Evaluation of the ventilatory effects on human subjects in prolonged hip-flexed/head-down restraint position.

BACKGROUND: The restraint chair is a tool used by law enforcement and correction personnel to control aggressive, agitated individuals. When initiating its use, subjects are often placed in a hip-flexed/head-down (HFHD) position to remove handcuffs. Usually, this period of time is less than two minutes but can become more prolonged in particularly agitated patients. Some have proposed this positioning limits ventilation and can result in asphyxia. The aim of this study is to evaluate if a prolonged HFHD restraint position causes significant ventilatory compromise. METHODS: Subjects exercised

Femoral nerve palsy following iliacus hematoma.

We report a rare case of femoral nerve palsy secondary to hematoma compression of the iliacus. The patient presented with pain 2 days after performing a double kick in karate. The femoral nerve symptoms (numbness and weakness) did not begin until 3 to 4 days after the initial trauma. Magnetic resonance imaging (MRI) confirmed that there was a hematoma compressing the iliacus (and femoral nerve). This hematoma was subperiosteal, within the iliac fossa. This case is rare because the femoral nerve palsy resolved without surgical intervention, the mechanism of injury, the imaging used to diagnose

[The effectiveness of constraint-induced movement therapy for children with hemiplegia following acquired brain injury].

UNLABELLED: Constraint-induced movement therapy is a promising technique for improving upper limb function in adults with hemiplegia. It involves restraint of the non-involved limb and intensive movement practice with the paretic limb. Although the technique has been applied successfully to children with cerebral palsy, only two studies have used it in children with acquired brain injury. OBJECTIVE: To assess the feasibility and efficacy of constraint-induced movement therapy in children with acquired brain injury. METHODS: We used a single-subject experimental design in three children (aged

Applying a DRO schedule and compliance training to reduce aggressive and self-injurious behavior in an autistic man: a case report.

An autistic man who displayed aggressive, self-injurious, and destructive behavior in a psychiatric hospital ward despite administration of psychotropic medications and repeated application of physical restraints was treated with a differential reinforcement of other behavior (DRO) schedule and compliance training. These were employed in conjunction with ongoing medical interventions. After introduction of behavioral procedures, the frequency of applying restraints for violent behavior declined from an average of more than once per day to less than once per month. Data taken while the program

Structured team approach to the agitated patient in the emergency department.

OBJECTIVE: Behavioural disturbance and aggression in the ED is an increasing problem. The present study describes the characteristics of patients with acute behavioural disturbance and their emergent treatment in an ED with a structured team approach. METHODS: This was a retrospective review of acute behavioural emergencies that required response from the Code Black (CB) Team (duress response team) in the ED during 2006. The hospital security log and hospital incident-reporting system identified all documented CB, and the patients' medical records were reviewed. Information extracted included

What is "effective treatment" for a schizophrenic inpatient with persistent treatment-resistant psychosis and severe violent behavior?: a case of ECT.

Aggressive behavior among treatment-resistant schizophrenic patients is a major clinical challenge whose prevalence is underestimated. In our 420-bed psychiatric hospital, some 15% of patients exhibit active psychosis and high rates of verbal/physical aggression necessitating physical restraints. In addition to their condition, these individuals endanger staff and other patients, consume extensive resources, and induce a sense of clinical helplessness.Physicians managing such complex patients face dilemmas regarding choice of treatment, criteria for treatment decisions, treatment goals, and out

The physiological impact of upper limb position in prone restraint.

Deaths occurring during and/or in close proximity to physical restraint have been attributed to positional asphyxia. This study investigated the physiological impact of three recognized prone-restraint positions with participants remaining passive. Position 3 (P3) the supported prone position (SPP) was designed to reduce the extent of pressure on the anterior chest wall (PAC) by bringing the upper limbs underneath the shoulder joint whereas for the other two positions (P1 and P2) the arms were abducted from the torso. Twenty-five adults participated. Forced vital capacity (FVC), expiratory vol

Neural correlates of arousal-induced circadian clock resetting: hypocretin/orexin and the intergeniculate leaflet.

In Syrian hamsters, some procedures for stimulating behavioural arousal (e.g. running in a novel wheel and sleep deprivation by gentle handling with minimal activity) markedly phase-advance circadian rhythms when applied during the middle of the daily rest period, while other arousal procedures do not (e.g. physical restraint, caffeine and modafinil). The neural basis for this differential effect of arousal procedures on clock resetting is unknown. We used c-fos expression as a marker for neuronal activation to determine whether these arousal procedures differentially activate two nonphotic in

Hyponatremia and rhabdomyolysis: a possible relationship.

