Oral premedication in children. A comparison of trimeprazine with a trimeprazine, droperidol and methadone mixture.

One hundred children who presented for minor general surgical procedures were randomly assigned to receive one of two oral premedications. Those in group A (n = 50) were given 3 mg/kg of trimeprazine and those in group B (n = 50) a mixture of trimeprazine 1.0 mg/kg, droperidol 0.15 mg/kg and methadone 0.08 mg/kg. Patients in group B were more likely to be asleep on arrival in the anaesthetic room (p less than 0.02) and were less likely to be distressed at induction of anaesthesia (p less than 0.02). Thiopentone requirements were less in group B (p less than 0.001). The incidence of side effect

[Comparative clinical studies of oral premedication in childhood with reference to various pharmaceuticals].

In a randomized prospective study of 206 children aged between one and 14, the influence of oral praemedication on the psychic condition and with regard to anaesthesiologic measures was investigated. Heart rate, inhibition of salivation and the kind and frequency of side-effects were also examined. The praemedication was carried out using diazepam (0.3 mg/kg), promazin (1.0 mg/kg), promethazin (1.0 mg/kg) and chlorphenaethazin (1.0 mg/kg). It was not possible to determine statistically relevant differences between the various drugs concerning the fixed criteria. In general, however, diazepam s

[Anesthesia induction in children: propofol in comparison with thiopental following premedication with midazolam].

Propofol provides smooth and rapid induction of anesthesia in adults and guarantees rapid recovery. The use of propofol in adults is frequently associated with pain on injection, but this can be reduced by: (1) injection into the relatively large veins in the forearm or the antecubital fossa: (2) addition of lignocaine to the propofol; or (3) injection of an opioid (alfentanil) before propofol. Compared with experience in adults, there is very little experience with propofol in pediatric anesthesia. The aim of this random prospective study was to compare the induction characteristics of propof

Evaluation of acepromazine/meperidine/atropine premedication followed by thiopental anesthesia in the cat.

Cardiopulmonary function was assessed in healthy cats premedicated with intramuscular acepromazine, meperidine, atropine combination (premix), followed by induction and maintenance with intravenous thiopental for 30 min. Cardiac output by thermodilution, heart rate, blood pressure and blood gas analysis were evaluated over 120 min. A minor degree of respiratory depression was noted in the cats. Cardiac index (cardiac output/kg) was significantly depressed following thiopental induction and for the entire 120 min studied. Stroke volume was significantly reduced after premix administration and f

The effect of premedication on the incidence of postoperative vomiting in children after E.N.T. surgery.

The influence of premedication and intraoperative narcotic administration on the incidence of vomiting after E.N.T. surgery was studied over a six-month period in 596 children. A high incidence of vomiting was found after intramuscular papaveretum and hyoscine and after oral diazepam. Oral trimeprazine tartrate was associated with a low incidence of vomiting and was found to have an anti-emetic effect when an opiate analgesic was administered after induction of anaesthesia.

Comparison of glucagon, atropine, and placebo as premedication for endoscopy of the upper gastrointestinal tract.

The effects of 0.5 mg glucagon and 0.5 mg atropine given intravenously as premedication in upper gastrointestinal endoscopy have been examined and compared to those of placebo (0.9% NaCl) in a double-blind study involving 36 patients and 1 endoscopist. The results showed no difference between atropine, glucagon, and placebo with regard to vomiting, opening of the pylorus, feeling of discomfort, or the success of the examination. Glucagon significantly reduced peristalsis compared with both atropine (p less than 0.01) and placebo (p less than 0.01). The reflux was also significantly reduced by

Premedication with steroids in breast cancer - supportive treatment or tumour growth promotion?

Glucocorticoids, most commonly dexamethasone, are widely used alongside breast cancer therapy to alleviate adverse effects of chemotherapy, particularly allergic reactions and nausea. However, growing evidence shows that the activation of the glucocorticoid receptor (GR) has context-dependent effects on tumour biology, with both tumour-suppressive and tumour-promoting consequences. This review summarises the molecular basis of GR signalling in breast cancer and its prognostic relevance across molecular subtypes. Particular emphasis is placed on ligand-dependent and ligand-independent GR activa

Premedication in pediatric Meckel scintigraphy: pantoprazole versus ranitidine for optimizing scan quality.

