Extra-articular elbow arthroscopy has to be considered as the evolution of the elbow surgery to a mini invasive endoscopically assisted surgery developed by the recent advance of the elbow arthroscopy. Various pathologies, such as lateral epicondylitis, ulnar nerve entrapment, distal Biceps tendon rupture, synovial cysts, or olecranon bursitis have been treated arthroscopically. Extra-articular pathologies can be treated through an intra-articular endoscopic approach. The true endoscopic extra-articular technique is proced through a real anatomical space or inside a space of work created de no

Rice bodies are most commonly found in inflammatory joints of patients with rheumatoid arthritis and have traditionally been considered to be a nonspecific response to inflammatory synovial disease. In the present study 18 rice bodies were found in the tibialis tendon sheath of a nine-year-old Omani boy subsequent to a date thorn injury. The rice bodies consisted of three major components: fibrin, collagen, and fibroblasts. In contrast to the findings of other authors there were no type A, B, or C synoviocytes, cartilage, or vascularisation of the rice body. At this extra-articulate site it wo

BACKGROUND: Population-based data for diabetic hand syndrome (DHS) are limited. The aim of the present epidemiological study was to estimate the overall and cause-specific prevalence and rate ratio (RR) of DHS in patients with diabetes.
METHODS: The present study was a cross-sectional study based on a random sample of 57 093 diabetics and matched controls, both identified from Taiwan National Health Insurance claims in 2010. The DHS analyzed in the present study included carpal tunnel syndrome (CTS), stenosing flexor tenosynovitis (SFT), limited joint mobility (LJM), and Dupuytren's disease (

BACKGROUND: The inability to extend the fingers at the metacarpophalangeal and interphalangeal joints leads to finger drop. While wrist drop and foot drop are well recognized, the causes of finger drop are poorly understood.
AIMS: This study describes the clinical, electrodiagnostic (EDX), and ultrasound (US) features in patients with finger drop.
MATERIALS AND METHODS: This is a retrospective study of 87 patients presenting with finger drop and referred for EDX studies during the past 10 years. We analyzed the clinical picture, EDX data, and US findings. The patients were categorized into g

In regard to the complex anatomical relationship of peripheral nerves and muscles, tendons, fasciae as well as their long course within those anatomical structures and additional close contact to bony structures, they are prone to suffer from local compression syndromes. Hence creating a vast majority of entrapment syndromes - well described in literature for almost every single nerve. The purpose of this article is to give an overview of symptoms, signs, diagnostic studies and treatment options, addressing especially the less known syndromes. Compression syndromes of the upper arm and shoulde

Checkrein deformity is characterized by the dynamic status of the hallux, in which flexion deformity is aggravated by ankle dorsiflexion and relieved by ankle plantarflexion. In most cases, a checkrein deformity occurs secondary to trauma or following surgery. It has been suggested that the flexor hallucis longus tendon tethers or entraps scar tissue or fracture sites. Improvement with conservative treatment is difficult once the deformity has already become entrenched, and surgical management is usually required in severe cases. Various surgical options are available for the correction of che

OBJECTIVE: To determine if the thread release technique can be applied to common peroneal nerve entrapment at the fibular neck.
METHODS: The thread common peroneal nerve release was performed on 15 fresh frozen cadaveric lower extremity specimens. All procedures were performed under ultrasound guidance and immediately underwent post-procedural gross anatomic inspection for completeness of decompression and presence or absence of iatrogenic neurovascular injury.
RESULTS: All 15 specimens demonstrated complete transection of the deep fascia of the peroneus longus overlying the common peroneal

Ultrasonography (US) of the elbow is an increasingly utilized modality for a variety of diagnoses. US is advantageous in many cases because of the ability to perform a dynamic examination while obtaining patient feedback. Furthermore, US is cost effective, widely available, and beautifully demonstrates superficial soft tissue structures. Finally, US is an excellent option for patients whose studies are degraded by motion artifact or those with claustrophobia concerns. The most common pathologies about the elbow are discussed in this article, including partial- and full-thickness tears of the b

Two main groups of soft-tissue disorders are identified: specific soft tissue syndromes and a non-specific disorder as yet not fully characterized. Specific soft tissue syndromes occur in joint, muscle or nerve and are associated with characteristic symptoms and physical signs. These include epicondylitis at the elbow, tendon disorders at the wrist and nerve entrapments such as carpal tunnel syndrome. The non-specific soft tissue syndrome is primarily described by symptoms of pain but may include muscular symptoms (such as weakness and cramp) or nerve symptoms (such as numbness, pins and needl

In idiopathic carpal tunnel syndrome (CTS), fibrosis and thickening of the subsynovial connective tissue (SSCT) increase pressure within the carpal tunnel, resulting in median nerve entrapment. Mitochondrial dysfunction in tissues reportedly leads to senescent cell accumulation and various diseases through reduced adenosine triphosphate (ATP) and excessive reactive oxygen species (ROS) production; however, no reports have linked this to CTS. Therefore, this study aimed to evaluate mitochondrial function in SSCTs of patients with CTS. This study investigated SSCTs obtained during carpal tunnel

