Structural Barriers to Pre-exposure Prophylaxis Use Among Young Sexual Minority Men: The P18 Cohort Study.

BACKGROUND: Despite decreasing rates of HIV among many populations, HIV-related health disparities among gay, bisexual and other men who have sex with men persist, with disproportional percentages of new HIV diagnoses among racial and ethnic minority men. Despite increasing awareness of HIV pre-exposure prophylaxis (PrEP), PrEP use remains low. In addition to exploring individual-level factors for this slow uptake, structural drivers of PrEP use must also be identified in order to maximize the effectiveness of biomedical HIV prevention strategies. METHOD: Using cross-sectional data from an on

Knowledge and Attitudes About Pre-Exposure Prophylaxis Among Young Adults Experiencing Homelessness in Seven U.S. Cities.

PURPOSE: Evidence suggests that young adults experiencing homelessness (YEH) are at elevated risk of HIV compared to housed youth. Given the limited research on pre-exposure prophylaxis (PrEP) awareness among YEH, this study examined their PrEP knowledge and attitudes. METHODS: Data from a cross-sectional survey among YEH (ages 18-26) (n = 1,427) in seven U.S. cities were used to assess their knowledge and attitudes regarding PrEP to inform HIV prevention efforts. RESULTS: Participants were primarily male youth of color. The mean age was 20.9years. While 66% felt at risk for HIV, only 14% st

Adherence to pre-exposure prophylaxis among men who have sex with men: A prospective cohort study.

OBJECTIVE: To evaluate adherence to pre-exposure prophylaxis (PrEP) in a clinical trial of a population of men who have sex with men (MSM), to explore the influence of social psychology and objective factors (e.g., forgetting, too busy) on adherence, and to provide evidence for subsequent PrEP research. METHODS: This study analyzed the data from the daily medication group within a PrEP study (a randomized placebo-controlled trial of oral tenofovir among MSM in Western China from April 2013 to March 2015). A total of 331 participants were analyzed. PrEP adherence was self-reported by the subje

A novel Online-to-Offline (O2O) model for pre-exposure prophylaxis and HIV testing scale up.

INTRODUCTION: PrEP awareness and uptake among men who have sex with men (MSM) and transgender women (TG) in Thailand remains low. Finding ways to increase HIV testing and PrEP uptake among high-risk groups is a critical priority. This study evaluates the effect of a novel Adam's Love Online-to-Offline (O2O) model on PrEP and HIV testing uptake among Thai MSM and TG and identifies factors associated with PrEP uptake. METHODS: The O2O model was piloted by Adam's Love (www.adamslove.org) HIV educational and counselling website. MSM and TG reached online by PrEP promotions and interested in free

Is pre-exposure prophylaxis effective for preventing HIV infection in men who have sex with men?

INTRODUCTION: Increasing rates of HIV infection remain of concern, especially for high-risk groups such as men who have sex with men. Oral pre-exposure prophylaxis has emerged as an alternative to prevention. However, doubts persist in patients and physicians about its effectiveness. METHODS: To answer this question we used Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic reviews, reanalyzed data of primary studies, conduct

[Pre-exposure prophylaxis for the prevention of sexual HIV transmission; new preventative strategy using tenofovir/emtricitabine].

The Netherlands has approximately 20,000 registered HIV-infected patients. The current HIV prevention policy consisting of condom use and active HIV testing does not effectively mitigate the HIV epidemic in all risk groups. In July of 2012, tenofovir/emtricitabine (TDF/FTC) was approved by the American Food and Drug Administration (FDA) for pre-exposure prophylaxis (PrEP) for long-term use in persons who exhibit frequent risky and unsafe sexual behaviour. With once-daily use and good therapy compliance, TDF/FTC has proved to be effective as PrEP, and few side effects are reported. Drawbacks in

Methodological considerations for indirect treatment comparisons of HIV incidence for long-acting injectable pre-exposure prophylaxis.

