Extra- and intracranial dumbbell-shaped hemangiopericytoma.

Hemangiopericytomas are malignant tumors arising from pericytic cells and account for less than 1% of all vascular neoplasms. We report a rare case of an extra- and intracranial dumbbell-shaped hemangiopericytoma originating from the soft tissue of the neck and penetrating the skull base with invasion into the posterior cranial fossa. The 59-year-old female patient presented with a large pulsating neck mass and reported weakness, abnormal fatigue and headache. MRI revealed an inhomogeneously enhancing tumor and cerebral angiography showed intensive vascularization. Preoperative embolization wa

Malignant giant cell tumor of the sphenoid.

Malignant giant cell tumors (MGCTs) of the sphenoid sinus are extremely rare neoplasms. They are challenging to diagnose and difficult to treat because of their skull base location. To the best of our knowledge, we report the first case of a primary MGCT of the sphenoid arising in a patient with Paget's disease. A 77-year-old man presented with epistaxis and a history of Paget's disease. There was normal cranial nerve function although radiographic images disclosed a large mass centered in the sphenoid sinus and extending into the ethmoid and maxillary sinuses. Excisional biopsy revealed a MGC

Unusual presentation of multiple myeloma with unilateral visual loss and numb chin syndrome in a young adult.

A 39-year-old man presented with unilateral visual loss as the first sign of multiple myeloma (MM). His visual loss was due to a plasmacytoma in the sphenoid sinus compressing the optic nerve with resultant optic nerve atrophy. Shortly after this presentation he developed numb chin syndrome due to a mandibular plasmocytoma compressing the mental nerve. His MM progressed rapidly despite treatment with high-dose steroids and alkylating agents and he required allogeneic bone marrow transplantation in order to achieve a remission. We reviewed the published medical literature on the presentation of

Subtotal petrosectomy in the management of advanced parotid neoplasms.

Circumferential growth of parotid neoplasms may involve the external auditory meatus posteriorly, the floor of the middle cranial fossa superiorly, and the neurovascular structures of the jugular foramen medially. Inadequate tumor resection in these anatomically complex regions will result in local disease recurrence at the lateral skull base. A subtotal petrosectomy approach has been combined with a standard total parotidectomy in the management of 27 patients with aggressive parotid tumors. Twenty-two patients had malignant lesions and 10 individuals had recurrent disease. The resultant cond

Safety profile and surgical outcomes of the endoscopic transorbital approach as a skull base surgical corridor: a systematic review and meta-analysis.

Endoscopic techniques have enabled minimally invasive approaches in neurosurgery, providing shorter recovery times and favorable outcomes. Among these, the endoscopic transorbital approach (ETOA) has emerged as a versatile surgical modality. Despite the growing body of evidence, complication rates associated with ETOA have not yet been systematically evaluated. We systematically searched PubMed, Embase, Scopus, and Web of Science up to March 2026. We included studies enrolling ≥ 5 patients who underwent ETOA as the sole surgical modality to treat both skull base and orbital lesions, providing

[Surgical Strategy for Skull Base Chordoma:Recent Advent of Neuroendoscopic Surgery].

Skull base chordoma is a rare bone tumor that is predominantly found in the clival area and considered to originate from the notochordal remnants. Chordoma is characterized by an aggressive nature and has a high risk of repeated recurrence despite multimodal treatments, including extensive surgical resection and high-dose radiotherapy. Thus, extensive surgical resection of the tumor and adjacent bony structures is strongly recommended. However, surgery was difficult because of the deep location of the lesion and involvement of important anatomies such as the cranial nerves and internal carotid

The "Gift Wrap" Technique - A Method that Simplifies the Placement of Fascia Lata in the Reconstruction of the Skull Base Following Endoscopic Endonasal Surgery: A Technical Note.

BACKGROUND: With the advancement of endoscopic endonasal surgery in the treatment of anterior skull base (ASB) pathologies, extended, watertight reconstructions are needed to prevent cerebrospinal fluid (CSF) leakage. This often involves the use of multilayers closure, with free fascia lata (FL) graft frequently used as an in- and/or outlay. However, positioning the FL properly can be challenging and time-consuming, particularly on wider defects. In this technical note, we present an easier and faster way to position FL using a silicone sheet. METHODS: FL graft is harvested using a standard t

[Skull base metastases with cranial nerve deficits : Clinical profile of a severe disease].

BACKGROUND AND PURPOSE: Skull base metastases are a severe complication of various malignant tumors. If cranial nerves are involved even small lesions can cause significant symptoms. Specific clinical characteristics like neurological symptoms, associated primary tumors, prognosis and optimal treatment are poorly defined and are systematically described in this article. METHODS: In a monocentric retrospective study patients with skull base metastases and cranial nerve deficits who received treatment between 2006 and 2018 were analyzed concerning clinical characteristics at initial diagnosis,

Training the skull base surgeon of the future: a comprehensive approach.

The tremendous advances in endoscopic skull base surgery added a new set of intricate less invasive approaches to the neurosurgical armamentarium. The new generations of neurosurgeons face the challenge of adopting the ever-changing technology and quickly learning modern surgical techniques beyond the traditional neurosurgery comfort zone. The future skull base surgeon must be well trained and knowledgeable in all surgical portals to the skull base in order to select the safest, least invasive, most direct approach that offers the optimum exposure to handle a complex skull base pathology. The

[Classification of Clinical Syndromes Associated with Skull-Base Metastasis in the MR Era].

