Screening of PCSK9 and LDLR genetic variants in Familial Hypercholesterolemia (FH) patients in India.

Familial Hypercholesterolemia (FH) is an autosomal, dominant, inherited disorder characterized by severely elevated LDL-cholesterol (LDL-C) levels with high risk for Coronary Artery Disease (CAD). There are limited genetic studies especially on genes other than Low Density Lipoprotein receptor (LDLR) conducted in Indian population. Thus, our aim was to screen the entire Proprotein Convertase Subtilisin/Kexin type 9 gene (PCSK9) gene & hotspot exons 3, 4 and 9 of LDLR gene in FH cases and controls. 50 FH cases were categorized into definite, probable and possible cases according to Dutch Lipid

Role of PCSK9 in the course of ejection fraction change after ST-segment elevation myocardial infarction: a pilot study.

AIMS: Proprotein convertase subtilisin/kexin type 9 (PCSK9) has emerged as a therapeutic target for reducing plasma low-density lipoprotein cholesterol. Beyond lipid control, recent findings suggest a deleterious effect of this protein in the pathogenesis of postmyocardial infarction left ventricle remodelling and heart failure-related complications. The aim of this study was to assess the relationship between circulating PCSK9 and 6 month cardiac magnetic resonance imaging-derived left ventricular ejection fraction (LVEF) after a first ST-segment elevation myocardial infarction (STEMI). Real-world lipid outcomes after switching PCSK9-targeting therapies in heterozygous familial hypercholesterolemia: the SHIFT-FH study.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibition represents a cornerstone of lipid-lowering therapy in heterozygous familial hypercholesterolemia (HeFH). Monoclonal antibodies (mAbs) and small interfering RNA (siRNA) targeting PCSK9 provide complementary therapeutic strategies; however, real-world data comparing lipid outcomes and target achievement after switching between these treatments remain limited. The SHIFT-FH study was a retrospective real-world cohort study including genetically confirmed HeFH patients receiving maximally tolerated lipid-lowering therapy and stable PC

Evolocumab treatment reduces carotid intima-media thickness in paediatric patients with heterozygous familial hypercholesterolaemia.

AIMS: Children with heterozygous familial hypercholesterolaemia (HeFH) show greater carotid intima-media thickness (cIMT). Evolocumab, a proprotein convertase subtilisin/kexin type 9 inhibitor monoclonal antibody, substantially reduced LDL cholesterol (LDL-C) in children with HeFH. We investigated evolocumab's effect on cIMT progression. METHODS AND RESULTS: HAUSER-RCT was a randomized, placebo-controlled trial. One hundred fifty-seven paediatric patients with FH (age: 10-17 years) and LDL-C > 130 mg/dL despite statin therapy received monthly evolocumab 420 mg or placebo for 24 weeks. Patient

Rationale, design, and baseline characteristicss of the effect of PCSK9 inhibition on cardiovascular risk in treated HIV infection: EPIC-HIV randomized clinical trial.

RATIONALE: People with HIV (PWH) are at increased risk of cardiovascular disease. Moderate lipid lowering with statins has been demonstrated to reduce cardiovascular risk among PWH. Accordingly, evaluation of more potent lipid-lowering strategies for prevention is needed, especially for PWH at higher risk. Prior research suggests that proprotein convertase subtilisin kexin type 9 (PCSK9) inhibitors safely lower low-density lipoprotein cholesterol by 60% among people with HIV, but the impact of PCSK9 inhibitors on arterial inflammation, endothelial function, coronary plaque, or markers of immun

Treatment with the natural FXR agonist chenodeoxycholic acid reduces clearance of plasma LDL whilst decreasing circulating PCSK9, lipoprotein(a) and apolipoprotein C-III.

BACKGROUND: The natural farnesoid X receptor (FXR) agonist chenodeoxycholic acid (CDCA) suppresses hepatic cholesterol and bile acid synthesis and reduces biliary cholesterol secretion and triglyceride production. Animal studies have shown that bile acids downregulate hepatic LDL receptors (LDLRs); however, information on LDL metabolism in humans is limited. METHODS: Kinetics of autologous 125 I-LDL were determined in 12 male subjects at baseline and during treatment with CDCA (15 mg kg-1 day-1 ). In seven patients with gallstones treated with CDCA for 3 weeks before cholecystectomy, liver bi

Low levels of low-density lipoprotein-C associated with proprotein convertase subtilisin kexin 9 inhibition do not increase the risk of hemorrhagic transformation.

