Impact of Urodynamic Parameters on Treatment Decision-Making in Men Under 50 With Treatment-Resistant Chronic Lower Urinary Tract Symptoms.

BACKGROUND: Lower urinary tract symptoms (LUTS) in young men are heterogeneous, and symptom-based management often fails to identify the underlying dysfunction, particularly in treatment-resistant cases. OBJECTIVE: To assess the impact of multichannel urodynamic studies (UDS) on clinical management decisions and identify urodynamic predictors of treatment modification in men under 50 years with chronic LUTS. METHODS: In this prospective cross-sectional study, men aged 18-49 years with LUTS persisting for ≥ 6 months despite prior guideline-concordant therapy underwent standardized multichanne

An Alternative Approach for Treating Female Underactive Bladders with Chronic Urine Retention: A Pilot Study on Combined Transvaginal Ultrasound-Guided Botulinum Toxin A External Sphincter Injection and Transurethral Incision of the Bladder Neck.

Background: Treating an underactive bladder (UAB) is challenging. Previously, we introduced a more precise method of transvaginal ultrasound-guided botulinum toxin A (BoNT-A) injection into the external urethral sphincter as a treatment option for patients with UABs. Although many patients experience good results, those with an UAB and excessive residual urine still require catheterization. Therefore, we developed a new method that combines transvaginal ultrasound-guided BoNT-A injection with a transurethral bladder neck incision. Methods: A prospective study was conducted on 16 patients who e

Urodynamic pattern distribution among aged male patients with lower urinary tract symptoms suggestive of bladder outlet obstruction.

OBJECTIVE: To develop a urodynamic study (UDS) pattern system for aged male patients who complained of non-neurogenic lower urinary tract symptoms (LUTS) to create a reference guideline for their diagnosis and treatment by a retrospective analysis. METHODS: A retrospective analysis of UDS data was carried out in 1984 male patients neurologically intact with symptoms suggestive of bladder outlet obstruction (BOO) aged older than 45 years (2002-2013). On the basis of their UDS characteristic findings, the patients were classified into 1 of 7 subgroups: equivocal or mild BOO with sphincter syner

TGF-β1 and connexin-43 expression in neurogenic bladder from rats with sacral spinal cord injury.

AIMS: Sacral spinal cord injury (SCI) could induce underactive bladder (UAB). Malfunction of connexin 43 (CX43) regulated by TGF-β1 might involve in urinary bladder dysfunction. We studied the changes of CX43 and TGF-β1/Smad3 signaling in detrusor of neurogenic bladder (NB) in sacral SCI rats. METHODS: Sacral SCI was produced by hemisection (SSCH) or transection (SSCT) of spinal cord between L4 and L5 in female Wistar rats. BBB scores, residual urine volume and bladder weight as well as characteristic cystometric parameters at 6th week were used to confirm the successful establishment of NB.

Underactive and overactive bladders are related to motor function and quality of life in Parkinson's disease.

AIM OF THE STUDY: Overactive bladder occurs in up to 70% and underactive bladder in up to 50% of patients with Parkinson's disease (PD), which is thought to reflect an altered frontal-basal ganglia circuit. However, it is not clear how bladder disorder is related to motor function and to quality of life (QOL) in PD. We studied these relationships by conducting multiple regression (MR) and bivariate correlation (BV) statistical analyses. METHODS: We recruited 58 PD patients, irrespective of the presence of bladder disorder: 35 men and 23 women of mean age 66.8 years, mean duration of the disea

Flexible ultrathin organic electrochemical transistor (OECT)-based portable system for monitoring and stimulation of bladder muscle.

Patients with bladder underactivity experience difficulty in sensing bladder fullness and emptying the bladder efficiently, leading to chronic urinary retention. An implantable bidirectional device that combines capturing bladder smooth muscle activities with stimulation could enable closed-loop assistance and offer benefits compared to the common timed intermittent catheterization. Most existing bidirectional systems lean on passive metal electrodes as the interface, which provide no intrinsic signal amplification for low-amplitude bladder electromyography sensing and offer limited charge-inj

Current consensus and controversy on the treatment of male lower urinary tract symptoms/benign prostatic hyperplasia.

