Osteomuscular latissimus dorsi scapula flap to repair chronic, posttraumatic osteomyelitis of the lower leg.

Fractures of the lower leg with soft-tissue loss may be complicated by osteomyelitis. Many treatment plans offered in the past have included the use of vascularized bone, but the soft tissue that accompanied the bone was sometimes less than ideal in quality as well as quantity. This study presents a one-step method for the surgical treatment of chronic, posttraumatic osteomyelitis that includes (1) radical debridement of the area affected by osteomyelitis, (2) use of revascularized bone for stabilization of the affected area, and (3) well-vascularized soft-tissue cover for the defect. The use

Nontraumatic First Rib Fracture in a Young Weightlifter Resulting in Winged Scapula: A Case Report.

INTRODUCTION: Shoulder pain is a common emergency department (ED) presentation. Scapular winging is a rare condition often associated with long thoracic nerve injury. This case report describes an even rarer case of dorsal scapular nerve injury caused by a nontraumatic first rib fracture in a young weightlifter, an injury mechanism not previously reported in the literature. CASE REPORT: A 17-year-old male presented to the ED with left shoulder pain following weightlifting. Physical examination demonstrated scapular winging, and a clinical diagnosis of dorsal scapular neuropraxia was made. Ima

Chondrosarcoma of the Flat Bones: Differential Survival Between High-Grade Lesions of the Pelvis and Scapula.

BACKGROUND: Flat bone chondrosarcomas have worse outcomes than extremity tumors, but there is no data directly comparing the different flat bones. The aim of this study was to examine differences in recurrence and survival between pelvic and scapular chondrosarcoma. METHODS: One hundred and sixty-nine (42 scapula, 127 pelvic) patients with chondrosarcoma who underwent surgical resection were reviewed. High-grade tumors were defined as lesions that were Grade 3 or dedifferentiated on surgical pathology. RESULTS: Patients with low-intermediate grade lesions of the scapula were more likely to h

An Incidental Finding of Unilateral Winged Scapula in a Patient Presenting With Acute Abdomen: A Case Report.

The primary cause of scapular winging, also known as scapula alata, is typically a malfunction of the serratus anterior, trapezius, and rhomboids, the three major scapular stabilizers. Scapular winging is often caused by injuries to the long thoracic nerve, which weakens the serratus anterior muscle. The long thoracic nerve is particularly vulnerable to both acute and nontraumatic damage due to its longer and superficial course. There are very few documented cases of isolated scapula winging. Here, we present the case of a 15-year-old Asian female who initially presented with right hypochondri

The relationship between the presence of scapula fracture and mortality and morbidity in cases with blunt thoracic trauma.

BACKGROUND: Scapula fractures (SFs) occur as a result of high-energy trauma and are significant in terms of life-threatening injuries. There are few studies showing the relationship between SFs and mortality and morbidity in patients with blunt thoracic trauma (BTT). Our study aims to investigate the relationship between SF and mortality and morbidity in BTT. METHODS: Adult patients admitted to the emergency department of Kahramanmaras Sutcu Imam University, School of Medicine with BTT between January 2019 and April 2021 were retrospectively scanned from hospital records. Patients' age, gende

Morphological evaluation of the scapula by three-dimensional computed tomography to elucidate shoulder joint function.

INTRODUCTION: There are many variations of scapula morphology. Evaluation of scapula morphology is necessary to elucidate shoulder joint movement. We aimed to analyze the bone morphology of various parts constituting the scapula to identify certain conserved features among them. MATERIALS AND METHODS: Thirty-one healthy individuals were the subjects. We created a scapular three-dimensional (3D) model using computed tomography (CT). X and Y axes were set on a glenoid surface. We measured the approximate plane of the upper and lower scapular bodies and scapular spine and the central axis of the

Successful Management of Gorham-Stout Disease in Scapula and Ribs: A Case Report and Literature Review.

Gorham-Stout disease (GSD) is an extremely rare bone condition of unknown etiology characterized by spontaneous and progressive resorption of bones. GSD can occur at any age and is not related to gender, genetic inheritance, or race. Any part of the skeleton can be affected and the symptoms correlate with the sites involved. The diagnosis of GSD is established based on the combination of clinical, radiologic, and histologic features after excluding other diseases. Because of its rarity, current knowledge is limited to case reports and there is no agreement on the best strategy for treatment. T

Surgical treatment of dislocated fracture of the scapula column and glenoid: a 22-year follow-up.

