We studied five cases of leukemia with invasion of the optic disk and optic nerve. Two patients had acute lymphoblastic leukemia and three had acute myelogenous leukemia. All patients received intrathecal injections of chemotherapeutic drugs including methotrexate, cytarabine, and prednisolone. Three patients underwent irradiation therapy of the whole brain and optic nerve. While initial improvement in the ophthalmic abnormalities, such as optic disk edema, flame-shaped retinal hemorrhages, and dilation of the retinal vein, was noted, optic atrophy was later found in all eyes.

Computerized image analysis, including fluorescein angiography, was used to evaluate the retinal, choroidal and optic disk blood flow in 16 patients with normal-tension glaucoma (NTG) and to correlate this measurement with visual fields, retinal vessel width, optic disk pallor and blood pressure (BP). The angle of the ascending slope of the fluorescein dye curve was measured as an index of blood flow from the densitometric and time curves of the fluorescein angiograms in the optic disk, peripapillary choroid, retinal artery and vein for each quadrant. While the ascending slope as well as the r

We report the incidental finding of megalopapilla in the contralateral eye of a 10-year-old black child with optic nerve hypoplasia. Megalopapilla is a rare entity characterized by an enlarged optic disk without other morphologic abnormalities except for an enlarged cup/disk ratio. To the best of our knowledge, the concurrence of megalopapilla and optic nerve hypoplasia in the same individual has not been reported previously.

From projected optic disk photographs we measured the size of the scleral canal in two samples of emmetropic patients: one of patients with unilateral pseudopapilledema and drusen and the other of the general normal population. Measurements on the non-drusen-containing optic disk of patients with unilateral drusen were taken to reflect the scleral canal size of the fellow, affected eye. For both trained (t = 6.642) and untrained (t = 4.274) observers, the average diameters of the non-drusen-containing optic disks of patients with unilateral drusen were significantly smaller than those of the o

PURPOSE: To assess the optic nerve head (ONH) parameters in normal male Saudi eyes using swept-source optical coherence tomography (SS-OCT).
MATERIALS AND METHODS: The study included 86 healthy men with a mean age of 23.6 ± 4.82 years. The metrics collected using SS-OCT from each individual was disk area (DA), rim area (RA), cup volume (CV), linear cup-disk ratio (LCDR), vertical cup-disk ratio (VCDR), and total retinal nerve fiber layer (RNFL) thickness, superior RNFL thickness, and inferior RNFL thickness. All the metrics were correlated with the DA. The right eye data were used for the cor

OBJECTIVE: Optic nerve head drusen often make evaluation of the nerve head difficult to interpret. In addition, visual field defects are known to occur in patients with optic disk drusen, resembling glaucomatous damage. The authors report two cases of coincident optic nerve head drusen and glaucoma, in which the use of optical coherence tomography (OCT) in evaluating the nerve fiber layer was beneficial.
PARTICIPANTS: Two patients with both optic nerve head drusen and glaucoma, one with primary open angle glaucoma, the other with pseudoexfoliation glaucoma were evaluated. Both patients had as

Seventeen optic disks with morning glory syndrome were measured in absolute and relative size units (millimeters and parapapillary retinal vessel caliber) in 5 and 12 eyes, respectively. Their respective mean areas were 7.47 +/- 2.63 mm2 and 7.84 +/- 4.96 mm2. They were significantly (Mann-Whitney test) larger than 457 unselected normal optic nerve heads (2.69 +/- 0.70 mm2; p less than 0.0001) and 15 optic disks with pits (4.84 +/- 1.42 mm2; p less than 0.001). They are the largest optic nerve heads, probably with constant postnatal size, measured so far, and form part of a spectrum of optic d

UNLABELLED: In patients with proliferative diabetic vitreoretinopathy the authors noticed tractive vitreoretinal distortion of the optic disk to the nasal side. As a result of this distortion a reduction in visual acuity, pallor of the optic disk, and changes in the visual evoked potentials were registered. The disease in called "tractive vitreoretinal opticopathy."
METHOD: We examined 15 diabetics with vitreoretinal traction and temporal pallor of the optic disk, as well as a reduction in visual acuity. Six patients were observed retrospectively and 9 patients prospectively before and after

