Treatment of Alice in Wonderland syndrome and verbal auditory hallucinations using repetitive transcranial magnetic stimulation: a case report with fMRI findings.

BACKGROUND: Alice in Wonderland syndrome (AIWS) is a rare cluster of CNS symptoms characterized by visual distortions (i.e. metamorphopsias), body image distortions, time distortions, and déjà experiences. Verbal auditory hallucinations (VAHs) are the most prevalent type of hallucination in adults with or without a history of psychiatric illness. Here, we report the case of a woman with AIWS, long-lasting VAHs, and various additional perceptual and mood symptoms. METHODS: Semi-structured interviews were used to assess symptoms, and functional MRI (fMRI) was employed to localize cerebral activ

Nonveridical visual perception in human amblyopia.

PURPOSE: Amblyopia is a developmental disorder of spatial vision. There is evidence to suggest that some amblyopes misperceive spatial structure when viewing with the affected eye. However, there are few examples of these perceptual errors in the literature. This study was an investigation of the prevalence and nature of misperceptions in human amblyopia. METHODS: Thirty amblyopes with strabismus and/or anisometropia participated in the study. Subjects viewed sinusoidal gratings of various spatial frequencies, orientations, and contrasts. After interocular comparison, subjects sketched the su

Alice in Wonderland syndrome as aura of migraine.

Alice in Wonderland syndrome (AIWS), named for Lewis Carroll's titular character, is a disorder characterized by transient episodes of visual hallucinations and perceptual distortions, during which objects or body parts are perceived as altered in various ways (metamorphopsia), including enlargement (macropsia) or reduction (micropsia) in the perceived size of a form. Migraine aura is a transient neurological symptom that most commonly involves the visual fields and occurs before the headache phase. Aura symptoms include the perception of flashing lights that begin in the center of vision and

A continuum hypothesis of psychotomimetic rapid antidepressants.

Ketamine, classical psychedelics and sleep deprivation are associated with rapid effects on depression. Interestingly, these interventions also have common psychotomimetic actions, mirroring aspects of psychosis such as an altered sense of self, perceptual distortions and distorted thinking. This raises the question whether these interventions might be acute antidepressants through the same mechanisms that underlie some of their psychotomimetic effects. That is, perhaps some symptoms of depression can be understood as occupying the opposite end of a spectrum where elements of psychosis can be

Data-Driven Approach for Auditory Profiling and Characterization of Individual Hearing Loss.

Pure-tone audiometry still represents the main measure to characterize individual hearing loss and the basis for hearing-aid fitting. However, the perceptual consequences of hearing loss are typically associated not only with a loss of sensitivity but also with a loss of clarity that is not captured by the audiogram. A detailed characterization of a hearing loss may be complex and needs to be simplified to efficiently explore the specific compensation needs of the individual listener. Here, it is hypothesized that any listener's hearing profile can be characterized along two dimensions of dist

Cerebellar Time and Relative Time: A Comparator-Based Dynamical Timing Model and its Relevance to Psychopathology and Therapies.

Time perception is fundamental to adaptive behavior, providing the scaffold for prediction, coordination, and learning. The cerebellum has long been recognized as a core hub for interval timing, yet its role extends beyond motor control to perceptual chronometry, reinforcement learning, and affective regulation. Here we introduce a novel framework, a Comparator-Based Dynamical Timing (CDT) model which describes distortions of subjective time as transformations that yield compression, dilation, and changes in temporal precision. In this account, subjective time is scaled by a gain factor κ. Whe

Cerebral perfusion in children with Alice in Wonderland syndrome.

Alice in Wonderland syndrome (AIWS) is characterized by visual hallucinations and bizarre perceptual distortions. Technetium-99m hexamethylpropyleneamine tomography (SPECT) brain scans were performed in four patients during the acute stage of AIWS. Two patients were demonstrated to have Epstein-Barr virus infections. One had abnormal (EEG) findings. The visual-evoked potential, cranial CT, and MRI findings were negative. The decreased cerebral perfusion areas in all patients were near the visual tract and visual cortex. All involved some regions of the temporal lobe. In most patients with AIWS

[REVIEW OF PSYCHODYSLEPTICS: I. CLASSIFICATION OF PSYCHODYSLEPTICS].

