The measurement of the range of hand joint movement is an essential part of clinical practice and rehabilitation. Current methods use three finger joint declination angles of the metacarpophalangeal, proximal interphalangeal and distal interphalangeal joints. In this paper we propose an alternate form of measurement for the finger movement. Using the notion of reachable space instead of declination angles has significant advantages. Firstly, it provides a visual and quantifiable method that therapists, insurance companies and patients can easily use to understand the functional capabilities of

Hyperflexion of the thumb interphalangeal (IP) joint interferes significantly with pinching between the thumb and the index finger in a paralyzed hand. The extensor pollicis longus-loop-knot (ELK) procedure was used successfully in 7 tetraplegic patients to balance the IP joint between strong restored flexor function and lacking or weak antagonism of the thumb extensors. A V-shaped incision was made over the extensor hood at the level of the IP joint, the extensor pollicis longus (EPL) tendon was elevated, a loop was formed, secured by sutures, and then turned proximally onto the EPL tendon it

PURPOSE: Chronic paralysis of the flexor pollicis longus (FPL) and the index flexor digitorum profundus (FDP) is commonly treated with brachioradialis transfer to the FPL and side-to-side suturing of the index FDP to the FDPs of the ulnar digits. We report the clinical outcomes of nine patients in whom the extensor carpi radialis brevis (ECRB) or longus (ECRL) was transferred to both the FPL and index FDP.
METHODS: We reviewed nine patients who underwent tendon transfer of the ECRB/L to both the FPL and index FDP. Postoperative assessments included strength grading of the FPL and index FDP us

Purpose: Many dental hygienists experience musculoskeletal pain during the course of their careers, often as a result of the sustained grips on instruments and repetitive movements employed during clinical practice. Current research suggests that lighter instruments with a larger diameter reduce force and load on the hand during scaling procedures; therefore, the texture and weight of silicone handles is designed to decrease the strain placed on the hand and fingers. The purpose of this research is to investigate and compare the effect of silicone instrument handles and traditional stainless s

Thumb tendon transfer surgical procedures in patients with cervical spinal injury engage the paralyzed flexor pollicis longus (FPL) muscle to enable lateral pinch grasp. However, functional outcomes are mixed, in part because the FPL cannot consistently produce force at the thumb-tip to promote a stable grasp. We used simulation to investigate whether a multiple recipient muscle tendon transfer, targeting sets of paralyzed muscles driven by a single donor muscle, could outperform a single recipient muscle tendon transfer with the FPL alone and restore lateral pinch. We formed 36 groups of 2 mu

The authors report and discuss the management of a 25-year-old male patient with a trapeziometacarpal joint dislocation complicated by a trapezium fracture. This injury combination is rare with only reported 15 cases. After closed reduction resulted in dorsal instability, anatomical reduction of the trapezium fracture and internal screw fixation were performed using an open approach. This fixation stabilized the joint but was fragile due to the small fragment size. For this reason, ligament reconstruction using half of the flexor carpi radialis tendon was added. At the last follow-up, 16 month

BACKGROUND: Carpal tunnel syndrome is a common compressive neuropathy of the median nerve. Compared with standard release, the efficacy, safety, and postoperative complications of limited carpal tunnel release remain controversial. The purpose of this study was to compare the effects of the 2 treatments.
METHODS: The English-language literature was searched using MEDLINE, Web of Science, and Embase. Randomized controlled trials that compared standard and limited incision for carpal tunnel release were included in the meta-analysis. Strength, interval to return to activities, the rate of adver

Fifty patients with rheumatoid arthritis had 59 Swanson implants of the metacarpal-phalangeal joint of the thumb. Eleven patients (15 implants) have since died and only 1 patient (1 implant) has been lost to follow-up leaving 43 implants available for study. The most common preoperative deformity was a boutonniere deformity with a flexible interphalangeal joint. All thumbs had less pain after operation. The average range of active motion is 25 degrees, with a flexion arc from 15 to 40 degrees. There is an average key pinch strength of 4 pounds (range, 0.5 to 10). Improvement in activities of d

Twenty-two patients with roping injuries to 38 digits, including 19 patients injured while team roping, are discussed. Ten digits in nine patients were successfully revascularized or replanted. Seven digits in three patients failed after initial success. One patient is included in both categories. The failure rate is 41 percent for all 17 digits. Average follow-up is 18 months. The dominant hand was injured in 83 percent of team roping injuries; the thumb is the most commonly injured digit. Average interphalangeal motion for thumb replants is zero; for revascularizations, it is 47 degrees. The

An arthroscopic technique was used to reduce Stener lesions associated with acute ulnar collateral ligament ruptures of the thumb. We operated on eight thumbs, seven dominant and one nondominant. The average follow-up period was 39 months (range, 30-48). At follow-up examination, no patient reported pain or functional limitation or had joint contracture. In most cases, key pinch, tip pinch, grip strength, and range of motion were equal to or greater than in the thumb on the unaffected side. The only complication, a pin track infection, resolved after wire removal and treatment with oral antibi

Background: A radial incision with radial plate fixation for distal radius fracture has historically been avoided due to its risk to the superficial branch of the radial nerve (SBRN). With careful technique, it is possible to avoid injury to the SBRN, thereby minimizing the soft tissue injury associated with other approaches. We compare subjective and objective functional outcomes of radial plate fixation surgeries that we performed with those of dorsal and volar plate fixation in current literature. Methods: Patients at a single center who underwent radial plate fixation for an AO type A or A

BACKGROUND: With advances in the surgical management for severe ulnar neuropathy with the introduction of the super charged-end-to-side (SETS) anterior interosseous nerve (AIN) to ulnar nerve transfer, a simple and reliable outcome measure is required. There is currently not "one" standardized outcome measure used to represent and compare results.
PURPOSE: To present the abduction hand diagram as a "novel", reproducible, and simple outcome measure for patients with severe ulnar neuropathy.
STUDY DESIGN: Retrospective case series.
METHODS: Nine patients with severe entrapment/compressive uln

Treatment of carpal tunnel syndrome (CTS) remains a challenge for hand surgeons. Carpal tunnel release (CTR) using nasal instruments has the advantages of both endoscopy and open surgery. In this study we aimed to explore the effectiveness of CTR using nasal instruments in Chinese patients. We present a case series of 49 cases of idiopathic CTS treated with the mini-incision technique using nasal instruments. The average recovery days before return to normal work and complications were recorded. The mean grip strength, pinch strength, and sensation were evaluated. Subjective results were evalua
