OBJECTIVES: To develop the first consensus to standardize the management of patients with Anorectal Malformations (ARMs) transitioning from childhood to adulthood.
METHODS: A dedicated task force of experts performed an extensive literature review and multiple meetings to define the most important aspects of transition of care. The findings were discussed with all ARM-net consortium members and a set of practical recommendations agreed upon at the annual meeting in 2016.
RESULT: We defined seven domains that are essential to provide an effective and practical transition process. Within each

BACKGROUND: There is evidence that the process of transition from paediatric (child) to adult health services is often associated with deterioration in the health of adolescents with chronic conditions.Transitional care is the term used to describe services that seek to bridge this care gap. It has been defined as 'the purposeful, planned movement of adolescents and young adults with chronic physical and medical conditions from child-centred to adult-oriented health care systems'. In order to develop appropriate services for adolescents, evidence of what works and what factors act as barriers …

An essential component of the learning process for nursing and other disciplines in health professions education is the structuring of successful clinical experiences for beginning students. The purpose of this research study was to explore and describe first patient care experiences of baccalaureate nursing students in order to better understand student perspectives and perceptions. Data were collected from three cohorts of baccalaureate nursing in two university settings in the first semester following completion of first patient care experiences. A basic, exploratory qualitative approach wi

Increasing numbers of children with congenital and chronic diseases are surviving beyond adolescence. Consequently, arrangements must be put in place to ensure that these groups of patients continue to receive high-quality care into adulthood. Although some medical disciplines have reacted positively to this growing challenge, perioperative care has lagged behind. This review explores some of the difficulties in delivering optimal perioperative care in the transitional period. Key issues, including the paucity of experience in the management of pediatric diseases among adult-trained clinicians

The clinical features, course and outcome of mood disorders in childhood and adolescence are discussed. The most specific symptoms of major depressive disorder in childhood and adolescence are irritability, social withdrawal, and comorbidity. Depression is a chronic and recurrent condition. The course of child onset depression resembles those with conduct disorder much more than those with mood disorders, without an increased risk for recurrence of depression into adult life. On the other hand, adolescent onset depression has a high risk of recurrent mood disorder into adulthood.

The increasing survival of children and young people with congenital disabilities such as spina bifida (SB) provides a challenge to health care systems globally about how best to respond to the multitude of health, developmental, and psychosocial needs of those affected by this complex disorder across the lifespan, not just in childhood and adolescence. The goal of transition to adult health care is to maximize lifelong functioning through the provision of quality, developmentally appropriate health care that continues uninterrupted as the individual moves from adolescence to adulthood. As sex reassignment surgeries become more common and advanced, health professionals are more likely to see patients with gender identity disorders (GID) in their clinics. This can be challenging in many ways, and the challenges continue even after gender reassignment surgery as each case may present with unique anatomy. This article reviews the definition and treatment of GID, service provision in National Health Service and post-transition care of such patients.

Adolescence is a time of major physical and psychological change. Young teenagers need to become comfortable with their developing sexuality, to begin moving towards independence with a greater reliance on peer group acceptance, to face the future and to make important decisions about their education, career and goals. Difficulties inherent in all of these steps to adult life are magnified by chronic ill health. It is harder for adolescent patients to break family ties, harder to feel accepted by their peer group, harder to plan realistically for the future. It must be the overriding aim of th

PURPOSE: This study aimed to calculate geospatial accessibility to optometrist and ophthalmologist using public transportation in Virginia, and to measure the sociodemographic factors associated with accessibility.
METHODS: We evaluated one-way public transit travel time and population-weighted accessibility score, defined as the number of eye care centers (optometry and ophthalmology) reachable within a one-hour walk or a two-hour transit commute, across seven regions in Virginia. Both objective and perceived ("feels-like") accessibility measures were calculated. Mann-Whitney U tests were us

Older people are at increased risk of medication-related problems, which may be identified by clinical pharmacists during hospital medication reviews. Some problems are better managed in primary care, where follow-up is possible. Medication-related communication by pharmacists may improve information transfer in care transitions, but this practice remains uncommon in Sweden. This qualitative study explored clinical pharmacists' experiences and perceptions of writing referrals to primary care. Data were collected through four focus groups and three supplementary interviews with 19 clinical phar

