[Anhedonia as a Transdiagnostic Phenomenon: Neurobiological, Clinical, and Therapeutic Approaches].

Anhedonia, classically defined as the inability to experience pleasure, was introduced in 1897 by the French philosopher and psychologist Théodule Armand Ribot. Although this term is still used, it has undergone conceptual modifications because the dichotomy of the original description is insufficient to capture the complexity of its components (Pérez Rincón, 2014). The hedonic experience is a dynamic and cyclical process that begins with a state with an individual reference range, known as hedonic tone, which determines each person’s capacity to experience pleasure and happiness. Currently, a

Working with clients by incorporating their preferences.

Working with clients by integrating their therapy preferences into the treatment decision-making process has been identified as an important part of evidence-based practice in psychology. Accommodating client preferences has also been demonstrated to lead to fewer treatment dropouts and improved therapy outcomes. In this article, we present a number of clinical interventions or techniques for addressing client preferences in psychotherapy. Clinical examples demonstrating the techniques are also provided.

The Psychopharmacology of Obsessive-Compulsive Disorder: A Preclinical Roadmap.

This review evaluates current knowledge about obsessive-compulsive disorder (OCD), with the goal of providing a roadmap for future directions in research on the psychopharmacology of the disorder. It first addresses issues in the description and diagnosis of OCD, including the structure, measurement, and appropriate description of the disorder and issues of differential diagnosis. Current pharmacotherapies for OCD are then reviewed, including monotherapy with serotonin reuptake inhibitors and augmentation with antipsychotic medication and with psychologic treatment. Neuromodulatory therapies f

Basal Exposure Therapy: A New Approach for Treatment-Resistant Patients with Severe and Composite Mental Disorders.

New treatment approaches are needed for patients with severe and composite mental disorders who are resistant to conventional treatments. Such treatment-resistant patients often have diagnoses of psychotic or bipolar disorders or severe personality disorders and comorbid conditions. In this study, we evaluate basal exposure therapy (BET), a novel ward-integrated psychotherapeutic approach for these patients. Central to BET is the conceptualization of undifferentiated existential fear as basic to the patients' problem, exposure to this fear, and the therapeutic platform complementary external r

[The relationship between forgivingness, mental health and psychotherapy].

Forgivingness occupies a prominent place in religions as well as in various philosophical systems of ethics and can be defined as the free, personal distancing from feelings of rage and resentment toward a person or persons having committed an injustice. The main psychological function of forgiveness consists in allowing for the replacement of negative emotions by positive ones, such as generosity, goodness, compassion, empathy, or even love toward the offender. It must be emphasized that forgiveness is independent from reconciliation, and it is not simply a form of tolerance, justification, o

Managing chemobrain: Translational insights and evidence-based psychological interventions.

With the improving survival rates of malignant tumors, the focus of attention in cancer research is shifting toward a better understanding of long-term treatment-related adverse effects that impact quality of life, with particular focus on cancer-related cognitive impairment. This phenomenon, commonly referred to as "chemobrain" in the literature, manifests as deficits in attention, memory, processing speed, and executive functions. Although the frequency of objectively measured cognitive deficits detected through neuropsychological assessments are moderate, subjective complaints often result

Psychedelic-assisted psychotherapy: The Australian and general practice perspective.

BACKGROUND: Since Australia's Therapeutic Goods Administration down-scheduled two psychedelic medicines from July 2023, psychotherapeutic treatment using these medicines has become available in the private sector, making it possible for general practitioners (GPs) to refer their patients with treatment-resistant depression or post-traumatic stress disorder (PTSD). Several other psychedelic medicines are also legally in use as off-label or unapproved medicines. OBJECTIVE: The aim of this article is to help GPs make sense of this new type of treatment by discussing the Australian context, with

Scorn Not Its Simplicity: Examining the Effectiveness of Simple Generalist Treatment for Personality Disorders.

Treatment guidelines for personality disorders have typically recommended specialized psychotherapeutic interventions. In this review, the author suggests that an intervention's effectiveness may be determined less by the specific method than by therapist competence, team culture, clinical process structure, and institutional context. The author argues that these elements determine variance in effectiveness between and within methods. Whereas initial studies of a specialized treatment may reflect the exceptional competencies of the treatment's developers and early adopters, in daily clinical p

[Psychiatric and psychological aspects of anorexia nervosa].

A group of 38 patients treated in the Department of Psychiatry, Medical Academy of Gdańsk, from 1972 to 1988, because of anorexia nervosa (0.33% patients treated in the Department) was studied. There was 36 women and 2 men aged 14 to 39 years (average 21 years). Half of the patients finished only secondary school, the others started university education--most of them were still students. Except one, they presented good intellectual abilities; 6 women were married. In 42% cases a conflict situation at school or family was found (adaptational problems, competition at home); in 18% cases problems

Sensory Processing Sensitivity and Maladaptive Personality Traits in Chronic Pain Conditions: A Network Analysis Perspective.

