Minimally invasive video-assisted thoracic surgery (VATS) lobectomy has proved to be equal and in some aspects superior to open lobectomy in T1 and T2 lung cancers. Indications for VATS pneumonectomy however are still not clearly defined and strictly limited. The minimally invasive VATS pneumonectomy can be undertaken in patients with centrally located tumors without extended invasion of the large pulmonary vessels, chest wall, pericardium, mediastinal structures or proximal part of the main bronchus and when sleeve resection is not feasible (T2). We present a case of a patient who underwent l

We report 4 patients with stage IVA thymic tumors who underwent extrapleural pneumonectomy and thymectomy with venous confluence resection using a temporary percutaneous venous jugular-femoral bypass technique. The superior vena cava was replaced in 2 patients, and the innominate vein was resected in 2 patients. Complete tumor resection was obtained in all patients. There was no 90-day postoperative mortality. One patient died at 6 months postoperatively of an unrelated cause, without recurrent disease, and 3 are alive and disease-free with a follow-up ranging from 19 to 80 months. Extrapleura

We report here on an unusual late postoperative presentation of extreme post-pneumonectomy dextrocardia and spontaneous contralateral pneumothorax presenting as late complications occurring approximately 2 years after right-sided pneumonectomy. Computed tomography is the diagnostic modality of choice to obtain information on anatomical changes within the post-pneumonectomy space. Knowledge of the spectrum of cardiopulmonary, pleural, and other complications after lung resection is important to properly manage complications in post-pneumonectomy patients.

PURPOSE: The morbidity and mortality for pneumonectomy in patients has been reported to be as high as 24%. To determine if a subset of patients undergoing pneumonectomy for a malignancy would have similar complication rates and appearances, we performed a review of the radiographic findings of patients at our institution.
METHOD: A retrospective review of a thoracic surgery database was performed at our institution for patients who underwent pneumonectomy between January 2001 and April 2004. All images were reviewed on the institutional patient archive communication system, by two experienced

Thymic hyperplasia occurs in a small proportion of patients receiving chemotherapy for various malignancies. It likely results from an immunologic rebound phenomenon. Fluorodeoxyglucose positron emission tomography-computed tomography is an important tool for staging malignant neoplasms. We report a case of rebound thymic hyperplasia manifesting as a hypermetabolic mass on fluorodeoxyglucose positron emission tomography-computed tomography after pneumonectomy and chemotherapy for primary pulmonary synovial sarcoma. We highlight the importance of recognizing the phenomenon of rebound thymic hyp

OBJECTIVE: Side- and sex-related differences were analysed to explain the occurrence of bronchopleural fistula (BPF) after pneumonectomy on the right-hand side in men.
PATIENTS AND METHODS: Surgical pathology reports on 209 patients (15 with BPF) were retrospectively reviewed regarding sex, age, side, TNM stage, outer diameter of the resection margin (mm) and intrabronchial distance between tumour and resection margin (mm). Patients without macroscopic bronchial invasion were categorised as peripheral tumours. The t-test, U-test (Mann-Whitney) and cross-tabulation using the chi 2-test were pe

Disruption of a mainstream bronchus is a rare but dreaded complication of pneumonectomy. When the bronchial tump measures at least 15 mm, the conventional therapeutic strategy is to drain the pleural cavity followed by closure of the fistula via trans-mediastinal sternotomy. After an experimental study on cadavers to test the technical feasibility of main bronchus closure via a cervical approach using a video-mediastinoscope, we used video-assisted mediastinal surgery successfully in a patient with a left main bronchus fistula.

We report the findings in a patient with shortness of breath due to pulmonary hypertension five years after left pneumonectomy. Mediastinal recurrence of an incompletely resected and slowly growing adenoid cystic carcinoma of the left main bronchus had encased the right main pulmonary artery.