Papilloarterial ischemia (PI) is the designation for a clinical picture characterized by circulatory insufficiency of the optic disk and the retinal arteries. Optic disk ischemia leads to visual loss, altitudinal field defects, pale papilledema and fluorescein angiographic filling defects of the disk capillaries. Retinal ischemia manifests itself by ophthalmoscopic and angiographic signs of caliber variations, focal constrictions or even occlusions of peripapillary retinal arteries. PI occurs more frequently in young females, in whom an inflammatory origin must be suspected in view of the angi

We report ocular findings of a patient with anterior ischemic optic neuropathy (AION) and cilioretinal artery occlusion in a female carrier of Fabry's disease. Fluorescein angiography revealed delayed filling of the upper and temporal part of peripapillary choroidal vessels and capillaries of the right optic disk and late filling of the cilioretinal artery. CT scanning was performed several times in early stages and demonstrated thickening of the intraorbital optic nerve due to ischemic edema. About 5 months later, the fellow eye showed optic disk edema, an early sign of AION, and was treated

The mere intraocular pressure (IOP) measurement and the visual field (VF) examination do not allow early primary open-angle glaucoma (POAG) diagnosis. At present, the morphological and morphometric analysis of the optic disk is considered very important for an early diagnosis and follow-up of the disease. The recent introduction of laser systems equipped with new polarimetry techniques (GDx) and confocal tomography (HRT) allows an objective, quantitative, and reproducible evaluation of the morphometry and morphology of the optic disk and retinal nerve fiber layer (RNFL). The GDx, scanning lase

Large cupping of the optic disk in a baby or a child can be indicative of primary congenital glaucoma. Primary congenital glaucoma is often refractory to treatment, and lifelong management and follow-up are necessary; therefore, diagnosis requires careful consideration. In this study, we describe the case of twins in whom primary congenital glaucoma was initially suspected due to large cupping of the optic disks. Twin babies (a boy and a girl weighing 455 g and 592 g respectively), born prematurely (at 28 gestational weeks), were referred to our hospital eight months after birth because large

Dominant optic atrophy is the most common heredodegenerative optic neuropathy. Typically, patients present with slowly progressive, bilaterally decreased central visual acuity. Subtle central or cecocentral visual field defect and normal peripheral isopters are demonstrated with perimetry. A defect in blue-yellow discrimination (tritan error axis) is the most common type of dyschromatopsia, however protan and deutan axes may be superimposed. A characteristic optic disk appearance includes temporal disk pallor with excavation. An autosomal dominant inheritance pattern can often be elicited from

A 37-year-old woman complained of headaches following bilateral visual loss in the past two years. She was obese and had undergone bariatric surgery three months earlier, followed by a considerable weight loss. Neuro-ophthalmic examination revealed a bilateral swollen optic disk. After a computerized analysis of the visual fields and magnetic resonance imaging of the brain and orbits, a diagnosis of idiopathic intracranial hypertension was made. At six months after the bariatric surgery, the patient reported no further headaches and exhibited better findings on computerized analysis of visual

By employing purple colour filters with a large blue and a small red transmission, normal and atrophic optic disks were photographed on colour transparencies. Normal optic disks reflect red light, and atrophic optic disks reflect blue light. The degree of atrophy estimated by colour contrast is quantitated by microdensitometry in the blue (470 nm) and in the red (640 nm) region of the transparencies. The difference between the blue and red density is used as an expression of optic disc colour.

PURPOSE: To report bilateral congenital optic nerve head pits in monozygotic siblings.
METHOD: Case reports.
RESULTS: Pits were found in abnormally large optic disks in both eyes of two otherwise healthy female monozygotic siblings aged 15 years. Pit size increased and visual acuity decreased with increased optic disk area. In one eye, nonrhegmatogenous retinal detachment developed that eventually necessitated pars plana vitrectomy. The siblings' parents were unremarkable.
CONCLUSIONS: Congenital optic nerve head pits can occur bilaterally in otherwise healthy monozygotic siblings with opht

We reviewed 170 eyes of 112 patients with optic atrophy from various causes. Special attention was directed towards measured cup:disk ratios as well as presence of glaucomatous-like cupping of the optic nerve head. We observed a small but significant increase in nerve head cupping in eyes with optic atrophy when compared to contralateral eyes, as well as to eyes of 50 diabetic patients. No characteristic glaucomatous disk changes were documented. We evaluated these findings with respect to possible causes of glaucomatous disk and field changes.
