Patterns of pulmonary perfusion scans in normal subjects.

A vital factor conditioning the usage of the pulmonary perfusion (Q) scan in the evaluation of patients suspected of pulmonary embolism is the prevalence of abnormal Q scans in subjects free of cardiopulmonary disease. Because this prevalence has not been well defined, we performed Q scans in 80 nonsmoking subjects 18 to 29 yr of age having no known active cardiopulmonary disease. Each subject underwent a history, physical examination, electrocardiogram, spirometry, and PA chest roentgenogram, followed by a 6-view Q scan. Two subjects in whom a Q defect was suspected underwent a 133Xe equilibr

The temporal variant of frontotemporal dementia.

Frontotemporal dementia is a dementia syndrome with diverse clinical characteristics. Based upon clinical parameters and single photon emission computed tomography, we identified 47 frontotemporal dementia subjects. In 10 of these 47 the primary site of brain dysfunction was anterior temporal and orbital-frontal with other frontal regions relatively spared. In this temporal lobe variant (TLV) of frontotemporal dementia, five of the subjects had more severe left-sided, and five had more right-sided, hypoperfusion. The clinical, neuropsychological and neuropsychiatric features of predominantly l

Cerebral blood flow in migraine accompaniments and vertebrobasilar ischemia.

BACKGROUND AND PURPOSE: Transient neurological symptoms of brain stem or occipital lobe origin may be caused by transient ischemic attack in the vertebrobasilar territory (VB-TIA) or late-onset (or late-life) migraine accompaniment (LOMA). It is often clinically difficult to distinguish between VB-TIA and LOMA. METHODS: Cerebral blood flow of 23 patients with VB-TIA, 24 with LOMA, and 28 age-matched control subjects was measured using the 133Xe inhalation regional cerebral blood flow (rCBF) technique. RESULTS: After adjusting for differences in baseline variables such as blood pressure, hema

CBF side-to-side asymmetries in stenosis-occlusion of internal carotid artery. Relevance of CT findings and collateral supply.

Cerebral blood flow (CBF) was studied at rest by 133-Xenon inhalation method in 15 normal subjects, in 10 patients with cerebral infarction and normal angiograms of major cerebral arteries and in 28 patients with unilateral stenosis-occlusion of the internal carotid artery (ICA), with or without cerebral infarction. All the normals and 20 patients with ICA stenosis-occlusion were tested again after cerebral vasodilatation induced by an intravenous bolus of acetazolamide. At rest the patients with cerebral infarction, irrespective of whether ICa stenosis-occlusion was present or not, showed abn

Superficial temporal artery to middle cerebral artery anastomosis. Intraoperative evaluation by fluorescein angiography and xenon-133 clearance.

Fluorescein angiography and xenon-133 (133Xe) clearance studies were performed during surgery on 15 patients who were undergoing superficial temporal artery (STA) to middle cerebral artery (MCA) anastomosis. Fourteen patients had occlusive disease of the internal carotid artery (ICA), and one patient had severe stenosis of the MCA. Before anastomosis, fluorescein angiography showed slow filling of the MCA branches through collateral channels. Focal areas of impaired microcirculatory filling and washout were seen in the territory of severely sclerotic cortical arteries. The findings of preanast

[The reliability of scintigraphic criteria of the lungs concerning the estimation of mediastinal involvement in case of bronchial carcinoma (prospective study) (author's transl)].

The goal of this prospective study is to determine the reliability of the scintigraphic diagnosis of mediastinal involvement in patients with bronchial carcinoma, as well as to establish the value of each individual scintigraphic criterion. Ninety-four patients, all of whom were hospitalized because of suspicion of bronchial carcinoma were examined using 133-Xenon i.v., 133-Xenon gas inhalation, and 99m-Tc-MAA i.v. (macroaggregated albumin or microspheres of human albumin). The suspicion was confirmed in 77 patients. -The most reliable scintigraphic criteria are: 1. a localized delayed appeara

A paradoxical cerebral hemodynamic effect of hydralazine.

Hydralazine is shown to have a very complex cerebral hemodynamic effect. It raises the intracranial pressure which, together with its effect upon systemic blood pressure, reduces the cerebral perfusion pressure. In spite of this and a concomitantly induced hyperventilation by hydralazine, CBF increases with some delay. The conclusion is that hydralazine is a cerebral vasodilator acting immediately upon cerebral capacitance vessels but later upon the resistance vessels as well.

Regional lung function after pneumonectomy.

Hall, D. R. (1974).Thorax, 29, 425-431. Regional lung function after pneumonectomy. Regional lung function was studied with xenon-133 in 10 patients with bronchial carcinoma before and again three to six months after pneumonectomy. It was found that ventilation and perfusion were often considerably reduced in the tumour-bearing lung and that the greater part of the cardiac output had apparently been diverted to the unaffected lung before operation. The regional distribution of ventilation and perfusion in the lung remaining after surgery was essentially unchanged from that measured in the same

[Cerebral blood flow disturbances after anterior choroidal artery infarcts. Anatomical and functional correlates].

We have investigated the cortical and subcortical regional cerebral blood flow (rCBF) disorders resulting from infarcts of the anterior choroidal artery (AChA), and correlations with the severity of lesions, the physical and cognitive deficits, and the functional impairment. Eighteen patients presenting with recent anterior choroidal artery infarct without any other brain injury were examined at the secondary phase post-stroke using the single photon emission computed tomography technique and 133 Xenon inhalation. The rCBF and asymmetry indexes (AI) were calculated for 12 symmetrical hemispher

Impaired chemical coupling of cerebral blood flow is compatible with intact neurological outcome in neonates with perinatal risk factors.