BACKGROUND: There is a growing interest in clarifying factors involved in the emergence of different chronic pain (CP) conditions, especially referring to the role of personality factors. Accordingly, the current study aims at exploring implications of temperamental (sensory processing sensitivity) and maladaptive personality traits across different CP syndromes (i.e., chronic headache, vulvodynia, and fibromyalgia) in patients with comorbid conditions compared to healthy controls (HCs). METHODS: The sample included 1144 women (chronic headache: 222; vulvodynia: 221; fibromyalgia: 201; comorb

Using the Trauma Reintegration Process to Treat Posttraumatic Stress Disorder with Dissociation and Somatic Features: A Case Series.

Given the suboptimal responses to medication and cognitive behavioral therapies in the treatment of post-traumatic stress disorder (PTSD), new approaches are needed. Background/Objectives: Therapies that include a somatic component such as Emotional Freedom Techniques (EFT) and Eye Movement Desensitization and Reprocessing (EMDR) have demonstrated efficacy in the treatment of PTSD in numerous clinical trials. This case series introduces the Trauma Reintegration Process (TRP), a psychotherapeutic process developed by the author that can be combined with somatic therapies to enhance their effect

Dropout in brief psychotherapy for major depressive disorder: Randomized clinical trial.

The aim of this paper is to analyse the factors associated with the dropout from brief psychotherapy for adults with major depressive disorder (MDD) treated at a mental health outpatient clinic. This is a randomized clinical trial with two models of psychotherapy: cognitive behavioural therapy (CBT) and supportive expressive dynamic psychotherapy (SEDP). MDD and anxiety disorders were evaluated through the Mini International Neuropsychiatric Interview-Plus. The personality disorders were evaluated by the Millon Clinical Multiaxial Inventory-III. The severity of depressive symptoms was measured

Using evidence-based psychotherapy to tailor treatment for patients with functional neurological disorders.

Functional neurological disorder (FND) frequently presents with comorbid psychopathology (e.g., anxiety, depressive, post-traumatic stress disorders (PTSD), somatic symptom and pain syndromes, and dissociative and personality disorders). It can become chronic and lead to unemployment and disability for many patients. Psychosocial factors play an important role in the onset and perpetuation of symptoms. Consequently, psychotherapy is recommended for the treatment of FND in general, and especially for the single symptom-based subtype of functional seizures (FS). Some of the psychotherapy approac

Mindfulness and emotion regulation--an fMRI study.

Mindfulness--an attentive non-judgmental focus on present experiences--is increasingly incorporated in psychotherapeutic treatments as a skill fostering emotion regulation. Neurobiological mechanisms of actively induced emotion regulation are associated with prefrontally mediated down-regulation of, for instance, the amygdala. We were interested in neurobiological correlates of a short mindfulness instruction during emotional arousal. Using functional magnetic resonance imaging, we investigated effects of a short mindfulness intervention during the cued expectation and perception of negative a

Anxiety disorders and the use and abuse of drugs.

Substance abuse continues to be a growing problem--both in the United States and worldwide. Moderate alcohol consumption reduces anxiety, but alcohol abuse actually increases anxiety and impairs judgment and performance. Research data indicate that phobic anxiety is more common than general anxiety as a prelude to alcohol dependence. Benzodiazepines are widely used throughout the world in the treatment of anxiety disorders. Patients who take benzodiazepines for clinical anxiety rarely exceed the prescribed dosage: benzodiazepine abuse is more likely to occur in those who abuse other drugs. For

Hyperbaric oxygen therapy as a potential treatment for MDD: a narrative review.

This narrative review explores the therapeutic potential of hyperbaric oxygen therapy (HBO2) in treating major depressive disorder (MDD). We highlight the common pathophysiological hypotheses for MDD, the strengths and limitations of current MDD treatments, and the potential for HBO2 to be a novel MDD therapeutic through anti-inflammatory and neuroprotective mechanisms. The current review discusses the monoamine, neuronal atrophy, and inflammation hypotheses of MDD pathophysiology origin, as well as how present MDD therapeutics target these MDD pathophysiological traits. While there are a mult

Psychological support can be distinguished from psychotherapy: Clarifications for future empirical work.

Classic psychedelic studies involve interpersonal interactions with participants before, during, and after the acute effects of psychedelics. In the field of psychedelic research, there is currently a debate about whether this interpersonal interaction is most accurately considered "psychological support," "psychotherapy," or something else. Proponents of the psychological support label describe the interpersonal interaction in most classic psychedelic studies as aimed at safety, while proponents of the psychotherapy label argue that the interpersonal interaction involves intervening on psycho

Emotional Changes during Imagery Rescripting of Aversive Social Memories in Social Anxiety Disorder: A Randomized Controlled Trial.

INTRODUCTION: Imagery rescripting (ImRs) is a psychotherapeutic intervention targeting aversive memories. During the three-phase intervention, patients reexperience their aversive memory (phase 1), observe the scene from their adult perspective, and intervene to help their former selves (phase 2), and reexperience it again with the positive changes (phase 3). Previous studies have rarely investigated emotional and regulatory processes taking place during the intervention. OBJECTIVE: This randomized controlled trial investigated self-reported affective and physiological responses during ImRs.