INTRODUCTION: Most scapular fractures are caused by high-impact blunt injuries, often as the result of motor vehicle accidents, fall from height, etc. In 80% to 90% of cases, scapula fractures are associated with multiple injuries (clavicle fracture, rib fractures, humeral fracture, pulmonary injury, brachial plexus injury). CASE REPORT: We presented scapular fracture in a 27-years-old male who had sustained a work-related injury when a ground soil brick machine pressed him. Fracture line was identified on radiotherapy and computed tomography scan from the distal scapular angle enclosing scap

Resting Orientations of Dinosaur Scapulae and Forelimbs: A Numerical Analysis, with Implications for Reconstructions and Museum Mounts.

The inclination of the scapular blade and the resting pose of the forelimb in dinosaurs differ among reconstructions and among skeletal mounts. For most dinosaurian taxa, no attempt has previously been made to quantify the correct resting positions of these elements. Here, we used data from skeletons preserved in articulation to quantify the resting orientations of the scapula and forelimb in dinosaurs. Specimens were included in the study only if they were preserved lying on their sides; for each specimen the angle between forelimb bones at a given joint was included in the analysis only if t

Pectoralis major transfer for the management of scapula winging secondary to serratus anterior injury or paralysis.

Pectoralis major transfer (partial or complete) is the most common transfer to the scapula for winging as a result of serratus anterior paralysis or injury. The most commonly described technique is to transfer the pectoralis major prolonged with tendon allograft or autograft to the scapula. We present a technique that entails direct transfer of the sternal head of the pectoralis major with its bony insertion to the lower scapula.

Scapula fractures: a marker for concomitant injury? A retrospective review of data in the National Trauma Database.

BACKGROUND: Many series have found that certain associated injuries occur with greater frequency in patients with scapula fractures than in patients without scapula fractures. However, several of the published series were limited by lack of a control group, inclusion of a patient population limited to the catchment area of one hospital, or inadequate control for injury severity. The goal of this study was to determine whether there was a relationship between scapula fractures and concomitant injury and which injuries related simply to the increased injury severity observed in this patient popu

[The 2 key muscles in thoracotomy for excision of the lung. The latissimus dorsi and the levator scapulae muscles].

Anatomical data related to the thoracotomies performed most frequently in lung surgery are described in some detail: continuity between serratus anterior and levator scapulae as a vide muscular sheet possessing a common deep aponeurosis (thoracolumbar fascia) extending Gilis' space to the vertebral column as the levator scapulae-thoraci space; presence of a "composite aponeurosis" in the posterior angle between serratus anterior and levator scapulae, covering the 8th rib triangle or triangle of auscultation; long costal insertion area and presence of two differently orientated muscle layers fo

[Congenital elevation of the scapula in children. Anatomo-pathological and therapeutic study apropos of 27 cases].

The authors have treated 27 cases of congenital elevation of the scapula in children. 21 were operated on. They describe 3 types of the deformity which was often associated with other congenital lesions local (spine, ribs) or general. Two types of operations were performed: Schrock's procedure (partial excision of the scapula) and Woodward's procedure (dissection and re-insertion of muscles). The second one was followed by better results. Operations must be cautiously decided for they may be the cause of severe complications.

3D reconstruction of the scapula from biplanar X-rays for pose estimation and morphological analysis.

BACKGROUND: Patient-specific scapular shape in functional posture can be highly relevant to clinical research. Biplanar radiography is a relevant modality for that purpose with already two existing assessment methods. However, they are either time-consuming or lack accuracy. The aim of this study was to propose a new, more user-friendly and accurate method to determine scapular shape. METHODS: The proposed method relied on simplified manual inputs and an upgraded version of the first 3D estimate based on statistical inferences and Moving-Least Square (MLS) deformation of a template. Then, man

A Cadaveric Study on the Utility of the Levator Scapulae Motor Nerve as a Donor for Brachial Plexus Reconstruction.

PURPOSE: The purpose of the study was to evaluate the utility of the levator scapulae motor nerve (LSN) as a donor nerve for brachial plexus nerve transfer. We hypothesized that the LSN could be transferred to the suprascapular nerve (SSN) or long thoracic nerve (LTN) with a reliable tension-free coaptation and appropriate donor-to-recipient axon count ratio. METHODS: Twelve brachial plexus dissections were performed on 6 adult cadavers, bilaterally. We identified the LSN, spinal accessory nerve (SAN), SSN, and LTN. Each nerve was prepared for transfer and nerve redundancies were calculated.
