The Natural History of Pediatric and Adolescent Shoulder Dislocation.

BACKGROUND: Twenty percent of shoulder dislocations occur in people younger than 20 years old. Management of anterior shoulder instability in young patients remains an evolving and controversial topic. Herein we review the natural history of anterior shoulder dislocation in young patients. METHODS: The English-language literature was searched for studies examining the natural history of pediatric and adolescent shoulder dislocation. Recurrent dislocation was the primary outcome of interest in most studies. RESULTS: Most studies found that recurrent instability was likely in young patients. S

Necrotizing vasculitis in immediate response to a shoulder dislocation.

The shoulder is one of the most commonly dislocated joints in the human body. Complications usually represent local damage. However, it is unclear whether joint dislocations can have systemic sequellae as wSell. Here we present the case of an 86-year-old female who developed necrotizing cryoglobulinaemic vasculitis in immediate response to a shoulder dislocation. We hypothesize there might be a link between trauma and systemic disease.

Anterior dislocation of shoulder in eclampsia: a case report.

This case report presents a 25-year-old female patient with anterior dislocation of right shoulder secondary to seizures as a complication of eclampsia. This is an unusual mechanism of injury, but similar to other uncontrolled muscular contractions caused by electroconvulsive therapy, etc. To the best of our knowledge only one such case has been reported in the English literature. Closed reduction under general anaesthesia was successfully achieved. High suspicion in patients complaining of pain over shoulder joint is necessary for early diagnosis of this condition.

[Pseudarthrosis of the coracoid process after dislocation of the shoulder].

PURPOSE OF THE STUDY: The aim of this study is to present an unusual lesion combining instability of the shoulder with a fracture of the coracoid process. MATERIAL AND METHODS: The authors have treated one case of recurrent anterior dislocation of the shoulder associated with pseudarthrosis of the coracoid process. The shoulder was stabilised by surgery using the Latarjet procedure. DISCUSSION: The association of an anterior dislocation of the shoulder and pseudarthrosis of the coracoid process is very unusual. It is often unrecognized because of poor knowledge of this lesion or poor quality

Percutaneous closed reduction of fracture-dislocation of the shoulder.

A technique using a cannulated tap in a percutaneous manner allows for easy reduction of the dislocated humeral head with minimal soft-tissue trauma and presents a useful alternative to repeated attempts at open or closed reduction.

[Injury pattern in shoulder dislocation in the elderly patient].

PROBLEM: Recurrent shoulder dislocation is reported to be rare in older patients. While injuries of the glenoid labrum and the anterior capsule following primary dislocation in younger patients seem to determine the rate of instability, the pattern of damages in the elderly has rarely been studied. METHODS: 91 patients with an age older than 40 years with the diagnosis of primary (group A, n = 50) or recurrent (group B, n = 41) anterior shoulder dislocation were included in a prospective study. By analysis of the findings in x-rays, MRI, CT-scans and in 36 patients of diagnostic arthroscopy t

Current concepts in the treatment of anterior shoulder dislocations.

Anterior shoulder dislocations are commonly seen in emergency departments. With the recent proliferation of shoulder arthroscopy, the pathoanatomy has been better delineated. Arthroscopic series have confirmed the very high percentage of Bankart lesions (avulsions of the inferior glenohumeral ligament-labral complex), especially in younger patients. A high rate of recurrent dislocation in young patients has been noted in the literature with standard conservative treatment, consisting of immobilization with or without rehabilitation. This high recurrence rate is thought to be due to the Bankart

Anterior dislocation of the shoulder and rotator cuff rupture.

Thirty-seven patients older than 40 years of age were seen after sustaining primary anterior dislocations of the shoulder. An associated rupture of the rotator cuff in each patient had been missed, often being mistaken for an axillary neuropathy. Eleven of these patients developed recurrent anterior instability that was due to rupture of the subscapularis and anterior capsule from the lesser tuberosity. In no patient was there a Bankart lesion. Repair of the capsule and subscapularis restored stability in all of the patients with recurrence.

