The Importance of Considering Olfactory Dysfunction During the COVID-19 Pandemic and in Clinical Practice.

The emergence of a worldwide pandemic due to coronavirus disease 2019 (COVID-19) and frequent reports of smell loss in COVID-19-infected patients have brought new attention to this very important sense. Data are emerging that smell impairment is a prominent symptom in COVID-19 and that this coronavirus behaves differently in causing olfactory dysfunction compared with other respiratory viruses. Anosmia and hyposmia, the complete and partial loss of smell, respectively, can result from many causes, most commonly from viral infections, sinonasal disease, and head trauma. Olfactory dysfunction ne

Identification of genes that promote or inhibit olfactory memory formation in Drosophila.

Genetic screens in Drosophila melanogaster and other organisms have been pursued to filter the genome for genetic functions important for memory formation. Such screens have employed primarily chemical or transposon-mediated mutagenesis and have identified numerous mutants including classical memory mutants, dunce and rutabaga. Here, we report the results of a large screen using panneuronal RNAi expression to identify additional genes critical for memory formation. We identified >500 genes that compromise memory when inhibited (low hits), either by disrupting the development and normal functio

Ethanol antagonizes human olfactory receptors through receptor-dependent competitive and noncompetitive mechanisms.

Mechanisms of ligand-receptor interactions at olfactory receptors (ORs), particularly those of antagonists, remain largely unclear. Here, we investigated how ethanol-empirically known to suppress human olfactory perception-modulates OR activity using a heterologous assay system. Ethanol did not activate OR1A1 or OR2W1 but significantly suppressed agonist-induced responses in a concentration-dependent manner. Pharmacological analyses revealed that ethanol acts as a competitive antagonist for OR1A1 and a noncompetitive antagonist for OR2W1, demonstrating receptor-dependent modes of inhibition. W

Effect of Olfactory and Gustatory Dysfunction and Motor Symptoms on Body Weight in Patients with Parkinson's Disease.

BACKGROUND: Non-motor symptoms in Parkinson's disease (PD) are often associated with a negative impact on the patients' quality of life and on their weight regulation. The aim of this study was to assess the effect of olfactory and gustatory dysfunction, apathy, fatigue, depression, and motor symptoms on weight regulation in PD patients. METHODS: We analyzed 112 participants, 63 PD patients (mean age ± SD: 69.2 ± 10.1), and 49 controls (mean age ± SD: 68 ± 9.6). For each participant we collected age, weight, height, BMI, olfactory and gustatory function, cognitive performance, apathy and fati

Eating Habits and Body Weight Changes Induced by Variation in Smell and Taste in Patients with Previous SARS-CoV-2 Infection.

Olfactory and gustatory dysfunction are recognized as common symptoms in patients with COVID-19, with a prevalence ranging, respectively, between 41-61% and 38.2-49%. This review focused on relating the variations in dietary habits with the reduction/loss of smell and/or taste in patients who contracted the COVID-19 infection. Primarily, we reviewed the main pathological mechanisms involved in COVID 19-induced anosmia/dysosmia and ageusia/dysgeusia. Then, we explored and summarized the behavioural changes in food intake and body weight during the COVID-19 pandemic in relation to sensory impair

Insulin Resistance Is Associated with Reduced Food Odor Sensitivity across a Wide Range of Body Weights.

The worldwide obesity epidemic is a major health problem driven by the modern food environment. Recently, it has been shown that smell perception plays a key role in eating behavior and is altered in obesity. However, the underlying mechanisms of this phenomenon are not well understood yet. Since the olfactory system is closely linked to the endocrine system, we hypothesized that hormonal shifts in obesity might explain this relationship. In a within-subject, repeated-measures design, we investigated sensitivity to a food and a non-food odor in the hungry and sated state in 75 young healthy (2

Pathophysiology and possible treatments for olfactory-gustatory disorders in patients affected by COVID-19.

