Characteristics of traumatic brain injuries sustained among veterans seeking homeless services.

This hypothesis-generating research describes the characteristics of traumatic brain injuries (TBIs) sustained among 229 Veterans seeking homeless services. Nearly all participants (83%) had sustained at least one TBI prior to their first episode of homelessness. Among participants with a TBI, assaults, transportation-related accidents, and falls were the most common causes of these injuries. Thirty percent of individuals sustained injuries with severity levels that would be expected to be associated with ongoing TBI-related deficits. Forty-three percent of the Veterans sustained at least one

Changes in cerebral compartmental compliances during mild hypocapnia in patients with traumatic brain injury.

The benefit of induced hyperventilation for intracranial pressure (ICP) control after severe traumatic brain injury (TBI) is controversial. In this study, we investigated the impact of early and sustained hyperventilation on compliances of the cerebral arteries and of the cerebrospinal (CSF) compartment during mild hyperventilation in severe TBI patients. We included 27 severe TBI patients (mean 39.5 ± 3.4 years, 6 women) in whom an increase in ventilation (20% increase in respiratory minute volume) was performed during 50 min as part of a standard clinical CO(2) reactivity test. Using a new m

The classification systems of stress fractures: a systematic review.

BACKGROUND: Stress fractures of the upper and lower extremity are troublesome overuse injuries in athletes and nonathletes alike. These injuries have a broad spectrum of severity and prognosis. We performed a systematic search of the literature, which revealed multiple classification systems; however, we did not uncover a general system that offered both validated radiographic and clinical parameters. METHODS: A literature search was conducted using Ovid/Medline, Embase, and the Cochrane Library, with publication dates ranging from 1960 to December 2009. Inclusion criteria included all studie

Injuries among male and female elite football players.

All 12 female football clubs (228 players) and 11 of 14 male clubs (239 players) in the Swedish premier league were followed prospectively during the 2005 season. Individual exposure (playing time), injuries (time loss), and injury severity (days lost due to injury) were recorded by the team medical staffs. Injury incidence was higher for male players during both training (4.7 vs 3.8 injuries/1000 h, P=0.018) and match play (28.1 vs 16.1, P<0.001). However, no difference was found in the incidence of severe injury (absence >4 weeks) (0.7/1000 h in both groups). The thigh, especially the hamstr

Massive internal injury in the absence of significant external injury after collisions of passenger vehicles with much larger vehicles.

We have encountered cases of motor vehicle collisions in which there was relatively little external trauma, but there were massive internal injuries that were much more extensive than might be expected from the external examination. Two cases were collisions between trains and pickup trucks, the third a collision between a semi trailer and a van. In all three cases, the external examination showed minor abrasions and lacerations. Internally, however, there were massive injuries which were fatal. While the significant injuries in our cases were not surprising given the force of the collisions i

Antioxidant micronutrients in major trauma and burns: evidence and practice.

There has been a growing interest in micronutrients as a result of their essential role in endogenous antioxidant defense mechanisms and immunity. Critically ill burn and trauma patients are characterized by an increased free radical production, which is proportional to the severity of the injury. In addition, they are at high risk of negative trace element balances, which contribute to the imbalance in endogenous antioxidant capacity and the extension of primary lesions. Although selenium, zinc, and vitamin C and E status are altered in all injured patients, patients with major burns are uniq

The effect of alcohol on incidence, pattern, severity and outcome from traumatic brain injury.

Alcohol consumption is known to be a major contributing factor for the occurrence of traumatic brain injuries (TBIs) in both developed and developing countries. It also influences diagnosis, management and recovery from TBIs, subsequent to injury occurrence. The present report examines the association of alcohol in injury occurrence, and its impact on severity and outcome from TBIs. Subjects were identified from 7 major hospitals in the city of Bangalore, India with data collection undertaken by standardised methods. Alcohol users (n = 243) and non-users (n = 1310) were compared on various cha

Prevalence of significant intraocular sequelae in blunt orbital trauma.

The purpose of this study was to describe the prevalence of significant intraocular sequelae (SIOS) and its correlation with the severity of blunt orbital trauma. Four hundred ten consecutive patients presenting to the ED who had sustained blunt orbital trauma were studied. The severity of orbital trauma was graded and SIOS was determined by the presence of an intraocular injury as listed in Table 2. The presence of SIOS was noted in 14 (41.2%) mild, 22 (59.5%) moderate, and 20 (29.4%) severe orbital trauma. In the severe group, the presence of SIOS was detected in 8 (23.5%) blowout fractures

Integrated treatment of the severely injured patient with coexistent comorbidities. A case report and literature review.

