tracing techniques in particular, can harbor some pitfalls and drawbacks that need to be carefully evaluated in order to come to the correct interpretation of obtained results. Structural characteristics of local cortical networks wired by distance dependent connectivity rules.

BACKGROUND: The function of the cerebral cortex is shaped by its anatomical connectivity, yet experimental findings on connection probabilities in local cortical networks remain inconsistent. This study explores structural characteristics of local cortical networks based on distance dependent, Gaussian connectivity profiles. Monolayers of 101 × 101 pyramidal neurons were examined. Their connectivity was based on experimental anatomical or electrophysiological data. In the anatomical setting the connection probability between neighboring neurons was 0.8. In the electrophysiological scenario con

Dynamical network models predict spectral response to single-pulse electrical stimulation.

Objective.Drug-resistant epilepsy can be treated by surgically removing the seizure onset zone (SOZ), but localizing this zone is challenging because no clinically accepted biomarker of the SOZ exists. A novel computational biomarker, neural resonance, utilizes control theory to model the brain's response to stimulation. However, model validation is limited, and it is necessary to further contextualize the relationship between the model's prediction and the brain's true response to stimulation.Approach.Single-pulse electrical stimulation (SPES) provides an opportunity to bridge this gap as Bod

Frequency-dependent disruption of morphology-constrained functional organization in Parkinson's disease.

Parkinson's disease (PD) is associated with widespread structural and functional brain abnormalities, yet whether morphology-constrained functional organization is disrupted in a frequency-specific manner remains unclear. To address this gap, we derived frequency-resolved features of morphology-constrained functional organization in PD by integrating morphological networks from T1-weighted magnetic resonance imaging with resting-state functional signals in the graph spectral domain. Low- and high-frequency eigenmodes and the relative decoupling index (RDI) were used to identify abnormal patter

A Proposed Novel Neurovascular Mechanism for Brain Network Dysfunction After Traumatic Injury.

Current research in predicting traumatic brain injury risk focuses on the relationship between head impacts and the likelihood of white matter damage, often overlooking the role of neurovascular coupling in the injury response. To fill this gap, we combined biomechanical and neurodynamic principles to simulate alterations to large-scale resting-state brain activity following head impacts. We simulated cortical neural activity with a network of Kuramoto phase oscillators, using structural connectivity to define coupling and a vascular-informed local resource term to capture neurovascular coupli

Cerebellar nuclei output pathways regulate motor and non-motor behaviors through forebrain circuits.

The adult cerebellum is involved in modulating most, if not all, behaviors via its direct or indirect projections throughout the central nervous system. Its involvement in higher order behaviors is most prominent in human, where the lateral cerebellum (hemispheres) has expanded along with areas in the cerebral cortex that regulate language, emotions and cognition. An important aspect of wiring of the brain, is how the circuitry is established during normal development and activity, compared to how it is established when a developmental defect occurs, such as an injury that kills cells or a gen

Development of the spinocerebellar system within the precerebellar network.

Mossy fiber and climbing fiber inputs coming into the cerebellum through the precerebellar system feed into a stereotyped circuit module at the heart of cerebellar computations. Building this circuitry requires exquisite coordination, from generating the diverse populations, to guiding axons to the appropriate targets, and finally establishing the correct synapses. In this chapter, we take a closer look at the development of the spinocerebellar system and then bring in aspects of the hindbrain precerebellar system to ask how these pieces fit together during development. We also emphasize how l

Supraspinal Circuit Mechanisms Underlying Itch Processing.

BACKGROUND: Itch and pain are distinct somatosensory modalities that share overlapping anatomical pathways, particularly in the peripheral nervous system. However, compared with pain, the supraspinal mechanisms underlying itch perception and modulation remain poorly understood, limiting progress in central therapeutic strategies. METHODS: This narrative review synthesizes recent mechanistic studies-primarily from rodent models-to examine supraspinal circuits underlying itch. Relevant literature was identified through targeted database searches and reference tracking. RESULTS: Accumulating ev

Anatomy and Structural Connectivity of the Opercula.

The cerebral operculum comprises the cortical folds of the frontal, central, parietal, and temporal lobes that overlie the insula. Although historically described as a simple anatomical "cover," the operculum is now recognized as a morphologically complex periSylvian region embedded within multiple cortico-cortical and cortico-subcortical networks. In this review, a comprehensive synthesis of opercular organization is provided, integrating topographic anatomy, structural connectivity, and vascular supply. The topographic anatomy of the operculum is described in relation to periSylvian sulcal l

Probing inter-areal computations with a two-photon holographic mesoscope.

Brain computation depends on intricately connected yet highly distributed neural networks. Owing to the absence of the requisite technologies, causally testing fundamental hypotheses on inter-areal processing has remained largely out of reach. Here, we developed a two-photon holographic mesoscope capable of simultaneously reading and writing neural activity patterns with near-single-cell resolution across large regions of the mouse cortex. We demonstrate the precise photoactivation of spatial and temporal sequences of neurons in one or multiple cortical areas while reading out the downstream e

Integrating endocannabinoid signaling, CCK interneurons, and hippocampal circuit dynamics in behaving animals.

The brain's endocannabinoid signaling system modulates a diverse range of physiological phenomena and is also involved in various psychiatric and neurological disorders. The basic components of the molecular machinery underlying endocannabinoid-mediated synaptic signaling have been known for decades. However, limitations associated with the short-lived nature of endocannabinoid lipid signals had made it challenging to determine the spatiotemporal specificity and dynamics of endocannabinoid signaling in vivo. Here, we discuss how novel technologies have recently enabled unprecedented insights i

Neural Metabolic Networks: Key Elements of Healthy Brain Function.

Neural networks are responsible for processing sensory stimuli and driving the synaptic activity required for brain function and behavior. This computational capacity is expensive and requires a steady supply of energy and building blocks to operate. Importantly, the neural networks are composed of different cell populations, whose metabolic profiles differ between each other, thus endowing them with different metabolic capacities, such as, for example, the ability to synthesize specific metabolic precursors or variable proficiency to manage their metabolic waste. These marked differences like

Advances in Bionic Therapies for Targeting Neural Circuit Reconstruction and Integration for Spinal Cord Injury.

Spinal cord injury (SCI) is one of the most common critical illnesses, which can cause neurological deficits and disabilities of motor, sensory and autonomic nervous system in mild cases, and lead to paralysis or even death following severe trauma. Although there are currently no effective and satisfactory clinical treatments, the efforts for repair SCI never stop. Besides the traditional strategies such as drugs, surgical interventions and rehabilitative care, the bionic therapies have attracted significant attention due to its considerable promise. The bionic therapies for SCI mainly include

Brain stimulation preferentially influences long-range projections.

Advances in brain stimulation have made it possible to target smaller and smaller regions for electromagnetic stimulation, in the hopes of producing increasingly focal neural effects. However, the brain is extensively interconnected, and the neurons comprising those connections may themselves be particularly susceptible to neurostimulation. Here, we test this hypothesis by identifying long-range projections in single-unit recordings from nonhuman primates receiving transcranial alternating current stimulation. We find that putative long-range projections are more strongly affected by stimulati
