Recognizing specific characteristics of nonspecific low back pain.

A retrospective review of 1293 cases of low back pain treated over a 12-year period revealed that sacroiliac joint syndrome and posterior joint syndromes were the most common referred-pain syndromes, whereas herniated nucleus pulposus and lateral spinal stenosis were the most common nerve root compression lesions. Referred pain syndromes occur nearly twice as often and frequently mimic the clinical presentation of nerve root compression syndromes. Combined lesions occurred in 33.5% of cases. Lateral spinal stenosis and herniated nucleus pulposus coexisted in 17.7%. In 30% of the cases of spond…

Right-sided low inguinal pain in young women.

Young women with right iliac fossa pain are commonly referred to general surgeons as possible acute appendicitis. The differential diagnosis that includes pelvic visceral disease may be very difficult to determine clinically, especially when the history and physical signs are equivocal. We believe that diagnostic accuracy may be improved by eliciting precisely the site of abdominal pain. Right-sided low inguinal pain may be referred from the female pelvic viscera in the same way as testicular pain may be referred to the groin, as these viscera derive their autonomic nerve supply from the T10-L

[Myofascial trigger point: an indicator of acupoint sensitization].

OBJECTIVE: The present study is to investigate the characteristic distribution of primary pain points and referred pain areas in patients with knee-joint pain diseases, therefore revealing the correlation between the myofascial trigger point and acupoint sensitization. METHODS: A total of 483 patients (320 men and 163 women, 36 to 72 years in age) with knee-joint pain recruited from 4 clinical centers were observed from July, 2017 to April, 2019 in the present study. The areas of primary and referred pain in these patients were detected by thumb-pressing and marked on human anatomic atlas. R…

Delayed referral increases the need for surgery and intervention in patients with acute pancreatitis.

BACKGROUND: The aim was to study the outcomes of acute pancreatitis (AP) patients who were referred from other facilities to a tertiary care centre. METHODS: Patients with AP were who were referred from other hospitals to a tertiary care centre between April 2013 and September 2019 were studied and their outcomes were analysed. Comparison was made between patients referred early (≤7 days) versus those referred late (>7 days). RESULTS: Of the 838 patients seen by us, 650 patients (77.6%) were referred from other centres. Median (interquartile range) onset to admission interval was 5 (4-7) day

Bladder pain relief by HMGB1 neutralization and soluble thrombomodulin in mice with cyclophosphamide-induced cystitis.

High mobility group box 1 (HMGB1), one of damage-associated molecular patterns (DAMPs), plays roles in not only inflammation but also processing of somatic pain. Given that no evidence for roles of HMGB1 in visceral pain signaling is available, we asked if HMGB1 participates in bladder pain accompanying cystitis caused by cyclophosphamide in mice, using the anti-HMGB1 neutralizing antibody and recombinant human soluble thrombomodulin (rhsTM) that sequesters HMGB1 and promotes its degradation by thrombin. Cyclophosphamide, administered i.p., caused bladder pain-like nociceptive behavior and ref

Myofascial trigger point pain.

Myofascial trigger point pain is an extremely prevalent cause of persistent pain disorders in all parts of the body, not just the head, neck, and face. Features include deep aching pain in any structure, referred from focally tender points in taut bands of skeletal muscle (the trigger points). Diagnosis depends on accurate palpation with 2-4 kg/cm2 of pressure for 10 to 20 seconds over the suspected trigger point to allow the referred pain pattern to develop. In the head and neck region, cervical muscle trigger points (key trigger points) often incite and perpetuate trigger points (satellite t…

Normative data for the balance error scoring system for patients with chronic neck pain.

BACKGROUND: Neck pain can have an adverse impact on balance control. The Balance Error Scoring System (BESS) is a widely used for assessing static balance across various populations. However, its application in evaluating balance in patients with chronic subclinical neck pain (SNP) has been limited by the absence of normative data. OBJECTIVE: This study aimed to develop normative values for the BESS in participants with chronic SNP, stratified by age and gender. METHODS: 310 participants aged 20-69 years with chronic neck pain and no balance-affecting disorders were included. Each age group

Experimentally induced muscle pain induces hypoalgesia in heterotopic deep tissues, but not in homotopic deep tissues.

The ability of muscle pain to generate somatosensory sensibility changes is controversial. Thus, in the present study, tonic infusion of hypertonic saline (5%, 7.1 ml administered over 15 min) into the tibialis anterior (TA) muscle was used as an experimental model to induce local and referred pain. The sensibility to high-intensity pressure stimuli applied to the local pain area, referred pain area and an arm was assessed in 14 healthy volunteers. Infusion of isotonic (0.9%) saline into the other leg served as control. The subject continuously scored the pain intensity on an electronic visual…

Spinal nociceptive transmission by mechanical stimulation of bone marrow.