Major trauma represents a paradigmatic clinical condition, needing multidisciplinary and structured approach. When a patient is affected by significant comorbidities such as diabetes mellitus and other conditions that compromise the immune competence the risk of infections in the presence of exposed fractures is remarkably high. Although usually managed by multidisciplinary teams, these patients often have unfavorable functional outcomes. We describe here the case of a young diabetic and immunosuppressed man, treated for a destroying injury of the left lower limb, with more than 60 centimeters

A prospective study of injury and activity profile in elite soccer referees and assistant referees.

Injuries to soccer players have been extensively examined, but not the injury experience of referees and assistant referees. This study aimed to determine the injury incidence and activity profile of soccer match officials. A 12 month prospective cohort study was used to collect activity and injury data of 31 participants who reported their training and match exposure and their injury incidence by means of weekly online questionnaire. Study participants spent a mean of 2632 hrs training and 1704 hrs officiating over the 12 month study period. Thirty eight injuries were recorded, (8.8 injuries/

[Study of homeostasis in children with traumatic shock].

The time course of changes in oxygen and acid-base homeostatic parameters was studied in children with grades 1, 2, 3, and 4 traumatic shock. The directionality of changes was ascertained in oxygen and acid-base balance and the influence of hemodynamic, hemocoagulation, hemorheological, and energy-generating functions in the destabilization of oxygen and acid-base homeostatic parameters were established. A package of medical measures that promote improved oxygen and acid-base balance in children with traumatic shock is stated.

Ipsilateral fibular transfer: a valuable option for treatment of large tibial defects in children.

Large tibial defects pose a great therapeutical challenge. Defects longer than 5 cm should be treated using various techniques with almost similar final results. Besides the method of bone lengthening and free fibular flap, there is also the possibility of reconstruction with ipsilateral fibular transfer. This method, mainly for children and adolescents, is now still an alternative with only few complications which should not be abandoned. We present the case of a 6-year-old boy with floating knee and tibial fracture with excessive bone loss which was solved by ipsilateral fibular transfer as

Statistical analysis of fatal bleeding pelvic fracture patients with severe associated injuries.

BACKGROUND: Bleeding pelvic fracture patients with severe associated injuries have a high mortality rate that is exacerbated by several factors. To gain deeper etiological insights into this injury, we investigated the specific risk factors associated with the high mortality rate. METHODS: A total of 102 bleeding pelvic fracture patients with severe associated injuries (abbreviated injury score > or =3) were treated at our level I trauma center between January 1994 and December 2004. Predictors of death within 24 h of arrival were determined by univariate and multivariate analyses using anato

A study of age-related variables among physically abused children.

One of every five substantiated cases of child abuse and over 50 percent of child-abuse-related deaths are the result of physical injury. The charts of 266 children, newborn to seventeen years of age, seen at Texas Children's Hospital during a two-year period were reviewed. Each child was assigned to one of four age-groups. Approximately 90 percent of all cases involved children five years old or younger, with 60 percent being less than two years of age. The results suggest that age is an important factor in the location, type, and severity of injuries due to the physical abuse of a child.

Influencing factors on the injury severity of restrained front seat occupants in car-to-car head-on collisions.

The results of 319 cases of belt-restrained front seat car occupants (234 drivers and 85 passengers) from 241 vehicles in car-to-car head-on collisions were examined. Ninety-five occupants were uninjured, 195 sustained a total injury severity of Maximum Abbreviated Injury Score (MAIS) 1-3 and 29, MAIS 4-6. There were 27 fatalities, and the main causes of death were polytrauma and hemorrhage. The main factors influencing occupant injury severity were: the energy equivalent speed (EES); the change of velocity (delta upsilon); the maximum deformation depth; and the collision angle. With an EES >

Circulatory response of young adult trauma victims with alcohol intoxication.

Young adults are a subgroup particularly susceptible to traumatic injury; alcohol intoxication is a major risk factor for trauma in young adults. During the first hours following multiple trauma, however, little is known about the specific effects of alcohol intoxication on the circulatory response of critically injured young adults. The purpose of this prospective study was to compare the circulatory response of 5 severely injured young adults (15-40 years old) with acute alcohol intoxication to the circulatory response of 5 severely injured, nonintoxicated young adults. Data were collected e