SARS-CoV-2 infects host cells mainly through the interaction between the virus's Spike protein and the viral receptors namely Angiotensin-Converting Enzyme 2 (ACE2) and transmembrane serine protease 2 (TMPRSS2). Both are highly expressed in the gastrointestinal tract, in the nasal and bronchial epithelium, as well as in the type II alveolar epithelial cells. The aim of this review is to report the evidences from the scientific literature on the pathophysiology and the available treatments for olfactory-gustatory disorders in patients with COVID-19. The mechanisms involved in these disorders ar

Nasal xenobiotic metabolizing enzymes: at the crossroads of toxicity, detoxification, and olfaction.

Enzymes exemplify the adaptability of biological systems through their multifunctional roles across tissues. Thus, xenobiotic metabolizing enzymes, traditionally recognized for their contributions to detoxification, biosynthesis, and signaling, also play specialized roles in the nasal cavity, where they encounter volatile compounds on a daily basis. In olfactory tissues, xenobiotic metabolizing enzymes act as odorant metabolizing enzymes, influencing not only volatile xenobiotic clearance but also sensory perception. These enzymes operate within a coordinated network of phase I, II, and III re

Intranasal odorant concentrations in relation to sniff behavior.

Knowledge on how odorants are transported through the nasal cavity to the olfactory epithelium is limited. One facet of this is how the sniffing behavior affects the abundance of odorants transferred to the olfactory cleft and in turn influences odor perception. A novel system that couples an online mass spectrometer with an odorant pulse delivery olfactometer was employed to characterize intranasal odorant concentrations of butane-2,3-dione (or butanedione, commonly known as diacetyl) at the interior naris and the olfactory cleft. Volunteers (n=12) were asked to perform different modes of sni

Increased squalene concentrations in the clitoral gland during the estrous cycle in rats: an estrus-indicating scent mark?

Squalene in the rat clitoral gland is reported to be semi-volatile and may serve as a chemo-signal. The objective was to determine squalene concentrations in the clitoral gland throughout the reproductive cycle. Clitoral glands were extracted with dichloromethane; 23 compounds were identified with Gas Chromatography linked Mass Spectrometry (GC-MS). Since squalene concentrations were significantly higher during proestrus and estrus, and remarkably reduced during metestrus and diestrus, we inferred that it could be an ovulation-indicating chemosignal in the female rat, acting as a scent mark fo

Central processing of odor concentration is a temporal phenomenon as revealed by chemosensory event-related potentials (CSERP).

Chemosensory event-related potentials (CSERP) can be used to examine central nervous odor processing. An important question for understanding odor perception is how different concentrations are processed. In the present study two odors were chosen which activate either the olfactory (linalool) or the trigeminal (menthol) system. Both odors were presented to 11 subjects in four different concentrations. Four subjects had to attend actively to the odors while the others perceived the odors under passive attention. The results showed that increased concentrations of the olfactory stimulus resulte

Long-term olfactory loss post-COVID-19: Pathobiology and potential therapeutic strategies.

An acute loss of smell emerged as a striking symptom present in roughly half of the people infected with the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus in the early phases of the COVID-19 pandemic. In most COVID-19 patients, olfaction recovers over the course of a few weeks. However, a lasting partial or complete loss of smell, often associated with distorted olfactory perceptions termed parosmia, has emerged as a widespread problem impacting at least 5%-10% of those who experience anosmia due to COVID-19. Our inability to offer effective therapies to this hyposmic or a

Brain lesion-pattern analysis in patients with olfactory dysfunctions following head trauma.

The presence of cerebral lesions in patients with neurosensory alterations provides a unique window into brain function. Using a fuzzy logic based combination of morphological information about 27 olfactory-eloquent brain regions acquired with four different brain imaging techniques, patterns of brain damage were analyzed in 127 patients who displayed anosmia, i.e., complete loss of the sense of smell (n = 81), or other and mechanistically still incompletely understood olfactory dysfunctions including parosmia, i.e., distorted perceptions of olfactory stimuli (n = 50), or phantosmia, i.e., olf

Limited observed efficacy of olfactory training in severe long-term post-COVID-19 syndrome: a prospective cohort study.

