BACKGROUND: Tuberculosis remains one of the primary infectious diseases in Bangladesh that affects both adults and children. Poor diagnostic criteria and insufficient reporting, especially of pulmonary tuberculosis (PTB) and extrapulmonary tuberculosis (EPTB) in children, limit the prevalence estimation, resulting in insufficient preventive strategies. This systematic review and meta-analysis aim to estimate the pooled proportions of PTB and EPTB among pediatric and adult diagnosed tuberculosis cases in Bangladesh, and to summarize the associated factors.
METHODS: We searched PubMed, Scopus,

INTRODUCTION: Monitoring treatment response in extrapulmonary tuberculosis (EPTB) remains challenging due to limited microbiological accessibility and the lack of validated biomarkers. [18F]FDG positron emission tomography/computed tomography [(18F)FDG-PET/CT] enables assessment of metabolic disease activity and has been explored in infectious diseases, but evidence supporting its use in EPTB is limited.
METHODS: We conducted a prospective observational study including adults diagnosed with EPTB at a tertiary referral center in Denmark between 2016 and 2018. Participants underwent baseline an

BACKGROUND: Globally, tuberculosis (TB) poses a severe threat to human health. Prior to the COVID-19 pandemic, TB was the commonest infectious cause of mortality, surpassing HIV/AIDS. A strong index of suspicion is required for the diagnosis of extrapulmonary tuberculosis (EPTB) because of the paucibacillary nature of biological specimens. Primary care physicians, as the first point of contact for patients, are in an ideal position to detect TB early due to its multisystemic nature and varied presentation. This study aims to determine microbiological, demographic, clinical, and changing trends

Tuberculosis (TB) is one of the leading causes of mortality in humans with clinical manifestations as pulmonary and extrapulmonary forms. TB pleural effusion (PITB) is one of the main forms of extrapulmonary TB, which is challenging to diagnose due to the overlapping clinical symptoms with other non-TB pleural effusions and poses severe complications in the treatment and management of disease. New biomarkers are urgently needed to improve the diagnostic accuracy for PlTB. Here we applied high-throughput targeted liquid chromatography-mass spectrometry (LC-MS/MS) metabolomics using a 96-well pl

Genitourinary tuberculosis accounts for 8-15 % of cases of extrapulmonary tuberculosis, with the testis being an infrequent site of involvement, seen in 3 % of individuals with genital tuberculosis. Testicular tuberculosis often presents with symptoms that mimic those of other testicular conditions, including neoplasms, infections and testicular torsion. We report the case of a 51-year-old male with a history of tuberculosis 11 years prior, who presented with left testicular pain, swelling, and an ulcer. Initial ultrasound findings suggested were unclear and only the histopathological examinat

BACKGROUND: The clinical presentation of extrapulmonary tuberculosis (EPTB) is atypical and it is easily confused with other diseases such as common infections, making prompt diagnosis a great challenge. This study aimed to evaluate the accuracy of targeted nanopore sequencing (TNS) in the diagnosis of EPTB. The diagnostic accuracy of TNS using different types of extrapulmonary specimens was also evaluated.
METHODS: We reviewed the clinical data of patients with suspected EPTB for whom TNS was conducted and who were hospitalized at our center. The true positive, false positive, false negative

In the United States, rates of Mycobacterium tuberculosis infection have been declining for decades. Osteoarticular tuberculosis of the ankle is rarely observed. We present the case of a 65-year-old man who immigrated to the United States from India 24 years before the onset of symptoms. The patient initially reported atraumatic swelling and pain of the left ankle and foot and was treated for venous insufficiency. Later, the patient was referred to a nonsurgical orthopaedic clinic for additional workup and was found to have elevated inflammatory markers. MRI showed septic arthritis and osteomy

BACKGROUND: Conventional diagnostics for Osteoarticular Tuberculosis (OATB) are slow and prone to inaccuracy due to low bacterial load, delaying treatment. Xpert MTB/RIF is WHO-recommended for TB cases, but there is a dearth of literature for non-spinal OATB.
METHODS: In a 4-year study (Sept 2016-Feb 2020), non-spinal osteoarticular infections were retrospectively analyzed. Xpert MTB/RIF sensitivity and specificity were evaluated against tuberculosis defined by histology and culture against patients having culture-isolated pyogenic infections as a reference standard for negative tuberculosis.

