Why, when and how to propose noninvasive ventilation in cystic fibrosis?

Cystic fibrosis (CF) lung disease is characterized by progressive airflow obstruction, due to mucus plugging and inflammation within the bronchial walls, and destruction of the lung parenchyma secondary to bronchiectasis. These alterations result in an increase of the work of breathing, leading to alveolar hypoventilation predominantly during sleep, exercise and acute respiratory exacerbations. Noninvasive positive pressure ventilation (NPPV) has been shown to unload the respiratory muscles in patients with CF, which increases alveolar ventilation and improves gas exchange. NPPV has been shown

Helmet noninvasive mechanical ventilation in patients with acute postoperative respiratory failure.

BACKGROUND: The physiological and clinical effects of noninvasive ventilation (NIV) on acute postoperative respiratory failure are relatively unknown. The aim of this study was to determine the prediction factors for failure in the use of NIV with a helmet in this context. METHODS: This was a prospective observational study. The use of NIV was assessed for a period of 2 years in a postoperative ICU. Demographic data were collected, as well as acute respiratory failure (ARF) and arterial gas readings. Hemodynamic changes were assessed using pulse contour cardiac output technology, and the clin

Noninvasive and invasive ventilation in acute respiratory failure associated with bronchiectasis.

PURPOSE: To describe the outcomes of patients with bronchiectasis and acute respiratory failure (ARF) treated with noninvasive ventilation (NIV) and invasive mechanical ventilation (IMV) after a failure of conservative measures, and to identify the predictors of hospital mortality and NIV failure. METHODS: Retrospective review of bronchiectatic patients on NIV (n = 31) or IMV (n = 26) for ARF over 8 years in a medical intensive care unit (ICU) experienced in NIV. RESULTS: At baseline, the NIV group had more patients with acute exacerbations without identified precipitating factors (87.1 vs.

Noninvasive ventilation in chronic respiratory failure: effects on quality of life.

BACKGROUND: Noninvasive ventilation (NIV) has been found to be an essential technique to treat chronic respiratory failure (CRF) resulting from restrictive thoracic disorders (RTD). The last decades were characterized by the expansion of NIV to treat patients suffering from various other conditions, such as chronic obstructive pulmonary disease (COPD) and obesity hypoventilation syndrome (OHS). OBJECTIVES: The aim of this study was to evaluate the effect of NIV on health-related quality of life (HRQoL) of patients with CRF during 2 years and to identify parameters associated with changes in H

Successful pregnancy and cesarean delivery via noninvasive ventilation in mitochondrial myopathy.

We report a case study of a 22-year-old woman with mitochondrial thymidine kinase 2 deficiency and chronic respiratory failure due to severe neuromuscular weakness requiring noninvasive positive pressure ventilation (NIPPV) since 12 years of age. During pregnancy and cesarean delivery, she was successfully supported with NIPPV. A multidisciplinary team approach should be used in pregnant patients with these disorders with specific attention to management of pulmonary complications, selection of route of delivery, anesthesia, and analgesia.

Noninvasive ventilation for acute respiratory failure near the end of life.

RATIONALE: For patients with acute respiratory failure who have declined intubation and resuscitation or have chosen comfort measures only, noninvasive ventilation (NIV) may help them achieve important health or personal goals, or merely prolong the dying process. OBJECTIVE: To determine clinicians' attitudes to and stated use of NIV for these patients. METHODS: We developed an instrument to assess the attitudes of intensivists, pulmonologists, and respiratory therapists (RTs) toward the use of NIV for patients with acute respiratory failure near or at the end of life. After assessing its ps

Practice guidelines as multipurpose tools: a qualitative study of noninvasive ventilation.

OBJECTIVE: Although practice guidelines for noninvasive ventilation (NIV) for patients with acute respiratory failure (ARF) have the potential to improve processes of care and patient outcomes, clinicians' views about life support technology guidelines are not well understood. The objective was to understand the knowledge about and attitudes toward an NIV guideline for patients with ARF and potential barriers to its use. DESIGN: Qualitative study based on individual, in-depth, semistructured interviews. SETTING: St. Joseph's Healthcare, Hamilton, Ontario. SUBJECTS: Thirty clinicians (six at

Noninvasive ventilation in the older patient who has acute respiratory failure.

