Idiopathic inflammatory myopathy: autoantibody update.

Several defined, specific autoantibodies have been associated with polymyositis and dermatomyositis. These include autoantibodies to at least six of the aminoacyl-transfer-ribonucleic-acid synthetases; to the signal recognition particle; to the protein complexes labeled Mi-2 and PM-Scl; and several autoantibodies, such as anti-U1nRNP and anti-Ro/SSA, that have recognized associations with other conditions. These autoantibodies are a continuing area of interest. Recent studies have involved the clinical implications of these autoantibodies, and their potential significance for etiology and path

Pulmonary arteriography in congenital heart disease.

Pulmonary outflow tract obstruction may occur at subvalve, valve, or pulmonary artery level. Selective ventriculography will usually define the intracardiac obstruction (whether valve, subvalve, or both) and may define pulmonary arterial abnormalities. Increasing experience in the surgical management of patients with tetralogy of Fallot and other conotruncal abnormalities characterized by significant pulmonary outflow tract obstruction suggests that preoperative recognition of pulmonary arterial stenosis is essential to the successful operative management of these patients (1-4). Because of th

Unraveling Chronic Pain: From Mechanisms and Risks to Diagnosis and Treatment.

Chronic pain is a globally prevalent and complex condition, encompassing three primary subtypes, that is, nociceptive, neuropathic, and nociplastic, each with distinct biopsychosocial mechanisms. Chronic pain was historically viewed as a monolithic symptom and managed with opioid-centric models, causing widespread therapeutic failure. While recognition of its heterogeneity has driven a paradigm shift toward precision medicine, tailoring multimodal strategies to the dominant pain mechanism, critical challenges persist. These include difficulty in identifying treatable root causes, limited long-

Zoon balanitis: not a distinct clinicopathological entity?

Zoon balanitis (ZB) was originally described in the 1950s in patients with clinical features resembling erythroplasia of Queyrat but with histology that demonstrated a plasma cell infiltrate without evidence of dysplasia. Subsequently, ZB has been extensively reported in the literature, reflecting widespread acknowledgement as an established distinct clinicopathological entity. However, its existence as such has been questioned, and there have been suggestions in the literature that ZB represents either a nonspecific irritant reaction pattern, or a part of the heterogeneous clinicopathological

Treatment of acute kidney injury in cancer patients.

Acute kidney injury (AKI), either of pre-renal, renal or post-renal origin, is an important complication in cancer patients, resulting in worse prognosis, withdrawal from effective oncological treatments, longer hospitalizations and increased costs. The aim of this article is to provide a literature review of general and cause-specific treatment strategies for AKI, providing a helpful guide for clinical practice. We propose to classify AKI as patient-related, cancer-related and treatment-related in order to optimize therapeutic interventions. In the setting of patient-related causes, proper as

Mechanisms underlying developmental programming of elevated blood pressure and vascular dysfunction: evidence from human studies and experimental animal models.

Cardiovascular-related diseases are the leading cause of death in the world in both men and women. In addition to the environmental and genetic factors, early life conditions are now also considered important contributing elements to these pathologies. The concept of 'fetal' or 'developmental' origins of adult diseases has received increased recognition over the last decade, yet the mechanism by which altered perinatal environment can lead to dysfunction mostly apparent in the adult are incompletely understood. This review will focus on the mechanisms and pathways that epidemiological studies

Adaptive threshold-stochastic resonance (AT-SR) in MHC clusters on the cell surface.

Highly conserved 2D receptor clusters (membrane rafts) of immunological signaling molecules with MHCI and MHCII antigens as their cores have been observed in the past on the surface of T- and B-cell lines of lymphoid origin, as well as on cells from patients with colon tumor and Crohn's disease. Conservativity is related to the ever presence of MHCI molecules. Although they are suspected to play a role in maintaining these clusters and facilitating transmembrane signaling, their exact role has been left largely enigmatic. Here we are suggesting stochastic resonance (SR), or "noise-assisted sig

Clinical characteristics of multifocal premature ventricular complexes originating from the basal septal region of the left ventricle: Anatomy, mapping, and ablation.

