Ankylosing spondylitis (AS) is a chronic rheumatic disease of young men that affects mainly the axial skeleton and is associated with HLA-B27 in 90% of the cases. Incidence of cardiovascular involvement in AS ranges between 10%-30%; conduction disturbances have been described in 1%-9% of the patients with AS. The majority of the series show a relationship with longstanding disease. To our knowledge, this is the first report of complete heart block in early AS.

OBJECTIVE: To investigate the diagnosis and effectiveness of surgical treatment for lumbar pyogenic spondylitis.
METHODS: Between February 2005 and June 2010, 15 cases of lumbar pyogenic spondylitis were treated. There were 10 males and 5 females with an average age of 48.6 years (range, 26-72 years). Affected segments included L2, 3 in 3 cases, L(3, 4) in 3 cases, L(4, 5) in 8 cases, and L5, S1 in 1 case. All cases had clinical manifestations of moderate or severe fever, local pain of lesion, and limitation of the spinal movement; 10 patients had nerve root symptoms, and 8 patients had sympt

UNLABELLED: Tuberculous spondylitis (TS) frequently poses both diagnostic and therapeutic challenges. The clinical symptoms, radiological imaging studies and laboratory tests are quite often inconclusive in the early stages of the disease.
GOAL: To identify early clinical symptoms, review results from radiological imaging studies and laboratory tests to establish their diagnostic value and determine the effect of conservative therapy in patients with early TS.
RESULTS: Twenty two (22) patients with TS subjected to conservative therapy were studied. Medullary compression syndrome was found in

BACKGROUND: Although the number of patients with tuberculous spondylitis in Japan is increasing slowly, the proportion of the elderly among these patients is increasing more quickly. The purpose of this study was to describe the clinical features and diagnostic imaging findings in elderly tuberculous spondylitis patients in order to enhance diagnosis of the condition in the elderly population.
METHODS: We conducted a retrospective review of 23 patients over 70 years of age previously diagnosed with tuberculous spondylitis. Clinical signs and symptoms, including local pain, fever, and neurolog

This is a case report of acute L3/4 vertebral osteomyelitis due to Salmonella paratyphi A confirmed by culture from vertebral needle biopsy. From a review of the literature this is the first reported case with bacteriological confirmation. The rarity of Salmonella paratyphi spondylitis and the options for treatment are discussed.

We describe a 22-year-old woman with coexisting Marfan's syndrome (MFS) and ankylosing spondylitis (AS). A change of fibrillin-1 due to mutation of the FBN1 gene (MFS) or a cell-mediated autoimmune response in AS could account for a common pathology.

PURPOSE: Spondylitis is a rare complication of vertebroplasty with only one case report having been published to date. We report a further case of spondylitis after vertebroplasty that was managed successfully with conservative therapy.
METHODS: The clinical course of a 55-year-old patient with secondary osteoporosis due to liver cirrhosis from alcohol abuse is reported, in whom percutaneous vertebroplasty of three fractured vertebral bodies (L3-L5) was complicated by spondylitis at these levels.
RESULTS: Spondylitis of L3-L5 with paravertebral abscess formation and progressive collapse of L

OBJECTIVE: : Brucellar spondylitis is one of the most serious complications of brucellosis. It should be considered in patients who have back pain and neurologic disorders as well as systemic symptoms and findings in or from endemic areas such as Turkey. This report describes important features of the disease.
METHODS: : Twenty-five patients with brucellar spondylitis were retrospectively evaluated in terms of their clinical, laboratory, and radiologic features and their response to different treatment regimens.
RESULTS: : The most common symptoms of brucellar spondylitis were back pain, fev

As advances in the treatment of ankylosing spondylitis continue, TNF-alpha blocking agents may eventually be used as a first-line treatment. MR imaging could then be used to aid in the early diagnosis of ankylosing spondylitis by identifying early sacroiliitis, followed by immediate initiation of treatment to prevent the progression of the disease with its accompanying morbidities. Currently, radiographic identification of sacroiliitis remains the mainstay in diagnosing ankylosing spondylitis. In ankylosing spondylitis and psoriasis, MR imaging can demonstrate areas that are undergoing active

Spinal histoplasmosis is a rare disease condition that must be differentiated from other common inflammatory lesions of the spine such as tuberculosis. A case is presented of a pathologically proven African spinal hisptoplasmosis in a 39-year-old female. Paraplegia and fever were the patient's clinical findings. Cervical plain radiography demonstrated a lytic destructive process of the lower cervical spine with radiographic signs similar to tuberculosis. The surgical management and chemotherapy of histoplasmosis are briefly discussed.

The incidence of tuberculosis is increasing in many of the industrialized countries, mainly due to the spreading of HIV infections, the migration from countries with high prevalence of tuberculosis and possibly through the increase of the aged population. In 10 to 25% of the cases, tuberculosis manifests itself in extrapulmonary organs. One of the most frequent locations is the spine. The symptoms of the TB spondylitis are unspecific, and the course is creeping, which makes an early diagnosis difficult. Because of the frequent, severe complications, this must be attempted. In patients belongin

A 37-year-old man with ankylosing spondylitis (AS) and psoriasis who was successfully treated with methotrexate (MTX) is reported. In 1980, he had low back pain, limited motion in the lumbar spine, radiological findings of bilateral sacroiliitis, and HLA-B27 positivity. In January 1991, he developed psoriasis and he had difficulty in performing desk work in spite of treatment with antirheumatic drugs. In May 1991, MTX 7.5 mg/week per os was started. Joint symptoms, psoriasis, and acute phase reactants improved within 1 month after the treatment and this improvement continued for more than 6 mo

A 23-year-old man is described with tuberculous spondylitis and paravertebral abscedation (Pott's disease). This is one of the rare cases of Pott's disease in a patient coming from industrialised Belgium, not belonging to one of the known risk groups such as intravenous drug users, immunodeficients or immigrants from southern countries. The epidemiology, differential diagnosis, complications and therapy are briefly discussed.

Four weeks after a slight scratch a 43-year-old man noted a severe back pain. Physical examination gave suspicion of a bacterial spondylitis. From the biopsy of the involved vertebral body Haemophilus aphrophilus was isolated under aerobic growth conditions.

Ankylosing spondylitis is one of the oldest diseases in humans; however, it is still one of the most fascinating and mysterious in human pathology. The unusual combination of both fundamental pathological processes: inflammation and ossification (which are mostly independent in respect to time and place) is unique. Until 1973, ankylosing spondylitis did not attract much immunological research. After the detection of an association between HLA B27 and the disease, clinical and immunological research was stimulated. It was supposed that HLA B27 may be a pathogenic factor. Meanwhile, it has becom
