Using the patient safety indicators to detect potential safety events among US veterans with psychotic disorders: clinical and research implications.

OBJECTIVE: Patients with psychotic disorders often experience poorer health outcomes, but whether they experience increased risks of medical errors/patient safety events is less clear. A single state-level US study found acute care inpatients with schizophrenia were at higher risk of incurring some of the Agency for Healthcare Research and Quality (AHRQ) patient safety indicators (PSIs). We examined a nationwide sample of US Veteran's Health Administration (VHA) acute care inpatients to determine the rates observed among patients with psychotic disorders for a larger set of PSIs. DESIGN: Retr

Improving Patient Safety Culture Perceptions in Resource-Limited Operating Rooms: A Nurse-Led Multifaceted Implementation of the WHO Surgical Safety Checklist.

BACKGROUND: Strengthening patient safety culture (PSC) is essential for enhancing clinical practice, teamwork, and nursing leadership in surgical settings. However, evidence on interventions that promote PSC in operating rooms (ORs) in low- and middle-income countries remains limited. OBJECTIVE: To evaluate the association between a multifaceted intervention and PSC perceptions among OR professionals in a resource-limited setting. METHODS: A quasiexperimental, nonrandomized before-and-after study with a concurrent control group was conducted over 21 months across 12 ORs in two tertiary hospi

Leadership and Organizational Responses After Patient Safety Incidents: A Systematic Review of Second-Victim Outcomes and Evidence Gaps in Safety Behaviors.

Background/Objectives: Patient safety incidents can adversely affect healthcare professionals, but relationships between leadership and organizational responses, clinician recovery, professional consequences, and safety-related outcomes remain incompletely synthesized. This review examined contemporary quantitative evidence on these relationships and the extent to which directly measured safety behaviors and patient-safety outcomes have been evaluated. Methods: This PROSPERO-registered systematic review (CRD420261453278) followed PRISMA 2020 and PRISMA-S. PubMed and Scopus were searched as the

SGLT2 Inhibitors and Safety in Older Patients.

SGLT2 inhibitors (SGLT2i) are effective in the management of diabetes and in reducing adverse cardiovascular and renal outcomes. Randomized clinical trials demonstrated safety and tolerability in older adults. Adverse effects associated with SGLT2i are impacted by patient frailty, comorbidities, and concomitant medication use and, therefore, must be thoroughly evaluated before initiating treatment. The risk of volume depletion, hypoglycemia, genital infections, and diabetic ketoacidosis can be minimized by appropriate patient selection, patient education, and early symptom recognition. Limited

Safety of radiotherapy for hemodialysis patients with cancer.

BACKGROUND: The number of hemodialysis (HD) patients is increasing worldwide, and they are at a higher risk of cancer than the general population. Because HD patients are more likely to have inflammation, radiotherapy (RT)-induced adverse effects (AEs) are theoretically expected to be worse for HD patients. Since only a few reports have been published on this subject, we aimed to evaluate the tolerability of RT in HD patients. METHODS: We retrospectively analyzed AEs related to RT for HD patients. Our study included patients from three institutions treated between January 2007 and July 2017.

Evaluating the safety of emicizumab in patients with hemophilia A.

Patients affected by hemophilia A often require frequent prophylactic and therapeutic self-infusion. For those who develop inhibitors, treatment options are limited and mortality is increased. Emicizumab, a bispecific antibody to Factors IXa and X that carries out the function of Factor VIII (FVIII), represents a novel therapeutic approach. Areas covered: We review the clinical trials and key laboratory assay research for emicizumab. Emicizumab reduced the annualized bleeding rate by 87% compared to placebo in patients with inhibitors. For patients without inhibitors, emicizumab reduced the an

Safety of Laparoscopy in Ventriculoperitoneal Shunt Patients.

There has been an increasing trend in Laparoscopic surgeries. There is also a higher incidence of patients with ventriculoperitoneal (VP) shunts due to the advances in the techniques of cerebral shunts. Surgeons may come across patients of VP shunts presenting with an indication for laparoscopic surgery. Although there is no absolute contraindication for laparoscopy in VP shunts, there is always a risk of raised intracranial pressure. We describe a case of VP shunt presenting with an ectopic pregnancy and undergoing laparoscopic salpingectomy. Patient withstood the procedure well and had an un

Efficacy and safety of rosuvastatin in Taiwanese patients.

BACKGROUND: Statins are effective in decreasing low-density lipoprotein cholesterol (LDL-C). The efficacy and safety of rosuvastatin, a newly launched statin, have not been determined in Taiwanese patients. METHODS: Patients with hypercholesterolemia receiving rosuvastatin 10 mg/d in this hospital were prospectively followed and retrospectively analyzed. Men and women with primary hypercholesterolemia were eligible for inclusion in the study if they were either lipid-lowering therapy (LTT)-naive or had been receiving starting doses of other LLT that had proved ineffective in reaching goals. [The safety of patients and staff in the operating room].

The risk of postoperative infection is determined by a number of factors including the underlying health of the patient, the type of surgery and the environment in which the procedure take place. The predominant infective organisms are Staphylococci. Coagulase-negative staphylococci are regarded as emerging pathogens, especially in clean surgery. The operating team is the major source of the contaminated bacteria. Bacteria dispersed from the skin is of greater importance than bacteria from the respiratory tract, so masks are not always necessary in the theatre during an operation except by tho

Safety of flexible bronchoscopy in elderly patients.

BACKGROUND: Flexible bronchoscopy is an indispensable tool in respiratory medicine. In the context of an aging society and increasing life expectancy, the number of elderly, often multimorbid patients is growing. This raises important questions regarding the safety of flexible bronchoscopy in this population. METHODS: In this retrospective study 1841 flexible bronchoscopies performed at two sites of Charité Universitätsmedizin Berlin in the years 2022 and 2023 were assessed and classified into two age groups: patients ≥ 70 years (elderly group) and patients < 70 years (non-elderly group). Efficacy and safety of hydrokinesitherapy in patients with dystrophinopathy.

Duchenne muscular dystrophy (DMD) is one of the most common forms of hereditary muscular dystrophies in childhood and is characterized by steady progression and early disability. It is known that physical therapy can slow down the rate of progression of the disease. According to global recommendations, pool exercises, along with stretching, are preferable for children with DMD, as these types of activities have a balanced effect on skeletal muscles and allow simultaneous breathing exercises. The present study aimed to evaluate the effectiveness of regular pool exercises in patients with Duchen