OBJECTIVES: Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy. Anatomic variations of the median nerve (MN) and carpal tunnel (CT) may predispose patients to CTS or increase procedural complexity and risk during carpal tunnel release (CTR). This study aimed to systematically evaluate the prevalence of 25 MN and CT anatomic variations and measurements using ultrasound (US) in patients referred for electrodiagnostic testing (EDX).
METHODS: In this cross-sectional observational study, patients underwent diagnostic US exams of the MN (mid-brachium to distal CT), foc

Heel pain is a common presenting symptom in ambulatory clinics. There are many causes, but a mechanical etiology is most common. Location of pain can be a guide to the proper diagnosis. The most common diagnosis is plantar fasciitis, a condition that leads to medial plantar heel pain, especially with the first weight-bearing steps in the morning and after long periods of rest. Other causes of plantar heel pain include calcaneal stress fracture (progressively worsening pain following an increase in activity level or change to a harder walking surface), nerve entrapment (pain accompanied by burn

BACKGROUND AND OBJECTIVE: In a preliminary consensus document the authors proposed criteria and methods to identify upper limb work-related MSDs due to biomechanical overload. With this document they intend to define severity according to shared models and procedures so as to fit behaviours to diagnostic procedures and their medical-legal assessment. This becomes especially important in view of Ministerial Decree of April 27 2004 fixing the new lists of diseases having a possible work-related origin that must be reported by law in accordance with art. 139 of law n. 1124, and also on account of

OBJECTIVE: In nonoperated patients, the MR diagnosis of carpal tunnel syndrome (CTS) is difficult. In the postoperative patient this difficulty is compounded. Consequently, we sought to evaluate for potential MR signs of postoperative CTS.
METHODS: At 1.5 T, 41 wrists in 37 patients with previous CTS release were evaluated by two observers for 1) flexor retinacular regrowth; 2) median nerve: a) high T2 signal, b) proximal enlargement, c) fibrous fixation, d) neuroma, and e) entrapment; 3) flexor tenosynovitis; 4) mass, bursitis, accessory muscle, distal belly progression, or excessive deep fa

INTRODUCTION: Sportsman (sports) hernia is a medially located bulge in the posterior wall of the inguinal canal that is common in football players. About 90% of cases occur in males. The injury is also found in the general population.
CLINICAL PRESENTATION: The presenting symptom is chronic groin pain which develops during exercise, aggravated by sudden movements, accompanied by subtle physical examination findings and a medial inguinal bulge on ultrasound. Pain persists after a game, abates during a period of lay-off, but returns on the resumption of sport. Frequently, sports hernia is one c

Neuropathy in rheumatoid arthritis (RA) may result secondary to entrapment, vasculitis, and drug toxicity. We aimed to study clinical and electrophysiological neuropathy and pathological changes in sural nerve in patients with RA. One hundred eight patients of RA, fulfilling American College of Rheumatology 1987 criteria (mean age, 45.83 years; M/F 1:3, 80.3% seropositive) were examined clinically and electrophysiologically for evidence of peripheral neuropathy. Sural nerve biopsies were performed in the involved cases. In all RA patient medications, disease activity, results of blood tests, a

BACKGROUND: The lateral antebrachial cutaneous nerve (LACN) is the terminal sensory branch of the musculocutaneous nerve and is rarely entrapped or injured. This study describes the electrodiagnostic (EDX) findings and etiologies of LACN neuropathy.
METHODS: This is a review of 49 patients with pain and/or paresthesia of the forearm who underwent EDX studies. The diagnosis of LACN neuropathy was based on clinical and sensory conduction abnormalities.
RESULTS: The most common etiology of LACN neuropathy was iatrogenic injury in 30 (61.2%) patients, primarily due to biceps tendon repair at the

BACKGROUND: Although carpal tunnel syndrome is diagnosed primarily on clinical grounds, a number of investigations can be helpful in confirming the diagnosis and providing therapeutic orientation. Electrodiagnostic testing is the most widely used method in everyday practice but can be inconclusive or inconsistent with the clinical findings. Magnetic resonance imaging is useful in such cases.
OBJECTIVES: To compare the diagnostic usefulness of electrodiagnostic testing and magnetic resonance imaging in patients with carpal tunnel syndrome.
PATIENTS AND METHODS: Thirty-three cases of clinicall

Os cuneo-I metatarsale-I dorsale is an exceptionally rare accessory bone located on the dorsal aspect of the medial cuneiform. This report describes a case of symptomatic bilateral os cuneo-I metatarsale-I dorsale in a 9-year-old boy who was a competitive kendo practitioner. Longitudinal imaging revealed development of a superficial subchondral irregularity into a distinct ossified fragment over a 1-year interval, providing novel evidence that an ossification disorder contributes to the etiology of os cuneo-I metatarsale-I dorsale. Unlike the cutaneous nerve entrapment typically associated wit

Bilateral variation of the semitendinosus (ST) and the long head of the biceps femoris (BFlh) muscles is an anatomically rare occurrence. In this case report, we describe a male cadaver exhibiting bilateral fusion of the ST and BFlh muscles at their proximal origin on the ischial tuberosity, with the muscles separating distally into their respective muscle bellies. As far as we are aware, this bilateral fusion has not been previously documented in the literature; the only known report of fusion between the ST and BFlh muscles describes a unilateral fusion in a female cadaver, which was associa