Background: Given the effectiveness of pre-exposure prophylaxis (PrEP), the HIV prevention landscape is expanding rapidly - including recently approved injectable options which have shown high efficacy in clinical trials. As head-to-head trials of injectable PrEPs are not feasible, indirect treatment comparisons (ITCs) are of value to generate comparative estimates of efficacy and safety. However, several methodological challenges must be carefully considered when conducting these ITCs. This review outlines these complexities to support informed decision-making. Just4Us: Acceptability and Feasibility of a Woman-Focused Pre-exposure Prophylaxis (PrEP) Randomized Controlled Trial Intervention.

Women comprise 19% of new HIV infections in the USA. Many women, particularly women of color, are unaware of pre-exposure prophylaxis (PrEP) as an HIV prevention option. We conducted a pilot randomized controlled trial to assess feasibility and acceptability of a theory-based, contextually relevant behavioral intervention, Just4Us, to promote PrEP initiation and adherence among women in New York City and Philadelphia. Eligibility criteria included: cisgender women, aged 18-55, not living with HIV, not currently taking PrEP, and meeting PrEP eligibility guidelines. Participants were randomized

Naltrexone as Pre-exposure Prophylaxis for Unintentional Opioid Overdose in Stimulant Use Disorder: A Case Report.

BACKGROUND: Stimulant-involved overdose deaths have increased sharply in the United States, largely driven by adulteration of the stimulant drug supply with fentanyl and other high-potency synthetic opioids. Individuals with stimulant use disorder (StUD) who do not intentionally use opioids remain at elevated risk for unintentional opioid overdose. Pharmacologic strategies for overdose prevention in this population remain limited. CASE PRESENTATION: We describe a 32-year-old woman with severe stimulant use disorder who smoked crack cocaine and denied intentional opioid use but experienced 2 n

Long-acting HIV pre-exposure prophylaxis adherence and discontinuation: a global systematic review and meta-analysis.

BACKGROUND: Long-acting HIV pre-exposure prophylaxis (PrEP) may overcome adherence and discontinuation challenges facing oral PrEP, which remain unsystematically reviewed. This study aimed to synthesize the evidence on long-acting PrEP adherence and discontinuation. METHODS: We conducted a systematic review and meta-analysis of studies published from database inception to July 1, 2026 in PubMed, Web of Science, EMBASE, and Cochrane Library. Eligible studies reported adherence (on-time doses receipt at time points, e.g., at 2 months) or discontinuation (stopping doses or loss to follow-up with

High Burden of Sexually Transmitted Infections Among Pre-exposure Prophylaxis (PrEP) Users: Testing and Prevention Needs.

Pre-exposure prophylaxis (PrEP) effectively prevents HIV acquisition but does not protect against other sexually transmitted infections (STIs). Rising STI rates among PrEP users have raised concerns about risk compensation and highlight the need for integrated sexual health strategies. We assessed the prevalence, anatomical distribution, and behavioural determinants of STIs among PrEP users in Spain. This retrospective observational study included 724 individuals receiving daily tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) at a tertiary hospital in Madrid between March and October 202

Online delivery of oral HIV pre- and post-exposure prophylaxis: findings from the ePrEP Kenya pilot.

INTRODUCTION: The expansion of telecommunication networks and smartphones in many African countries could be leveraged to deliver HIV prevention products directly to consumers. In collaboration with a private e-commerce platform and online pharmacy in Kenya, MYDAWA, we piloted a new model of HIV pre- and post-exposure prophylaxis (PrEP/PEP) delivery. METHODS: In the ePrEP Kenya pilot (NCT05377138), individuals living in Nairobi and Mombasa Counties could complete a free telehealth visit with a remote clinician to assess eligibility for online PrEP/PEP (i.e. ≥18 years; no medical contraindicat

Sexual health assessments, testing, and pre-exposure prophylaxis referrals among veterans with a sexually transmitted infection.

