High-quality acoustic energy harvesting via topology-optimized quasi-bound states in the continuum.

Harvesting energy from ambient sound offers a sustainable power solution for distributed sensors and intelligent devices, yet the inherently low energy density of acoustic waves severely limits conversion efficiency. Overcoming this bottleneck requires confining sound with minimal radiation leakage to maximize electromechanical coupling. Here, we introduce a topology-optimized waveguide-resonator structure that exploits bound states in the continuum (BICs) to achieve high-quality acoustic resonance for efficient energy harvesting. The optimized geometry enforces symmetry-protected confinement,

Pathophysiological insights and therapeutic developments in age-related hearing loss: a narrative review.

Age-related hearing loss (ARHL), or presbycusis, is characterized by a progressive decline in binaural auditory sensitivity, particularly affecting high-frequency hearing and sound localization. The pathogenesis of ARHL is still unclear, correspondingly reflected in a lack of clinically effective intervention strategies. Recent advancements in audiology and neurobiology have illuminated the black box of the pathogenesis of ARHL. The intricate mechanisms underlying ARHL involve inflammation, oxidative stress, excessive autophagy, cellular signaling dysregulation, and metabolic alterations, whic

[New hearing test for functional deafness--the procedure to estimate the genuine hearing threshold].

This report describes a new procedure for examining functional deafness with binaural sound stimulation. This new hearing test can estimate the genuine pure tone hearing threshold quantitatively at every frequency by using the ordinary audiometer. In the case of the nonorganic deafness, even if the hearing disorders are bilateral or hemilateral, we believe that measuring auditory threshold separately, causes the deterioration of the threshold. Therefore, this procedure is designed so that the subject may not be aware of testing the auditory acuity of each ear, and utilizes the response of the

Local Application of Sodium Salicylate Enhances Auditory Responses in the Rat's Dorsal Cortex of the Inferior Colliculus.

Sodium salicylate (SS) is a widely used medication with side effects on hearing. In order to understand these side effects, we recorded sound-driven local-field potentials in a neural structure, the dorsal cortex of the inferior colliculus (ICd). Using a microiontophoretic technique, we applied SS at sites of recording and studied how auditory responses were affected by the drug. Furthermore, we studied how the responses were affected by combined local application of SS and an agonists/antagonist of the type-A or type-B γ-aminobutyric acid receptor (GABAA or GABAB receptor). Results revealed t

Imaging evaluation of the suprahyoid neck.

Evaluating the complex anatomy of the suprahyoid neck on imaging studies can be a daunting task without a sound understanding of anatomy and a systematic approach. In this article, the suprahyoid neck is divided into characteristic anatomic spaces, which allow for the accurate localization of both normal structures and abnormal pathology in the neck. Once a lesion is localized to a specific suprahyoid space, imaging characteristics and clinical data can be used in a logical fashion to provide a clinically useful imaging differential diagnosis.

Middle Ear Mechanics in the Barn Owl.

The barn owl is a common research subject in auditory science due to its exceptional capacity for high frequency hearing and superb sound source localization capabilities. Despite longstanding interest in the auditory performance of barn owls, the function of its middle ear has attracted remarkably little attention. Here, we report the middle ear transfer function measured by laser Doppler vibrometry and direct measurements of inner ear pressures. Our results illustrate that the barn owl middle ear produces a pressure gain between the ear canal and the inner ear vestibule of up to 35 dB, which

Lung sounds in bronchial asthma.

Modern understanding of lung sounds started with a historical article by Forgacs. Since then, many studies have clarified the changes of lung sounds due to airway narrowing as well as the mechanism of genesis for these sounds. Studies using bronchoprovocation have shown that an increase of the frequency and/or intensity of lung sounds was a common finding of airway narrowing and correlated well with lung function. Bronchoprovocation studies have also disclosed that wheezing may not be as sensitive as changes in basic lung sounds in acute airway narrowing. A forced expiratory wheeze (FEW) may b

An algorithm underlying directional hearing in fish.