The discovery of new dysleptic drugs has prompted the present review of these substances which influence neuropsychic activity. It is possible to divide dysleptics into two categories: under the first heading fall mescaline, LSD-25, psilocybine, adrenochrome, bufotenine and dimethyltryptamin; the other group includes Ditran, Butoxamine, WH-4849, and AHR-379. The place of Sernyl could not be ascertained and it might well constitute a class by itself. The mescaline type of reaction is mainly characterized by the induction of perceptual distortions without alteration of the state of consciousness

Identifying segmental and prosodic errors associated with the increasing word length effect in acquired apraxia of speech.

Purpose: Individuals with stroke-related apraxia of speech (AOS) plus aphasia tend to produce more speech errors with increasing word length. The Words of Increasing Length task (WIL) uses a 3-point scale to score word accuracy but penalises for error types that can arise either from language or motor impairment, reducing the test's sensitivity and specificity. The purpose here was to identify error types explaining variance in the WIL score, and those associated with AOS and word length.Method: Speech errors were perceptually identified on the WIL task for 51 Australian English-speaking adult

The right cerebral hemisphere: emotion, music, visual-spatial skills, body-image, dreams, and awareness.

Based on a review of numerous studies conducted on normal, neurosurgical and brain-injured individuals, the right cerebral hemisphere appears to be dominant in the perception and identification of environmental and nonverbal sounds; the analysis of geometric and visual space (e.g., depth perception, visual closure); somesthesis, stereognosis, the maintenance of the body image; the production of dreams during REM sleep; the perception of most aspects of musical stimuli; and the comprehension and expression of prosodic, melodic, visual, facial, and verbal emotion. Associations of Cognitive Expectancies With Auditory Hallucinations and Hallucinatory-Like Experiences in Patients With Schizophrenia.

BACKGROUND: Various neurocognitive models explore perceptual distortions and hallucinations in schizophrenia and the general population. A variant of predictive coding account suggests that strong priors, like cognitive expectancy, may influence perception. This study examines if stronger cognitive expectancies result in more auditory false percepts in clinical and healthy control groups, investigates group differences, and explores the association between false percepts and hallucinations. STUDY DESIGN: Patients diagnosed with schizophrenia with current auditory hallucinations (n = 51) and w

Assessment of Childhood Apraxia of Speech: A Review/Tutorial of Objective Measurement Techniques.

Background With respect to the clinical criteria for diagnosing childhood apraxia of speech (commonly defined as a disorder of speech motor planning and/or programming), research has made important progress in recent years. Three segmental and suprasegmental speech characteristics-error inconsistency, lengthened and disrupted coarticulation, and inappropriate prosody-have gained wide acceptance in the literature for purposes of participant selection. However, little research has sought to empirically test the diagnostic validity of these features. One major obstacle to such empirical study is

[Facets of Body Image Disorder: A Differential Analysis of the TEK-KJ (Test for the Detection of Body Disorder in Children and Adolescents) for the Direct Measurement of Haptic-Perceptual Parts].

There is presently a lack of instruments that measure the haptic-perceptive component of body-image distortion – a central criterion for the eating disorders anorexia nervosa and bulimia nervosa. We present a differential analysis for the Test for Body Image Distortion in Children and Adolescents (BID-CA, German: TEK-KJ) using a large child and adolescents sample, including for the first time male participants. We analyze convergent validity with the Contour Drawing Rating Scale (CDRS) as well as differences between children and adolescents from different type of schools and different BMI perc

[Psychometric properties of the French version of the How I Think Questionnaire].