Pediatric extrapulmonary tuberculosis (EPTB) is a diagnostic challenge. A new immunochemistry based MPT64 antigen detection test has shown improved sensitivity compared to current laboratory tests. The aim of this study was to implement and validate the test performance in a resource limited African setting. Presumptive pediatric (0-18 y) EPTB patients were prospectively enrolled at Mbeya Zonal Referral Hospital, and followed to the end of treatment or until a final diagnosis was reached. Specimens from suspected sites of infection were subject to routine diagnostics, GeneXpert MTB/RIF assay a

Bovine tuberculosis (bTB) caused by Mycobacterium bovis (M. bovis) represents one of major zoonotic diseases among cattle, it also affects the health of human, other domestic animals and wild life populations. Inhalation of infected aerosol droplets is considered as the most frequent route of the infection. This study aims to investigate the current forms of tuberculosis in cattle and identify the possible transmission modes in dairy farms of China. 13,345 cows from eight dairy farms in three provinces were comprehensively diagnosed by a multitude of assays, including SIT, CIT, IFN-γ assay and

Miliary tuberculosis results from the lymphohematogenous spread of the tubercle bacilli to the vascular beds in the lungs and other organs. Diagnosis is made by clinical judgment and chest X-ray showing miliary mottling of the lung fields. Another imaging study like computed tomography imaging of the lungs and abdomen can also be supportive in diagnosing miliary tuberculosis. We present a case of miliary tuberculosis in an immunocompetent young male with atypical manifestation of a left-sided pleural effusion and a life-threatening complication of acute respiratory distress syndrome during hos

The real time PCR Xpert ® MTB/RIF is fundamental for rapid diagnosis in paucibacillary respiratory samples and for the detection of multidrug-resistant TB cases. This paper aimed to determine its performance on different extrapulmonary samples. We determined sensitivity, specificity, positive and negative predictive value on respiratory and non-respiratory samples collected from January 2010 to June 2014. The protocol for the Xpert ® MTB/RIF PCR suggested by Cepheid was strictly followed for all specimens. In 12257 respiratory samples we observed a sensitivity of 87.1% and a specificity of 99.

It is often difficult to establish confirmatory diagnosis in cases of extrapulmonary tuberculosis (TB) because of its paucibacillary nature and difficulty in accessing the involved organs. In several cases, empirical anti-tubercular treatment is started, and the patient is followed-up closely for response. In countries with high prevalence of TB, it is a reasonably good strategy and works most of the times. However, catastrophe may occur when aggressive lymphomas masquerade as TB.

PURPOSE: Tuberculous paradoxical reactions (PR) have been seldom studied in non-immunocompromised patients. We conducted a study to describe the incidence, clinical and biological features, treatment and outcome of PR in human immunodeficiency virus (HIV)-negative patients treated for extrapulmonary tuberculosis (TB) and to identify predictive factors of PR.
METHODS: A single-center retrospective study was conducted in consecutive HIV-negative patients presenting with TB with at least one extrapulmonary manifestation who were hospitalized in an internal medicine department between 2000 and 20

BACKGROUND: The proportion of extrapulmonary tuberculosis (EPTB) reported in the United States has been gradually increasing. HIV infection and foreign birth are increasingly associated with tuberculosis and understanding their effect on the clinical presentation of tuberculosis is important.
METHODS: Case-control study of 6,124 persons with tuberculosis reported to the North Carolina Division of Public health from January 1, 1993 to December 31, 2006. Multivariate logistic regression was used to obtain adjusted odds ratios measuring the associations of foreign birth region and US born race/e

AIMS: Extrapulmonary tuberculosis (EPTB) still constitutes an important clinical problem. We aimed to evaluate the incidence and features of extrapulmonary tuberculosis.
MATERIALS AND METHODS: We retrospectively evaluated 14,266 tuberculosis patients diagnosed between January 1999 and December 2003 in a tertiary care hospital in Istanbul. As many as 2,435 patients (17.1%) with EPTB were evaluated for the incidence and features.
RESULTS: Of the 14,266 patients, 4,154 were female (29%) and 10,112 were male (71%) and were aged between 14 and 86 years with a mean age of 35 +/- 14 years. As many

STUDY OBJECTIVES: To characterize the incidence of extrapulmonary tuberculosis (EPTB) in Estonia, by site and age, in conditions of marked pulmonary tuberculosis (TB) increase, ie, during the period of fundamental societal changes, but where TB incidence has not been influenced by HIV infection.
DESIGN AND SETTING: A retrospective study.
PATIENTS: All new cases of EPTB (n = 622) detected in Estonia from 1991 to 2000.
RESULTS: The incidence of pulmonary TB in Estonia increased from 21.5 in 1991 to 44.6/100,000 in 2000. In contrast, the incidence of EPTB did not increase at the same pace, cha

BACKGROUND: Mycobacterium tuberculosis (MTb) infection remains the cause of higher morbidity and mortality than any other infectious disease in the world. Intact cellular immunity is necessary to resist the disease, and therefore the AIDS epidemic has greatly contributed to the resurgence of MTb. Depending on the degree of immunosuppression, the presentation of MTb in patients with AIDS can be atypical and difficult to diagnose as compared to the classical presentation of MTb in the nonimmunocompromised population.
CASE: A patient who was not known to be HIV positive had a clinical picture of

Among 777,512 Indian adults with extrapulmonary tuberculosis, in-treatment mortality varied over tenfold by site, highest for tuberculous meningitis (adjusted odds ratio 12.48 vs. lymph node TB), miliary (5.17), and pericardial TB (3.86). Associations of HIV, diabetes, alcohol, and tobacco use with mortality varied across sites. Risk-stratified, comorbidity-aware care is warranted.