OBJECTIVE: Describe sexual history completion among U.S. Veterans with a sexually transmitted infection (STI) and its association with number of ordered STI laboratory tests and offer of human immunodeficiency virus (HIV) pre-exposure prophylaxis (PrEP). METHODS: We assembled a retrospective cohort of Veterans with a STI seen at five sites in the southern United States and excluded those with diagnosis of HIV. We conducted a systematic chart review of sexual history using Centers for Disease Control and Prevention's five 'Ps.' The outcome was STI testing and PrEP offer. Charts were reviewed b

Influencing Factors for the Acceptability of Accessing HIV Pre-exposure Prophylaxis via Community Pharmacies in Wales.

INTRODUCTION: HIV prevention methods, such as pre-exposure prophylaxis (PrEP), have been a significant contributing factor to a global decline in HIV transmission. PrEP has been available through the NHS in Wales since 2017. However, access is exclusively via sexual health clinics, and those accessing PrEP do not reflect those being diagnosed with HIV. Widening access beyond sexual health clinics may be one approach to encourage more equitable uptake, and there is growing interest in offering PrEP services in community pharmacies. We, therefore, aimed to explore the acceptability of PrEP servi

Routine childhood rabies pre-exposure prophylaxis can be cost effective in low- and middle-income countries.

BACKGROUND: Pre-exposure prophylactic rabies vaccination (PrEP) is advised for travellers to countries with high rabies incidence, but rarely available for local residents. Some studies suggest poor cost-effectiveness of PrEP in such settings, but have generally focused upon post-exposure prophylaxis (PEP) cost savings as the main benefit of PrEP, without considering lives saved by PrEP efficacy. METHODS: We compared incremental cost-effectiveness ratios (ICERs) of use of rabies PrEP, against an alternative of using only PEP, by adapting a decision-tree model previously used to inform Gavi's

Long-Term Immunogenicity of Rabies Pre-Exposure Prophylaxis in Japanese Adult Travelers: Comparison of Dosing Regimens.

BACKGROUND/OBJECTIVES: Japan is a rabies-free country; therefore, pre-exposure prophylaxis (PrEP) is primarily recommended for travelers to rabies-endemic regions. However, no prior studies in Japan have assessed long-term immunogenicity after PrEP vaccination. METHODS: This descriptive study evaluated the long-term persistence of rabies virus-neutralizing antibodies among Japanese adult travelers who had received PrEP. Neutralizing antibody levels were measured using the rabies rapid fluorescent focus inhibition test more than two years post-vaccination. RESULTS: Among 97 participants, 86.6

Poor Oral HIV Pre-Exposure Prophylaxis (PrEP) Persistence in an Integrated PrEP/STI Program in Malawi.

The integration of HIV pre-exposure prophylaxis (PrEP) into STI services can improve PrEP uptake among a population at elevated risk of acquiring HIV. The effectiveness of PrEP relies on ongoing coverage during periods of HIV risk, however, and little is known about longitudinal PrEP use among people accessing PrEP through STI clinics in sub-Saharan Africa. In this study, we analyzed routine records data from people who newly initiated PrEP at an STI clinic in Lilongwe, Malawi in March-December 2022. We assessed PrEP persistence among clients who received Malawi's standard-of-care PrEP service

Structural Racism and HIV Pre-exposure Prophylaxis Use in the Nationwide US: A County-Level Analysis.

BACKGROUND: Structural racism contributes to geographical inequalities in pre-exposure prophylaxis (PrEP) coverage in the United States (US). This study aims to investigate county-level variability in PrEP utilization across diverse dimensions of structural racism. METHODS: The 2013-2021 nationwide county-level PrEP rate and PrEP-to-need ratio (PNR) data were retrieved from AIDSVu. PrEP rate was defined as the number of PrEP users per 100,000 population, and PNR was defined as the ratio of PrEP users to new HIV diagnoses per calendar year. Linear mixed effect regression was employed to identi
