Challenges and Controversies in Treating Massive Rotator Cuff Tears.

Massive rotator cuff tears present several challenges for orthopaedic surgeons. Many rotator cuff tears can be repaired; however, some chronic rotator cuff tears require advanced reconstructive techniques. Repair, if possible, is the optimal treatment for rotator cuff tears. In general, muscle transfers are an option for patients younger than 60 years who do not have pseudoparalysis. Arthroplasty is an option for older patients who have concomitant arthritis and for patients who have pseudoparalysis. Biologic augmentation in the setting of rotator cuff tears continues to evolve, and the applic

Muscle architectural changes after massive human rotator cuff tear.

Rotator cuff (RC) tendon tears lead to negative structural and functional changes in the associated musculature. The structural features of muscle that predict function are termed "muscle architecture." Although the architectural features of "normal" rotator cuff muscles are known, they are poorly understood in the context of cuff pathology. The purpose of this study was to investigate the effects of tear and repair on RC muscle architecture. To this end thirty cadaveric shoulders were grouped into one of four categories based on tear magnitude: Intact, Full-thickness tear (FTT), Massive tear

[Pyoderma Gangrenosum Mimicking Necrotising Fasciitis after Rotator Cuff Reconstruction].

UNLABELLED: Gangrenous pyoderma is a rare complication of surgical treatment mimicking bacterial necrotizing fasciitis. The characteristic symptoms include a pre-existing immune disorder, negative results of the relevant microbiological examinations, no response to any antibiotic therapy, and deteriorating of the condition after radical debridement. Another typical feature is a discrepancy between an ominous appearance of the wound accompanied by leucocytosis and an elevated CRP level, and the patient s good general condition with low-grade fever and no signs of sepsis. The case of a 68-year-o

Evaluation and nonsurgical management of rotator cuff calcific tendinopathy.

Rotator cuff calcific tendinopathy is a common finding that accounts for about 7% of patients with shoulder pain. There are numerous theories on the pathogenesis of rotator cuff calcific tendinopathy. The diagnosis is confirmed with radiography, MRI or ultrasound. There are numerous conservative treatment options available and most patients can be managed successfully without surgical intervention. Nonsteroidal anti-inflammatory drugs and multiple modalities are often used to manage pain and inflammation; physical therapy can help improve scapular mechanics and decrease dynamic impingement; ul

Partial rotator cuff injury in athletes: bursal or articular?

UNLABELLED: A painful shoulder is a very common complaint among athletes, especially in the case of those in sports involving throwing. Partial lesions of the rotator cuff may be very painful and cause significant functional limitation to athletes' sports practice. The incidence of partial lesions of the cuff is variable (13-37%). It is difficult to make the clinical and radiological diagnosis, and this condition should be borne in mind in the cases of all athletes who present symptoms of rotator cuff syndrome, including in patients who are diagnosed only with tendinopathy. OBJECTIVE: To eval

Current concepts on imaging diagnosis of rotator cuff disease.

Rotator cuff disease is common and may be clinically silent or a cause of shoulder pain. Evaluation of the rotator cuff before and after surgery is challenging for the orthopedic surgeon, but the radiologist can make an accurate diagnosis and facilitate appropriate management. This article reviews the current concepts on imaging diagnosis of rotator cuff disease, beginning with a discussion of the complex anatomy of the rotator cuff, continuing into the normal and pathologic MR imaging appearances of the rotator cuff including tendinosis and tearing, and concluding with a review of the postope

Postoperative imaging of bioabsorbable anchors in rotator cuff repair.

BACKGROUND: Bioabsorbable anchors may lead to osteolysis and cyst formation. However, the prevalence of these outcomes is not known for rotator cuff repairs. PURPOSE: To evaluate cyst formation after placement of bioabsorbable anchors for rotator cuff repairs and to verify whether bioabsorbable anchors degraded as intended and preserved bone stock for possible revision compared with metal anchors. The null hypothesis was that the rate and severity of cyst formation around the anchor are negligible. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Between April 2008 and November 201

The effect of shoulder manipulation on rotator cuff integrity.

The use of shoulder manipulation in the treatment of frozen shoulder remains controversial. Humeral fractures and neurological damage are the risks associated with the procedure. A concern of causing a rotator cuff tear exists but the incidence of iatrogenic rotator cuff tears is not reported. The purpose of this study was to assess the effect of shoulder manipulation for frozen shoulder on the integrity of the rotator cuff. In a prospective study, 32 consecutive patients (33 shoulders) with the diagnosis of frozen shoulder underwent manipulation of the shoulder under anaesthesia (MUA), 18 fem

Partial rotator cuff tears in adolescents: factors affecting outcomes.

INTRODUCTION: In the adult population, rotator cuff tears are common and established treatment methods yield satisfactory results. In adolescents, however, these injuries are uncommon and few treatment methods and outcome reports exist. The purpose of this study was to examine a series of adolescent rotator cuff tears, identify associated pathology, and report treatment outcomes. METHODS: A retrospective comparative analysis of adolescent patients treated for rotator cuff tears diagnosed by magnetic resonance imaging (MRI) or arthroscopy between 2008 and 2010 was performed. Patients were divi

Isometric evaluation of rotator cuff muscles in volleyball athletes.

