Inflammatory signals and network connections implicate cell-mediated immunity in chronic venous insufficiency.

BACKGROUND: The role of inflammation in superficial venous reflux in varicose veins (VVs) is unknown. Computational network modeling has deduced inflammation in experimental and clinical settings. We measured immune mediators in plasma from competent and incompetent leg veins inferring the role of cellular immunity based on cytokine networks. METHODS: Temperature was assessed using infrared thermography (IRT) to measure inflammation. Blood was obtained during sclerotherapy or endovenous thermal ablation for VVs. Control subjects underwent phlebotomy from saphenous and forearm veins. Vein segm

Fibula Free Flap Salvage From 10-hour Prolonged Arterial Insufficiency and Venous Thrombosis.

Fibula free flaps are standard in facial reconstructions after head and neck cancer resection and typically have short flap ischemia times, often under 2 hours. Methods for short-term flap ischemia are characterized, but methods for extremely prolonged flap ischemia have not been well-described. We describe a 72-year-old patient who underwent composite floor of mouth and mandible resection with fibula free flap reconstruction. Patient intraoperative instability forced flap ischemia of over 10 hours with arterial insufficiency and venous thrombosis. Despite complicated and extremely prolonged i

[Surgical treatment of patients with chronic lower limb venous insufficiency and trophic disorders].

The experience in surgical treatment of 326 patients with IV-VI degree chronic lower limb venous insufficiency (CEAP) is analyzed. Correction of disorders of venous hemodynamics was performed with surgical methods and sclerotherapy. The best clinical and functional results in long-term period were achieved in patients undergone surgery in an early stage of the disease without deep veins valves insufficiency and external trophic disorders. Pathology of deep veins required correction of valves function. It is concluded that these patients require complex approach to the treatment.

Treatment of severe chronic venous insufficiency using the subfascial endoscopic perforator vein procedure.

BACKGROUND: Before 1985, corrective surgical procedures for patients with chronic venous insufficiency and venous ulcers were performed using long incisions through diseased skin and subcutaneous tissues. The procedure, involving ligation of incompetent perforator veins, known as "the Linton operation," often was complicated by wound infections and poor healing. This changed in 1985 when G. Hauer demonstrated a new surgical technique for direct visual identification of the incompetent perforator veins using an endoscope in the subfascial space. This seminal contribution marked the advent of su

Does air plethysmography correlate with duplex scanning in patients with chronic venous insufficiency?

PURPOSE: Duplex ultrasonography with distal cuff deflation was used to determine the presence and size of incompetent veins and compare the results with those of air plethysmography in patients with chronic venous insufficiency. METHODS: Thirty-two legs underwent a detailed study with both modalities. Sixteen legs had venous ulceration, six had stasis dermatitis, and ten had symptomatic varicose veins without skin changes. RESULTS: Although the venous filling index (VFI) in limbs with ulcers (5.4 +/- 3.8 ml/sec) and dermatitis (7.7 +/- 4.6 ml/sec) was significantly higher (p < 0.05) than it

Chronic venous insufficiency disease. Its etiology. A new technique for vein valve transplantation.

This is a two-year study of 25 patients with venous insufficiency who were found to be in the terminal stage, referred to in this paper as chronic venous insufficiency disease (CVID). Apart from the usual signs and symptoms of venous insufficiency, the patients were found to have severe pain on weight bearing, inability to ambulate, and nonhealing lower extremity ulcers as common features that did not respond to conservative management. The most common underlying etiology was valvular incompetency. In the surgical management of these patients, at present, all the procedures described in the li

[Measurement of capillary permeability in chronic venous insufficiency using sodium fluorescein video-densitometry].

The measurement of capillary permeability in chronic venous insufficiency (CVI) requires techniques able to modify local microcirculatory hemodynamics and therefore the results. As for instance, venous occlusion in the Landis test or occlusive plethysmography. We are presenting a selective method which visualizes and quantifies capillary permeability without modifying the hemodynamics. A skin area on the lower extremity is observed on a microscope with blue filters. This optical device is linked with a camera transmitting information to a TV monitor and a video cassette recording system. When

Efficacy of a 6-month treatment with Daflon 500 mg in patients with venous leg ulcers associated with chronic venous insufficiency.