PURPOSE: Persistent chemosensory dysfunction is a debilitating symptom of Post-COVID-19 Syndrome (PCS). While olfactory training (OT) is effective in early post-viral smell loss, its efficacy in chronic, multisystemic PCS remains unclear. This study aimed to characterize a severely affected cohort and to evaluate changes in olfactory function during a structured OT intervention. METHODS: In this prospective study, 29 patients with persistent PCS lasting > 12 weeks (mean duration approximately 30 months) were recruited from a specialized post-COVID outpatient clinic. All patients underwent str

Developmental Profile of Brain Neprilysin Expression Correlates with Olfactory Behaviour of Rats.

A neuropeptidase, neprilysin (NEP), is a major amyloid (Aβ)-degrading enzyme involved in the pathogenesis of Alzheimer's disease (AD). The olfactory system is affected early in AD with characteristic Aβ accumulation, but data on the dynamics of NEP expression in the olfactory system are absent. Our study demonstrates that NEP mRNA expression in rat olfactory bulbs (OB), entorhinal cortex (ECx), hippocampus (Hip), parietal cortex (PCx) and striatum (Str) increases during the first postnatal month being the highest in the OB and Str. By 3 months, NEP mRNA levels sharply decrease in the ECx, Hip

Facial Characteristics and Olfactory Dysfunction: Two Endophenotypes Related to Nonsyndromic Cleft Lip and/or Palate.

Evidence exists for the presence of a specific facial phenotype in nonaffected first-degree relatives of persons with CL/P. An increased risk for olfactory dysfunction has also been reported in CL/P-relatives. These phenotypic features can probably be explained via the presence of CL/P-related susceptibility genes. We aimed at confirming the occurrence of these endophenotypic traits in first-degree CL/P-relatives, and we investigated the link between the facial phenotype and the smell capacity in this group. We studied the facial morphology of 88 nonaffected first-degree relatives of patients

Human male superiority in olfactory sensitivity to the sperm attractant odorant bourgeonal.

Recent studies have shown that sperm chemotaxis critically involves the human olfactory receptor OR1D2, which is activated by the aromatic aldehyde bourgeonal. Given that both natural and sexual selection may act upon the expression of receptors, we hypothesized that human males are more sensitive than human females for bourgeonal. Using a 3-alternative forced-choice test procedure, olfactory detection thresholds were determined for a total of 500 subjects, 250 males, and 250 females between 18 and 40 years of age. We found that male subjects detected bourgeonal at significantly lower concentr

Olfactory Neuroblastoma: An Unusual Ocular Presentation.

We report a case of 51-year-old medically healthy male who presented with gradual painless diminution of vision in both eyes for 3 months. His visual acuity was hand movement perception in the right eye, and no light perception in the left eye. The intraocular pressure, external eye examination, ocular motility and anterior segment examinations were normal. Fundus examination revealed severe bilateral papilloedema and engorged tortuous veins in both eyes. Imaging exhibited a large intracranial tumour causing raised intracranial pressure. This was debulked by neurosurgery and histological exami

Anosmia: a missing link in the neuroimmunology of coronavirus disease 2019 (COVID-19).

Just before 2020 began, a novel coronavirus, severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), brought for humans a potentially fatal disease known as coronavirus disease 2019 (COVID-19). The world has thoroughly been affected by COVID-19, while there has been little progress towards understanding the pathogenesis of COVID-19. Patients with a severe phenotype of disease and those who died from the disease have shown hyperinflammation and were more likely to develop neurological manifestations, linking the clinical disease with neuroimmunological features. Anosmia frequently occurs

Clinical assessment and mutation analysis of Kallmann syndrome 1 (KAL1) and fibroblast growth factor receptor 1 (FGFR1, or KAL2) in five families and 18 sporadic patients.

We report on the clinical and molecular findings in 25 males and three females with Kallmann syndrome (KS) aged 10-53 yr. Ten males were from five families, and the remaining 15 males and three females were apparently sporadic cases. Molecular studies were performed for Kallmann syndrome 1 (KAL1) and fibroblast growth factor receptor 1 (FGFR1, also known as KAL2) by sequence analysis for all the coding exons, by PCR-based deletion analysis, and by fluorescence in situ hybridization (FISH) analysis, showing six novel and two recurrent intragenic KAL1 mutations in seven familial and four sporadi
