Spectrum of disease due to Branhamella catarrhalis in children with particular reference to acute otitis media.

For many years Branhamella catarrhalis was regarded as a non-pathogenic inhabitant of the respiratory tract. This article outlines the spectrum of B. catarrhalis disease in childhood and the extent of the evidence for a pathogenic role of the organism. B. catarrhalis is a rare etiologic agent in septicemia, meningitis, and other systemic illness in both apparently normal and immunocompromised infants and children. It is an unusual cause of ophthalmia neonatorum, but can be confused with Neisseria gonorrhoeae. Whether or not B. catarrhalis is acquired from the birth canal in these cases has not

Pulmonary lesions in disseminated visceral coccidiosis of sandhill and whooping cranes.

Fifty cranes, consisting of 46 sandhill (Grus canadensis) and four whooping cranes (Grus americana), were studied. Eighteen sandhill cranes and the four whooping cranes were naturally infected with disseminated visceral coccidiosis (DVC). The remaining sandhill cranes were chicks experimentally infected with oocysts of Eimeria reichenowi and/or E. gruis; five chicks served as controls. There were no clinical signs attributed to respiratory infection. Necropsy of naturally infected adult birds revealed nodules in many organs, including the lung, air sacs, trachea and nares. Artificially infecte

Clinical aspects on bacterial infections in the upper respiratory tract.

Acute respiratory tract infections represent the major cause of morbidity in younger age groups. Most of these infections involve the upper respiratory tract. The frequency of respiratory tract infections vary not only with age, but also with season of the year and the epidemiological situation. Surveys of the incidence and aetiology of these infections must therefore cover large populations during relatively long periods of time. In the developed countries, the mortality in respiratory tract infections in patients below the age of 60 years is low, while it increases markedly in elderly patien

Tracheobronchitis in Noncritically Ill Children With Tracheostomy: Can Quantitative Tracheal Cultures Assist in the Management?

BACKGROUND AND OBJECTIVES: Bacteria in tracheal aspirate samples from children with tracheostomy can indicate infection or colonization. Our study aimed to determine whether bacterial counts > 104 or > 105 CFU (colony forming units)/mL are more frequently associated with tracheobronchitis. Additionally, we aimed to examine the association between bacterial count and variables distinguishing colonization from infection in tracheobronchitis, along with clinical severity indicators. METHODS: Retrospective cohort study of children with tracheostomy at La Paz University Hospital, Madrid (June 2018

CO2 Narcosis in a Patient with Histopathological Features Suggestive of Aspergillus Tracheobronchitis during Treatment for Malignant Lymphoma.

Aspergillus tracheobronchitis (ATB) is a rare but severe infection in immunocompromised patients that is distinct from invasive pulmonary aspergillosis. A 50-year-old man undergoing dexamethasone, methotrexate, ifosfamide, L-asparaginase, and etoposide chemotherapy for NK/T-cell lymphoma developed febrile neutropenia and worsening respiratory failure. Computed tomography revealed airway epithelial detachment and bronchoscopy confirmed pseudomembrane formation. Due to his "Do Not Intubate" status, mechanical ventilation was not initiated, and he died of CO2 narcosis. Autopsy histopathologically

Bacterial Laryngotracheitis and Associated Upper Airway Obstruction: A Case Series.

BACKGROUND: Iatrogenic laryngotracheal stenosis (LTS) continues to be a known complication of indwelling endotracheal tubes (ETTs). It is well established that secondary scar formation caused by inflammation and mucosal injury are the main mechanisms by which stenosis occurs. Additionally, there are reports of bacterial colonization of ETTs and its potential association with tracheal scar formation. We describe 4 cases of patients with history of intubation and/or tracheostomy and presumed LTS that improved with the management of concurrent bacterial laryngotracheitis. METHODS: A retrospectiv

Airway and parenchymal manifestations of pulmonary aspergillosis.

Pulmonary aspergillosis encompasses a heterogeneous group of mycoses that result from either colonisation or pathogenic damage of lung tissue by Aspergillus fungi. These clinical entities range from relatively benign saprophytic hypersensitivity associated with fungal inhabitation to life threatening invasive disease. The diagnosis of pulmonary disorders related to Aspergillus is on the increase and it is more important than ever those both general and respiratory physicians have a good understanding of these disorders. This paper reviews the contemporary understanding of the clinical, radiogr

Differentiation of epiglottitis from laryngotracheitis in the child with stridor.

