Assessment of the Forearm Tendon Transfer with Irreparable Radial Nerve Injuries Caused by War Projectiles.

INTRODUCTION: Injuries to the radial nerve can occur at any point along its anatomical route, and the etiology quite varies. A particular entity are war injuries of the extremities, which have high morbidity but low mortality. After irreparable radial nerve injury, the only treatment is tendon transfer (if we neglect arthrodesis) with over then 40 methods. Four tendon transfers are considered as better than the other and two of them are the subject of our article flexor carpi radialis (FCR) and flexor carpi ulnaris (FCU). AIM: To evaluate the ultimate functional results of forearm tendon tran

Where do injectable stem cell treatments apply in treatment of muscle, tendon, and ligament injuries?

Treatment options for muscle, tendon, and ligament injuries span a constantly evolving spectrum. For years, treatments focused on symptomatic relief. Closer scrutiny of symptomatic treatment suggests that the provision of transient relief of symptoms may have caused more harm than good. Cortisone injections provide a trade-off of short-term relief for poorer long-term outcomes. When conventional treatment failed, patients have faced limited options including surgery, which has increased risk and limited efficacy. Regenerative injections offer a more robust option for soft tissue disease. Basic

Outcome after tendon transfers to restore wrist extension in children with brachial plexus birth injuries.

Children with brachial plexus birth injuries often require tendon transfer to restore active wrist extension and maximize hand function. The purpose of this study is to assess the clinical results in children with brachial plexus birth injuries after tendon transfer to reconstruct active wrist extension. Over a 10-year period, 21 children (11 male, 10 female) underwent tendon transfer to reconstruct active wrist extension by a single surgeon. Eight patients had C5/C6/C7 injury and 13 patients had global palsy (C5-T1). The average age at surgery was 5.5 years (range, 3 to 8 y). Restoration of w

Effects of a serotonin S2-receptor blocker on healing of acute and chronic tendon injuries.

The beneficial effects of serotonin S(2)-receptor blockers on healing skin and muscle ulcers and refractory lesions such as leprosy and diabetic and ischemic ulcers have been reported previously, but their mechanisms of actions are not clear. The present study sought to elucidate the action of an S(2)-receptor blocker, metrenperone, on the healing of collagenase-induced injuries in superficial digital flexor tendons of two groups of rabbits. In one group, oral and topical therapy for 28 days with metrenperone, was started within 48 hr of a single acute injury. The animals were then left untrea

[Primary sonographic diagnosis and follow-up of muscular and tendon injuries of the lower extremity].

Ultrasound examination has become an important diagnostic method in muscle and tendon injuries. In addition dynamic ultrasound examination allows evaluation of the course of healing from various aspects. Starting with the sonoanatomy, the pathologic ultrasound patterns typical for injuries to muscle and to Achilles, patellar and quadriceps tendons are demonstrated. As complement to clinical examination, ultrasonography enables appropriate adaptation of the therapy concept to the course of healing. In the hands of experienced operators, ultrasound examination has reached a high level of sensiti

[Interdisciplinary experimental studies on the biomechanics of extensor and flexor tendon injuries of the hand].

The interdisciplinary experimental computer test for determination of the biomechanics of extensor and flexor tendon injuries at the distal phalanx of the fingers were performed with the aid of the finite element method. By input of the extreme boundary values (stress, biomechanic system, deformation and movement behavior, material coefficent distribution), it was possible to simulate clinically occurring injuries. Pathogenetically crucial for the type of injury are chance factors, which lead to a particular constellation of static or dynamic stress conditions. Accordingly, maximal stress exce

Selective FDP repair in zone 2B flexor tendon injuries: a better outcome by doing less?

PURPOSE: Zone 2B flexor tendon injuries present unique surgical challenges, with ongoing debate regarding optimal management of concurrent flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) ruptures. This study compared functional outcomes between isolated FDP repair versus combined FDS-FDP repair in Zone 2B injuries.. METHODS: We retrospectively analyzed 69 patients (97 fingers) with complete Zone 2B flexor tendon injuries between January 2017 and December 2020. Group 1 underwent isolated FDP repair (35 patients, 49 fingers) while Group 2 received combined FDS-FDP repa

Achilles Tendon Injuries Requiring Surgical Treatment in the Pediatric and Adolescent Population: A Case Series.

