Pyomyositis associated with abscess formation caused by streptococcus pneumoniae in children: a case report and review of literature.

BACKGROUND: Pyomyositis is an unusual bacterial infection but potential severe in children. Staphylococcus Aureus is the main caused of this disease (70-90%), following by Streptococcus Pyogenes (4-16%). Streptococcus Pneumoniae rarely caused invasive muscular infections. We describe a case of pyomyositis caused by Streptococcus Pneumonia in an adolescent 12-year-old female. CASE PRESENTATION: I.L. referred to our hospital for high fever associated with right hip and abdominal pain. The blood exams showed increase of leukocytes with prevalence of neutrophils with high level of inflammatory ma

Histoplasma Pyomyositis in a Patient with Disseminated Histoplasmosis and Anti-Synthetase Syndrome: Case-Based Review of Literature.

BACKGROUND: Histoplasmosis is an endemic mycosis caused by Histoplasma Capsulatum, a thermally dimorphic fungus with mycelial and yeast forms. Muscle involvement is infrequent in Histoplasmosis. CASE: A 49-year-old lady presented with generalized myalgia and arthritis of two-year duration, which had responded partly to glucocorticoids. The lady reported to us two years into the illness with ulcerative eyelid lesions, worsening myalgia, and painful skin nodules. Eventually, it turned out that anti-synthetase syndrome was the primary diagnosis with Histoplasma infection in the muscles, subcutan

Pyomyositis mimicking leptospirosis: two cases in a Brazilian tertiary care hospital in a non-tropical area.

Pyomyositis is a subacute, deep suppurative bacterial infection of skeletal muscle not arising from contiguous infection. It is presumably haematogenous in origin, and characterized by muscle pain and swelling. We report on two patients who presented with pyomyositis in a tertiary care hospital in temperate region located in southern Brazil with a clinical presentation, which was initially suggestive of leptospirosis. This report discusses the pathogenesis, clinical presentation, diagnosis and management of pyomyositis. Physicians living in non-tropical areas should note that pyomyositis might

Severe pyomyositis caused by Panton-Valentine leucocidin-positive methicillin-sensitive Staphylococcus aureus complicating a pilonidal cyst.

BACKGROUND: To our best knowledge, Panton-Valentine leucocidin (PVL)-positive methicillin-sensitive Staphylococcus aureus (MSSA) has not been described yet as cause for severe pyomyositis. CASE REPORT: We present a 23-year-old apparently healthy male patient without any typical predisposing findings who developed severe pyomyositis secondary to an operated pilonidal cyst. In the follow-up, the patient showed signs of immunocompromisation. The causative agent for purulent infection of multiple muscles was a MSSA strain harbouring PVL toxin. RESULTS: In the reported case, aggressive antibiotic

Early diagnosis of disseminated nocardiosis in a case of pyomyositis and brain abscesses exemplifies the importance of smear examination.

We report a 73-year-old immunocompromised man with multiple muscle abscesses, in a toxic state with neurological deficits, with a history of panhypopituitarism, diabetes, hypothyroidism, and recent SARS-CoV-2 infection. MRI showed abscesses in the right chest wall, right thigh (peri-implant), and left adductor. Elevated inflammatory markers, comorbidities and clinical presentation suggested disseminated fungal or Nocardial infection. Immediate aspiration of the anterior chest wall abscess and protocolized sample collection identified Nocardial filaments in the smear within 4 hours, allowing fo

[Tropical pyomyositis of the iliacus, obturator internus, piriformis and psoas major muscles in an immunocompetent patient with claudication].

INTRODUCTION: Pyomyositis is a term that denotes pyogenic infection usually primary skeletal muscle, associated with hematogenous dispersion due to transient bacteremia, or penetrating trauma, usually forming abscesses. Classically described frequently in tropical areas and predominantly affecting the lower limb musculature, however, in recent decades it has increasingly been associated with areas of temperate climates and relatively more frequently in immunosuppressed patients, being the patients under 30 years the most affected. CLINICAL CASE: Male of 15 years without relevant medical histo

Case of acute paraspinal pyomyositis in an elderly diabetic secondary to spread from urinary tract infection.

A 73-year-old woman with medical history of diabetes and hypertension presented with right-sided back pain and night sweats of 1 month duration. On physical examination, there was costovertebral angle tenderness and her urinalysis was indicative for urinary tract infection. She was admitted and managed with intravenous antibiotics. On initial imaging, there was a right-sided retroperitoneal mass adjacent to right kidney. This was investigated further with MRI, which showed diffuse inflammation of right paraspinal muscles with two loculated abscesses. The abscesses were aspirated and the cultur

Obturator internus pyomyositis manifested as sciatica in a patient with subacute bacterial endocarditis: A rare case report.

