[What change was caused under short-time treatment by the Ueda method for severely disabled infants with cerebral palsy? Report of two cases].

A new physical therapy, Ueda method (UM), were performed on two patients with severe motor and intellectual disabilities syndrome and spastic cerebral palsy (CP). The period of the UM therapy was 5 days in one case with spastic CP and 10 days in another case with rigo-spastic CP. They showed the remarkable reduction of abnormal muscle tone and interesting phenomena such as postural changes of extremities and changes of some primitive reflexes. First, peculiar postures of treated extremities appeared on the first day of the UM therapy, associated with the reducing muscle tone of extremities onl

Reorganization of flexion reflexes in the upper extremity of hemiparetic subjects.

We examined spatiotemporal abnormalities in the flexor reflex response in the impaired upper extremity of hemiparetic subjects. Electrical stimulation was used to elicit flexion reflexes in both upper extremities of 8 hemiparetic brain-injured and 6 control subjects. Electromyograms (EMGs) were recorded from 12 arm muscles, and reflex forces and moments were recorded at the wrist with a load cell, and converted to shoulder and elbow torques. We found that the onset of reflex torque and EMG was delayed in the impaired arm and delays were greater at the shoulder than at the elbow. The normal ref

Reflex blepharospasm associated with bilateral basal ganglia lesion.

A patient with a bilateral striatal lesion secondary to anoxia presented reflex blepharospasm associated with parkinsonism and dystonia in the limbs. The blink reflex excitability curve was enhanced and the R-2 response prolonged as in patients with essential blepharospasm. The findings in this patient support the notion that blepharospasm may be secondary to basal ganglia dysfunction through abnormal facilitation of reticular formation neurons controlling facial nucleus motoneuron excitability.

Clinical Neurophysiological Study for the Diagnosis of Functional Cranial-Cervical Dystonia.

OBJECTIVES: The differential diagnosis of functional and other etiologies of dystonia can be difficult. We performed a clinical neurophysiological study in a female patient with cranial-cervical dystonia, providing strong evidence for the diagnosis of functional dystonia. METHODS: The patient had torticollis and intermittent facial pulling with downward deviation of the left angle of the mouth. She had left laterocollis and right torticollis with left shoulder elevation. She additionally experienced limited anterior-posterior flexion of the neck and moderate limitation of lateral flexion and

Detection of anomalies in the red reflex test requires adequate training.

CLINICAL RELEVANCE: Midwives should receive appropriate training to enable them to reliably detect an abnormality in the red reflex, so as to detect potentially sight- (and possibly life-) threatening visual anomalies in newborns. BACKGROUND: Midwives who had completed the relevant Newborn and Infant Physical Examination (NIPE) training in the UK, were tested to determine whether they could reliably detect an abnormality in the red reflex of a model eye. Their results were compared to a group of undergraduate optometry students in their final year, who had considerably more experience with op

Assessment of nerve conduction study to establish most common electrophysiological predictor of lumbosacral radiculopathy among radiologically diagnosed L5S1 neural foramina compression cases.

Magnetic resonance imaging (MRI) and electromyography (EMG) are complimentary investigations in diagnosis of lumbosacral radiculopathy (LSR). With changing pattern of S1 electrodiagnosis by H-reflex study measures, electrophysiological studies were conducted to establish most common electrophysiological predictors of LSR in MRI diagnosed L5S1 neural foramina compression subjects. Fifty subjects, with definite L5S1 neural foramina compression underwent electrophysiological evaluation and the data was analyzed using established electrodiagnostic criteria. Reduced H/M ratio in combination with ab

Stapedial reflexes in normal versus recruiting ears.

The latency and magnitude of the stapedial reflex was examined in a group of normal-hearing Ss (N: 15) and in 15 Ss with unilateral sensorineural impairment exhibiting abnormal loudness growth in the impaired ear. Reflexes were determined at successive 2-db steps from threshold to 16 db above using a 2-kc/s stimulus tone. Latency and magnitude measurements were not statistically different between groups nor between ears within the pathological group. These results substantiate dependence of the stapedial reflex on loudness. Furthermore, there appears to be a dependence of the reflex magnitude

Neurophysiologic findings in early acute inflammatory demyelinating polyradiculoneuropathy.

BACKGROUND: Patients with early acute inflammatory demyelinating polyradiculoneuropathy (AIDP) may not meet the current neurophysiologic criteria. OBJECTIVE: To document neurophysiologic findings in early AIDP. METHODS: Clinical and neurophysiologic data from 38 AIDP patients, assessed within 10 days of symptom onset were reviewed. RESULTS: In addition to absent H reflexes and abnormal F-wave responses in the acute stage of AIDP, abnormalities of blink reflexes, upper limb sensory responses abnormalities with spared sural responses, presence of distal CMAP dispersion, and A-waves were frequ

Determinants of carotid sinus hypersensitivity in patients with suspected reflex syncope.