BACKGROUND: Clinical recognition of multifocal premature ventricular complexes (PVCs) poses diagnostic and therapeutic challenges. Despite their morphological diversity, these arrhythmias may originate from a confined anatomical region, allowing a systematic mapping and ablation strategy. OBJECTIVE: To describe clinical characteristics and propose a stepwise approach to mapping and ablation of multifocal PVCs originating from the basal septal left ventricular (LV) ostium. METHODS: Patients with multifocal septal PVCs who underwent coronary venous mapping were included. Ablation targeted the

An Adaptive Multi-Representation Fusion Framework for EMG Pattern Recognition in Transradial Amputees.

Electromyography (EMG)-based pattern recognition is a key enabling technology for intuitive upper-limb prosthetic control; however, amputee EMG signals show substantial intersubject variability and nonstationary characteristics, which can limit the effectiveness of conventional fixed feature representations. This study proposes an Adaptive Multi-Representation Fusion Framework that combines an enhanced Adaptive Linear Series Decomposition Learner (Adaptive LSDL) with Time-Domain Power Spectral Descriptors (TDPSD) for amputee movement classification. The proposed Adaptive LSDL extends conventio

Structural Comparison and Drug Screening of Spike Proteins of Ten SARS-CoV-2 Variants.

SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) has evolved many variants with stronger infectivity and immune evasion than the original strain, including Alpha, Beta, Gamma, Delta, Epsilon, Kappa, Iota, Lambda, and 21H strains. Amino acid mutations are enriched in the spike protein of SARS-CoV-2, which plays a crucial role in cell infection. However, the impact of these mutations on protein structure and function is unclear. Understanding the pathophysiology and pandemic features of these SARS-CoV-2 variants requires knowledge of the spike protein structures. Here, we obtained th

Aptamer Sensors for Inflammaging Biomarkers: Principles of Molecular Recognition, Analytical Platforms, and Unaddressed Targets.

Inflammaging, a chronic, low-grade systemic inflammatory state that develops with advancing age, is one of the key drivers of age-associated pathology and promotes the emergence of the senescence-associated secretory phenotype (SASP). Timely quantitative monitoring of pro-inflammatory and SASP-associated biomarkers therefore demands highly sensitive analytical platforms capable of reliable performance in complex biological matrices. Aptamer-based biosensors (aptasensors), which employ short single-stranded oligonucleotides offering high affinity and specificity for molecular recognition, have

A Rare Case of Cerebral Amyloidoma Mimicking Thalamic Glioma in a Rheumatoid Arthritis Patient.

Amyloidosis, often referred to as "the great imitator", is a condition characterized by the abnormal deposition of amyloid proteins in various tissues, potentially leading to organ dysfunction. When these deposits localize in the brain, they can disrupt neurological function and present with diverse clinical manifestations, making diagnosis particularly challenging. Cerebral amyloidosis is a rare entity that frequently mimics other neurological disorders, often resulting in significant delays in recognition and management. This case highlights the diagnostic challenge posed by cerebral amyloid

Responses of mental health professionals to man-made trauma: the Israeli experience.

The reactions and responses of mental health professionals in the area of armed conflict is the focus of this paper. It examines the way the therapeutic community has dealt with the survivors of two catastrophes-the Holocaust and warfare. A parallel process of a gradual change of attitudes towards the survivors was observed: emotional detachment, lack of recognition in the early stages and, eventually, social acceptance and empathy. The origins of these attitudes will be discussed, and three explanations will be offered. Israel is a small, stress-ridden country that has known seven full-scale

Sustainable graphene-based molecularly imprinted polymer for selective solid-phase extraction of triclosan: From tap water to serum matrices.