Early detection of pathophysiological factors associated with permanent brain damage is a major issue in neonatal medicine. The aim of our study was to evaluate the significance of the CO2 reactivity of cerebral blood flow (CBF) in neonates with perinatal risk factors. Fourteen ventilated neonates with perinatal risk factors (pathological cardiotocogramm, low cord pH, postpartal encephalopathy) were enrolled into this prospective study. The study was performed 18-123 h after birth. CBF was measured using the noninvasive intravenous 133Xe method. Two measurements were taken with a minimal PaCO2

Respiratory failure and pulmonary hypertension associated with Klippel-Feil syndrome.

A 28-year-old woman with a deformed thorax and kyphoscoliosis associated with Klippel-Feil syndrome developed respiratory failure with pulmonary hypertension. Pulmonary 133Xe ventilation and 99mTc-MAA perfusion scintigraphies showed maldistributions of lung ventilation and perfusion, and noticeably delayed 133Xe washout from the lungs. Dynamic breathing MR imaging showed poor and/or asynchronous respiratory movements of the chest wall and diaphragm. These findings indicate that the perfusion-ventilation imbalance, the decreased ventilatory turnover, and expiratory flow from the alveolar space

Inferior temporal lobe atrophy and APOE genotypes in Alzheimer's disease. X-ray computed tomography, magnetic resonance imaging and Xe-133 SPECT studies.

The apolipoprotein E (APOE) gene epsilon 4 allele is known to be associated with late-onset familial and sporadic Alzheimer's disease (AD). We assessed the possible relationship between APOE genotypes and morphological or functional changes in AD brains by x-ray computed tomography (CT), magnetic resonance imaging (MRI) and Xe-133 single photon emission CT (SPECT). First, we estimated the change in size of the whole brain and total ventricular system by using two x-ray CT indices, the cerebral index (CI) and ventricular index (VI), respectively. Neither CI nor VI differed significantly among A

Cerebral blood flow and plasma hypoxanthine in relation to surfactant treatment.

We have previously reported reduction in EEG activity in preterm babies after tracheal instillation of Curosurf. To elucidate the cause of EEG depression, we have examined cerebral blood flow (CFB), amplitude-integrated EEG (aEEG), mean arterial blood pressure (MABP) and plasma hypoxanthine (Hx) concentration in a group of preterm babies before and immediately after administration of surfactant. No change occurred in CBF immediately after surfactant treatment despite a significant decrease in MABP. At 60 min after surfactant administration, a significant reduction in CBF occurred (p < 0.05). [Regional cerebral blood flow as dependent on the nature of the occlusive lesion of the branches of the aortic arch].

A 133Xe radionuclide investigation was employed for examination of regional cerebral circulation with 5% carbogen mixture loading in patients with occlusions of the aortic arch major arteries. It was established that a positive response to carbogen mixture inhalation indicates a compensated cerebral circulation irrespective of the arterial lesion type and number. The negative vascular response is associated with uni- or bilateral reduction of the regional cerebral circulation depending on the type and severity of the arterial involvement.

Cerebral blood flow in moyamoya disease. Part 1: Correlation with age and regional distribution.

A study has been made of the cerebral blood flow (CBF) in moyamoya disease from the perspective of hemispheric cerebral blood flow and regional CBF (rCBF). The material includes 21 children between the ages of 5 and 15 years with moyamoya disease, and 19 adult moyamoya cases--all of which had virtually no neurological symptoms at the time of the study. CBF was measured using the 133Xe intravenous injection method. Comparison was made with the measurements from 16 normal children and 14 normal adults. Study was also made of the relationship between the angiographic stage of the disease and the

Stable xenon CT CBF measurements in prevalent cerebrovascular disorders (stroke).

Local cerebral blood flow (LCBF) and local tissue: blood partition coefficient (L lambda) values were measured for small volumes of gray or white matter by CT CBF. Single compartment analysis was used but fitted to infinity in normal volunteers aged between 20 to 100 years (N = 20). Hemispheric LCBF and L lambda values were compared to those of 61 age matched patients with transient ischemic attacks (TIAs, N = 10), reversible ischemic neurologic deficits (RINDS, N = 10), acute and chronic cerebral infarctions associated with emboli from atherosclerotic plaques or complete occlusion of internal

Mapping cerebral blood flow by xenon-enhanced computed tomography: clinical experience.

Local cerebral blood flow was measured and mapped using xenon-enhanced x-ray transmission computed tomography. Studies involving 4-6 minutes of xenon-oxygen inhalation can be performed routinely in awake and anesthetized patients with acceptable patient tolerance and compliance. Several case studies of patients with acute and chronic ischemic injuries and other cerebral abnormalities are presented to illustrate characterization of flow pattern in normal and abnormal tissue, as well as the relevance of this flow information to clinical patient management.

Pulmonary abnormalities in mitral valve disease. Comparison between pulmonary wedge pressure, regional pulmonary blood flow and chest films.

Chest films, right sided heart catheterization, and measurement of the regional lung perfusion, using 133Xe, were carried out 31 times on patients with mitral valve disease. A relationship was found between the radiologic evaluation in 3 grades, and the values of pulmonary wedge pressure and the apical and basal perfusion. Changes in flow distribution as reflected in altered appearance of the vessels and the presence of interstitial edema were found to be the most sensitive factors in the evaluation of pulmonary wedge pressure. Chest radiography was thus found suitable for the evaluation of pu