PURPOSE: To elucidate the pathophysiology of the carotid sinus reflex in a large cohort of patients with suspected reflex syncope. METHOD: We reviewed an electronic database of patients with suspected reflex syncope who underwent carotid sinus massage. The diagnostic protocol included both right and left massage, in supine and upright positions, followed by tilt testing. RESULTS: The cohort included 2249 patients (52% male), median age 73 years. The conventional abnormality threshold of a maximum RR interval ≥ 3000 ms was present in 14.7% of patients and of a systolic blood pressure fall > 5

Vestibular function in severe GH deficiency due to an inactivating mutation in the GH-releasing hormone receptor gene.

PURPOSE: Body balance involves the vestibular, visual, and proprioceptive systems. IGF-I is a GH-dependent key factor in the development and postnatal differentiation of the inner ear in mice and men, but its role in the vestibular function in adult humans is unknown. We have previously described a cohort of individuals with severe isolated GH deficiency (IGHD) caused by a mutation in the GHRH receptor (GHRHR) gene. These individuals complain of dizziness, exhibit mild sensorineural loss, but have normal postural balance, without increase in falls risk. The aim of this study was to evaluate th

Assessment of spasticity in hemiplegic cerebral palsy. II: Distal lower-limb reflex excitability and function.

A clinical method for measuring the stretch reflex threshold and gain of muscles acting across the ankle joint in children with congenital hemiplegia is described. The stretch reflexes of all limbs were velocity-dependent. Hemiparetic limbs were not necessarily spastic compared with non-paretic limbs, suggesting that the term 'spastic hemiplegia' should be used more selectively and emphasis be placed on the heterogeneity of the hemisyndromes of childhood. Abnormal motor control, planning and dexterity, the hemipostures and plastic (non-electrical) muscle changes may be more important measures

The relationship between acoustic reflex threshold and air-bone gap.

Neither air-bone gap nor acoustic reflex presence/absence gives an unequivocal indication of abnormality in middle-ear sound transmission. In the present study, a stratified population sample was investigated to examine the interrelationships between these and other parameters of middle-ear status. Of a total of 1725 ears, 1450 had both normal acoustic reflex threshold (ART less than or equal to 100 dB; see text) and air-bone gap (ABG less than or equal to 15 dB); 70 had both abnormal ART and ABG; 160 had abnormal ART but normal ABG; 45 had abnormal ABG but normal ART. Examination of data on t

Aspiration in bilateral stroke patients.

Seventy patients with bilateral strokes underwent neurologic and videofluoroscopic barium swallowing examinations; 34 (48.6%) aspirated. Patients with aspiration were more likely to have posterior circulation strokes, abnormal cough, abnormal gag, and dysphonia. However, patients likely to aspirate can be identified best by the presence of an abnormal voluntary cough, an abnormal gag reflex, or both. The prediction of patients at risk for aspiration was not improved by additional clinical information (ie, presence of dysphonia or bilateral neurologic signs).

Temporal course of the threshold and size of the receptive field of the Babinski sign.

The changes of threshold and receptive field of the Babinski reflex, with time, were assessed in ten patients with acute cerebrovascular lesions. It was found that the threshold of the reflex decreased gradually and the size of the cutaneous afferent reflex field enlarged with the passage of time. These findings suggest that there is a gradual release or organisation of the reflex flexor synergy of which the Babinski sign is a modified part.

The blink reflex recovery cycle differs between essential and presumed psychogenic blepharospasm.

BACKGROUND: Psychogenic blepharospasm is difficult to distinguish clinically from benign essential blepharospasm (BEB). The blink reflex recovery cycle measures the excitability of human brainstem interneurons and is abnormal in BEB. We wished to study the blink reflex recovery cycle in patients with atypical (presumed psychogenic) blepharospasm (AB). METHODS: This was a prospective data collection study investigating the R2 blink reflex recovery cycle at interstimulus intervals (ISI) of 200, 300, 500, 1,000, and 3,000 msec in 10 patients with BEB, 9 patients with AB, and 9 healthy controls.

Electrical study of jaw and orbicularis oculi reflexes after trigeminal nerve surgery.

Trigeminal nerve ophthalmic and motor division function was assessed clinically and electrically in 32 patients who had undergone various surgical procedures for trigeminal neuralgia. Using known electrophysiological techniques, the orbicularis oculi and jaw reflexes were tested in all subjects. Abnormalities of the orbicularis oculi reflex were anticipated on the basis of ophthalmic division anaesthesia. However, jaw reflex abnormalities appeared in operated cases with no clinical or electromyographic evidence of masseter denervation. These results were unexpected, and imply that the proprioc
