Management of Penetrating Thoracoabdominal Trauma in a Resource-Limited Setting: A Case Report.

Trauma remains a major global health challenge, particularly in low- and middle-income countries where access to advanced surgical resources is limited. This case report describes the management of penetrating thoracoabdominal impalement in a resource-constrained hospital in Honduras during a Medical Readiness Training Exercise (MEDRTE). An adult male presented after being impaled by a metal spike through the left thoracoabdominal region. With limited imaging and no access to blood products, the patient underwent emergent exploratory laparotomy and thoracotomy. Injuries included gastric perfor

Adenosquamous carcinoma of the cervix: a case report from a resource-limited setting.

INTRODUCTION AND IMPORTANCE: Adenosquamous carcinoma of the cervix (ASCC) is a rare, aggressive histological variant representing 1.8% to 5% of cervical malignancies. It is generally associated with a poorer prognosis than squamous cell carcinoma. In resource-limited settings, systemic barriers often preclude standardized multimodal management, necessitating management decisions shaped by limited resources. This case is presented following the SCARE criteria. CASE PRESENTATION: A 64-year-old multiparous female presented to a tertiary hospital in Uganda with a multi-month history of dyspareuni

Prognostic Markers of Diabetic Foot Ulcer Healing in A Resource-Limited Setting.

This study aimed to identify the prognostic markers for Diabetic Foot Ulcer (DFU) healing using routinely collected clinical data. A prospective cohort study of 1075 (M/F: 664/411) patients who attended the foot clinic with a DFU was conducted. At baseline, detailed clinical data were collected and ulcer characteristics were systematically assessed and classified. During 167 ± 408 days follow-up, ulcers of 1039 patients healed and 36 did not. Ulcers with SINBAD classifications 4, 5 and 6 (compared to SINBAD-3) showed 50%, 60% and 70% increase in the risk of not healing and 2.2, 2.8 and 3.3 fol

Successful Surgical Management of an Advanced Abdominal Pregnancy in a Resource-Limited Setting.

INTRODUCTION: Abdominal pregnancy (AP) is a rare ectopic pregnancy (EP), often misdiagnosed, with no standardized treatment due to limited literature. Most APs result in early pregnancy loss, making advanced AP cases even rarer. We present a case of advanced AP in a limited resource setting. CASE PRESENTATION: A 23-year-old woman with a previous cesarean delivery presented with a 2-month history of amenorrhea, chest pain, and lower abdominal pain. Despite a positive pregnancy test, pelvic ultrasound showed no intrauterine or EP but revealed a paracecal mass, raising suspicion of an appendicea

Diagnostic Challenges of Syphilis in a Resource-Limited Rural Setting: A Case Report.

Syphilis, a sexually transmitted infection caused by Treponema pallidum, continues to represent an important public health challenge worldwide, especially in underserved and resource-limited settings. We report a 65-year-old man from a rural area who presented with progressive weight loss, malaise, generalized lymphadenopathy, and a trunk rash, features that can mimic HIV, tuberculosis, viral infections, or lymphoma. A rapid HIV/Syphilis Duo test (SD BIOLINE, Standard Diagnostics Inc., South Korea) was reactive for syphilis and negative for HIV, but no nontreponemal or confirmatory assays were

Evaluation of psychosocial outcomes of pediatric cancer survivors in a resource limited setting.

Pediatric cancer survivorship presents significant challenges globally, with varying survival rates and survivorship care models across different income settings. This present study focuses on the psycho-social outcomes of pediatric cancer survivors attending an After Completion Therapy (ACT) Clinic in a low-middle-income country, where survivorship care resources are limited compared to high-income countries. The study included 394 pediatric cancer survivors, aged 18 years and above, who had completed two years of disease-free survival post-treatment. Study data was collected from the ACT Cli

A Rare Diagnosis in a Resource-Limited Setting: Alkaptonuria in a Young Dominican Child.

Alkaptonuria (AKU) is a rare metabolic genetic disorder transmitted in an autosomal recessive pattern caused by a deficiency in the activity of the enzyme homogentisate 1,2-dioxygenase (HGD), leading to progressive accumulation of oxidized homogentisic acid (HGA) as a dark pigment in connective tissues, cartilage, and organs in a process called ochronosis. Polymerization of HGA causes urine darkening after a few hours of exposure to the atmosphere, which is a hallmark of AKU. We present the case of a 3-year-old boy from a rural community in the Dominican Republic assessed at the pediatric hosp

Challenges in managing osteogenesis imperfecta in a resource-limited setting: a case report.

INTRODUCTION: Osteogenesis imperfecta is a rare inherited connective tissue disorder that results in excessive bone fragility due to defects in collagen production. The majority of osteogenesis imperfecta cases are inherited in an autosomal dominant pattern, and 17 genetic causes have been identified. Diagnosis is usually based on clinical presentation and low bone mineral density scores, while treatment involves a multidisciplinary approach using medical therapies such as bisphosphonates, vitamin C, and pamidronate. Recent research has also explored new therapies, including teriparatide, deno

Ovarian response to controlled stimulation and its predictors in a limited-resource setting.