BACKGROUND: Advances in medical care have dramatically improved survival rates for children and youth with special health care needs (SHCN), with more than 90% now reaching adulthood. As a result, pediatric-to-adult health care transition has become a critical component of lifespan care. Despite national consensus recommendations, fewer than one in four adolescents receive the transition services needed to prepare them for adult-oriented care. Standardized, validated instruments to assess and promote transition readiness are essential to close this gap.
OBJECTIVE: This review describes the pr

BACKGROUND: Rheumatic and musculoskeletal disorders (RMDs) of childhood onset are chronic conditions that often persist into adulthood, necessitating a structured transition from Pediatric to adult healthcare. In Saudi Arabia, no standardized protocol exists, leading to inconsistencies and care gaps for adolescents with RMDs. Saudi health transformation goals include improving overall health, enhancing the quality and accessibility of healthcare services, and increasing value by improving outcomes and controlling costs.

Hypertensive disorders of pregnancy (HDP) increase the lifetime risk of cardiovascular disease (CVD), and national guidelines recommend primary care provider (PCP) transition in the first year postpartum to offer preventive measures. However, only 15-40% of individuals with HDP see a PCP, and the quality of this transition remains unknown. This secondary analysis of a randomized controlled trial assesses whether postpartum preventive care for individuals with HDP followed guideline-concordant recommendations. Only 44% of primary care or continuity visits in the first six months postpartum touc

The transition of care from paediatric to adult healthcare services is a vulnerable period for patients with inborn errors of immunity (IEI), a heterogeneous group of primary immunodeficiency disorders characterized by chronic immune dysfunction and significant morbidity. With advances in diagnosis and treatment, an increasing number of patients with IEI are now reaching adulthood, making the management of this transition a growing clinical priority. Inadequate transition is associated with medical complications, treatment non-adherence, discontinuity of care, increased healthcare utilization,

BACKGROUND: Ustekinumab, approved for the treatment of moderate to severe Crohn disease (CD) and ulcerative colitis (UC), requires a clinic-administered intravenous (IV) induction infusion (loading dose) followed by transition to self-administered, subcutaneous (SC) injection for maintenance every 8 weeks. Many patients need subsequent dose escalations to obtain or maintain effectiveness. As an increasing number of CD and UC therapies require a similar dosing and escalation strategy, research evaluating care coordination requirements and clinical outcomes for patients prescribed ustekinumab ca

BACKGROUND: Adolescents and young adults (AYA) with rare forms of diabetes - including Wolfram syndrome (WS), Alström syndrome (AS), Bardet-Biedl syndrome (BBS), and maturity-onset diabetes of the young (MODY) - face unique challenges during the transition to adult care. These challenges are intensified by multisystem endocrine involvement, neurocognitive and sensory impairments, and limited adult provider expertise.
OBJECTIVE: This narrative review describes transition-specific barriers in rare diabetes syndromes, explores current initiatives, and proposes recommendations for care models and

PURPOSE OF REVIEW: Pediatric healthcare providers have increasingly become aware of the need for timely and informative transition of adolescents and young adults with chronic medical conditions such as diabetes and cystic fibrosis. However, there is paucity of published data on the importance of and most effective way to transition youth with lipid disorders who are at increased risk of premature cardiovascular disease.
RECENT FINDINGS: Evidence shows that atherosclerosis begins at a young age. However, there are no guidelines on the transition of adolescents and young adults with dyslipidem

INTRODUCTION: Turner Syndrome is a rare condition secondary to a complete or partial loss of one X chromosome, leading to a wide spectrum of clinical manifestations. Short stature, gonadal dysgenesis, cardiovascular malformations, and dysmorphic features characterize its common clinical picture.
AREAS COVERED: The main endocrine challenges in adolescent girls with Turner Syndrome are puberty induction (closely intertwined with growth) and fertility preservation. We discuss the most important clinical aspects that should be faced when planning an appropriate and seamless transition for girls w
