Endogenous opiates: 1998.

This paper is the twenty-first installment of our annual review of research concerning the opiate system. It summarizes papers published during 1998 that studied the behavioral effects of the opiate peptides and antagonists, excluding the purely analgesic effects, although stress-induced analgesia is included. The specific topics covered this year include stress; tolerance and dependence; eating and drinking; alcohol; gastrointestinal, renal, and hepatic function; mental illness and mood; learning, memory, and reward; cardiovascular responses; respiration and thermoregulation; seizures and oth

[The hyperventilation syndrome: the treatment problems].

The principal methods of the treatment of the hyperventilation syndrome (HVS) are reviewed. The following methods of the action on the patient are distinguished: psychotherapy, pharmacotherapy, remedial gymnastics, correction of respiratory disorders according to the principle of the biological feed-back mechanism, recurrent respiration or carbon dioxide inhalations. Emphasis is laid on the importance of an individual approach to the institution of the treatment measures. The relationship between the HVS and respiratory diseases is discussed. The authors advance an opinion that every patient m

[Characteristics of motion sickness during prolonged otolith stimulation in anti-orthostatic position].

Thirty healthy volunteers were tested for an extended period of time (up to 4 hours) on modified parallel swings during a head-down tilt (-8 degrees) study. The pattern of motion sickness that developed in this situation was evaluated in terms of physiological parameters (electrocardiography, rheoencephalography, arterial pressure, respiration rate electrooculography, etc.), hormonal status, fluid-electrolyte balance, sensory function. The test subjects developed a vestibulo-hemodynamic syndrome that included vestibular disorders, spatial illusions, fluid displacement in the cranial direction,

Photo-evoked eyelid microvibration in newborn infants and children--reflex arc and maturational change.

We studied maturational process of the photo-evoked eyelid microvibration (PEMV) in 166 newborns, 72 children and 10 adults free of any neurologic disorders. The results indicate that the latency of PEMV shortens during growth due to the myelination of the nerve fibers of the involved pathway. Age-related normal values were established. The pathway of PEMV probably consists of retina, optic nerve, pretectum, facial nerve, and orbicularis oculi muscle. We also evaluated the effect on this reflex of retinoblastoma, facial nerve paralysis, holoprosencephaly anomalad, and a muscle relaxant used fo

The muscles of the upper airways.

The upper airways are a complex structure with multiple functions; many of the muscles participate in the act of respiration. The neural control of upper airway muscles is distinct from that of chest wall muscles under a variety of circumstances. Coordinated activation of upper airway muscles, both regionally and inter-regionally, results in changes in upper airway size and resistance, alterations in the route of airflow, and increases in the ability of the airways to resist collapse. Several disorders have now been described in which neuromuscular drive abnormalities and/or mechanical dysfunc

Mitochondrial functions in chronic spinal muscular atrophy.

We determined the respiration rate, respiratory control and ADP/O ratios, with different substrates in mitochondria isolated from seven patients with chronic spinal muscular atrophy and compared them with normal human muscle. In all cases studied, a severe alteration of the respiratory control with variable derangement of oxidative phosphorylation was found. Similar findings have been described in other neuromuscular disorders including the so-called "mitochondrial myopathy". We believe that this disturbance of mitochondrial function is non specific and only the hypermetabolic syndrome of Luft

Open and clean: the healthy nose.

The nose exerts many functions, mainly for the respiration and the olfaction and represents the first doorway for the oxygen, but also for pathogens. The present Supplement reports some clinical experiences concerning the use of a new internal nasal dilator in different settings, including nasal obstructive disorders, obstructive sleep apnea syndrome, continuous positive active pressure (CPAP), and sport activity. The outcomes support the concept that a healthy nose should be maintained ever patent and free from secretions, as impaired nasal function can significantly affect quality of life. T

Recent Advances on Nitric Oxide in the Upper Airways.

Exhaled nitric oxide (NO) originates from the upper airways, and takes action, to varying extents, in regulation, protection and defense, as well as in noxious processes. Nitric oxide retains important functions in a wide range of physiological and pathophysiological processes of the human body, including vaso-regulation, antimicrobial activity, neurotransmission and respiration. This review article reports the ongoing investigations regarding the source, biology and relevance of NO within upper respiratory tract. In addition, we discuss the role of NO, originating from nasal and paranasal sin

Sleep attacks--apparent relationship to atlantoaxial dislocation.

Sleep attacks, while being characteristic of narcolepsy, have been reported in several metabolic and central nervous system disorders--most of which are refractory to treatment. We have recently treated a 47-year-old man with disabling sleep attacks who was found to have atlantoaxial dislocation. Despite narrowing of the sagittal cervical canal diameter to 12 mm (behind the dens), symptoms of spinal cord compression were mild and intermittent, while signs were absent. Posterior fusion of the C-1 and C-2 vertebrae resulted in relief of symptoms. We suggest that this sleep disorder was a functio

Infantile or malignant osteopetrosis: case report of two siblings.

Infantile or Malignant osteopetrosis is a rare congenital disorder of bone resorption. It is caused by failure of osteoclasts to reabsorb immature bone. Severe infantile or malignant osteopetrosis present at birth or develops within the first few months of life. We are reporting here a case of two siblings with malignant osteopetrosis. Prominent clinical features included marked pallor, noisy respiration, progressive blindness and developmental delay.

The forgotten cause of stridor in the emergency department.

Paradoxical Vocal Fold Movement Disorder is where the larynx exhibits paradoxical vocal cords closure during respiration, creating partial airway obstruction. Causes of vocal fold movement disorder are multifactorial, and patients describe tightness of throat, difficulty getting air in, have stridor, and do not respond to inhalers. We propose using transnasal laryngoscopy examination, which will show narrowing of vocal cords on inspiration, and The Pittsburgh Vocal Cord Dysfunction Index with a cutoff score of ≥4 to distinguish vocal fold movement disorder from asthma and other causes of strid

Metabolic Changes of Hepatocytes in NAFLD.

Nonalcoholic fatty liver disease (NAFLD) is often accompanied by systemic metabolic disorders such as hyperglycemia, insulin resistance, and obesity. The relationship between NAFLD and systemic metabolic disorders has been well reviewed before, however, the metabolic changes that occur in hepatocyte itself have not been discussed. In NAFLD, many metabolic pathways have undergone significant changes in hepatocyte, such as enhanced glycolysis, gluconeogenesis, lactate production, tricarboxylic acid (TCA) cycle, and decreased ketone body production, mitochondrial respiration, and adenosine tripho

[Intralymphatic drug therapy--a pathogenetically substantiated method of treatment in peritonitis].

The method of endolymphatic drug therapy (EDT) suggested by the authors has some advantages over the method of endoarterial therapy (which has proved to be effective in clinical practice) because it is aimed not only at control of the microbial flora, but also at correction of rheologic and microcirculatory disorders, improvement of the processes of tissue respiration and metabolism, stimulation of the intestine, prevention of the damaging effect of enzymes, toxins, and poisons on the organs and tissues of the body, and at correction of disorders of the immune status with promotion of the barr