Pediatric Achilles tendon injuries requiring surgical treatment are considered rare and have not been well described. A retrospective chart review was conducted from 2010 to 2020 to identify cases of acute Achilles tendon rupture or laceration that required surgical repair in individuals 19 years or younger. A total of 24 individuals with acute Achilles tendon ruptures (n = 8) and lacerations (n = 16) were identified. All spontaneous ruptures occurred in skeletally mature individuals during sports. One subject was on minocycline at the time of injury, while two had a body mass index (BMI) ≥ 99

Combined nerve, vessel, and tendon injuries of the volar wrist: Multidisciplinary treatment and functional outcomes.

The superficial location of critical structures, including tendons, nerves and vessels, in the volar surface of the wrist makes them vulnerable to penetrating trauma. Extensive injuries to these structures are described as "spaghetti wrist". The main objective of this study was to report functional outcome in spaghetti-wrist injuries. The records of patients presenting to our clinic with extensive volar wrist injuries between January 2016 and January 2019 were reviewed retrospectively. Age, gender, comorbidities, date of injury, injury mechanism, affected hand and transected structures were no

Suture and Splint Compared With K-Wire Fixation for Open Zone 1 Extensor Tendon Injuries.

Extensor tendon injuries are classified as per the zone of injury. Zone 1 injuries disrupt the lateral bands with resultant inability to extend the distal interphalangeal joint. Open Zone 1 injuries have many well-described treatment options. To date, none of these have been compared directly. Our aim was to compare the benefit of additional Kirschner-wire fixation with suture repair and splinting of open Zone 1 extensor tendon injuries. We performed a retrospective cohort analysis comparing 2 different surgical procedures, "Suture and Splint" versus "Suture, Splint, and Kirschner wire." The 2

Utility of early active motion for flexor tendon repair with concomitant injuries: A multivariate analysis.

INTRODUCTION: Flexor tendon injury often occurs with concomitant injuries such as fracture, vascular injury, and extensor tendon injury. These injuries are repaired independently, without a comprehensive strategy. We aimed to identify the effect of concomitant injuries and treatment choice on the outcome of flexor tendon repair. PATIENTS AND METHODS: We evaluated 118 fingers of 102 adult patients with zone 1-3 flexor digitorum profundus (FDP) tendon injuries who underwent primary surgery at our hospital between April 2009 and December 2017. Cricket fast bowling workload patterns as risk factors for tendon, muscle, bone and joint injuries.

OBJECTIVE: To assess workload-related risk factors for injuries to particular tissue types in cricket fast bowlers. DESIGN: 235 fast bowlers who bowled in 14600 player innings over a period of 15 years were followed in a prospective cohort risk factor study to compare overs bowled in each match (including preceding workload patterns) and injury risk in the 3-4 weeks subsequent to the match. Injuries were categorised according to the affected tissue type as either: bone stress, tendon injuries, muscle strain or joint injuries. Workload risk factors were examined using binomial logistic regress

A new technique for repair of acute or chronic extensor tendon injuries in zone 1.

We describe a new surgical technique for the treatment of lacerations of the extensor tendon in zone I, which involves a tenodesis using a length of palmaris longus tendon one-quarter of its width. After exposing the dorsal aspect of the distal interphalangeal joint and harvesting the tendon, a 1.5 mm drill bit is passed through the insertion of the extensor tendon into the distal phalanx where it penetrates through the skin of the pulp of the digit. The palmaris longus tendon is threaded through the drill hole from dorsal to ventral and the ventral end is tied in a simple knot and trimmed. Th

Delayed repair of tendon to bone injuries leads to decreased biomechanical properties and bone loss.

INTRODUCTION: Repair of the torn rotator cuff tendon is a common procedure performed in the shoulder. In the clinical setting, a significant delay between rotator cuff tear and subsequent repair often exists. The purpose of this study was to investigate the biomechanical properties and bone density of the tendon to bone repair site after acute and delayed repair. METHODS: The supraspinatus tendons in bilateral shoulders of 60 rats were transected from the bone. In the acute group, the tendons were immediately repaired with suture. In the delayed group, the tendons were allowed to retract and

[Surgical results of various injuries related to central slip in extensor tendon of the hand].

In this article the results of surgical repair in 41 cases with different degrees of the central tendon band injuries of hand were reported and the results were compared for different kinds of operation. In this series all patients with degree I injury (3 cases) achieved excellent result by direct repairing disrupted tendons. 25 cases with degree II injury were repaired using three different procedures, among which the method of splitting lateral bands with cross suture yielded better result. However, only 7 patients (47%) reached excellent and good result, The reason for that is due to attent