To identify which clinical findings serve to differentiate acute epiglottitis from laryngotracheitis and also to evaluate the role of direct inspection of the epiglottis in the evaluation of children initially thought to have laryngotracheitis, we prospectively evaluated 155 children presenting to the emergency room with acute stridor. Three of the findings on physical examination were associated with epiglottitis: absence of spontaneous cough, drooling, and agitation. The diagnosis assigned prior to inspection of the epiglottis was incorrect in two of six patients with epiglottitis and in thr

Adverse effects of racemic epinephrine in epiglottitis.

Acute epiglottitis, a potentially fatal disease, may be difficult to diagnose if the presentation is atypical. We present three such patients who were initially diagnosed as acute laryngotracheitis. They deteriorated rapidly after administration of racemic epinephrine via nebulizer. We caution that: diagnosis of acute epiglottitis should be considered in similar patients; and appropriate precautions must be taken prior to visualization of the airway.

Tracheobronchitis caused by methicillin-resistant Staphylococcus aureus as a cause of chronic wheezing in a non-ventilated adult patient with tracheobronchomalacia.

Bacterial tracheobronchitis is an extremely rare entity, which was long considered to be a pediatric disease. We report the case of a 65-year-old woman who presented with persistent wheezing, worsening productive cough and sore throat. Computed tomography of the chest revealed the presence of tracheomalacia, confirmed at bronchoscopy. The presence of purulent exudate, coating the trachea and main bronchi, was consistent with bacterial tracheobronchitis. Culture of the tracheal aspirates grew methicillin-resistant Staphylococcus aureus (MRSA). As the patient was afebrile and not systemically il

Aspergillus niger causing tracheobronchitis and invasive pulmonary aspergillosis in a lung transplant recipient: case report.

A case of invasive aspergillosis caused by Aspergillus niger in a lung transplant recipient is described. The patient presented hyperglycemia starting postoperatively, with other complications such as cytomegalovirus infection. The associated predisposing factors and other implications are discussed. Aspergillus niger seems to be a fungal species of low virulence that requires the presence of a severely immunosuppressed host to cause invasive disease.

Sudden death in a toddler with laryngotracheitis caused by human parainfluenza virus-1.

Laryngotracheitis caused by human parainfluenza virus (HPIV) and not complicated by bacterial superinfection rarely causes sudden unexpected death in infants and toddlers, especially in the absence of stridor and a barking cough. We therefore describe a 15-month-old white male who died suddenly and unexpectedly with clinical and pathological features of laryngotracheitis caused by culture-proven HPIV-1 infection. Given the presence of mucosal inflammation extending into the vocalis muscle of the larynx without associated significant narrowing of the laryngotracheal airway lumen, we propose his

Tracheostomy in the management of laryngotracheobronchitis. Red Cross War Memorial Children's Hospital experience, 1980-1985.

In the 6-year period 1980-1985 162 children required tracheostomy during treatment of laryngotracheobronchitis at Red Cross War Memorial Children's Hospital, Cape Town. This represents 4.6% of 3,500 children with this disorder and 28% of those requiring airway intervention. Fifty-eight percent of the children were decannulated within 4 weeks and 75% within 10 weeks. Fifty-four per cent of the children required one or more further procedures before decannulation, including 7 children who required a laryngotracheoplasty. Obstructing stomal granulation tissue had to be removed from 24 children an

An inactivated influenza D virus vaccine partially protects cattle from respiratory disease caused by homologous challenge.

Originally isolated from swine, the proposed influenza D virus has since been shown to be common in cattle. Inoculation of IDV to naïve calves resulted in mild respiratory disease histologically characterized by tracheitis. As several studies have associated the presence of IDV with acute bovine respiratory disease (BRD), we sought to investigate the efficacy of an inactivated IDV vaccine. Vaccinated calves seroconverted with hemagglutination inhibition titers 137-169 following two doses. Non-vaccinated calves challenged with a homologous virus exhibited signs of mild respiratory disease from