BACKGROUND: Triclosan (TCS), a widely used antimicrobial agent, is recurrently detected in environmental and biological matrices. Its environmental persistence and potential endocrine-disrupting effects raise significant toxicological concerns. Sensitive and selective analytical strategies are therefore essential for trace-level monitoring in complex samples. RESULTS: A straightforward graphene-based molecularly imprinted polymer (Gr@MIP) was developed to produce highly selective sorbents for TCS. Graphene (Gr), obtained by electrochemical exfoliation, was functionalized with 9-vinylcarbazole

Word recognition memory and serum levels of Borna disease virus specific circulating immune complexes in obsessive-compulsive disorder.

BACKGROUND: Borna disease virus 1 (BoDV-1) is a non-segmented, negative-strand RNA virus that persistently infects mammals including humans. BoDV-1 worldwide occurring strains display highly conserved genomes with overlapping genetic signatures between those of either human or animal origin. BoDV-1 infection may cause behavioral and cognitive disturbances in animals but has also been found in human major depression and obsessive-compulsive disorder (OCD). However, the impact of BoDV-1 on memory functions in OCD is unknown. METHOD: To evaluate the cognitive impact of BoDV-1 in OCD, event-relat

Optimised fluorescence-activated nuclei sorting for epigenomic analysis of cortical cell types.

Increased understanding of the functional complexity of the genome has led to growing recognition of the role of non-sequence-based regulatory variation in disorders of the human central nervous system. Most genomic analyses of the brain are limited by the use of bulk tissue, which comprises a heterogeneous mix of different neural cell types with distinct epigenetic profiles, thereby limiting the ability to attribute regulatory changes to specific cell populations. Given the limited availability of human post-mortem tissue resources and the importance of integrating multi-omic data from the sa

An Amino-Acid-Derived Metal-Organic Framework with Large Pores for Unspecific Enantioseparation.

The selective separation of enantiomers is critical in pharmaceutical production, while conventional chiral sorbents always suffer from the trade-off between selectivity and the substrate scope. Herein, inspired by a natural unspecific peroxygenase with large protein channels, we developed a homochiral metal-organic framework (MOF) constructed from flexible phenylalanine-derived ligands and zinc ions. This MOF features a giant chiral cavity with a size of 2.2 × 3.1 nm, decorated with 42 chiral phenylalanine residues, which serves as a solid sorbent for the highly enantioselective adsorption an

Integrative Migraine Therapy: From Current Concepts to Future Directions-A Plastic Surgeon's Perspective.

Migraine is a prevalent and disabling neurological disorder with multifactorial origins and complex clinical manifestations. While pharmacologic therapies remain the cornerstone of management, a growing body of evidence highlights the role of extracranial peripheral nerve compression as a significant contributor to migraine pathophysiology in selected patients. This recognition has expanded the therapeutic role of plastic surgery, offering anatomically targeted interventions that complement or surpass traditional medical approaches for refractory cases. From a plastic surgeon's perspective, op

The Stylohyoid Complex: An Update on Its Embryology, Comparative Anatomy and Human Variations.

The stylohyoid complex (SHC), comprising the styloid process (SP), stylohyoid ligament, and lesser horn of the hyoid bone, arises from Reichert's cartilage and plays a central role in head and neck organization. Although anatomically small, it occupies a strategic position in the parapharyngeal space, linking neural, vascular, and visceral compartments. This review integrates embryological, comparative, anatomical, and clinical perspectives to provide an updated synthesis of SHC morphology and significance. Developmental studies highlight the early segmentation of Reichert's cartilage, its tra

Update on clinical characteristics in the evaluation of phaeochromocytoma and paraganglioma.

Pheochromocytomas and sympathetic paragangliomas (PPGL) are rare neuroendocrine tumors originating from chromaffin tissue of the adrenal medulla and extra-adrenal sympathetic paraganglia. Historically, many of these tumors were diagnosed postmortem, earning pheochromocytomas the moniker "great mimic" due to their diverse clinical manifestations that can resemble various other conditions. Over time, the clinical presentation of PPGL has evolved, with a shift from symptomatic or postmortem diagnoses to more frequent incidental discoveries or diagnoses through screening, with postmortem identific