Older patients are at significantly increased risk of acute respiratory failure from multiple causes. Noninvasive positive pressure ventilation has been shown to dramatically improve care of patients with acute respiratory failure. Patient selection is important in all patients being treated with noninvasive positive pressure ventilation but is especially important in older patients. Delirium, confusion, and dementia can lead to difficulty for patients in tolerating this procedure and lead to a worsening respiratory status. The presence of a do-not-intubate order does not necessarily preclude

Application of noninvasive ventilation in acute respiratory failure after cardiovascular surgery.

OBJECTIVE: To examine ventilatory response, oxygenation-related, and hemodynamics of patients with hypoxemic acute respiratory failure (ARF) submitted to noninvasive mechanical ventilation (NIV) during the postoperative phase of cardiovascular surgery in order to identify predictive variables of success, and to compare the different types of NIV. METHODS: Seventy patients with hypoxemic ARF were randomized to one of three modalities of NIV--continuous positive airway pressure (CPAP) and ventilation with two pressure levels (PEEP + SP and BiPAP). Ventilation, oxygenation-related, and hemodynam

Occurrence of pneumothorax during noninvasive positive pressure ventilation through a helmet.

A 79-year-old woman presented with hypoxemic acute respiratory failure secondary to pneumonia and was started on continuous noninvasive positive pressure ventilation (NPPV) by helmet. Patient improved over first two days of NPPV, but worsened suddenly on the third day because of development of a pneumothorax. Pneumothorax may have been caused by barotrauma from desynchronization between patient and ventilator.

Weight gain in cachectic COPD patients receiving noninvasive positive-pressure ventilation.

BACKGROUND: In chronic obstructive pulmonary disease (COPD), body mass index (BMI) is an important predictor of survival. Little is known about the prevalence of malnutrition or longitudinal changes of BMI in patients undergoing noninvasive positive-pressure ventilation (NPPV). METHODS: In a cohort study of 141 patients with COPD and severe chronic respiratory failure (mean forced expiratory volume in the first second [FEV1] 0.80 +/- 0.27 L, mean P(aCO2) 55.6 +/- 8.8 mm Hg), we investigated nutritional status in relation to respiratory impairment. Changes in BMI were evaluated at 6 and 12 mon

Noninvasive pressure support versus proportional assist ventilation in acute respiratory failure.

BACKGROUND: Although conventional pressure ventilation (PSV) decreases the rate of intubation in acute respiratory failure, patient-ventilator dyssynchrony is a frequent cause of failure. In proportional assist ventilation (PAV), pressure is applied by the ventilator in proportion to the patient-generated volume and flow; therefore, there is automatic synchrony between the patient's effort and the ventilatory cycle. OBJECTIVE: The aim of this study was to compare the effects of PSV and PAV during noninvasive ventilation in the treatment of acute respiratory failure. DESIGN: Prospective rando

Noninvasive positive pressure ventilation in trauma patients with acute respiratory failure.

The effectiveness of noninvasive pressure support ventilation (NIPSV) in treating trauma patients with acute respiratory failure (ARF) was evaluated in a retrospective clinical study. Forty-six conscious patients with ARF admitted to the general intensive care units (ICUs) of three hospitals between July 1988 and July 1991 were surveyed. Patients received NIPSV after a period of spontaneous breathing with supplemental oxygen. Blood gas levels and respiratory parameters were measured before the application of the mask and after 1, 6 and 12 h of NIPSV. Thirty-three (72%) patients were successful

Automated Detection of Trigger Asynchronies During Initiation of Noninvasive Nocturnal Ventilation.

BACKGROUND: Nocturnal noninvasive ventilation (NIV) can improve outcomes in severe COPD. However, optimal ventilator titration is challenging, and patient-ventilator asynchrony (PVA) is common, reducing comfort and perceived benefits. Optimal PVA detection in nocturnal NIV has yet to be optimized, with continuous manual review of ventilator tracings being practically unfeasible. This study compared how noninvasive surrogate measures of subject-initiated breathing activity, namely respiratory inductance plethysmography (RIP) and surface electromyography (sEMG), influence automated PVA detection
