Nasogastric Tube Syndrome Treated by Replacement With a Smaller-Diameter Tube: A Case Report.

Nasogastric tube syndrome (NGTS) is a severe and potentially life-threatening complication of the nasogastric tube. NGTS is characterized by throat pain and abductor dysfunction of vocal cords due to the presence of the NGT. NGTS sometimes progresses to acute upper airway obstruction caused by vocal cord paralysis or laryngeal infection. Early recognition and appropriate management are essential to prevent serious outcomes. Removing the nasogastric tube is generally recommended for treatment. Here, we present a case of NGTS, wherein the symptoms were improved by replacing the nasogastric tube

Inhaled Mannitol as a Laryngeal and Bronchial Provocation Test.

OBJECTIVES: Timely diagnosis of vocal cord dysfunction (VCD), more recently termed "inducible laryngeal obstruction," is important because VCD is often misdiagnosed as asthma, resulting in delayed diagnosis and inappropriate treatment. Visualization of paradoxical vocal cord movement on laryngoscopy is the gold standard for diagnosis, but is limited by poor test sensitivity. Provocation tests may improve the diagnosis of VCD, but the diagnostic performance of current tests is less than ideal. Alternative provocation tests are required. This pilot study demonstrates the feasibility of using inh

Measuring Quality of Life in Pediatric Paradoxical Vocal Fold Motion Using the SF-36v2.

OBJECTIVES: Paradoxical vocal fold motion (PVFM) consists of intermittent adduction of the vocal folds during inspiration, resulting in stridor and worsened by anxiety and stress. The purpose of this study was to assess the impact of PVFM on quality of life in our pediatric patient population. STUDY DESIGN: This is a prospective, descriptive survey study. METHODS: Thirty-nine consecutive patients (ages 12-17 years) presenting with a PVFM diagnosis for respiratory retraining sessions with speech-language pathology were recruited. Patients completed a brief demographic questionnaire and the Sh

Deep brain stimulation for vocal tremor: a comprehensive, multidisciplinary methodology.

Tremulous voice is a characteristic feature of a multitude of movement disorders, but when it occurs in individuals diagnosed with essential tremor, it is referred to as essential vocal tremor (EVT). For individuals with EVT, their tremulous voice is associated with significant social embarrassment and in severe cases may result in the discontinuation of employment and hobbies. Management of EVT is extremely difficult, and current behavioral and medical interventions for vocal tremor result in suboptimal outcomes. Deep brain stimulation (DBS) has been proposed as a potential therapeutic avenue

Masqueraders in clinical allergy: laryngeal dysfunction causing dyspnea.

The allergist may be called upon to evaluate patients with dyspnea that is labeled "refractory asthma." In some patients, the site of origin of their dyspnea is the larynx. The diagnosis of laryngeal disorders requires a thorough understanding of the normal function of the laryngeal anatomy and physiology. Some patients with asthma may have a component of their airway obstruction in the larynx and large airways. Paradoxical vocal cord motion occurs when the true vocal cords approximate during inspiration, and although severe subjective dyspnea may occur, in most cases there is no evidence of h

Paradoxical vocal cord movement during sleep - A unique case with review of literature.

Paradoxical vocal cord movement (PVCM) refers to paradoxical adduction of the true vocal cords during inspiration resulting in variable upper airway obstruction. The condition often presents with shortness of breath, wheeze or stridor, mimicking bronchial asthma. Both organic and non-organic causes have been described. PVCM occurring only during sleep has rarely been reported in patients with CNS disease. We present a case of PVCM occurring only during sleep in a 14-year-old boy without any CNS disease, who was initially misdiagnosed as bronchial asthma.

Nasolaryngoscopy.

Nasolaryngoscopy is a low-risk, quick means of making a specific diagnosis for voice complaints; it should be performed before empiric treatment based on history and general examination alone. The most common indications for nasolaryngoscopy are hoarseness, globus sensation, and chronic cough. The most common findings in a primary care setting include laryngopharyngeal reflux (43%), chronic rhinitis (32%), and vocal cord lesions (13%).

Differential diagnosis of chronic cough.

Coughing that lasts >3 weeks is generally considered chronic. The causes of chronic cough are many. Most often, chronic cough is due to postnasal drainage, asthma, and/or gastroesophageal disease. However, other causes such as laryngopharyngeal reflux, vocal cord dysfunction, occult sinusitis, pertussis, and angiotensin-converting enzyme inhibitor should be considered. Even rarer entities will be discussed as well as some of the pathophysiology associated with the cough reflex.

Ansa-recurrent nerve anastomosis for vocal cord paralysis due to mediastinal lesions.

Recurrent laryngeal or vagus nerve injuries in the mediastinum are repaired rarely because of technical difficulties. Impairment in phonation is especially severe in patients with respiratory dysfunction. We performed a simple and less invasive reconstruction, ansa cervicalis-recurrent laryngeal nerve anastomosis in the neck, to improve phonation in 2 patients. Although the reinnervated vocal cord did not regain normal movement, both of the patients obtained excellent improvement in phonation.

The digital AcuBlade laser system to remove huge vocal fold granulations following subglottic airway stent.