BACKGROUND: Evaluate the efficacy and safety of pneumonectomy in the treatment of destroyed lung caused by benign diseases, and identify the risk factors for postoperative complications.
METHODS: We retrospectively analyzed 158 patients with destroyed lung caused by benign diseases who underwent pneumonectomy in a single center from January 2014 to December 2024. Cases were assigned to tuberculosis (TB) group and non-tuberculosis (non-TB) group. Clinical characteristics and postoperative complications were analyzed and compared between groups. Univariate and multivariate analyses were perform

OBJECTIVE: To improve treatment effectiveness in patients with bilateral pulmonary tuberculosis and indications for pneumonectomy.
MATERIAL AND METHODS: A retrospective analysis enrolled 25 patients who underwent surgery between 2012 and 2025. All patients underwent staged surgical treatment, including pneumonectomy and surgery on the contralateral lung. Patients were divided into two groups: group 1 (n=12) - pneumonectomy combined with lung resection; group 2 (n=13) - pneumonectomy combined with collapse surgery (predominantly extrapleural pneumonolysis with silicone implant placement).

Pulmonary mucormycosis (PM) is a rare and life-threatening condition, most prevalent in immunocompromised patients. Early signs and symptoms are often nonspecific. A high index of suspicion in at risk patients should prompt early infectious work-up, including bronchoscopy, followed by aggressive antifungal therapy and early surgical resection when indicated. We demonstrate these core tenants of diagnosis and management of PM via two patient presentations, the first involving a kidney transplant recipient who presented with a mild cough, found to have a lung lesion with rapid growth over a few

OBJECTIVES: Post-pneumonectomy syndrome (PPS) is rare and predominantly characterised by dynamic airway obstruction due to mediastinal rotation at any time point following pneumonectomy. The objective of this systematic review was to identify the optimal treatment strategy for PPS based on subjective symptomatic relief, objective radiological imaging, and treatment durability.
METHODS: A systematic review was performed up to and including February 2022 based on the "Preferred Reporting Items for Systematic Reviews and Meta-Analyses" guidelines. All studies that presented the management of sym

A 57-year-old man with a history of right pneumonectomy for squamous cell lung cancer who presented with dyspnea and hypotension, was found to have pericardial effusion complicated by cardiac tamponade, associated with pembrolizumab therapy. Pericardiocentesis could not be safely attempted due to presence of right-sided mediastinal tissue shift in the setting of previous right pneumonectomy. The patient improved significantly with surgical placement of pericardial window. Analysis of the pericardial fluid was negative for malignancy and was consistent with acute inflammation. Pembrolizumab and

In this protocol, we detail the correct procedural steps and necessary precautions to successfully perform a left pneumonectomy and induce PAH in rats with the additional administration of monocrotaline (MCT) or SU5416 (Sugen). We also compare these two models to other PAH models commonly used in research. In the last few years, the focus of animal PAH models has moved towards studying the mechanism of angioproliferation of plexiform lesions, in which the role of increased pulmonary blood flow is considered as an important trigger in the development of severe pulmonary vascular remodeling. OBJECTIVES: The objective of this retrospective study was to assess perioperative outcomes, overall survival and freedom from recurrence after induction chemotherapy followed by extrapleural pneumonectomy (EPP) or pleurectomy/decortication (P/D) in patients with mesothelioma in a propensity score matched analysis.
METHODS: Between September 1999 and August 2015, 167 patients received multimodality treatment (platinum-based chemotherapy followed by EPP [ n = 141] or P/D [ n = 26]). We performed 2:1 propensity score matching for gender, laterality, epithelioid histological subtype and Interna

The post-pneumonectomy syndrome is a rare complication consisting of rotation and herniation of the mediastinal structures, the remaining lung and the respective bronchi, into the contralateral hemithorax. This situation may produce symptomatic airway obstruction and varies in its presentation and severity. We describe one case of right and one of left pneumonectomy syndrome as well as one case of post-lobectomy syndrome. We review the literature on the pathophysiology, the clinical, radiological and bronchoscopic characteristics of this rare entity and discuss all available therapeutic altern