Humans and other land vertebrates localize sound by comparing the signals in each ear. Even though these differences are virtually absent underwater, fish are still able to sense the direction of sound. In 1975, Arie Schuijf proposed that this ability could arise from a comparison of the particle motion phase and the pressure phase of sound, a prediction that was recently confirmed experimentally for near-field sounds. However, in natural environments, sounds arrive from variable distances, altering the motion-pressure-phase relationship. Thus, directional hearing and distance hearing are pote

Microsecond interaural time difference discrimination restored by cochlear implants after neonatal deafness.

Spatial hearing in cochlear implant (CI) patients remains a major challenge, with many early deaf users reported to have no measurable sensitivity to interaural time differences (ITDs). Deprivation of binaural experience during an early critical period is often hypothesized to be the cause of this shortcoming. However, we show that neonatally deafened (ND) rats provided with precisely synchronized CI stimulation in adulthood can be trained to lateralize ITDs with essentially normal behavioral thresholds near 50 μs. Furthermore, comparable ND rats show high physiological sensitivity to ITDs imm

Modulation of electrocortical brain activity by attention in individuals with and without tinnitus.

Age and hearing-level matched tinnitus and control groups were presented with a 40 Hz AM sound using a carrier frequency of either 5 kHz (in the tinnitus frequency region of the tinnitus subjects) or 500 Hz (below this region). On attended blocks subjects pressed a button after each sound indicating whether a single 40 Hz AM pulse of variable increased amplitude (target, probability 0.67) had or had not occurred. On passive blocks subjects rested and ignored the sounds. The amplitude of the 40 Hz auditory steady-state response (ASSR) localizing to primary auditory cortex (A1) increased with at

Cochlear Implantation for Unilateral Hearing Loss.

Pediatric cochlear implantation for unilateral hearing loss is a new application of cochlear implants with Federal Drug Administration approval for this indication in 2019. The criteria for cochlear implant candidacy include evaluation of the etiology of hearing loss, the duration of deafness, child and family motivation, and the child's comorbidities. Studies have demonstrated significant improvements in speech perception in quiet and noise, sound localization, and speech and language development after implantation. Additional research is required to determine the best candidate and device fo

[Progress in the treatment of single-sided deafness with artificial hearing aids].

Single-sided deafness(SSD) is a common disease. Compared with normal hearing people, unilateral deaf patients often suffer from the loss of sound source localization ability and speech recognition in complex environments due to the lack of binaural summation effect, squelch effect, spatial demasking effect, and the presence of head shadow effect. Patients' quality of life and professional activities are always poor. Therefore, this article will review the related definitions of SSD, epidemiology, hazards, and artificial hearing aids in the application of SSD patients.

[Research progress on cochlear implantation in patients with unilateral deafness].

Cochlear implantation (CI) is a relatively new therapeutic method for patients with severe-to-profound unilateral sensorineural hearing loss. Acoustic signal can stimulate the ear directly, thus patients can gain better binaural hearing. This passage has reviewed the recent researches about CI for patients suffered from single side deafness. It can improve the ability of sound localization and speech recognition in noisy environment for patients, in addition, it may relieve tinnitus to some degree. But the long-term effect and indication selection still need further study.

An essential role for modulation of hyperpolarization-activated current in the development of binaural temporal precision.

In sensory circuits of the brain, developmental changes in the expression and modulation of voltage-gated ion channels are a common occurrence, but such changes are often difficult to assign to clear functional roles. We have explored this issue in the binaural neurons of the medial superior olive (MSO), whose temporal precision in detecting the coincidence of binaural inputs dictates the resolution of azimuthal sound localization. We show that in MSO principal neurons of gerbils during the first week of hearing, a hyperpolarization-activated current (I(h)) progressively undergoes a 13-fold in

Constrictive pericarditis localized to left ventricle presented with left pleural effusion: a case report.

An 18-year old woman presented with progressive dyspnea and cough. Physical examination revealed decreased breath sounds at the left hemithorax and distant heart sounds with no murmurs or rub. Electrocardiogram revealed low voltage. Chest X-ray showed unilateral left-sided pleural effusion with no cardiomegaly. Transthoracic echocardiogram showed thickened pericardium localized throughout the left ventricle impairing the diastolic filling. Doppler waveforms were suggestive of localized constrictive pericarditis. A computerized tomographic scan of the chest confirmed the presence of unilateral