AIM: The aim of the present study was to evaluate, in a group of semi-professional volleyball players, the function of the rotator cuff muscles with a new specific device. METHODS: We have studied 30 asymptomatic volleyball players, 15 females and 15 males, all right handed central or lateral spikers, with some specific functional tests. Every subject was evaluated by some common clinical tests to detect shoulder range of motion (ROM) and rotator cuff muscles strength in both sides. Athletes were also submitted to an isometric bilateral shoulder strength test, with a specific upper limb dynam

Rotator cuff tears: association with acromion angulation on MRI.

OBJECTIVE: Using magnetic resonance imaging (MRI), evaluate the correlation of acromion angulation with thickening of the coracoacromial ligament (CAL) and narrowing of the subacromial space resulting in impingement upon the rotator cuff tendons. MATERIALS AND METHODS: Eighty-nine shoulder MRI studies performed on a 3T scanner were retrospectively analyzed by two blinded independent reviewers. Measurements of the acromion angle (delta angle), CAL thickness and distance between the CAL and humeral head were obtained. The data were categorized into two groups, delta angle less that and greater

[Arthroscopic rotator cuff construction : Current state of refixation techniques].

The treatment of rotator cuff tears is affected by numerous factors. In addition to the patient's age, quality of blood supply, age and size of the tear and fatty infiltration, the refixation technique is important to ensure successful treatment. In recent years a trend towards arthroscopic operation techniques has been observed due to new developments in the field. The most important principles of arthroscopic refixation techniques currently used are presented. Special attention is paid to the type of suture, suture anchors and anchor arrangements used. Biomechanics and footprint coverage of d

[Rotator cuff tear: correlation between ultrasound and arthroscopic findings].

OBJECTIVE: To analyze the preoperative shoulder ultrasound (US) findings in patients with a diagnosis of rotator cuff (RC) tear and correlate them with the arthroscopic findings. METHODS: Retrospective analysis of a consecutive series of patients with a diagnosis of RC pathology in whom US was performed and who underwent surgery (shoulder arthroscopy). The US/arthroscopic findings were correlated with Fisher's and chi2 tests. RESULTS: The total number of patients was 53. We identified US/arthroscopic correlation in 22/34 cases for the total tears (64.7%) and in 7/19 cases for the partial tea

Shoulder arthroplasty for the treatment of rotator cuff insufficiency.

Irreparable rotator cuff tendon tears result from chronic tears, failed cuff repairs, and fracture sequelae and occur in patients with rheumatoid arthritis. The management of patients with cuff tear arthropathy can be challenging. When pain is severe and function is poor, surgical options include hemiarthroplasty, bipolar arthroplasty, extended head arthroplasty, arthroplasty with tendon transfer, reverse shoulder arthroplasty, and fusion. A review of the literature shows good pain relief with hemiarthroplasty in carefully selected patients; however, the reverse prosthesis has been found to be

Understanding and preventing complications in repairing rotator cuff tears.

Repair of rotator cuff tears is a common procedure. Prior to approaching this surgery, it should be realized that each surgical step can lead to complications, including those related to positioning and anaesthesia. Stiffness, infection and failure of repair are the more frequent complications reported.

Biomechanical evaluation of arthroscopic rotator cuff repairs over time.

PURPOSE: The aim of this study was to assess the contact pressure, force, and area over time for 4 common arthroscopic rotator cuff repair techniques. METHODS: The transosseous-equivalent, single-row, triangle double-row, and suture-chain transosseous repair techniques were used to repair a full-thickness tear of the supraspinatus in 16 cadaveric shoulders. Continuous data points were collected immediately after repair and for 160 minutes at set time intervals by use of a custom thin film pressure sensor. RESULTS: Each of the 4 rotator cuff repair techniques showed decreased contact force, p

Open versus two forms of arthroscopic rotator cuff repair.

UNLABELLED: There have been technologic advances in the methods for repairing torn rotator cuffs. We compared the clinical and structural outcomes of three different forms of rotator cuff repair with up to 24 months' followup. We wished to assess how surgical technique affected clinical outcomes and see how these correlated to repair integrity. Three cohorts of patients had repair of a symptomatic rotator cuff tear using (1) an open technique (n = 49); (2) arthroscopic knotted (n = 53); or (3) arthroscopic knotless (n = 57) by one surgeon. Standardized patient- and examiner-determined outcomes

A novel transosseous button technique for rotator cuff repair.

In an attempt to maximize stability by improving the lateral footprint compression of our repair in rotator cuff tears, we have been using a rotator cuff button (Arthrex, Naples, FL) passed through a transosseous tunnel as an anchor for our transosseous sutures. Our new innovation is to pass a rotator cuff button fully loaded with 4 strands around the central post, with 2 leading strands and 2 trailing strands on either end, through our transosseous tunnel. In this way, we can use the 4 central strands through our tunnel to obtain 2 good mattress sutures as a primary repair and the peripheral