Anterior dislocation of the shoulder: a simple method of reduction.

An effective method for reducing anterior dislocation of the glenohumeral joint which does not require either sedation or traction is described. The patient performs the manoeuvre. A series of 32 consecutive patients were treated by this method. Easy reduction was achieved in 72 per cent, with no complications and patients spent less than half as long in the accident and emergency department as when it is not successful. We recommend this simple technique as a first method of reduction in patients presenting to accident and emergency departments.

The fate of traumatic anterior dislocation of the shoulder in children.

The cases of twenty-one patients who had open physes and were treated for radiographic evidence of traumatic anterior dislocation of the shoulder at either the Adelaide Childrens Hospital or The Hospital for Sick Children, Toronto, during a fifteen-year period, were reviewed. All twenty-one patients had had one or more recurrent dislocations. Treatment, which included immobilization in a sling and swathe for as long as six weeks, had no effect on the rate of recurrence.

[The value of MR tomography in acute shoulder dislocations].

24 patients up to two weeks after primary traumatic shoulder dislocation were examined at 0.5 and 1.5 T. Surgical and/or arthroscopic correlation was available in 13, CT-arthrographic correlation in 16 patients. A joint effusion allowing sufficient evaluation of the capsulolabral complex was present in 21/24 (87.5%) cases. 11/14 patients with combined dislocated detachments of the glenoid labrum and capsular lesions were subsequently operated upon. Marrow edema of the humeral head was found in 16/19 Hill-Sachs lesions and in 4/5 fractures of the greater tuberosity. Two patients presented with

[Reduction of traumatic dislocation of the shoulder. For a gentler approach....].

Early traumatic anterior dislocation of the shoulder may be reduced very easily using a method combining gentle traction along the axis of the arm, slow external rotation and outwards pressure on the humeral head with the patient seated. This gentle method does not require sedation or analgesia.

Brachial plexus lesions associated with dislocated shoulders.

We reviewed 28 patients with brachial plexus lesions caused by shoulder dislocation. Contrary to most other reports, we found that the neurological lesions involved the infraclavicular and the supraclavicular brachial plexus. With supraclavicular lesions the involvement was always of the suprascapular nerve, and this always recovered spontaneously. Isolated axillary nerve lesions had the poorest prognosis for spontaneous recovery. We explored all lesions that showed no recovery after three to five months and performed either grafting or neurolysis. We discuss the combinations of nerve lesions,

The surgical treatment of recurrent anterior dislocation of the shoulder.

The authors analysed the results obtained in 94 recurrent dislocations of the shoulder submitted to surgery between 1979 and 1988 according to the Latarjet method. After considering the reasons for selecting this method, they discuss some aspects of the physiopathology of recurrent dislocation and instability of the shoulder. the principles of the Latarjet method are described and the Patte modification, used by the authors since 1986, is briefly illustrated. The long-term results were evaluated according to the walch and Duplay evaluation scheme. Sixty-nine patients with a follow-up ranging f

Prosthetic replacement for chronic unreduced dislocations of the shoulder.

Seven patients with chronic dislocations of the shoulder were treated by humeral head or total shoulder replacement. All patients had significant humeral head damage. Three dislocations were posterior and four were anterior. All had been unreduced for at least two months. A Neer prosthesis was used in each patient and the version of the humeral component was altered by 30 degrees-50 degrees away from the direction of the dislocation. Only a sling and swathe was used postoperatively, and the patient was allowed early motion in a limited-goals physical therapy program. There were no redislocatio

Latissimus dorsi transfer for recurrent dislocation of the shoulder.

The results of latissimus dorsi transfer for recurrent anterior dislocation of the shoulder performed during the past three decades were subjected to a failure analysis. Twenty patients operated on in the light of this had no episodes of dislocation and obtained full function. The mode of action of the latissimus dorsi transfer explains the apparent paradox of an operation that produces external rotation resulting in the prevention of anterior dislocation. The action of the transfer in pulling the humeral head posteriorly is the most important feature. If the transfer is not located correctly,