BACKGROUND: Infertility remains a serious health concern for Ethiopian women. Most of its treatment approaches entail controlled ovarian stimulation, the responses of which vary. However, there are no data on ovarian response to stimulation or its predictors in our situation. Thus, the current study aimed to assess the ovarian response to controlled stimulation and identify predictors. METHODS: A retrospective follow-up study was undertaken from April 1, 2021, to March 31, 2022, among patients who had first-cycle controlled ovarian stimulation at St.Paul's Hospital Fertility Center in Addis A

Optical iridectomy: A stop-gap for penetrating keratoplasty in a resource-limited setting.

Corneal opacities are a major source of corneal morbidity in Africa and many resource-limited parts of the world. Unfortunately, there is a dearth of specialist corneal services either from lack of manpower or non-availability of materials and tools. This makes penetrating keratoplasty inaccessible from prohibitive cost or lack of donors. The index case was a 45-year-old indigent female farmer who presented with a right atrophic eye and defective vision on the left eye due to stick injury from farm work. Examination revealed a 2/60 eccentric vision and extensive adherent leukoma on the left ey

Practice of enhanced recovery after cesarean delivery in resource-limited setting.

BACKGROUND: Caesarean section is one of the most common procedures and clinician faces dual challenges with feto-maternal morbidity and mortality after caesarean delivery. Enhanced recovery after caesarean delivery protocols might effectively reduce postoperative feto-maternal morbidity and mortality. Therefore, this study aimed to assess practice of enhanced recovery after caesarean delivery among parturients who underwent elective caesarean delivery. METHODS: A cross-sectional study design was conducted from March to June 2021 on 225 consecutive parturients scheduled for elective caesarean

Case Report: Anti-MDA-5 dermatomyositis in a resource-limited setting.

Anti-Melanoma Differentiation-Associated gene 5 (Anti-MDA-5) dermatomyositis is a rare subtype of inflammatory myopathy characterized by unique skin lesions, rapidly progressive interstitial lung disease, and skeletal muscle inflammation. It has a high mortality rate in the absence of early treatment. However, diagnosis of this entity is challenging in a country like Nepal because of various constraints such as lack of expert rheumatologists and resource limitations. Here we describe a case of one patient who had presented to us with generalized weakness, cough and shortness of breath who was

Screening Strategy for Detecting Double-Hit Lymphoma in a Resource-Limited Setting.

AIM: High-grade B-cell lymphomas with MYC and BCL2 and/or BCL6 rearrangements [double-hit lymphomas (DHL)] are aggressive lymphomas. Current literature recommends fluorescent in-situ hybridization analysis (FISH) in all cases of diffuse large B-cell lymphoma (DLBCL) to identify cases of DHL. However, this approach is not feasible in a resource-limited setting. We analyzed cases of de novo high-grade B-cell non-Hodgkin lymphoma using histomorphology, immunohistochemistry and FISH to identify which cases need to undergo FISH testing in a resource-limited setting. MATERIALS AND METHODS: Cases of

The effect of COVID-19 on burn management and outcomes in a resource-limited setting.

INTRODUCTION: Optimal burn care includes fluid resuscitation and early excision and grafting. During the COVID-19 pandemic, resource-constrained environments were susceptible to interruptions in burn care. We sought to characterize pre- and intra-pandemic burn-associated outcomes at a busy tertiary hospital in Malawi. METHODS: This is a retrospective analysis of burn patients that presented to Kamuzu Central Hospital Lilongwe from 2011 through December 2021. We compared patients based on whether they presented pre- or intra-pandemic, starting on March 11, 2020, the date of official WHO design

Primary bladder neck obstruction (PBNO) in children in a resource-limited setting.

PBNO although rare when missed may have the debilitating effect of obstructive uropathy in children. The gold standard for its diagnosis requires videourodynamics and electromyography (EMG) studies which are not available in nearly all hospitals in developing countries. We present a case demonstrating the usefulness of readily available imaging in diagnosis of PBNO as well as demonstrate effectiveness of alpha blockade in children. MCUG and cystoscopy should suffice in cases of suspected PBNO in neurologically normal children to inform trial of therapy in resource limited settings.

Selecting topic areas for developing quality standards in a resource-limited setting.

Variation in practices of and access to health promotion and disease prevention (P&P) across geographical areas have been studied in Thailand as well as other healthcare settings. The implementation of quality standards (QS)-a concise set of evidence-informed quality statements designed to drive and measure priority quality improvements-can be an option to solve the problem. This paper aims to provide an overview of the priority setting process of topic areas for developing QS and describes the criteria used. Topic selection consisted of an iterative process involving several steps and relevan
