Intraoperative neuromonitoring (IONM) involves repeated electrical stimulation of the nervous system to assess integrity of neural pathways. Transcranial motor evoked potentials (tcMEPs) require high levels of electrical stimulation and are not recommended for patients with neurologic stimulators in situ, such as a deep brain stimulator (DBS) system, due to the risk of device damage and patient harm. We describe a case of a patient with bilateral DBS undergoing a surgery with IONM, in which direct electrical response wave (D-wave) monitoring via an epidural electrode was used, rather than tcME

Acute brain injury is a leading cause of ICU admission and long-term disability. Clinical neurological examination is frequently limited by sedation and mechanical ventilation, making objective physiological monitoring essential. While intracranial pressure remains the cornerstone of neurocritical care, pressure alone provides an incomplete picture of the complex pathophysiology underlying secondary brain injury, and contemporary practice has progressively moved toward multimodal neuromonitoring strategies. This narrative review summarizes the most widely used neuromonitoring techniques beyond

Background and Objectives: Thyroid surgery with intraoperative neural monitoring (IONM) of the recurrent laryngeal nerve (RLN) requires precise anesthetic management. This narrative review compares non-selective (neostigmine) and selective (sugammadex) reversal agents for neuromuscular blockade (NMB), discussing their mechanisms of action and the challenges of achieving optimal NMB reversal without compromising surgical conditions or IONM quality. Materials and Methods: A literature search was conducted using PubMed, MEDLINE, and Google Scholar for studies published up to November 2023. Releva

There is an urgent unmet need for a reliable noninvasive tool to detect elevations in intracranial pressure (ICP) above guideline-recommended thresholds for treatment. Gold standard invasive ICP monitoring is unavailable in many settings, including resource-limited environments, and in situations such as liver failure in which coagulopathy increases the risk of invasive monitoring. Although a large number of noninvasive techniques have been evaluated, this article reviews the potential clinical role, if any, of the techniques that have undergone the most extensive evaluation and are already in

Intraoperative neurophysiological monitoring of the vagus and recurrent laryngeal nerves is increasingly used during thyroidectomy, parathyroidectomy, skull base surgery, and cervical discectomy with fusion. Monitoring can assist in nerve localization and in reducing the incidence of neural trauma. To be effective, however, monitoring must be correctly implemented and the results interpreted based on an in-depth understanding of technique and the surgical structures at risk. Because "poor monitoring is worse than no monitoring" all members of the surgical monitoring team must have training spe

BACKGROUND: Lateral lumbar interbody fusion (LLIF) carries a characteristic risk of lumbar plexus injury. Intraoperative neuromonitoring (IONM) is widely used to mitigate this risk, yet its aggregate diagnostic accuracy has not been formally quantified.
METHODS: We conducted a systematic review and meta-analysis following PRISMA 2020 guidelines. PubMed, Embase, and Cochrane Library were searched through January 24, 2026. Eligible studies were prospective or retrospective observational cohorts of adults undergoing elective LLIF or XLIF with IONM correlated to postoperative neurologic outcomes

Intraoperative neurophysiology (ION) plays a crucial role in dorsal rhizotomy, a procedure aimed at reducing spasticity while preserving neural function. ION encompasses monitoring and mapping, with the mapping of the root/rootlet constituting a significant component. Despite the acknowledged roles of ION, persistent controversies exist, and a universally accepted standard for integrating ION procedures into dorsal rhizotomy remains elusive. This chapter provides a detailed explanation of the author's approach to dorsal rhizotomy, with a primary focus on the decision-making process for root/ro

Preservation of facial nerve (FN) function is the most important goal in acoustic neuroma (AN) surgery. We have been using intraoperative continuous facial nerve monitoring (ICFNm) of evoked electromyography during AN surgery. ICFNm is very useful, and we can identify the real-time functions of the FN. Some surgeons have experienced difficulty with placing the ICFN stimulating electrode (SE). We therefore show how to place the ICFN SE. We mostly perform AN surgery with a retrosigmoid approach (RSA). A craniotomy with four burr holes is performed. We dissect the arachnoid membrane along the acc

The purpose of this study was to perform a systematic, scoping review of commonly described intermittent/semi-intermittent autoregulation measurement techniques in adult traumatic brain injury (TBI). Nine separate systematic reviews were conducted for each intermittent technique: computed tomographic perfusion (CTP)/Xenon-CT (Xe-CT), positron emission tomography (PET), magnetic resonance imaging (MRI), arteriovenous difference in oxygen (AVDO2) technique, thigh cuff deflation technique (TCDT), transient hyperemic response test (THRT), orthostatic hypotension test (OHT), mean flow index (Mx), a