BACKGROUND: Since bone marrow receives innervation from A-delta and C-fibers and since an increase in intramedullary pressure in bone marrow may induce acute pain in orthopedic patients during surgery and chronic pain in patients with bone marrow edema, skeletal pain may partly originate from bone marrow. Intraosseous lesions, such as osteomyelitis and bone cancer, are also known to produce cutaneous hypersensitivity, which might be referred pain from bone. However, little is known about pain perception in bone marrow and referred pain induced by bone disease. Thus, we carried out an in vivo e…

Illness behaviour and personality in intractable facial pain syndromes.

Management of intractable facial pain is often referred to a specialised Pain Relieving Clinic, especially in light of the evidence that psychological factors play a part in the maintenance of the pain. A group of 60 patients so referred were divided into two groups according to diagnosis. The 32 diagnosed as suffering from trigeminal neuralgia were found to be more likely to deny non-pain problems, less irritable, but less convinced that there was a physical disease process responsible for their pain, than the 28 patients with non-neuralgic facial pain. Personality factors did not discriminat

Bilateral temporomandibular joint pain as the first and only symptom of ischemic cardiac disease: a case report.

Pain of ischemic and non-ischemic cardiac disease can be referred to the craniofacial region. Also, in 6% of patients, craniofacial pain can be the first and only and symptom of cardiac ischemia. Missed diagnoses of these cases may lead to unnecessary dental treatment and a significant number of deaths in patients with atypical symptoms of coronary disease. Therefore the aim of this report, was to present a 48-year-old man with a chief compliant of severe bilateral pain in the temporomandibular joint who was referred to us for evaluation of a suspected temporomandibular disorder. Clinical and

Chest pain in the emergency department: the broad spectrum of causes.

We performed a prospective study to describe the broad spectrum of causes of chest pain in patients presenting to the emergency department and to compare the diagnoses in referred patients, self-referred patients and patients rushed in by ambulance. The final diagnosis in a consecutive case series of 578 chest pain patients was established after discharge from the hospital. The underlying disorders were grouped into cardiac, respiratory, gastro-oesophageal disorders, musculoskeletal pathology, somatization disorders, other diseases and unknown. For comparison of the frequencies of the disease

Referred Somatic Hyperalgesia Mediates Cardiac Regulation by the Activation of Sympathetic Nerves in a Rat Model of Myocardial Ischemia.

Myocardial ischemia (MI) causes somatic referred pain and sympathetic hyperactivity, and the role of sensory inputs from referred areas in cardiac function and sympathetic hyperactivity remain unclear. Here, in a rat model, we showed that MI not only led to referred mechanical hypersensitivity on the forelimbs and upper back, but also elicited sympathetic sprouting in the skin of the referred area and C8-T6 dorsal root ganglia, and increased cardiac sympathetic tone, indicating sympathetic-sensory coupling. Moreover, intensifying referred hyperalgesic inputs with noxious mechanical, thermal, a…

Trigger points and myofascial pain: toward understanding how they affect headaches.

Myofascial pain, referred from hyperalgesic trigger points located in skeletal muscle and its associated fascia, is a common cause of persistent regional pain. Clinical and experimental literature on manifestations, pathophysiology, and management of pain from myofascial trigger points is reviewed with priority given to how pain referred from trigger points generates, triggers, and maintains headaches--especially chronic and recurrent ones. Because treating myofascial problems may be the only way to offer complete relief from certain types of headache, clinicians must learn to diagnose and man

A mixed-method study of pain management practice in a UK children's hospital: identification of barriers and developing strategies to maintain effective in-patient paediatric pain management.

AIM: To assess Acute Pain Service and paediatric pain management efficacy in a UK specialist paediatric hospital to inform wider recommendations for future sustainability. BACKGROUND: UK paediatric acute pain services vary. Although comprehensive pain management guidelines exist, consensus on the best model of care is lacking. Worldwide, medical and pharmacological advances and rapid patient turnover have increased the challenges of managing hospitalized children's pain. Simultaneously nurses, who deliver the bulk of pain management, have experienced reduction in skill mix and training opport

Review for the generalist: evaluation of pediatric hip pain.

Hip pathology may cause groin pain, referred thigh or knee pain, refusal to bear weight or altered gait in the absence of pain. A young child with an irritable hip poses a diagnostic challenge. Transient synovitis, one of the most common causes of hip pain in children, must be differentiated from septic arthritis. Hip pain may be caused by conditions unique to the growing pediatric skeleton including Perthes disease, slipped capital femoral epiphysis and apophyseal avulsion fractures of the pelvis. Hip pain may also be referred from low back or pelvic pathology. Evaluation and management requi

Referred otalgia.

Otalgia is the sensation of pain in the ear, while referred otalgia is pain felt in the ear but originating from a nonotologic source. The rich sensory innervation of the ear derives from four cranial nerves and two cervical nerves which also supply other areas in the head, neck, thorax, and abdomen. Pain referred to the ear may originate in areas far removed from the ear itself. Neoplastic, infectious, inflammatory, musculoskeletal, and vascular disorders must all be considered when evaluating the patient with otalgia and a normal ear examination. Otalgia is frequently the presenting complain
