Surgical correction of supination deformity in children with obstetric brachial plexus palsy.

We present a series of 40 children who were operated on for supination contracture following severe obstetric brachial plexus palsy. Surgery was done at an average age of 7 years and the mean postoperative follow-up was 4 years. In the 23 cases treated by an open or closed radial osteotomy, the mean intraoperative derotation was 78 degrees, the immediate postoperative position was 29 degrees pronation and it stabilized at follow-up at 17 degrees pronation. Biceps rerouting was performed in 17 cases without any recurrence of supination deformity and the final position was 22 degrees pronation.

Nerve injuries in neonates.

A clinical study of 40 cases of nerve injuries in neonates with one-year follow-up is presented. The total incidence of such injuries was 1.50%. Thirty-three cases sustained nerve injury due to obstetrical trauma and in 7 cases it was due to postnatal trauma like intragluteal injection and tight splintage. Almost in all cases there was history of prolonged or obstructed labour. Upper limb nerve injuries were commonest followed by facial palsy. Twenty-three cases (76%) showed complete recovery while 17% had partial and 7% no recovery at all. Hence nerve injuries are not uncommon in neonates. If

[Birth palsy of the brachial plexus--surgical exploration and attempted repair in twenty one cases (author's transl)].

Twenty one cases of birth palsy of the brachial plexus have been explored surgically. The patients were aged from seven weeks to nine years. The results of pre-operative examination are described and myelography is considered to be especially important. It was found that ruptures of the upper roots at the junction of C5-C6 were frequent and could be repaired by nerve graft. Avulsion from the spinal cord was more frequent in the lower roots. It is concluded that surgical exploration and attempted repair should be done early, preferably at the age of two or three months in cases of absence of sp

[Palsy of the upper limb: Obstetrical brachial plexus palsy, arthrogryposis, cerebral palsy].

"Palsy of the upper limb" in children includes various diseases which leads to hypomobility of the member: cerebral palsy, arthrogryposis and obstetrical brachial plexus palsy. These pathologies which differ on brain damage or not, have the same consequences due to the early achievement: negligence, stiffness and deformities. Regular entire clinical examination of the member, an assessment of needs in daily life, knowledge of the social and family environment, are key points for management. In these pathologies, the rehabilitation is an emergency, which began at birth and intensively. Splints

[Obstetric brachial plexus injury].

Obstetric palsy is a birth injury that occurs when the brachial plexus is damaged by traction. In the majority of patients spontaneous recovery will occur; however, in case of incomplete spontaneous recovery early neurosurgical intervention may be indicated. We present 3 case reports in this article, as well as describing the strategy favoured in our clinic. We recommend referring patients who have incomplete spontaneous recovery at the age of 1 month. At that age a good prediction of prognosis can be made by combining neurological examination with needle electromyography (EMG) of the biceps m

[Radius reed osteotomy for supination deformity in children with obstetrical brachial plexus palsy].

We report our experience and results in the use of reed pronating osteotomy in supination deformities secondary to obstetrical brachial plexus injury. This retrospective study involved 11 patients with paralytic supination of the forearm due to a brachial plexus injury. Other causes of paralytic supination were excluded. The surgical technique consisted of a proximal osteotomy of the ulna fixed by an intramedullary nail and a stable elastic reed osteotomy of the radius. The minimum postoperative follow-up was 2 years. Four boys and seven girls mean aged 8 years (5-12) were operated on between

Birth injuries of the brachial plexus.

Birth injuries of the brachial plexus are fairly common, but the majority of affected newborns make quick recoveries without any specific intervention. A minority suffer more severe injuries that lead to varying degrees of life-long disability. Happily, modern microsurgical techniques permit reconstruction of certain plexus injuries and, in carefully selected patients, can restore voluntary activity to target muscle groups. To what degree reanimation of paralyzed muscles improves function and quality of life for these children is a more important matter that has not yet been addressed at the l

Reliability of three classification systems measuring active motion in brachial plexus birth palsy.

BACKGROUND: Several classification systems for the categorization of function in patients with brachial plexus birth palsy have been proposed. The purpose of this investigation was to determine the intraobserver and interobserver reliability of the modified Mallet Classification, Toronto Test Score, and Hospital for Sick Children Active Movement Scale in the evaluation of these patients. METHODS: Eighty children with brachial plexus birth palsy were evaluated by two trained examiners on two different occasions. Intraobserver and interobserver reliability was determined with use of the kappa s

Tendon transfer to reconstruct wrist extension in children with obstetric brachial plexus palsy.

