Neuroendoscopic third ventriculostomy in the management of noncommunicating hydrocephalus secondary to giant basilar artery bifurcation aneurysm - case report and review of literature.

Recently neuroendoscopic third ventriculostomy has become a well-established procedure for the treatment of noncommunicating hydrocephalus. However, morphological changes of the third ventricular floor can make endoscopic fenestration technically difficult, and increase the risk of injury to the basilar artery and perforators. The risk is especially high in patients with basilar artery bifurcation aneurysms, in whom aneurysm exclusion should precede treatment of hydrocephalus. If definitive aneurysm treatment is impossible or hazardous, endoscopic septostomy or shunting may be beneficial. Howe

Neuroendoscopic management of a third ventricle hydatid cyst presenting with obstructive hydrocephalus in a 4-year-old female: a case report and review of the literature.

BACKGROUND: Hydatidosis is a parasitic disease caused by the larvae of Echinococcus granulosus, typically acquired through ingestion of contaminated food. While hydatid cysts commonly develop in the liver and the lungs, the central nervous system is rarely affected, and the cyst predominantly develops within the cerebral parenchyma. However, the ventricular system involvement remains extremely rare. Among the cases of intraventricular hydatid cysts, the lateral ventricles are the mostly affected, while the involvement of the third ventricle remains exceedingly rare, with only five cases previo

Combination of neuroendoscopic hematoma evacuation and endovascular coil embolization for a ruptured anterior choroidal artery aneurysm in patients with moyamoya disease: illustrative cases.

BACKGROUND: The treatment strategy for hemorrhagic moyamoya disease (MMD) due to a ruptured aneurysm at the distal portion of the anterior choroidal artery remains controversial. The authors successfully treated the ruptured aneurysm with neuroendoscopic hematoma evacuation, followed by endovascular coil embolization. OBSERVATIONS: The authors encountered two patients with massive hemorrhagic MMD whose MMD had already been diagnosed and who had a periventricular anastomosis due to a ruptured aneurysm of the distal portion of the anterior choroidal artery involving the periventricular anastomo

Endoscopic Trans-Aqueductal Procedures for Juxta 4th Ventricular and Posterior Fossa Arachnoid Cyst using Flexible/Video Neuroendoscope: A Novel Approach.

Objective  Fourth ventricular and juxta fourth ventricular arachnoid cysts (ACs) are rare clinical entities. Conventionally, ACs are managed with either micro-surgical excision or cerebrospinal fluid (CSF) diversionary procedures such as a shunt. Effective treatment modality still remains controversial. Advances in neuroendoscopy have helped in the effective management of this benign condition in a minimally invasive method. Description of a subset of patients with fourth ventricular and juxta fourth ventricular ACs and hydrocephalus who underwent transaqueductal cysto-ventriculostomy with a f

Electromagnetic navigation-guided neuroendoscopic transfrontal transaqueductal fenestration of expansive posterior fossa arachnoid cyst with simultaneous endoscopic third ventriculostomy in an infant.

INTRODUCTION: We present an infant with an expansive posterior fossa arachnoid cyst and severe clinical deterioration due to decompensated obstructive hydrocephalus. Given the dilated Sylvius aqueduct, we favoured the endoscopic transfrontal transaqueductal route to approach the cyst. CASE REPORT: A 12-month-old boy was acutely admitted for severe symptoms of intracranial hypertension. Imaging revealed spacious cystic formation in the posterior fossa with expansive behaviour towards the brain stem, fourth ventricle and cerebellum associated with obstructive triventricular hydrocephalus. Neuroendoscopic Resection of Trigeminal Schwannoma in the Pterygopalatine/Infratemporal Fossa via the Transnasal Perpendicular Plate Palatine Bone or Transnasal Maxillary Sinus Approach.

BACKGROUND: Both the pterygopalatine fossa (PPF) and the infratemporal fossa (ITF) lie outside the midline of the skull base. Lesions in the PPF or ITF include trigeminal schwannoma (trigeminal schwannoma, TS), which originates from the second or third branch of the trigeminal nerve (maxillary nerve or mandibular nerve). Due to their typically deep anatomic location, lesions in the PPF or ITF can be difficult to treat using traditional surgical approaches. In recent years, because of their advantages, which include the fact that they allow the problem to be observed close up, neuroendoscopic t

Neuroendoscopic Trans-ventricular Cyst Drainage Prior to Tumour Resection Using a Two-Staged Approach for Cystic Craniopharyngioma: A Case Report and Literature Review.

Craniopharyngiomas are benign suprasellar tumours that can present with symptoms of raised intracranial pressure, visual impairment, or endocrine dysfunction. They are classified into the more common adamantinomatous and less common papillary tumours. Cystic variants may cause obstructive hydrocephalus, necessitating urgent intervention. This video case-based review presents the case of a 61-year-old male who presented with a two-month history of headaches, cognitive decline, and visual changes. Imaging revealed a large 2.8 x 2.2 x 2.2 cm suprasellar mass with cystic components extending into

Effective bilateral neuroendoscopic approach for managing intermittent triventricular hydrocephalus with septated choroid plexus cysts of the third ventricle: a case report and comprehensive literature review.

INTRODUCTION: Choroid plexus cysts (CPCs) are rare, benign neuroepithelial cysts that can cause obstructive hydrocephalus, although most are asymptomatic. Symptomatic CPCs are uncommon, and hydrocephalus from these cysts, especially intermittent in nature, is rarely reported. Diagnosing CPC-induced intermittent hydrocephalus can be challenging due to the cysts' CSF-like content and thin walls, often requiring advanced MRI sequences for detection. CASE REPORT: We present the case of a previously healthy 6-month-old male who developed intermittent triventricular hydrocephalus, presenting with p

Neuroendoscopic-Assisted vs. Conventional Burr-Hole Drainage for Septated Chronic Subdural Hematoma: A Comparative Cohort Study on Efficacy and Long-Term Functional Outcomes.

OBJECTIVE: Chronic subdural hematoma (CSDH) is prevalent in the elderly. Its septated variant poses a significant surgical challenge, as conventional burr-hole drainage often yields suboptimal results due to incomplete evacuation. Neuroendoscopy has emerged as a potential solution, but robust comparative evidence is lacking. METHODS: In this single-center retrospective cohort study, we enrolled 100 patients with radiologically confirmed septated CSDH. Fifty-two underwent neuroendoscopic-assisted evacuation (Endoscopic Group), and 48 underwent conventional burr-hole drainage (Burr-Hole Group).