A 40-year-old schizophrenic presenting with an acute toxic psychosis was found to be severely hyponatremic with a serum osmolality of 217 mOsm/kg. The patient was combative and required physical restraints. Shortly after admission the patient's serum transaminase level was elevated, and the next day the CPK value was 175,000 IU. In addition, his serum and urinary myoglobin levels were markedly raised. Shortly after rhabdomyolysis was diagnosed, the patient developed reversible acute renal failure presumably secondary to the myoglobinuria. This report examines the possibility that the severe hy

Sedative protocols for the restraint of coatis (Nasua nasua) for management and monitoring.

Coatis (Nasua nasua) are medium-sized mammals of the family Procyonidae, native of South America, characterized by an elongated nose and a long tail with alternating light and dark rings. The Instituto Chico Mendes de Conservação da Biodiversidade (ICMBio) plays a crucial role in preserving this and other species within the Iguaçu National Park, where animal capture for tracking, data collection, and medical interventions is performed. In this context, ensuring an effective sedation method is critical. This study aimed to compare the effects of two sedation protocols (dexmedetomidine-ketamine

Curcumin alleviates restraint stress-induced learning and memory deficit and activity via modulation of biochemical, morphology changes, and apoptosis in the prefrontal cortex and hippocampus.

Restraint stress indicated induction of morphology, biochemistry, and behavioral impairments. Several investigations have reported that curcumin has a protective effect against stress disturbance. The present study is designed to investigate the effects of curcumin on learning and memory, activity, biochemical, morphology changes, and apoptosis in the hippocampus and prefrontal cortex of restraint stress rats. For chronic restraint stress, the rats were kept in the restrainers for 2.5 h per day for 21 consecutive days. The animals received the gavage of curcumin every other day for 21 days. Af

Circulating adrenocorticotropic hormone levels, lactate levels, hematocrit and osmolality in relation to capture stress in Atlantic sharpnose sharks, Rhizoprionodon terraenovae.

Incidental capture of sharks during commercial and recreational fishing is of major conservation concern because of the potential effects it can have on physiological stress responses and survival. Endocrine aspects of the stress response are, however, poorly understood in elasmobranchs because of difficulties in measuring the primary glucocorticoid (1α-hydroxycorticosterone). Here, we combined measures of plasma adrenocorticotropic hormone (ACTH), the highly conserved pituitary hormone responsible for stimulating the release of adrenal/interrenal glucocorticoids, with measures of plasma lacta

Vertebral heart size in healthy Australian cattle dog.

The vertebral heart score or size (VHS) measurement is routinely used to provide a more objective measurement of cardiomegaly in dogs. However, breed or body conformation can influence the VHS. To assess the specific VHS for the Australian Cattle Dog, left-to-right lateral, right-to-left lateral, dorsoventral and ventrodorsal thoracic radiographs from 20 individuals free from cardiac and pulmonary disease were obtained. The mean VHS was significantly higher in Australian Cattle Dog (10.5 ± 0.4 vertebrae), when compared with the average VHS for 100 normal dogs of different breeds that had been

Aggressive attempted escape behavior during head-lock restraint reduced reproductive performances in Holstein heifers.

The objective was to determine associations between behavior during head-lock restraint and reproductive performance in dairy heifers. Behavior of 817 Holstein heifers from four farms was evaluated at feeding (Days 0 and 7) while restrained in a self-locking stanchion. All heifers were assigned a body condition score (BCS; 1, emaciated to 5, obese) on Day 0. Heifers were timed-inseminated on a maximum of three occasions to determine impact of behavior for first service pregnancy per AI (FS-P/AI) and cumulative P/AI (C-P/AI). Ovulation was synchronized with an Ovsynch synchronization protocol f

Morphological substantiation for acute immobilization stress-related disorders of adaptation mechanisms.

INTRODUCTION: Nowadays, an individual is being constantly accompanied by stresses in his/her everyday life. Stress reactions, produced in the process of evolution, have become the organisms' response to emergency action or pathological factors and are the important link in adaptation process. However, the adverse course of stress reaction can lead to derangement of the adaptation mechanisms in the body and become the element of the pathogenesis of various diseases. THE AIM: The study was aimed at morphological substantiation of derangement of adaptation mechanisms in white Wistar rats caused

Evaluation of intraperitoneal and intrahepatic administration of a euthanasia agent in animal shelter cats.

One hundred eighty-one adult cats, with body weight greater than 1.8 kg, were obtained from animal shelters, then were administered a sodium pentobarbital-lidocaine euthanasia agent by either the intraperitoneal (IP; n = 77) or intrahepatic (IH; n = 85) route. A preliminary study (n = 19 cats) indicated that most cats gave no indication of perception of injection (responding) if restraint was minimal and injection was rapid. During IP injection, 3 of the 77 cats (4%) responded (turned the head backward or vocalized). Of the 85 cats given IH injection, 8 (9%) responded; however, no response app