Lower urinary tract symptoms (LUTS) are highly prevalent in men and increase with age. Because LUTS are common among elderly men, they are usually considered synonymous with benign prostatic hyperplasia (BPH). Drugs should be the first-line treatment for BPH and surgical intervention should be performed only when there are complications or LUTS refractory to medical treatment. In addition to medical treatment, several minimally invasive therapies, such as thermal therapy, prostatic lift, laser evaporation, or laser enucleation techniques have been developed. Recent investigations have also rev

Hemiobturator Nerve-Innervated Latissimus Dorsi Muscle for Restoring Voluntary Voiding: Anatomic Study and Clinical Application.

This study presents a modified latissimus dorsi detrusor myoplasty technique using the hemiobturator nerve for neurogenic underactive bladder reconstruction. Anatomical studies ( n = 22 hemipelves) revealed that the diameters of the anterior (mean, 0.209 cm) and posterior (mean, 0.199 cm) branches matched the diameter of the thoracodorsal nerve (1-way analysis of variance, P = 0.557), confirming their ideal donor potential. Latissimus dorsi detrusor myoplasty using the posterior branch of the intrapelvic obturator nerve as the donor nerve was performed in 5 patients with neurogenic underactive

Severe Lower Urinary Tract Dysfunction in Otherwise Healthy Children: A Three-Case Series and Narrative Review.

BACKGROUND: Severe lower urinary tract dysfunction (LUTD) in neurologically and anatomically normal children is uncommon and frequently underdiagnosed. When severe, functional voiding disorders may closely mimic obstructive or reflux pathology, leading to diagnostic errors, unnecessary invasive procedures, and potential risk to the upper urinary tract. CASE PRESENTATION: We present three pediatric cases (aged 3-10 years) referred for recurrent febrile urinary tract infections, incontinence, or acute urinary retention in the absence of neurological or structural abnormalities. Coexistent Detrusor Overactivity-Underactivity in Patients with Pelvic Floor Disorders.

Pelvic floor disorders represent a series of conditions that share, in part, the same etiological mechanisms, so they tend to be concomitant. Recently, awareness of a new lower urinary tract clinical syndrome has risen, namely the coexisting overactive-underactive bladder (COUB). The etiopathogenetic process, prevalence, and related instrumental findings of COUB are not well-established. We aimed to evaluate the prevalence, clinical features, and urodynamic findings of patients with COUB in a large cohort of patients with pelvic floor disorders. Methods: A cohort of 2092 women was retrospectiv

Resection of Bilateral Endometriosis of the Uterosacral Ligaments Resolves Voiding Dysfunction: A Case Report.

Patients with deep endometriosis of the posterior pelvic compartment might present various lower urinary tract symptoms, including urinary retention, without any prior pelvic surgery in their medical history. These symptoms, including urinary retention, pollakiuria or urge incontinence, urinary urgency, and decreased bladder sensitivity, imply an infiltration of the pelvic autonomic nerves by endometriosis lesions, in combination with inflammatory phenomena. Although there are numerous publications dealing with de novo urinary dysfunction after endometriosis surgery, significantly less scienti

The potential of soft robotics for the restoration of urinary voiding.

Current treatment options for chronic urinary retention secondary to detrusor underactivity are limited, and the mainstay is simply drainage by catheterization. The interdisciplinary field of bioelectronics could help to advance the prospect of artificial bladder emptying: first, by restoring continence control using human-in-the-loop mechanical compression to facilitate voiding and, second, by providing some of the components for managing detrusor overactivity, possibly using autonomous, closed-loop neuromodulation to inhibit aberrant contractions during storage. Pneumatic, magnetic and diele
