The redemption of thalidomide: standardizing the risk of birth defects.

In this paper we examine how a standardized drug distribution system contributed to a therapeutic and symbolic make-over of thalidomide. In the 1960s, thalidomide was seen as a horror drug that caused severe birth defects among over 10,000 babies who were exposed to it in utero. Currently, thalidomide is viewed as a potentially life-saving drug which is being distributed in the USA. We discuss this transformation from a social worlds perspective, showing how the standardized drug distribution system normalized the risk of foetal birth defects, while preserving the autonomy of health care profe

Factors That Influence Genetic Counselors' Professional Identity-A Qualitative Study.

Genetic counselors describe their professional identity as who they are (personal characteristics and attributes), what they do (actions and skills), and what they possess (training and knowledge). A shared multidimensional perspective highlights the transferability of their skills across diverse roles and settings. This qualitative study aimed to identify factors that influence genetic counselors' perceptions and formation of their professional identities. Using Social Identity Theory as a theoretical framework, semi-structured recorded interviews were conducted with 50 genetic counselors who

Why shouldn't we do that on placement if we're doing it in the real world? Differences between undergraduate and graduate identities in speech and language therapy.

Healthcare graduates are often characterised as ill-prepared for workplace entry. Historically, research on health professional's work preparedness has focused on the quality of graduates' clinical knowledge, skills and problem-solving. This ignores the role of professional identity formation in determining readiness for clinical practice. Yet, professional identity defines graduate self-perception, how others perceive them and informs clinical behaviour. The scholarship of identity formation at the transition from undergraduate to graduate is characterised by individual (cognitive) rather tha

[Autonomy or compassion? Values to be taken into account in the treatment of patients suffering Alzheimer's disease].

Advance directives applied to decision making in assisting people with neurodegenerative disorders like Alzheimer disease faces professionals with an ethical dilemma that seems to have a pole on the autonomy enjoyed by the subject when gave such directions and in the other the compassion that awakens the individual at the time of their implementation. In this paper we take the concepts of R. Dworkin of experiential interests(i.e. things that people do just because they like the experience of doing them) and critical interests (interests, which if not satisfied, people would think they were wor

Ethical considerations of genetic presymptomatic testing for Huntington's disease.

The aim of this literature review was to determine if there is adequate ethical justification for presymptomatic genetic testing on potential Huntington's disease patients. Huntington's disease is a neurological genetic disorder characterized by midlife onset which consists of cognitive, physical, and emotional deterioration. Although genetic testing has traditionally been guided by the principle of autonomy, severe psychological consequences such as depression, anxiety, survival guilt, and suicide have complicated the ethical issue of providing a presymptomatic yet definitive diagnosis for an

The changing landscape of travel health services in Canada.

Canadians are increasingly travelling to areas that would necessitate a pre-travel consultation. Changes in professional regulations in Canada allow greater autonomy of nurses and pharmacists, resulting in shifts in provision of travel health services. We surveyed 824 Canadian travel clinics, 270 (33%) of whom responded. Private clinics were most common, and more likely to offer extended hours and drop-in appointments. In one province, pharmacies dominated. Half the services were relatively new and a similar proportion saw fewer than 10 patients weekly; 1/3 had a single provider. The increased

Physician resiliency?

The profound changes in the health care industry have led to the anger, frustration, and unhappiness that physicians are feeling. It is important to examine physicians' responses to the threats to their professional autonomy, image, lifestyles, and relationships with their patients. The "learned helplessness" behavior exhibited by physicians is astounding, considering the education, status, and reputation of physicians as healers for those in need. This article explores the concept of resiliency among physicians and describes why physicians as a group may be less resilient than other individua

A qualitative study to explore how professionals in the United Kingdom make decisions to test children for a sickle cell carrier status.

