PURPOSE: To investigate the temporal sequence of vascular, neural, and extracellular matrix (ECM) changes during the progression of collagenase-induced patellar tendinopathy and to test the hypothesis that early angiogenesis initiates a cascade leading to neural remodeling and tendon degeneration.
METHODS: A collagenase-induced rat patellar tendinopathy model was established in 40 male Sprague-Dawley rats. Animals were divided into one control group (PBS injection, n = 8, sacrificed at 12 weeks) and four experimental groups (collagenase type I injection, n = 8 each, sacrificed at 2, 4, 8, and

Tendinopathy is a multifactorial condition driven by dysregulated mechanotransduction, chronic low-grade inflammation, oxidative stress, and impaired matrix homoeostasis. Despite its prevalence, current treatments focus largely on symptomatic relief, with limited impact on underlying pathology. This narrative review synthesizes emerging evidence on the molecular mechanisms underpinning tendinopathy, including integrin - FAK/Src, YAP/TAZ, and PIEZO1 signalling pathways, and highlights how these insights inform stage-specific rehabilitation strategies. In particular, isometric loading is explore

UNLABELLED: Proximal hamstring tendinopathy (PHT) can occur in active and sedentary people and presents clinically as localised lower buttock pain with activities including prolonged sitting, squatting, and walking uphill or running. A recent systematic review of interventions for PHT reported insufficient evidence to recommend one intervention over another. The most commonly recommended treatment for PHT by experts is education and exercise. Exercise for PHT aims to restore kinetic chain function and loading capacity of the hamstring complex. The purpose of this commentary is to outline an ed

Achilles tendinopathy (AT) is the most common running-related pathology among masters runners. Previous evidence suggests there are no differences in submaximal running biomechanics between masters runners with and without AT. Evidence suggests lower extremity power deficits are common among ageing individuals and those with AT. The single-leg horizontal hop (SLHH) test is a valid and reliable lower extremity power test. The purpose of this study was to investigate differences in SLHH biomechanics between masters runners with and without AT. 32 masters runners age 50 and older with AT (60.31 ±

Tendinopathies represent about 45% of musculoskeletal lesions and they are a big burden in clinics characterized by activity-related pain, focal tendon tenderness and intra-tendinous imaging changes. Many approaches have been proposed for tendinopathies' management (e.g., nonsteroidal anti-inflammatory drugs, corticosteroids, eccentric exercises, laser therapy), unfortunately with very little support of efficacy or serious side effects, thus making the identification of new treatments fundamental. The aim of the study was to test the protective and pain reliever effect of thymoquinone (TQ)-loa

The patient is a 65-year-old female recreational skier and avid walker who presented with a several-month history of right ankle and foot pain. The patient's pain began without inciting event and was described as a constant aching pain aggravated by downhill walking and alleviated with rest. She was diagnosed with right distal tibialis anterior tendinopathy with partial thickness tear noted on magnetic resonance imaging and musculoskeletal ultrasound. Given symptoms recalcitrant to conservative measures, the patient opted to pursue an ultrasound-guided prolotherapy injection and a course of ph

A 62-year-old right hand-dominant man with a 20-year history of left shoulder pain and 6-month insidious, progressively worsening symptoms was referred to physical therapy with clinical and radiographic evidence of left supraspinatus tendon calcification. Following examination, the patient was prescribed rest, isometrics, and pain-free range of motion. He was treated with iontophoresis for 3 visits per week for 5 weeks. Radiographs, repeated 1 week post treatment and 9 weeks since his initial radiographs, demonstrated marked resorption of the calcific deposit. J Orthop Sports Phys Ther 2020;50

BACKGROUND: Most knowledge regarding conservative management for lower limb tendinopathy (LLT) is for persistent symptoms, with less known about conservative management of acute LLT. Sub-optimal management of acute LLT is detrimental in many regards, not least the likely conversion to persistent symptoms.
OBJECTIVES: To synthesise existing literature on conservative management of acute LLTs.
DESIGN: Systematic review of relevant literature (PROSPERO [ID: CRD42018117882]).
METHOD: A search was made of multiple databases (MEDLINE, CINAHL and EMBASE) using relevant search terms. Titles, abstra

The aim is to report the long-term clinical results after pisiform excision in patients with refractory flexor carpi ulnaris (FCU) tendinopathy. We performed pisiform excision in 14 patients with recalcitrant FCU tendinopathy, who had failed conservative treatment. Nine patients were followed-up for more than 2 years. Pre-operative pain visual analog scale (VAS) was extracted from the electronic medical records. Post-operative symptoms and function were assessed with pain VAS, quick disabilities of arm, shoulder and hand (DASH) score, patient rated wrist evaluation (PRWE) score, and satisfacti

OBJECTIVES: Patellar tendinopathy (PT) is a debilitating and prevalent condition that tends to affect those who are physically active or engaged in jumping sports. Although tendinopathies are known to have a genetic basis, the role of DNA methylation as an epigenetic factor and risk determinant for human PT has never been described. We sought to determine whether differences existed between the methylation profiles of both the TIMP2 and ADAMTS4 gene promoter sequences in a cohort of males having undergone surgery for patellar tendinopathy compared to controls.
DESIGN: Case-control epigenetic

Dialysis-related amyloidosis (DRA) is a specific subtype of amyloidosis with several clinical presentations. Herein we report a case of severe destructive tendinopathy around the wrist associated with long-standing hemodialysis (HD). A 63-year-old female patient who had been on regular HD for 23 years suffered from symptoms of pain and a palpable mass around the wrist. Magnetic resonance imaging showed an ill-defined soft tissue mass around the extensor tendons that partially invaded the wrist joint. We performed surgical excision and tenolysis for the mass. The operative finding revealed an i

Insertional Achilles tendinopathy is a degenerative enthesopathy associated with pain and dysfunction. Nonsurgical management is first attempted for a period of 3 to 6 months and may consist of physical therapy with eccentric training and other modalities. Surgical treatment can be successful with a variety of approaches. A thorough debridement through a midline tendon-splitting approach is associated with high satisfaction rates. Flexor hallucis longus transfer to augment the repair is considered in older, heavier patients or if more than 50% of the tendon was debrided. Early functional rehab

BACKGROUND: Although gastrocnemius recession has been proposed and used in the treatment of chronic noninsertional Achilles tendinopathy, only weak evidence exists to support this operative indication. The purpose of our study was to assess the clinical and functional outcomes of patients treated with gastrocnemius recession at 2 institutions for this problem.
METHODS: Thirty-four patients were identified through our medical records and asked to participate in this study. Thirty patients (35 legs) responded to the invitation. Sixteen patients were eligible for clinical follow-up, and 14 patie
