Retroperitoneal perivascular epithelioid cell tumours: A case report and review of literature.

BACKGROUND: The perivascular epithelioid cell tumour (PEComa) family of tumours mainly includes renal and hepatic angiomyolipomas, pulmonary lymphangioleiomyomatosis and clear cell "sugar" tumour of the lung. Several uncommon tumours with similar morphological and immunophenotypical characteristics arising at a variety of sites (abdominal cavity, digestive tract, retroperitoneum, skin, soft tissue and bones) are also included in the PEComa family and are referred to as PEComas not otherwise specified. CASE SUMMARY: We present a 37-year-old female patient who underwent resection of an 8.5 cm ×

Transarterial embolization for serious renal hemorrhage following renal biopsy.

The goal of this study is to evaluate the feasibility and efficacy of percutaneous transarterial embolization for the treatment of serious renal hemorrhage after renal biopsy. Nine patients with renal hemorrhage had frank pain and gross hematuria as main symptoms after renal biopsy. Intrarenal arterial injuries and perinephric hematoma were confirmed by angiography in all cases. The arterial injuries led to two types of renal hemorrhage, Type I: severe renal injure or intrarenal renal artery rupture (n=5), with contrast medium spilling out of the artery and spreading into renal pelvis or kidne

Prognostic factors in renal cell carcinoma: analysis of 227 patients treated at the Brazilian National Cancer Institute.

PURPOSE: We evaluated the role of several prognostic factors in predicting death and/or progressive disease in patients with renal cell carcinoma. MATERIALS AND METHODS: Between 2004 and 2010, 227 consecutive patients with renal cell carcinoma underwent radical nephrectomy at our Institute. All histological specimens were examined by the same pathologist. We considered certain histological parameters, including histological subtype, conventional Fuhrman grade, presence of sarcomatoid features, adrenal gland infiltration, invasion of the perinephric fat, vascular embolization, collecting syste

Xanthogranulomatous Pyelonephritis in a male child with renal vein thrombus extending into the inferior vena cava: a Case Report.

BACKGROUND: We present a case of Xanthogranulomatous pyelonephritis (XGPN) in a male child with renal vein thrombus extending into the inferior vena cava. This is a rare presentation. XGPN is a rare type of renal infection characterised by granulomatous inflammation with giant cells and foamy histiocytes. The peak incidence is in the sixth to seventh decade with a female predominance. XGPN is rare in children. CASE PRESENTATION: An 11 year old male child presented with a history of high grade fever and chills, right flank pain and progressive pyuria for two months. He had a history of vesical

Distant metastasis of renal cell carcinoma with a diameter of 3 cm or less-which is aggressive cancer?

PURPOSE: It was previously thought that renal cell carcinoma with a diameter of 3 cm or less has low potential to cause distant metastasis. However, metastasis develops in a small number of cases, which cannot be ignored. We investigated the clinicopathological characters of small renal cell carcinoma with metastasis to further understand this condition. MATERIALS AND METHODS: From January 1983 to February 2009, 165 cases of sporadic renal cell carcinoma 3 cm or less were treated at our department. Bilateral and von Hippel-Lindau disease were excluded from study. Clinicopathological parameter

Pretreatment of choriocarcinoma syndrome: a rare complication of metastatic germ cell tumours.

A 27-year-old man newly diagnosed with metastatic testicular choriocarcinoma developed a large right perinephric haematoma secondary to a metastatic deposit in his right kidney. His presentation was also complicated by bilateral iliac venous thrombosis and pulmonary embolism identified prior to initiation of chemotherapy. He underwent multiple attempts at angioembolisation of the bleeding vessels and ultimately angioembolisation of the main renal artery had to be performed to control the bleeding. Following resolution and commencement of chemotherapy, the patient also developed spontaneous int

Girl with xanthogranulomatous pyelonephritis and bronchiectasis: case report.