European guidelines recommend that, unless there are clear benefits of autosomal recessive carrier testing in childhood, it should be deferred to protect children's autonomous decision making. Although it is believed that children receive testing in the United Kingdom, it is unclear how or why professionals make decisions to provide tests. Semi-structured interviews were conducted with 25 professionals in the United Kingdom who advise about, and undertake, childhood sickle cell trait testing. Data were analysed using thematic analysis. Few professionals were aware of, or used, guidelines to in

The ethical role of dermatologists in smoking cessation counseling.

Smoking has been associated with increased disease severity, poorer treatment response, and adverse outcomes in several dermatologic conditions, including hidradenitis suppurativa, psoriasis, cutaneous lupus erythematosus, and chronic wounds. Although smoking cessation counseling is traditionally viewed as the responsibility of primary care clinicians, dermatologists frequently encounter patients whose skin disease may be influenced by tobacco use. This raises an important ethical question: when does counseling aboutlifestyle behaviors fall within the scope of dermatologic practice? We discuss

An Interprofessional Commentary on Justice, Equity, Diversity, and Inclusion Efforts in Allied Health Education.

As allied health professionals are front-line workers and drivers of population health, it is vital that their practice is rooted in and centered on quality care and equitable health for all. In recent years, justice, equity, diversity, and inclusion (JEDI) initiatives have multiplied and become more robust, particularly within allied health professions. JEDI initiatives help eliminate health disparities related to social factors like race, class, and gender, largely by creating an allied healthcare workforce that represents the culture and ideology of the patients it serves. However, growing

Sterilisations at delivery or after childbirth: Addressing continuing abuses in the consent process.

Non-consensual sterilisation is not only a historic abuse. Cases of unethical treatment of women around the time of a pregnancy continue in the Twenty-First Century in five continents. Sterilisation is being carried out by some healthcare professionals at the time of delivery, or soon afterwards, without valid consent. A range of contemporary examples of such practices is given. Respecting women's autonomy should be the touchstone of the consent process. Avoidance of force, duress, deception and manipulation should go without saying. Increasing prebunking effectiveness to aid disinformation-susceptible publics in overcoming vaccine-related disinformation.

In the dynamic intersection of health communication and disinformation, this study investigates tailored intervention strategies for disinformation-susceptible public segments classified by the motivation-knowledge-attitude framework. Using a two-phase survey and experimental design with American adults, participants were segmented into four public types (disinformation-immune, vulnerable, receptive, and amplifying) and exposed to one of three inoculation message conditions: autonomy supportive, autonomy controlling, or control. Data were analyzed using ANCOVA with Bonferroni-adjusted post-hoc

A synthesis of the literature on trauma-informed care.

Patients with a history of traumatic life events can become distressed or re-traumatized as the result of healthcare experiences. These patients can benefit from trauma-informed care that is sensitive to their unique needs. However, despite the widespread prevalence of traumatic life experiences such as sexual assault and intimate partner violence, trauma-informed care has not been widely researched or implemented. The purpose of this synthesis of the literature is to examine existing research on trauma-informed care for survivors of physical and sexual abuse. The following themes are discusse

Application of an empowerment model to improve civility in nursing education.

Incivility within nursing education presents clear challenges to both students and faculty. The consequences of incivility extend beyond the educational process and into practice, thereby creating an urgent need for a framework that can guide faculty in the efforts to improve and maintain civility in nursing education. This article reviews the complex problem of incivility in nursing education and utilizes evidence in the application of an empowerment model (Worrell et al., 1996) as a framework to set the standard of civility and one that will guide intervention in the unfortunate event that i

Display rules versus display autonomy: emotion regulation, emotional exhaustion, and task performance in a call center simulation.

"Service with a smile" is satisfying for the customer, but such display rules may be costly to the employee and the organization. Most previous research on such costs has used self-reported and cross-sectional designs. The authors use an experimental approach to test tenets of resource depletion theories; specifically, whether the self-regulation of emotions required by display rules depletes energy and attentional resources during a service encounter. Using a call center simulation with three "customer" interactions, the authors found that participants given positive display rules (e.g., be e