BACKGROUND: Skull-base metastases frequently cause progressive ipsilateral disturbances of cranial nerves or pain, resulting in poor quality of life in patients with cancer. Magnetic resonance(MR)imaging is the method to detect skull-base metastases, which demonstrates focal lesions of low-intensity signal and enhancement on T1-weighted MR images. METHODS: We reviewed clinical data and MR images from 127 patients with skull-base metastases diagnosed at Shizuoka Cancer Center Hospital between August 2012 and November 2017. RESULTS: The most common primary site was the lung(44 patients), follo

Uncontrolled spread following radiosurgery for a skull base aspergilloma misdiagnosed as schwannoma: is radiosurgery responsible?

BACKGROUND: Little is known about the natural history of skull base fungal lesions in immunocompetent individuals and the effect of high-dose radiation on fungal lesions. METHOD AND RESULTS: We report a case where radiosurgery was given to a skull base aspergilloma, mistaking it to be a trigeminal schwannoma. There was dramatic spread of the lesion to periventricular region with significant increase in the skull base lesion. The patient never received steroids. A stereotactic biopsy established the diagnosis, and treatment with voriconazole helped. CONCLUSIONS: It is possible that radiosurge

Long-Term Outcomes of Stereotactic Radiosurgery for Postoperative World Health Organization Grade I Skull Base Meningioma: Utility of Ki-67 Labeling Index as a Prognostic Indicator.

BACKGROUND: Gross total resection, without causing neurological deficits, is challenging in skull base meningioma (SBM). Therefore, stereotactic radiosurgery (SRS) is an important approach for SBMs; however, it is difficult to predict the long-term prognosis. OBJECTIVE: To identify the predictive factors for tumor progression after SRS for World Health Organization (WHO) grade I SBMs, focusing on the Ki-67 labeling index (LI). METHODS: In this single-center retrospective study, factors affecting progression-free survival rates (PFSs) and neurological outcomes in patients undergoing SRS for p

Calvian Endo-pen: New Coagulation Forceps for Endoscopic Endonasal Transsphenoidal Surgery.

OBJECTIVE: Over the past few years bipolar electrocoagulation techniques in neurosurgery have been continually improving. However, limited access during endoscopic endonasal transsphenoidal surgery (EETS) for central skull base pathologies and the requirement of very precise coagulation in that dedicated anatomical area requires further refinement of bipolar coagulation instruments. We describe our experience (effectiveness of coagulation, intraoperative handling, and the use as a dissecting tool) with a new type of coagulation forceps, the Calvian endo-pen (Sutter Medizintechnik, Freiburg, Ge

Head and Neck: Skull Base Imaging.

There are a myriad of head and neck pathologies that extend from the extracranial to the intracranial compartment, traversing the skull base, and knowledge of the imaging appearance of this pathology is critical to practicing neurosurgeons. This article reviews some of the important inflammatory or acquired head and neck pathology along the skull base, neoplastic skull base lesions, and the intracranial extension of head and neck malignancy. Focus will be on the relevant anatomy, appropriate imaging protocols to evaluate these processes, as well as the differentiating imaging findings on compu

The Making of a Skull Base Team and the Value of Multidisciplinary Approach in the Management of Sinonasal and Ventral Skull Base Malignancies.

The management of sinonasal and ventral skull base malignancies is best performed by a team. Although the composition of the team may vary, it is important to have multidisciplinary representation. There are multiple obstacles, both individual and institutional, that must be overcome to develop a highly functioning team. Adequate training is an important part of team-building and can be fostered with surgical telementoring. A quality improvement program should be incorporated into the activities of a skull base team.

The use of the Olympus EndoArm for spinal and skull-based transsphenoidal neurosurgery.

Minimally invasive surgical techniques have evolved to reduce soft-tissue injury associated with open surgical techniques. The use of endoscopic visualization allows the exposure of deep structures and provides a mechanism to perform all the components of an open surgical procedure through small portals, thus satisfying a basic requirement of minimally invasive surgical procedures. Surgeons in the field of skull-base and spine surgery are now taking advantage of the benefits of such endoscopes. The pneumatically powered EndoArm endoscopic holder has been used extensively in both cranial and sp

Posttreatment imaging of the skull base.

Imaging the skull base after surgery can be challenging because anatomic structures may have been destroyed by an underlying process or removed at surgery. Foreign substances may be introduced to fill a void left by tumor resection, for hemostasis, and to repair dural defects. Previous imaging studies must be available for comparison to understand the characteristics of an underlying lesion. By following the progression of a lesion on subsequent imaging studies, the nature of treatment-related changes and residual or recurrent pathology is best realized.

[A combination of a meningioma of the base of the skull and an arteriovenous micromalformation of the brain stem].

The paper describes a rare combination of atypical meningioma of the base of the skull, which was greatly extended, with a life-time unrecognized small vascular malformation of the brain stem at the level of the pons. Intraoperative hemorrhage into the stem from the micromalformation was a cause of the female patient's death. A detailed account of the operation and the postoperative period and a review of literature on the main aspects of the problem in the diagnosis of cerebral micromalformations are given.