AIM: Epidemiological data show that standard compression therapy for leg ulceration in chronic venous insufficiency (CVI) often fails to effectively improve patients' condition. This study assesses the contribution of Daflon 500 mg added to conventional therapy in the healing of hypostatic ulcers of CVI patients. METHODS: Patients of about 65 years were included, with ulcers > or = 2 and > or = 10 cm diameter on 1 or 2 limbs, Doppler ankle/arm pressure index > 0.9, and no recent history of skin graft. Controls (n=68) remained on compression alone while the tested group (n=82) also received Da

Chronic Venous Insufficiency - A One-dimensional Blood Flow Modelling Assessment of Venous Valves, Perforators, Muscle Pump Activity and External Compression.

OBJECTIVE: Compression therapy is the primary treatment for chronic venous insufficiency, yet hemodynamic mechanisms through which it improves venous function remain incompletely understood. This study investigates the effects of venous valves and external compression on flow and pressure distribution in the lower-limbs using a novel one-dimensional computational model of venous hemodynamics. METHODS: The model includes major deep and superficial veins interconnected by perforators, collapsible vessel mechanics, dynamic valve behavior, gravitational effects, and calf muscle activity to repres

The Vicious Cycle of Biofilm, Host Inflammation and Microvascular Insufficiency in Venous Leg Ulcers.

Chronic venous leg ulcers (VLUs) affect approximately 1% of adults, and up to 30% remain unhealed by 12 months of standard therapy, with rates of recurrence approaching 70%. The chronicity and treatment resistance cannot be explained by the traditional view that venous hypertension alone causes the pathogenesis of VLUs. This narrative review synthesizes evidence from PubMed, Web of Science, and Scopus (2016-April 2026), including original research, systematic reviews, meta-analyses, and clinical trials on the pathophysiology, diagnosis, and treatment of VLU, with a focus on biofilm, inflammati

Dystrophic Calcinosis Cutis Leading to a Nonhealing Wound in a Patient with Chronic Venous Insufficiency.

The presence of bony-appearing fragments and calcifications appearing superficially in a chronic, nonhealing wound raises suspicion for osteomyelitis. When radiological imaging and tissue biopsy of the lesion return negative for osteomyelitis, however, the differentials must be widened to successfully manage and heal a chronic wound. In this report, we discuss a case of an 80-year-old morbidly obese woman with a history of chronic venous insufficiency, hereditary hemochromatosis, and squamous cell carcinoma who presented to the wound clinic with a 5-month history of a nonhealing wound with bon

Video-capillaroscopy, a promising tool for the clinical evaluation of patients with chronic venous insufficiency.

BACKGROUND: The cutaneous microangiopathy plays a key role in the development of the skin clinical lesions of venous insufficiency. Capillaroscopy allows a non-invasive observation of the superficial skin capillaries of the lower leg, which have previously been shown to be altered in patients with advanced venous disease. As it is now available in a friendly, easy to handle way through modern video devices, we report our findings in a short series of patients with C3-C5 chronic venous disorders using this technique. METHODS: A total of 21 patients with venous insufficiency (C3-C5 on at least

Validation of a Gravitational Model to Study Local Endogenous Biomarkers in Chronic Venous Insufficiency.

OBJECTIVE/BACKGROUND: Unlike most systemic chronic diseases, chronic venous insufficiency (CVI) is ideal to study using endogenous biomarkers. The stimulus causing damage can be turned on and off with gravitational positioning and venous blood samples can be taken locally. Annexin V (apoptosis) and microparticles (cell membrane debris) were used as markers of cell destruction, with matrix metalloproteinases (MMPs) as markers of tissue remodelling. The aim of this proof of concept study was to validate a gravitational model by investigating whether standing induced biochemical stress and whethe

Toenail abnormalities and onychomycosis in chronic venous insufficiency of the legs: should we treat?

BACKGROUND: Toenail manifestations of chronic venous insufficiency (CVI) may often mimic the nail changes of onychomycosis. The current study aims to determine the frequency of toenails deformations in patients with CVI, onychomycosis prevalence among deformed toenails and the outcome of itraconazole treatment. METHODS: Patients with clinical evidence of (CVI) were enrolled. All patients with toenails deformations and proven onychomycosis were treated by itraconazole pulse therapy for 4 months and then followed-up for additional 5 to 6 months. RESULTS: Forty-four patients with CVI entered th