UNLABELLED: Certain research has pointed out the relative importance of cognitive distortions in the development of antisocial behavior. Distortions of social cognitions that facilitate aggression and other types of antisocial behavior have been described as self-serving cognitive distortions (Barriga et al., 2001 [2]). Considering the importance of the assessment of delinquent cognitive distortions, an instrument has been validated to measure self-serving cognitive distortions: the How I Think Questionnaire (HIT-Q, Barriga et al., 2001 [2]). Thus, the aim of the present study was to evaluate

The effects of reality distortion syndrome on salient stimuli processing in patients with schizophrenia: an fMRI study.

Schizophrenia is associated with interpersonal difficulties related to impairments in the processing of facial emotional expressions. The aim of the present study was to identify brain regions associated with reality distortion syndrome reduction in a group of patients with schizophrenia during processing of emotionally salient stimuli. We used functional magnetic resonance imaging to measure cerebral blood oxygenation changes during an implicit emotional task in 11 patients with schizophrenia, who were scanned twice with an interval of 6-8 weeks. We found that reality distortion syndrome redu

Tinnitus, diminished sound-level tolerance, and elevated auditory activity in humans with clinically normal hearing sensitivity.

Phantom sensations and sensory hypersensitivity are disordered perceptions that characterize a variety of intractable conditions involving the somatosensory, visual, and auditory modalities. We report physiological correlates of two perceptual abnormalities in the auditory domain: tinnitus, the phantom perception of sound, and hyperacusis, a decreased tolerance of sound based on loudness. Here, subjects with and without tinnitus, all with clinically normal hearing thresholds, underwent 1) behavioral testing to assess sound-level tolerance and 2) functional MRI to measure sound-evoked activatio

Body images of patients with anorexia nervosa, bulimia nervosa and female control subjects: a comparison with male ideals of female attractiveness.

Body images of female patients with anorexia nervosa and bulimia nervosa were assessed against females without eating disorders and compared with male ideals of female attractiveness. A computer program was applied to examine body images of 62 patients with anorexia nervosa, 45 patients with bulimia nervosa, and 40 female and 39 male control subjects. Body size overestimation was most distinct in the two patient groups. Self-ideal discrepancy was highest in bulimia nervosa. Estimation of the society's ideal female body in all three female groups did not differ from men's perception of the most

Effects of anesthesia on efferent-mediated adaptation of the DPOAE.

Distortion product otoacoustic emissions (DPOAE) adapt after primary tone onset, with an approximately 100 ms time constant, due to feedback effects of medial olivocochlear (MOC) activity elicited by the primary tones. We tracked DPOAE postonset adaptation as a metric of MOC reflex strength, before during and after induction of anesthesia in guinea pigs. Reflex strength was significantly diminished by the barbiturate/neuroleptic anesthesia most commonly used in this species. The MOC reflex recovered more slowly than toe-pinch or startle reflexes, correlating better with recovery of general mob

Characterization of hearing in an X,0 'Turner mouse'.

Turner's syndrome is due to total (45,X) or partial (mosaicism) loss of one X-chromosome. The main features are short stature, ovarian dysgenesis with no estrogen production and infertility. In addition to ear and hearing disorders, middle ear problems including acute/serous otitis media and chronic middle ear disease are frequent. Sensorineural hearing loss is often seen with a dip in the mid-frequencies and also an early high frequency loss. In this study, middle-and inner-ear pathology was characterized using physiological and morphological techniques in a 'Turner mouse' that has been gener

Body size over-estimation in women with anorexia nervosa is not qualitatively different from female controls.

Over-estimation of body size is a cardinal feature of anorexia nervosa (AN), usually revealed by comparing individuals who have AN with non-AN individuals, the inference being that over-estimation is pathological. We show that the same result can be reproduced by sampling selectively from a single distribution of performance in body size judgement by comparing low BMI individuals with normal BMI individuals. Over-estimation of body size in AN is not necessarily pathological and can be predicted by normal psychophysical biases in magnitude estimation. We confirm this prediction in a dataset fro
