Long-Term Survival After Pelvic Exenteration for Locally Recurrent Rectal Cancer Associated With Crohn's Disease With, Adjuvant Chemotherapy, and Immunosuppressive Therapy.

Crohn's disease (CD) is associated with an increased risk of developing colorectal cancer. In particular, cases in which long-term survival is achieved by patients with local recurrence of CD-associated rectal cancer are rare. We report a case in which curative resection was achieved for a 47-year-old man with long-standing CD and locally recurrent rectal cancer. In this case, the patient obtained a long-term survival without recurrence after surgical resection with adjuvant chemotherapy and immunosuppressive therapy. In the management of inflammatory bowel disease patients with cancer, the ma

Intestinal fistulae formation following pelvic exenteration: a review of the University of Texas M. D. Anderson Cancer Center experience, 1957-1990.

Intestinal fistulae are an uncommon but serious complication of pelvic exenteration. To characterize factors leading to fistula formation and to define optimal management of this complication, we reviewed 533 cases of patients who underwent pelvic exenteration at the University of Texas M. D. Anderson Cancer Center between 1957 and 1990. Forty-two of those patients developed an intestinal fistula following total (n = 29), anterior (n = 12), or posterior (n = 1) exenteration which was not tumor related. Prior to routine pelvic floor reconstruction, the fistula rate was 16%. With the advent of o

Reconstruction of a Large Perineal Defect After Radical Oncological Pelvic Exenteration in a Patient With Anal Cancer: An Interdisciplinary Approach and Literature Review.

INTRODUCTION: Recurrent anal cancer (AC) often requires surgical intervention, especially when large perineal defects must be reconstructed. These cases are complicated by poor tissue vascularity and comorbid conditions such as peripheral arterial disease (PAD). METHODS: We report the case of a 64-year-old male with PAD and recurrent AC, presenting with a large perineal defect following radical pelvic exenteration. Preoperative iliac artery stenting was performed to ensure adequate perfusion. Surgical reconstruction was conducted in two stages: first, a vertical rectus abdominis myocutaneous

Laparoscopic Posterior Pelvic Exenteration (Complete and Supralevator) for Locally Advanced Adenocarcinoma of the Rectum in Females: Surgical Technique and Short-Term Outcomes.

Background: Laparoscopic posterior exenteration (total and supralevator) is a complex and rarely done procedure. In this study we describe the surgical technique and short-term perioperative outcomes in 7 female patients of locally advanced carcinoma rectum operated with laparoscopic pelvic exenteration. Materials and Methods: We report 7 cases of carcinoma rectum involving either posterior wall of the uterus or vagina, which were operated with a laparoscopic procedure. All perioperative and intraoperative data were collected retrospectively from prospectively maintained electronic data. Resul

Combined serratus anterior and latissimus dorsi myocutaneous flap for obliteration of an irradiated pelvic exenteration defect and simultaneous site for colostomy revision.

BACKGROUND: Usually, several surgical methods are used, with re-suturing, free skin grafting and local flaps, for the reconstruction of wall defects after abdominoperineal resection. However, or larger defects, free flaps have been preferred because they can provide a large area of well-vascularized soft tissue, which is suitable for defect repair. We present the case of a large abdominal wall defect, which was treated with a free combined serratus anterior and latissimus dorsi myocutaneous flap, resulting in a successful outcome. CASE PRESENTATION: A 38-year-old female originally had squamou

Systematic Review and Meta-analysis of Perioperative Outcomes of Minimally Invasive and Open Pelvic Exenteration for Locally Advanced and Recurrent Colorectal Cancer.

BACKGROUND: Pelvic exenteration (PE) is a complex surgery used for locally advanced and recurrent colorectal cancer. With most PE procedures performed via laparotomy, the role of minimally invasive surgery (MIS) in PE remains unclear. This systematic review aimed to assess perioperative outcomes of MI compared to open PE in locally advanced and recurrent colorectal cancer. METHODS: A PRISMA-compliant systematic search of PubMed, Scopus, and Google Scholar was conducted through March 2026. Studies comparing MI (laparoscopic or robotic) with open PE for locally advanced colorectal cancer were i

The impact of staple transection of the dorsal venous complex and urethra on intraoperative blood loss in cooperative laparoscopic and transperineal endoscopic pelvic exenteration.