This study reports on 20 children with obstetric brachial plexus palsy who underwent a tendon transfer to reconstruct wrist extension. The mean age at the time of tendon transfer was 8 years. There were seven patients with Erb's palsy and the remaining 13 had total palsy. The flexor carpi ulnaris was utilized 15 times and the flexor carpi radialis five times. The transferred tendon was sutured to the tendon of the extensor carpi radialis brevis. The result of the transfer was assessed according to a modified Medical Research Council (MRC) muscle grading system. A good result was obtained in 18

Management of obstetrical brachial plexus palsy evaluation, prognosis, and primary surgical treatment.

Primary surgery for obstetrical brachial plexus lesions is a young field of surgical expertise that offers the possibility of improved functional ability in carefully selected patients who would otherwise be faced with lifelong impairment and secondary skeletal deformities. One major challenge in this area of peripheral nerve surgery is the selection of patients most likely to derive benefit from surgical intervention. The key to the development of selection criteria and to the resolution of other considerations (such as the determination of root avulsion) is consistency, accuracy, and careful

Ultrasound diagnosis of shoulder congruity in chronic obstetric brachial plexus palsy.

Ultrasound (US) was used to determine the congruity of the shoulder in 22 children with a deformity of the shoulder secondary to chronic obstetric brachial plexus palsy. There were 11 boys and 11 girls with a mean age of 4.75 years (0.83 to 13.92). The shoulder was scanned in the axial plane using a posterior approach with the arm internally rotated. The humeral head was classified as being either congruent or incongruent. The US appearance was compared with that on clinical examination and related to the intraoperative findings. All 17 shoulders diagnosed as incongruent on US were found to be

Focal upper extremity neuropathy in neonates.

Focal upper extremity neuropathies are common in neonates. The brachial plexus is the most common site involved. Brachial plexus injuries may involve different structures, thus producing different clinical presentations: complete brachial plexus palsy, Duchenne-Erb palsy, upper-middle trunk brachial plexus palsy, Klumpke palsy, fascicular brachial plexus palsy, and bilateral brachial plexus palsy. The causes of brachial plexus palsy are obstetric injury, intrauterine compression, humeral osteomyelitis, hemangioma, exostosis of the first rib, neck compression, and neoplasm. The differential dia

Medico-legal aspects of brachial plexus injury: the obstetrician's point of view.

The author reviews the obstetric literature with regard to factors predisposing to or predicting the occurrence of brachial plexus injury in the newborn. Based on the evaluated data, it is concluded that, whereas there are identifiable predisposing factors for shoulder dystocia, to which the occurrence of brachial plexus injury is widely attributed, the predictive power of the various identifiable factors is generally low. The writer also quotes literary data which suggest that injury to the brachial plexus may occur in utero before labor and, thus, unrelated to arrest of the shoulders during

Obstetric brachial plexus palsy.

In a retrospective survey of 46 conservatively treated patients with obstetric brachial plexus palsy (OBPP) the following conclusions were reached. An immediate examination of the neurological status of the neonate root by root is essential for proper analysis of the course of the recovery. Mild cases with C5-C6 root injury (Erb's palsy) have a good outcome and may be treated conservatively. Intermediate cases with C5-C7 root injury with additional drop hand require close observation as early as age 3 weeks when the first electromyographic (EMG) examination is indicated. The second EMG should

Obstetrical brachial plexus lesions.

The renewal of interest in brachial plexus surgery has been led by the success of microsurgical techniques in other peripheral nerve repairs. The results in adults remain poor, but in obstetrical palsy the regenerative capacity of babies allows for dramatic recovery after nerve repair or reconstruction. Obstetrical brachial plexus injuries represent a unique group with similar aetiology, age and prognosis.

Upper cervical spinal cord injury in neonates: the use of magnetic resonance imaging.

Neonatal upper cervical spinal cord injury is associated with rotational forceps delivery and presents with quadriparesis and diaphragmatic paralysis. The underlying pathology determines neurologic outcome but is difficult to assess clinically or with simple radiographic techniques. We report 4 cases in which early magnetic resonance imaging demonstrated the extent and severity of the injury and guided management.
