Current concepts in the management of distal radius fractures.

The treatment of fractures at the distal end of the radius continues to challenge orthopaedic and upper extremity surgeons. As our understanding of the injury mechanism and local anatomy continues to improve, so too have our surgical techniques in helping patients regain functional use of the injured extremity. The purpose of this manuscript is to review the treatment methods available for distal radius fracture management.

Thumb polydactyly with radius hypoplasia--a case report.

Triplication of the thumb is a rare condition described only in several case reports. Radial longitudinal deficiency is a relatively more common entity. These 2 congenital abnormalities generally do not occur within the same extremity. We present a patient with a combination of 3 thumbs (triplication of the thumb) and hypoplastic radius in the same limb.

Rotational deformity in malunited fractures of the distal radius.

PURPOSE: To evaluate rotational deformity in malunited fractures of the distal radius and its effect on forearm rotation. METHODS: Thirty-seven patients with a symptomatic malunion of the distal radius (25 with dorsal angulation and 12 with volar angulation) were assessed for rotational deformity of the distal fragment. Spiral computed tomographic scans were taken of both wrists. Rotational deformity was evaluated by comparing the radial torsion angle of the injured and uninjured sides according to Frahm. Multivariable regression analyses were used to identify the radiologic parameter that ha

The functional importance of malunion in distal radius fractures.

The purpose of our study was to investigate which radiological parameter had the maximal detrimental effect on functional outcome following malunion of distal radius fractures. Sixty-four women over 40 years of age who had sustained low-energy wrist fractures were included in the study. The mean age of the patients was 64 years. We compared the ulnar variance, radial inclination and palmar tilt of the fractured wrist to the non-injured contralateral side on radiographs obtained after fracture healing. The functional outcome was assessed at a mean follow-up of 28 months. Wrist mobility, grip st

Intra-articular fractures of the distal aspect of the radius.

Intra-articular distal radius fractures are a heterogeneous group of injuries with different fracture patterns. The existing classification systems are helpful for describing the fractures but not for assessing their stability or for deciding which surgical approach to use. Patients who have a fracture with at least 1.0 mm of displacement of the articular surface may benefit from open surgical treatment. Improved diagnostic imaging with CT is helpful for fracture classification and surgical planning. The options for surgical treatment include limited open reduction and internal fixation, arthr

[Prognosis of proximal radius fractures in the growth period].

Fractures of the proximal radius in children may lead to deformities of the radial head and functional disturbance of pro- and supination. However, traumatization is not only caused by the injury itself, but may also occur secondary to surgical reduction, manipulation of fracture fragments and excessive physiotherapy. In a prospective long-term follow-study (2-20 years after trauma) of 38 children with displaced proximal radius fractures we found functional disturbances in 11% of children only. These were not caused by radial head deformities, which were present on 83% of follow-up radiographs

Plate osteosynthesis of diaphyseal fractures of the radius and ulna.

Between January 1980 and December 1989, 133 consecutive patients were treated for a fracture of the shaft of one or both forearm bones (134 forearms in total). All fractures were stabilized with AO/ASIF 3.5 mm stainless-steel dynamic compression plates. The 1 year follow-up rate was 99 per cent; the long-term follow-up rate was 92 per cent (the mean long-term follow-up was 10.2 years (range, 2.7-15.2)) so there were 96 men and 35 women, with an average age of 37.5 years (range, 16-63). Twenty-two per cent of the forearms had open fractures, 26 per cent of patients had sustained multiple injuri

Complications and problems in the management of distal radius fractures.

Despite the best of treatments, problems and complications may be inherent in the fracture, depending on the nature of its severity. For that reason, patients should be followed for a minimum of 2 years after injury and treatment to assess the result and rule out a degenerative processes. A careful, thoughtful analysis at the time of presentation using a descriptive classification system that provides guidance for therapeutic intervention followed by meticulous execution of the mode of treatment employed, regular follow-up care, and rehabilitation will provide the surgeon with a means to treat

Compartment syndrome after fracture of the distal radius.

A compartment syndrome of the forearm after a fracture of the distal radius is presented. This rare complication should be considered after high velocity trauma in young people. In this setting, we prefer general anesthesia for fracture reduction. Fracture fixation and avoidance of circular casts are recommended. Repeated evaluation allows early detection of compartment syndrome. Fasciotomy should be done if this syndrome occurs.

[Osteosynthesis by plate in fractures of the distal end of the radius].

The authors recall the place of apposed plate fixation of fractures of the lower end of the radius, most of which are anterior. The ideal indication is the simple fracture by articular or supra-articular, compression-flexion. The importance of modelling and positioning of the plate is emphasised. The use of other types of fixation for composite systems is possible in complex fractures. Anatomical reconstruction must be obtained to ensure a satisfactory functional result.

[Kienboeck disease: treatment by shortening osteotomy of the radius].

INTRODUCTION: A series of twenty eight cases of Kienbock's disease treated by a shortening osteotomy of the radius are reported. MATERIAL AND METHODS: The series consisted of sixteen male and twelve female patients with an average age of twenty-five and followed for an average of five years. Seventeen patients were manual workers. The dominant side was affected in twenty cases; there were no bilateral cases. The clinical grades before surgery were one case in stage I, ten stage II, thirteen stage III, and four stage IV, using the Decoulx classification. The preoperative distal radio ulnar ind

The rationale for precise management of distal radius fractures.

Fractures of the distal radius continue to be one of the most common skeletal injuries treated by orthopedic or trauma surgeons. These injuries account for one sixth of all fractures seen and treated in emergency rooms. The rapid expansion of knowledge regarding the functional anatomy of the hand and wrist, the recognition by treating physicians of the ever increasing functional demands of senior citizens, and improved methodologies of achieving and maintaining anatomic restoration of these fractures have generated a renewed interest in addressing these fractures in a more precise manner.

Operative management of fractures of the shaft of the radius and ulna.

Diaphyseal fractures of the radius and ulna usually can be managed by closed reduction and immobilization in a long arm cast. Open fractures may benefit from pinning if the reduction will be difficult to maintain. Fractures that cannot be reduced or for which satisfactory alignment maintained are best managed operatively. Our preference is for intramedullary fixation. Operative management will be necessary most frequently in children older than age 10.

Bipolar fixation of fractures of the distal end of the radius.

The frequency fo secondary displacement in fractures of the distal end of the radius is very high and varies from 40 to 60% in various series. This results from the void in the epiphysis after reduction due to crushing of the cancellous bone. Secondary displacement can be easily avoided by the bipolar fixation method. This method is quite simple. Two strong Kirschner wires are driven into the radius and into the first metacarpal, and then incorporated in the cast after the reduction of the fracture. In this study we have followed 102 out of 250 patients treated by this method. The number of se
