Arthroscopic microfracture treatment for osteonecrosis of the knee.

PURPOSE: This study evaluated the results of arthroscopic subchondral microfracture performed on patients with spontaneous osteonecrosis (ON) (group 1) or secondary ON (group 2) of the knee joint. TYPE OF STUDY: Retrospective clinical study. METHODS: Group 1 included 26 patients (mean age, 48 years) who had spontaneous ON. Group 2 included 15 patients (mean age, 32 years) with ON secondary to inflammatory disease or steroid therapy. Seventy-six percent of the chondral defects were located in the medial femoral condyle. The average defect sizes in group 1 was 162 mm2 and in group 2 was 362 mm

Spontaneous osteonecrosis of the knee: tibial plateaus.

Spontaneous osteonecrosis of the medial tibial plateau is less recognized than osteonecrosis of the medial femoral condyle, but it presents in a similar manner. These patients have a sudden onset of pain on the medial side of the knee associated with a spectrum of MRI changes in the tibial subchondral bone. The small lesions can resolve with only minimal residual scar remaining in the subchondral zone. If the lesion is large, it can collapse or show MRI changes of osteonecrosis. Recognition of this problem may help avoid unnecessary intra-articular surgical intervention.

Outcome of hemiresurfacing in osteonecrosis of the femoral head.

Hemiresurfacing of the femoral head for treatment of osteonecrosis has been proposed as a reasonable alternative to total hip arthroplasty. The results of 59 patients with Ficat Stage III osteonecrosis done by a single surgeon are reviewed. At an average followup of 4.5 years, 16 patients were considered failures because of conversion to total hip arthroplasty or considerable groin pain requiring medication. Failure did not correlate with age, body mass index, preoperative length of symptoms, acetabular articular cartilage status at the time of surgery, or cause of the underlying disease. The

Spontaneous resolution of osteonecrosis of the femoral head.

BACKGROUND: The decision to proceed with surgical treatment of asymptomatic osteonecrosis of the femoral head is controversial. The purpose of the present study was to determine (1) if spontaneous resolution of osteonecrosis of the femoral head occurs, (2) how long it takes for resolution to occur, and (3) if there are predictors of spontaneous resolution. METHODS: Patients with asymptomatic osteonecrosis of the femoral head were identified from two National Institutes of Health-funded screening studies: a prospective screening study for osteonecrosis of the femoral head after organ transplan

Idiopathic osteonecrosis of the medial tibial plateau.

Osteonecrosis of the medial tibial plateau is characterized by acute pain on the medial aspect of the knee. Progression can lead to articular collapse and requires early diagnosis and treatment. We studied seven patients affected of idiopathic osteonecrosis of the tibial plateau. The mean age was 62 years and the mean follow-up 42 months. We performed roentgenograms in all patients, bone scans in three patients and magnetic resonance image (MRI) in five. MRI shows T1-weighted low-intensity signal and T2-weighted high-intensity signal with a surrounding area of intermediate low-intensity signal

Can osteonecrosis of the femoral head be recurrent?

Three patients with osteonecrosis of the femoral head in whom the extent of necrosis increased by repeated episodes of osteonecrosis are described. In all cases the evidence of two attacks of osteonecrosis was detected clearly on magnetic resonance images. The significance of the recurrent attacks of osteonecrosis is not clear and the detectable incidence of the recurrence seems to be low. The authors propose the possibility that the osteonecrosis may be the result of multiple episodes of necrosis.

Osteonecrosis: what to do, what to do!

Despite extensive literature on the subject, the treatment approaches for osteonecrosis of the femoral head (ONFH) remain controversial. The literature presently available to the orthopaedist suggests that i) ONFH can be identified early; ii) there are reasonable treatments for early stages of ONFH; iii) no single treatment is sufficient for all hips; and iv) there is as yet no predictable permanent treatment for early ONFH. The author's recommended approach for the practicing orthopaedist includes the following: i) develop a high degree of clinical suspicion that the disease is present and un

Osteonecrosis of the humeral head: results of replacement.

One hundred twenty-seven shoulders treated with replacement arthroplasty for osteonecrosis of the humeral head were reviewed and assessed by mailed questionnaire for results, durability, and complications. These included 71 humeral head replacements and 56 total shoulder replacements. Thirty-six shoulders were lost to follow-up because of death of the patient; an additional 3 shoulders were locking survey follow-up. Thus current follow-up was available for 88 shoulders. At an average follow-up of 8.9 years, subjective improvement was expressed for 70 (79.5%) of the shoulders and no to occasion

A clinicopathologic study of osteonecrosis in the osteoarthritic hip.

OBJECTIVE: To investigate the incidence and nature of secondary osteonecrosis observed in osteoarthritis (OA) of the femoral head. DESIGN: This study is based on a retrospective review of the histopathologic and radiologic materials obtained from 906 consecutive cases (1007 femoral heads) diagnosed as OA. RESULTS: Secondary osteonecrosis was recognized grossly and confirmed microscopically in 38.2% of the femoral heads. The lesions were categorized into two types based on shape, size and depth; 'shallow' flat lesion (median axis 3-10 mm, depth 2-3 mm) with or without cysts (368 cases, 36.5%)

Osteonecrosis of the femoral head associated with pregnancy.

A 31-year-old pregnant woman suddenly complained of bilateral hip pain 2 weeks before delivery. She was delivered of triplets by Caesarean section. She had been treated with human menopausal gonadotropin and human chorionic gonadotropin (hMG-hCG) before pregnancy. Radiograms of the hip joint showed collapse of the femoral heads bilaterally. Magnetic resonance imaging revealed a band pattern of low signal intensity for both hips on T1- and T2-weighted images. She had no history of steroid therapy or alcohol abuse. Osteonecrosis of the femoral heads bilaterally associated with pregnancy was conf

Acetabular involvement in osteonecrosis of the femoral head.

When an arthroplasty is needed to treat osteonecrosis of the femoral head, the use of a component that replaces or resurfaces only the femoral head is often considered as an alternative to total hip replacement if the acetabulum appears radiographically normal. However, the long-term results of the use of endoprostheses have often been poor, secondary in part to progressive degeneration of the acetabular cartilage and to protrusio acetabuli. To help to explain these observations, we examined the acetabular cartilage in forty-one hips in which a primary total hip replacement had been performed

Osteonecrosis of the knee: current clinical concepts.

Osteonecrosis of the knee should be differentiated into two main categories: (1) primary, spontaneous, or idiopathic osteonecrosis and (2) secondary osteonecrosis (e.g., secondary to factors such as steroid therapy, systemic lupus erythematosus, alcoholism, Caisson decompression sickness, Gaucher's disease, hemoglobinopathies, etc.). Spontaneous or primary osteonecrosis of the knee presents with an acute knee pain in elderly patients. It is three times more common in women than in men. Traumatic and vascular theories have been proposed as a causative factor of osteonecrosis of the knee, but th

Benign osteonecrosis of the external ear canal.

Benign osteonecrosis (BON) of the external ear canal (EEC), also termed as focal or circumscribed necrotizing lesion, is an infrequent phenomenon with distinctive features and of an obscure origin. Five patients with BON of the EEC presented with aggressiveness and extension of varying degree including involvement of the middle ear. It seems that the disease might have a self limited course (two patients) though, at times, extensive measures including hyperbarric oxygen therapy (one patient) should be applied.