We report a case of granulations that complicated subglottic stent placement and completely destroyed vocal folds with luminal stent obstruction. A microbial aetiology significantly contributed to the occurrence of granulations associated with mechanical irritation. The granulations were successfully resected using a digital AcuBlade laser system, a new generation of CO2 laser used in otorhinolaryngology, particularly in vocal cord disease. It permitted a precise control of the scan line between vocal fold and granulation for several reasons. The scan line was completely electronic and integra

Pseudo-asthma: when cough, wheezing, and dyspnea are not asthma.

Although asthma is the most common cause of cough, wheeze, and dyspnea in children and adults, asthma is often attributed inappropriately to symptoms from other causes. Cough that is misdiagnosed as asthma can occur with pertussis, cystic fibrosis, primary ciliary dyskinesia, airway abnormalities such as tracheomalacia and bronchomalacia, chronic purulent or suppurative bronchitis in young children, and habit-cough syndrome. The respiratory sounds that occur with the upper airway obstruction caused by the various manifestations of the vocal cord dysfunction syndrome or the less common exercise

The Fortuitous Discovery of the Olin EILOBI Breathing Techniques: A Case Study.

Exercise-induced laryngeal obstruction (EILO) is the term for the condition previously named vocal cord dysfunction and paradoxical vocal fold motion. It is defined by glottic or supraglottic obstruction during periods of intense exercise. Not all patients respond to conventional therapy with speech-language pathology, behavioral health interventions, and treatment of contributing conditions. In this edition of Journal of Voice, the authors describe a novel series of respiratory retraining techniques, now called Olin EILOBI (EILO biphasic inspiratory) breathing techniques, specifically designe

Inducible Laryngeal Obstruction Differential Diagnosis in Adolescents and Adults: A Tutorial.

Inducible laryngeal obstruction (ILO), formerly referred to as paradoxical vocal fold motion and vocal cord dysfunction, is a complex disorder of the upper airway that requires skillful differential diagnosis. There are several medical conditions that may mimic ILO (or which ILO may mimic) that should be considered in the differential diagnosis before evidence-supported behavioral intervention is initiated to mitigate or eliminate this upper airway condition. A key in treatment planning is determination of an isolated presentation of ILO or ILO concurrent with other conditions that affect the

The Association between Paradoxical Vocal Fold Motion and Dysphonia in Adolescents.

OBJECTIVE: Associations between dysphonia and paradoxical vocal fold motion (PVFM) have been previously reported in adults, but it is unclear whether similar associations exist for adolescents. The goals of this study were to identify the prevalence and severity of voice disorders in adolescent patients with PVFM, identify differences between those with and without clinician-identified dysphonia, and investigate what factors were associated with voice handicap in this population. METHODS: A retrospective review of eligible adolescent patients diagnosed with PVFM over a 1-year period at a sing

Speech pathology for chronic cough: a new approach.

Chronic cough may persist despite systematic evaluation and medical treatment of known associated diseases such as asthma, rhinitis, and gastro-esophageal reflux. These patients have refractory chronic cough and many exhibit laryngeal hypersensitivity that is manifest at both a sensory and motor level. Examples of this are heightened sensitivity of the cough reflex to capsaicin, and laryngeal motor dysfunction with hoarse vocal quality and paradoxical vocal cord movement. Chronic cough that persists despite medical treatment may respond to speech pathology intervention. A multidimensional spee

Relative Fundamental Frequency in Children With and Without Vocal Fold Nodules.

Purpose Relative fundamental frequency (RFF) is an acoustic measure that is sensitive to functional voice differences in adults. The aim of the current study was to evaluate RFF in children, as there are known structural and functional differences between the pediatric and adult vocal mechanisms. Method RFF was analyzed in 28 children with vocal fold nodules (CwVN, M = 9.0 years) and 28 children with typical voices (CwTV, M = 8.9 years). RFF is the instantaneous fundamental frequency (f 0) of the 10 vocalic cycles during devoicing (vocal offset) and 10 vocalic cycles during the revoicing (voca

Validation of ultrasound as a diagnostic tool to assess vocal cord motion in an animal feasibility study.

BACKGROUND: Post-thyroidectomy dysphonia can result from recurrent laryngeal nerve (RLN) injury. Confirmation of postoperative recurrent nerve function has prompted many surgeons to advocate laryngoscopic examination. Indirect and flexible laryngoscopy permit visualization of vocal cord motion, but not all thyroid surgeons are skilled in these techniques. Indirect laryngoscopy has a significant failure rate due to gag reflexes or anatomical obstruction. Flexible fiberoptic laryngoscopy, the current gold standard, allows reliable visualization of the cords, but perioperative examination is not

[Vocal cord paralysis after thyroid surgery : Current medicolegal aspects of intraoperative neuromonitoring].

Intraoperative neuromonitoring (IONM) has been commercially available for approximately 15 years and is highly predictive in thyroid gland surgery concerning either postoperative vocal fold mobility in the case of an intact signal for muscle action electromyogram (EMG, > 99 % right negative) or vocal fold dysfunction in the case of loss of signal (> 70 % right positive). The use of IONM improves the intraoperative identification of recurrent laryngeal nerve function and due to the high predictive value with respect to the expected vocal cord function the result of IONM has to be integrated int