Although intracranial pressure (ICP) is essential to guide management of patients suffering from acute brain diseases, this signal is often neglected outside the neurocritical care environment. This is mainly attributed to the intrinsic risks of the available invasive techniques, which have prevented ICP monitoring in many conditions affecting the intracranial homeostasis, from mild traumatic brain injury to liver encephalopathy. In such scenario, methods for non-invasive monitoring of ICP (nICP) could improve clinical management of these conditions. A review of the literature was performed on

Intraoperative neurophysiology (ION) in brainstem surgery evolved as brainstem surgery advanced. The original idea of brainstem mapping (BSM) is a neurophysiological procedure to locate cranial nerve motor nuclei (CNMN) on the floor of the fourth ventricle. With the introduction of various skull base approaches to the brainstem, BSM is carried out on any surface of the brainstem to expose the safe entry zone to the intrinsic brainstem lesion. It is the modern concept of BSM, a broader definition of BSM. BSM enables to avoid direct damage to the CNMN when approaching the brainstem through the ne

Cardiac arrest survivors comprise a heterogeneous population, in which the etiology of arrest, systemic and neurologic comorbidities, and sequelae of post-cardiac arrest syndrome influence the severity of secondary brain injury. The degree of secondary neurologic injury can be modifiable and is influenced by factors that alter cerebral physiology. Neuromonitoring techniques provide tools for evaluating the evolution of physiologic variables over time. This article reviews the pathophysiology of hypoxic-ischemic brain injury, provides an overview of the neuromonitoring tools available to identi

Monitoring brain integrity and neurocognitive function is a new and important target for the management of a patient treated with extracorporeal membrane oxygenation (ECMO), in particular because of the increasing awareness of cerebral abnormalities that may potentially occur in this setting. Continuous regular monitoring, as well as repeated assessment for cerebral complications has become an essential element of the ECMO patient management. Besides well-known complications, like bleeding, ischemic stroke, seizures, and brain hypoperfusion, other less defined yet relevant injury and clinical

Intraoperative use of brain monitoring during anesthesia has increased in recent years due to the development of more advanced monitors. However, it is still not a standard practice, even in spine fusion with instrumentation surgeries. This case report describes the use of a brain monitor with real-time changes in spectral edge frequency (SEF) to assess anesthetic levels and to guide the neurophysiologist to perform timely motor evoked potential (MEP) testing during a wake-up test process in a pediatric patient when MEPs were lost. We should consider using SEF monitoring more regularly when pr

Morquio syndrome, a lysosomal storage disease, is associated with an increased risk of spinal cord ischemia with positioning changes.1 Though uncommon in non-spine surgeries, intraoperative neuromonitoring (IONM) can facilitate prompt detection of ischemia.2,3 We utilized IONM in an 11-year-old undergoing bilateral distal femoral osteotomies and lumbar epidural placement. After induction and supine baseline signal acquisition, the patient was positioned laterally. Signals were lost, and hypotension ensued. Phenylephrine and fluids were administered, but hypotension resolved only after supine r

INTRODUCTION: Neuromonitoring represents a cornerstone in the comprehensive management of patients with traumatic brain injury (TBI), allowing for early detection of complications such as increased intracranial pressure (ICP) [1]. This has led to a search for noninvasive modalities that are reliable and deployable at bedside. Among these, ultrasonographic optic nerve sheath diameter (ONSD) measurement is a strong contender, estimating ICP by quantifying the distension of the optic nerve at higher ICP values. Thus, this scoping review seeks to describe the existing evidence for the use of ONSD

Action monitoring is crucial to the successful execution of an action and understanding the actions of others. It is often impaired due to brain lesions, in particular after stroke. This systematic review aims to map the literature on the neurophysiological correlates of action monitoring in patients with brain lesions. Eighteen studies were identified and divided into two groups: studies on monitoring of one's own actions and studies on monitoring of the actions of others. The first group included EEG studies on monitoring of self-performed erroneous and correct actions. Impaired error detect

STUDY DESIGN: This study is a meta-analysis of prospective and retrospective studies identified in PubMed, Web of Science, and Embase with outcomes of patients who received intraoperative somatosensory-evoked potential (SSEP) monitoring during lumbar spine surgery.
OBJECTIVE: The objective of this study is to determine the diagnostic accuracy of intraoperative lower extremity SSEP changes for predicting postoperative neurological deficit. As a secondary analysis, we evaluated three subtypes of intraoperative SSEP changes: reversible, irreversible, and total signal loss.

Standardization of the intraoperative neuromonitoring (IONM) technique is an essential aspect of modern monitored thyroid surgery. The standardized technique involves vagal nerve stimulation. VN stimulation is useful for technical problem solving, detecting non-recurrent laryngeal nerve (non-RLN), recognizing any recurrent laryngeal nerve (RLN) lesions, and precisely predicting RLN postoperative function. Herein, we present technical notes for the VN identification to achieve the critical view of safety of the VN stimulation with or without dissection.
