Epidural lipomatosis and congenital small spinal canal in spinal anaesthesia: a case report and review of the literature.

INTRODUCTION: Complications after lumbar anaesthesia and epidural blood patch have been described in patients with congenital small spinal canal and increased epidural fat or epidural lipomatosis. These conditions, whether occurring separately or in combination, require magnetic resonance imaging for diagnosis and grading, but their clinical significance is still unclear. CASE PRESENTATION: A 35-year-old Caucasian woman who was undergoing a Caesarean section developed a longstanding L4-L5 unilateral neuropathy after the administration of spinal anaesthesia. There were several attempts to corr

Normal anterior-posterior diameters of the spinal cord and spinal canal in healthy term newborns on sonography.

BACKGROUND: There are no published normal values for spinal cord and canal diameters in newborns. Spinal cord and spinal canal diameters are assessed subjectively by radiologists without any objective values for the upper limit of normal. OBJECTIVE: To determine normal values for anteroposterior (AP) diameters of the spinal cord and spinal canal on sonography in healthy term newborns. MATERIALS AND METHODS: We performed ultrasound of the entire spine on 37 healthy newborns (23 male, 14 female). The AP diameters of the spinal canal and spinal cord were measured at representative levels of the

[Spread of hyperbaric local anesthetics in a spinal canal model. The influence of Trendelenburg position and spinal configuration].

BACKGROUND: The influence of Trendelenburg positions and variations in spinal canal configuration on the spread of hyperbaric spinal anesthetics was examined in two models of the subarachnoid space. METHODS: Both models included simulations of the spinal cord, filum terminale and cerebrospinal fluid. Model I had a straight shape, thus omitting replications of lumbar lordosis and thoracic kyphosis. It allowed the evaluation of fluid dynamics and the spread of 0.5% hyperbaric bupivacaine and 4% hyperbaric mepivacaine in 0° (supine position), 5° and 10° head-down tilt positions. Model II include

Management of degenerative changes and stenosis of the lumbar spinal canal secondary to cervical spinal cord injury.

We describe the case of a 47-year-old female who sustained a C5/6 fracture with C6 complete spinal cord injury 26 years ago. She presented with increased spasticity of the lower extremities, the abdominal wall and episodes of autonomic dysreflexia. Imaging of the spine revealed post-traumatic kyphosis at the level of the injury and degenerative changes of the lumbar spine with marked facet joint hypertrophy at the level of L4/5 causing severe spinal canal stenosis. Discussants of this case comment on the possible pathophysiological mechanisms causing autonomic dysreflexia, especially the devel

[The spinal canal, dural sac and spinal cord on cervical myelograms. Possibilities of quantification using a computer].

The objective of the presented work was to investigate anatomical structures of the cervial spine and their interrelations on cervical myelograms in a neutral position and in retroflexion. Cervical myelograms were made in 34 patients from a lateral approach between the first and second vertebra. In a neutral position and in retroflexion the relations of the dural sac and spinal canal were investigated in 26 subjects with a normal antero-posterior diameter of the spinal canal and in eight patients with congenital stenosis of the spinal canal. Quantification by means of a computer revealed that

Extrarenal malignant rhabdoid tumor infiltrating the brachial plexus and spinal canal.

Extrarenal malignant rhabdoid tumors are rare, aggressive lesions that primarily affect infants and children with characteristic SMARCB1/INI1 mutations. While rhabdoid tumors are most commonly found in the kidneys and central nervous system, they have been reported in virtually every soft tissue in the body. A 20-year-old previously healthy male presented with a 4-month history of left upper extremity weakness and pain and a 1-week history of lower extremity weakness. MRI showed a combined intradural and extradural mass arising from the C8 root and involving the lower trunk of the brachial ple

Postoperative anemia following posterior decompression surgery for lumbar spinal canal stenosis.

The incidence of lumbar spinal canal stenosis (LSCS) is increasing in Japan. Posterior lumbar decompression surgery, wide fenestration and laminectomy, for LSCS is a common treatment modality. Compared with posterior fusion surgery, posterior decompression surgery has been considered as less invasive. However, no reports regarding postoperative anemia following posterior decompression surgery have been published. In this retrospective study, we evaluated changes in hemoglobin values following wide fenestration in 80 patients with LSCS based on the number of operated levels, and also analyzed t

A new ceramic interspinous process spacer for lumbar spinal canal stenosis.

OBJECTIVE: Because surgery in elderly patients should be minimally invasive, interspinous process distraction has been widely used in this group to treat lumbar canal stenosis. We developed a new interspinous process distraction spacer composed of hydroxyapatite ceramic. In this work, we demonstrate the usefulness of this novel device. METHODS: Since 2003, we operated on 19 elderly patients with lumbar canal stenosis, including 14 men and five women. Their mean age was 70.1 years. We compared the intervertebral angle, posterior disc height, and interspinous process distance on midsagittal mag

Primary malignant fibrous histiocytoma extending into spinal canal through intervertebral foramina.

A 60-year-old man was referred to us because of gradual-onset radiculopathy in his left leg for 2 months. Magnetic Resonance Imaging (MRI) of the lumbar spine demonstrated a relatively homogeneous huge mass in the left paravertebral area from L1 to L3 (Fig. 1A, B). Axial MRI scan (Fig. 1C) confirmed the extension of the tumour into the adjacent spinal canal through the neural foramina, forming an extradural mass, which resulted in the compression of both the dural sac and the nerve roots. The patient was operated on by a staged-resection. First, a left retroperitoneal approach was performed fo

Extensive intradural arachnoid cyst of the lumbar spinal canal: case report.

BACKGROUND: Spinal intradural arachnoid cysts are rare outpouchings of arachnoid lining occurring mainly in the thoracic and cervical regions. MRI is considered the diagnostic procedure of choice; however, some arachnoid cysts have been reported to elude diagnosis by MRI due to the similar signal intensity of the cyst and the subarachnoid space. CASE DESCRIPTION: We present a case of a 41-year-old woman with an intradural arachnoid cyst of the lumbar spinal canal. Diagnostic studies demonstrated a herniated L4-5 disk, which led to two operations. Despite intractable pain postoperatively, the

Muscle sympathetic nerve activity in patients with lumbar spinal canal stenosis.

The present study aimed to measure sensory nerve conduction velocity (SNCV) and muscle sympathetic nerve activity (MSA) in both normal subjects and patients with lumbar spinal canal stenosis (LSCS), and to determine what sensory and sympathetic nerve systems relate to the development of abnormal sensation in the lower limbs of the patients. The study population was 12 patients and 10 age-matched healthy control subjects. A statistical difference in the mean MSA intervals was found between the LSCS patients and the normal subjects. There was a fairly large difference between them in the values

Lateral lumbar spinal canal stenosis: classification, pathologic anatomy and surgical decompression.

The lateral region of the lumbar spinal canal is subject to pathologic alterations which encroach upon the neural structures located within. In order to further understanding and facilitate communication among specialists in diverse disciplines, a subdivision of the lateral region is proposed to comprise three zones: entrance zone, mid-zone and exit zone. The anatomical boundaries of these zones are defined and the pathologic conditions of each are described, and serve as the basis for techniques of surgical decompression when clinically required.