PURPOSE: While laparoscopic pelvic exenteration reduces intraoperative blood loss, dorsal venous complex bleeding during this procedure causes issues. We previously introduced a method to transect the dorsal venous complex and urethra using a linear stapler during cooperative laparoscopic and transperineal endoscopic (two-team) pelvic exenteration. The present study assessed its effectiveness in reducing intraoperative blood loss by comparing it with conventional laparoscopic pelvic exenteration. METHODS: This retrospective cohort study was conducted at a Japanese tertiary referral center. El

Total Exenteration En Bloc with a Nephrectomy for Locally Advanced Cervical Cancer Invading a Pelvic Kidney-a Case Report and Literature Review.

Introduction: Extended pelvic resection might be the option of choice in patients presenting locally advanced cervical cancer. However, the possibility of a co-existence of an ectopic, pelvic kidney that is invaded by such a tumor is extremely rare. Case Presentation: A 54-year-old female patient, diagnosed with locally advanced cervical cancer in the presence of a pelvic kidney, was submitted to surgery with curative intent. A large, abscessed cervical tumor invading the urinary bladder and the rectum was found, so a total exenteration was planned. Intraoperatively, tumor invasion of the left

Transperineal retropubic approach in total pelvic exenteration for advanced and recurrent colorectal and anal cancer involving the penile base: technique and outcomes.

BACKGROUND: Complete pathological resection of locally advanced and recurrent anorectal cancer is considered the most important determinant of survival outcome. Involvement of the retropubic space with cancer threatening or involving the penile base poses specific challenges due to the potential for margin involvement and blood loss from the dorsal venous plexus. In the present study we evaluate a new transperineal surgical approach to excision of anterior compartment organs involved or threatened by cancer which facilitates exposure and visualisation of the bulbar urethra and the deep vein of

[A case of total pelvic exenteration that closed the perineal wound with a rectus abdominis myocutaneous flap for lower rectum cancer with cutaneous fistula].

The patient was a 60-year-old man with lower rectum cancer invading into the prostate gland and urinary bladder. The tumor expanded locally and made perineal cutaneous fistula. We performed a total pelvic exenteration with extended resection of perineal skin and subcutaneous tissue, followed by reconstruction using vertical rectus abdominis myocutaneous flap (VRAM flap). Although a mild pelvic abscess developed after the operation, it was controllable by drainage because the dead cavity of pelvis was relatively small due to the filing effect of VRAM flap. The VRAM flap is useful for both pelvi

The Preoperative Prognosticators of Surgical Margins (R0 vs. R1) in Pelvic Exenteration-A 14-Year Retrospective Study from a Tertiary Referral Centre.

BACKGROUND/OBJECTIVES: Pelvic exenteration is a complex surgery considered for locally advanced or recurrent pelvic malignancies, entailing a radical en-block resection of multiple adjacent pelvic organs, followed by reconstructive surgery. Achieving R0 resection (complete removal of macroscopic and microscopic disease) is critical for improving survival outcomes. This study aimed to define patient, tumour, and surgical predictors of R0 resection in an irradiated field, thereby optimising patient selection and establishing a surgical roadmap for pelvic exenterations. METHODS: Our retrospectiv

[A case successfully treated with total pelvic exenteration after preoperative chemotherapy FOLFOX4 plus bevacizumab for unresectable sigmoid colon cancer with extramural progression].

The patient was a 73-year-old woman. Her chief complaints were abdominal pain and lower abdominal distension. After some examinations, we diagnosed pelvic tumor, bladder cancer (adenocarcinoma) and sigmoid colon cancer. We performed a first operation, but the pelvic tumor was firmly fixed anterior to the sacrum and right common pelvic artery. We judged it unresectable and performed tumor biopsy and ileostomy. The pathological findings were very similar to sigmoid colon cancer, so we diagnosed that the pelvic tumor was sigmoid colon cancer with extramural progression. Later, the patient was tre