BACKGROUND AND PURPOSE: Low levels of low-density lipoprotein-cholesterol (LDL-C) are suspected to be associated with a risk of hemorrhagic transformation after ischemic stroke. We assessed the risk of hemorrhagic transformation after cerebral ischemia/reperfusion in mice with low levels of LDL-C resulting from proprotein convertase subtilisin kexin 9 (PCSK9) deficiency. METHODS: PCSK9-/- and PCSK9+/+ mice were fed with a high-fat/high-cholesterol (21%/0.15%) diet for 1 month. Plasma lipids were measured using colorimetric assays. PCSK9-/- and PCSK9+/+ mice (n=15 per group) were subjected to

PCSK9 Inhibitors in Peripheral Arterial Disease: A Systematic Review.

BACKGROUND: Dyslipidaemia is a key modifiable risk factor for peripheral arterial disease (PAD), and statin therapy for lipid-lowering is well established in its management. Proprotein convertase subtilisin/kexin type-9 (PCSK9) inhibitors lower lipids and cardiovascular risk; however, their impact on limb-based outcomes in PAD remains uncertain. METHODS: A systematic review was conducted in accordance with Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. Studies evaluating PCSK9 inhibitor therapy in patients with PAD were eligible for inclusion. RESULTS

Evolocumab in patients with multivessel coronary disease after acute myocardial infarction: A target trial emulation.

BACKGROUND AND AIMS: The proprotein convertase subtilisin/kexin type 9 inhibitors, i.e. evolocumab, promote coronary plaque regression in patients with acute myocardial infarction (AMI). However, its clinical benefit for non-culprit intermediate lesions deferred for revascularization remains uncertain. This study evaluated whether early evolocumab initiation improves outcomes in this specific population. METHODS: Using target trial emulation to a multicenter registry of AMI patients in Shanghai (2021-2022), this study enrolled patients with successful culprit-vessel revascularization but ≥1 d

LDL-C target achievement after adding evinacumab in 2 patients with autosomal recessive hypercholesterolemia.

BACKGROUND: Autosomal recessive hypercholesterolemia (ARH) is a rare form of genetic hypercholesterolemia consequent to pathogenic variants in the low-density lipoprotein receptor adaptor protein 1 (LDLRAP1) gene, coding for a protein responsible for moving low-density lipoprotein-receptor (LDL-R) to its site of activity. ARH is characterized by very high levels of low-density lipoprotein cholesterol (LDL-C), leading to aggressive and frequently premature atherosclerotic cardiovascular disease (ASCVD). Lowering of LDL-C is the main target of treatment; however, classical lipid-lowering agents,

N-terminal domains and site-specific glycosylation regulate the secretion of avian melanocortin inverse agonists, agouti signaling protein (ASIP) and agouti-related protein (AGRP).

Agouti signaling protein (ASIP) and agouti-related protein (AGRP) are paralogous inverse agonists of melanocortin receptors with distinct physiological roles, but their structural and biochemical properties in birds remain poorly understood. Here, we characterized chicken ASIP and AGRP proteins. Analysis of available sequences revealed that a motif resembling the mammalian proprotein convertase 1/3 (PC1/3, also known as PCSK1) cleavage site is conserved across a broad range of avian orders, but Western blot analysis of transfected Chinese hamster ovary (CHO-K1) cells and chicken hypothalamus d

Hypercholesterolemia Successfully Treated With Two Different PCSK9 Inhibitors in a Patient With Glycogen Storage Disease IXd: Phosphorylase Kinase Deficiency.

OBJECTIVE: This case report describes a novel use of proprotein convertase subtilisin-kexin type 9 (PCSK9) inhibitors in a patient whose glycogen storage disease (GSD) was unmasked by statin therapy. Evidence-based guidelines for the management of hyperlipidemia in patients with concurrent neuromuscular disorders (NMD) remain limited. METHODS: We report the case of a 46-year-old man with hyperlipidemia, treated with simvastatin, who presented with 2 days of thigh pain without weakness or dark urine. He was diagnosed with statin-associated muscle symptoms. On further interview, the patient rec
