Brachial plexus injury after shoulder dislocation: a literature review.

Brachial plexus injuries are among the rarest but at the same time the most severe complications of shoulder dislocation. The symptoms range from transient weakening or tingling sensation of the upper limb to total permanent paralysis of the limb associated with chronic pain and disability. Conflicting opinions exist as to whether these injuries should be treated operatively and if so when surgery should be performed. In this review, available literature dedicated to neurological complications of shoulder dislocation has been analysed and management algorithm has been proposed. Neurological co

The effects of shoulder injury on kinaesthesia: a systematic review and meta-analysis.

The aim of this systematic review was to synthesize the evidence for changes for proprioceptive variables consisting of movement and position sense in participants with glenohumeral musculoskeletal disorders. Five databases were searched until 13th August 2013. Methodological quality was assessed and meta-analyses were performed for active and passive joint reposition sense (AJPS and PJPS) and movement sense, determined with threshold to detection of passive motion (TTDPM). The search yielded 17 studies, four of which were classified as having high methodological quality, seven as moderate and

Reducing the risk of shoulder dystocia and associated brachial plexus injury.

Despite persisting controversy over shoulder dystocia prediction, prevention, and injury causation, the authors find considerable evidence in recent research in the field to recommend additional guidelines beyond the current American College of Obstetricians and Gynecologists and Royal College of Obstetricians and Gynecologists guidelines to improve clinical practice in managing patients at risk for experiencing shoulder dystocia. In this article, the authors offer health care providers information, practical direction, and advice on how to limit shoulder dystocia risk and, more importantly, t

Surgical normalization of the shoulder joint in obstetric brachial plexus injury.

Obstetric brachial plexus injuries (OBPI) result in bony derangements that include posterior humeral head subluxation, glenoid retroversion, and joint incongruity. Often these deformities are accompanied by scapular hypoplasia, elevation, and rotation, which further exacerbate shoulder dysfunction. The purpose of the current study was to investigate the effects of triangle tilt surgery on glenohumeral joint anatomy in 100 OBPI patients. The triangle tilt surgery restores the distal acromioclavicular triangle from an abnormal superiorly angled position to a neutral position, thereby restoring n

Plexus injury after reduction of anterior caudal dislocation of the shoulder.

The case is reported of a 45-year-old male patient who fell when his ladder slipped away underneath him. He sustained a bilateral dislocation of the shoulders (bilateral luxatio erecta humeri). The injury on the one hand a glenohumeral dislocation of the humerus head right to ventral and caudal, accompanied by a suspected fracture of the tuberculum major and on the other hand a glenohumeral dislocation of the left humerus head to ventral and caudal, accompanied by a suspected total collum fracture. Based on this case and the pertinent literature, the patho-physiology, diagnosis and treatment o

Nerve injury following shoulder dislocation: the emergency physician's perspective.

We describe the case of a 57-year-old woman who presented to the emergency department with a right anterior shoulder dislocation following a fall onto the right shoulder and right upper arm. She also complained of numbness in the right forearm and dorsum of the right hand. The examination revealed a bruise to the upper aspect of the right arm resulting from the impact following the fall. The patient also had a right wrist drop and loss of sensation in the lateral border of the right forearm and on the dorsum of the right hand, suggesting a radial nerve injury. She also had altered sensation in

Risk factors for brachial plexus injury with and without shoulder dystocia.

OBJECTIVE: To ascertain whether brachial plexus palsy (BPP) that occurs without shoulder dystocia (SD) represents a traction injury during unrecognized SD or a natural phenomenon with a different mechanism of injury, we compared risk factors and outcomes between SD-associated and non-SD-associated BPP. STUDY DESIGN: Neonates with BPP after cephalic vaginal delivery were pooled from all deliveries at Johns Hopkins (June, 1993-December, 2004) and a dataset of litigated permanent BPP from multiple institutions (1986-2003), grouped by SD association based on clinician documentation and compared b

Axillary nerve injury after thermal capsular shrinkage of the shoulder.

The purpose of this study was to evaluate 4 cases in which axillary nerve injury was observed after radiofrequency capsular shrinkage of the shoulder. These cases were used to evaluate the clinical circumstances under which axillary nerve injury occurred, the time frame over which these injuries recovered, and the ultimate outcome observed in these patients. The case histories of each of these 4 patients were carefully evaluated. In 2 of the 4 cases, purely sensory findings were found, and in 2 cases both sensory and motor injuries occurred. In each case the inferior axillary recess was treate

Obstetric shoulder injury. Associated risk factors, prediction and prognosis.

A prospective case-control study was made to estimate the incidence of clavicular fracture and brachial plexus palsy, to find out possible risk factors during pregnancy and labor associated with these injuries, and to ascertain the prognosis of injured infants. Clavicular fracture occurred in 165 (3.2%) and brachial plexus palsy in 10 (0.2%) of the 5082 infants born during the study period. From maternal characteristics the Body Mass Index (kg/m2) and pregnancy weight gain were significantly greater in the cases than in the controls. Symphysis-to-fundus height was, on average, higher in the ca

Shoulder pain and functional disability in spinal cord injury patients.

Sixty spinal cord injured patients were examined to determine the incidence of nontraumatic shoulder pain and associated functional disability during the first 18 months after spinal cord injury (SCI). Seventy-eight percent of quadriplegics and 35% of paraplegics had pain in the first six months. When reexamined six to 18 months after SCI, 33% of the quadriplegics and 35% of the paraplegics continued to have pain. The functional disability resulting from shoulder pain was not a significant problem for the paraplegics; however, 84% of the quadriplegics having pain had either moderate or severe

The 'teres major first' approach for deltoid reinnervation in brachial plexus injuries with shoulder fracture-dislocations.

BACKGROUND: In upper brachial plexus (C5-C6) avulsion injuries, deltoid reinnervation via radial-to-axillary nerve transfer is standard. However, fracture-dislocations around the shoulder complicate surgical exposure due to fibrosis and distorted anatomy. METHODS: Fifteen male patients (mean age: 26.1 years) with C5-C6 avulsion injuries and concomitant shoulder fractures were treated between October 2019 and April 2022 using a modified posterior approach. The "teres major first" technique utilized the inferior border of the teres major muscle as a consistent landmark to identify and mobilize