Xanthogranulomatous pyelonephritis (XGP) is a rare chronic inflammatory disorder of the kidney. Infiltration to lung and liver can occur. We present a rare complication of locally invasive XGP extending beyond the diaphragm to the lung to cause bronchiectasis in an adolescent girl with chronic productive cough, weight loss and no urinary symptoms. The patient underwent open left radical nephrectomy, where it was noted that the left kidney lay very high with significant perinephric inflammation and was densely adherent to the diaphragm and partially adherent to the spleen. XGP was confirmed on

A rare case of renal vein thrombosis secondary to Klebsiella pneumoniae pyelonephritis.

Renal vein thrombosis (RVT) is most often an implication of nephrotic syndrome. Pyelonephritis has been associated at a much lower rate, with the incidence of Klebsiella pneumoniae causation being extremely rare. In our case, a 35-year-old female patient presented with right-sided K. pneumoniae-positive acute pyelonephritis complicated by perinephric abscess and renal vein thrombosis. She was successfully treated with anticoagulation and extended antibiotic therapy. The possibility of RVT in patients with K. pneumoniae-induced pyelonephritis warrants consideration.

[Two cases of emphysematous urinary tract infections successfully treated with conservative management].

Emphysematous urinary tract infection is a rare, fulminant complication that is characterized by the presence of gas in the pelvicaliceal system, renal parenchyma, perinephric tissues and retroperitoneum. Surgical resection is usually regarded as the treatment of choice, however several studies have shown the safety and efficacy of conservative management under the correct diagnosis and appropriate antibiotic administration. We herein report on two cases of emphysematous urinary tract infection, pyelonephritis and cystitis infected with ESBL-producing Eschrerichia coli, complicated with diabet

Spontaneous rupture of renal pelvis.

Spontaneous urine extravasation from the pelvicalyceal system into the perinephric space is an uncommon condition. It is most commonly seen in the setting of obstructive ureteric calculus. Other rare causes include neoplasms, trauma, and iatrogenic procedures. Most commonly described phenomenon is forniceal rupture, but renal pelvis rupture without forniceal rupture is extremely rare. We present a short series of 3 cases of spontaneous pelvis rupture, each with a different etiology. Diagnosis was confirmed by computed tomography. The follow-up and therapeutic approaches are discussed with a sh

Epithelioid angiomyolipoma of the kidney in an adult male with tuberous sclerosis.

Epithelioid angiomyolipoma is a rare variant of angiomyolipoma and a part of the microphthalmia transcription factor (MiTF) tumors. We report a case of epithelioid angiomyolipoma of the kidney that occurred in a 40-year-old male. The tumor was composed of sheets of uniform epithelioid cells with clear to granular eosinophilic cytoplasm and hyperchromatic nuclei. The tumor extended into the vascular margin and perinephric fat with identified angiolymphatic invasion. Immunohistochemistry showed diffuse staining for HMB45, Melan A, and focal staining for SMA. No expression for EMA, AE1/AE3, CK7,

An unusual presentation of neonatal adrenal hemorrhage.

Case report of a neonate presenting within 1 h of birth with right side scrotal swelling and a dusky skin discoloration thought clinically to be a hydrocele or testicular torsion. Ultrasound examination demonstrated bilateral hydroceles, more prominent on the right, scrotal oedema and a perinephric loculated fluid collection secondary to right adrenal hemorrhage. We review the ultrasound appearance of the normal and haemorrhagic neonatal adrenal and discuss the differential diagnosis of both neonatal adrenal lesions and scrotal swellings. Ultrasound in the investigation of renal disease in infants and children.

The author reports the case histories of a number of neonates and children with renal disease to illustrate the use of grey-scale ultrasound in the investigation of such patients. In the series, ultrasound proved to be an excellent diagnostic tool in the investigation of masses of suspected renal origin, both in ascertaining their site of origin and in evaluating their nature. Other applications of diagnostic ultrasound included investigation of the kidney not visualised by urography, and suspected urinary tract obstruction, and renal or peri-renal infection. The characteristic ultrasound appe