BACKGROUND: The standard method for diagnosing Meckel diverticulum and identifying ectopic gastric mucosa is 99 mTc-pertechnetate imaging. Premedication with H2 receptor antagonists enhances the scan's sensitivity by reducing the washout of 99 mTc-pertechnetate activity from the intestinal lumen. OBJECTIVE: After the withdrawal of ranitidine, we compared the efficacy of the proton pump inhibitor pantoprazole as an alternative premedication agent for 99 mTc-pertechnetate Meckel diverticulum imaging. MATERIALS AND METHODS: This study assessed the scan quality of 141 children (aged 1 month to 2

Oxidative Stress Reduction by Midazolam Premedication during Oocyte Retrieval Procedure: Pilot Study.

Infertility is one of the major medical problems nowadays. Couples who opt for In Vitro Fertilization (IVF) face a great deal of stress which certainly affects the outcome of the procedure. Therefore, we aimed to reduce the stress during the oocyte retrieval procedure by applying midazolam. Total oxidant (TOC) and antioxidant (TAC) capacities of serum, as well as glutathione (GSH) content and catalase activity, were measured in both control and midazolam groups. Follicular fluid was also tested for oxidant capacity and IL1β. Results implied that the midazolam group increased TAC at the end of

Midazolam Premedication Immediately Before Surgery Is Not Associated With Early Postoperative Delirium.

BACKGROUND: Postoperative delirium is common among older surgical patients and may be associated with anesthetic management during the perioperative period. The aim of this study is to assess whether intravenous midazolam, a short-acting benzodiazepine used frequently as premedication, increased the incidence of postoperative delirium. METHODS: Analyses of existing data were conducted using a database created from 3 prospective studies in patients aged 65 years or older who underwent elective major noncardiac surgery. Postoperative delirium occurring on the first postoperative day was measure

The added value of H2 antagonists in premedication regimens during paclitaxel treatment.

BACKGROUND: Ranitidine, a histamine 2 blocker, is the standard of care to prevent hypersensitivity reactions (HSRs) caused by paclitaxel infusion. However, the added value of ranitidine in this premedication regimen is controversial. Therefore, we compared the incidence of HSRs during paclitaxel treatment between a standard regimen including ranitidine and a regimen without ranitidine. METHODS: This prospective, pre-post interventional, non-inferiority study compared the standard premedication regimen (N = 183) with dexamethasone, clemastine and ranitidine with a premedication regimen without

Effect of Premedication with Oral Clonidine on Hemodynamic Response during Electroconvulsive Therapy.

BACKGROUND: Electroconvulsive therapy (ECT) is the most effective treatment available for the acute treatment of depression in patients who do not respond to medications. It is generally used as a second line treatment for many psychological disorders, mainly major depression and schizophrenia where medication is not effective. ECT is often associated with some complications such as hypertension, tachycardia arrhythmia and even myocardial infarction. Various methods have been used for prevention or control of these cardiovascular side effects. AIM: The aim of this study was evaluating the eff

Fluoroscopy-guided hydrostatic reduction of intussusception in infancy: role of pharmacological premedication.

PURPOSE: Intussusception is one of the most common causes of paediatric emergency. Fluoroscopy-guided hydrostatic reduction is a common nonoperative management strategy for the treatment of intussusception. The role of pharmacological premedication in increasing the success rate of hydrostatic reduction is still controversial. The purpose of this study was to verify the presence of a possible correlation between pharmacological premedication and the percentage of hydrostatic reduction of intussusception in paediatric patients. Premedication with midazolam prior to caesarean section has no neonatal adverse effects.

Like all surgical patients, obstetric patients also feel operative stress and anxiety. This can be prevented by giving patients detailed information about their operation and with preoperative pharmacological medications. Because of depressive effects of sedatives on newborns, pharmacological medications are omitted, especially in obstetric patients. The literature contains few studies concerning preoperative midazolam use in Caesarian section (C/S) patients. Our aim in this study was to help patients undergoing C/S surgery. One group scheduled for elective C/S received midazolam 0.025 mg kg(-