Pyomyositis is a pyogenic infection of the skeletal muscles causing myalgia and fever in patients. Hematogenous seeding engendered by persistent bacteremia and septic embolism is usually the underlying cause of the disease. Trauma, intravenous drug use, and immunodeficiency are the main predisposing factors.Obturator internus pyomyositis with sciatica has not previously been reported. We report a rare case of a patient with subacute bacterial endocarditis presenting with left buttock pain and sciatica.Computed tomography confirmed the diagnosis of obturator internus pyomyositis. Clinical characteristics and predictors of mortality in 67 patients with primary pyomyositis: a study from North India.

Primary pyomyositis is infection of the skeletal muscles in the absence of adjacent skin, soft tissue, and bone infection. This study was undertaken to look at the spectrum of clinical presentations, therapeutic interventions, and their outcomes and also to evaluate the association of various risk factors with mortality. This was a retrospective study in which the patients admitted with the diagnosis of primary pyomyositis from January 2000 to June 2007 were included. Their demographic details, clinical and laboratory data, Sequential Organ Failure Assessment (SOFA) score at presentation, trea

Gluteal pyomyositis in a non-tropical region as a rare cause of sciatic nerve compression: a case report.

INTRODUCTION: Pyomyositis, or isolated abscess formation within a skeletal muscle, is a relatively common condition in tropical climates but it is only encountered rarely in temperate zones. CASE PRESENTATION: We present a case of non-tropical pyomyositis of the gluteal muscle in a 26-year-old, previously healthy man from the United Kingdom, who initially presented with sciatica-like symptoms which began 3 days after a mosquito bite on his nose, which had become infected and discharged pus. CONCLUSION: Gluteal pyomyositis involving the sciatic nerve may initially present as radiculopathy. Mo

Role of Whole-body 18F-fluorodeoxyglucose Positron Emission Tomography/Computed Tomography in Difficult to Treat Multifocal Pyomyositis.

18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) has been used previously for the diagnosis of sepsis of unknown focus and pyrexia of unknown origin. Its use in the diagnosis of pyomyositis has not been fully explored. Pyomyositis was found to be an important cause of sepsis, but its cause has not been fully elucidated in previous studies exploring the role of PET. We studied the role of whole-body 18F-FDG PET/CT in the diagnosis and management of 10 patients with suspected multifocal pyomyositis. In our study, we were able to find a distant focus in 7 o

Non-typhoidal, Non-paratyphoidal Salmonella Species Causing Sacroiliitis and Pyomyositis in a Healthy 19-Year-Old Athlete.

Salmonella is a gram-negative bacterium, subdivided into typhoidal and non-typhoidal Salmonella. It is usually caused by eating raw or undercooked meat, poultry, eggs, or egg products. The clinical manifestations of Salmonella infection can be divided into five syndromes: enterocolitis (food poisoning), enteric (typhoid) fever, bacteremia/septicemia, focal infection, and a chronic carrier state, which is usually asymptomatic. The most common clinical presentation is diarrhea. Salmonella osteomyelitis occurs most frequently in patients with sickle-cell disease; other risk factors include other

63-year-old man with right biceps and right pectoralis major abscesses: an unusual case of pyomyositis.

Pyomyositis is considered a great mimicker and masquerader. This case is of a 63-year-old man with diabetes who initially presented to the outpatient clinic afebrile with right shoulder pain. His work-up was negative, and he was discharged home. He subsequently presented to the emergency room (ER) two times for worsening right shoulder pain. During his first visit to the ER, his work-up was unremarkable, and he was discharged home. On his return to the ER, he was now febrile with inflammation involving his right upper extremity and right chest wall. Imaging studies of his right upper extremity

Subscapularis pyomyositis, a rare cause of shoulder pain, in a patient without apparent risk factors: A case report.

INTRODUCTION: Pyomyositis, previously tropical, now seen more in temperate zones, particularly in those with HIV, cancer, or diabetes. Even healthy individuals, like athletes, can get it post-exercise or minor injuries. Our patient, in a desert-like area, adds an interesting aspect. Diagnosing is challenging due to deep tissue occurrence and subtle symptoms, leading to delayed detection and higher risks of morbidity. PRESENTATION OF CASE: A 45-year-old female patient presented with acute left shoulder pain and functional impairment, exacerbated by movement. Clinical examination revealed tende
