Rates of Septic Arthritis After ACL Reconstruction: A Single-Center Analysis Highlighting Quadriceps Tendon Grafts.

BACKGROUND: Although the infection rates for bone-patellar tendon-bone autograft (BTB), hamstring tendon autograft (HT), and allograft have been reported previously, there are limited data available for a large cohort of individuals undergoing anterior cruciate ligament (ACL) reconstruction (ACLR) using quadriceps tendon autograft (QT). PURPOSE: The aims of this study are (1) to compare rates of septic arthritis after primary and revision ACLR with QT, BTB, HT, and allograft and (2) to evaluate the association between an infection after ACLR and potential risk factors in a large single-system

Factors Associated With Pain and Function Before Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is performed to treat recurrent patellar instability. Measurement of joint pain and function at the time of surgery has been demonstrated to be a predictor of the final outcomes in many surgical procedures. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the relationship between baseline patient characteristics, mental health, and intraoperative findings and patient-reported knee pain and function at the time of MPFL reconstruction. We hypothesized that patient characteristics and associated pathology would be asso

Is Quadriceps Tendon Autograft a Better Choice Than Hamstring Autograft for Anterior Cruciate Ligament Reconstruction? A Comparative Study With a Mean Follow-up of 3.6 Years.

BACKGROUND: The quadriceps tendon (QT) autograft is known as an effective graft for anterior cruciate ligament (ACL) reconstruction and shows a similar functional outcome to the bone-patellar tendon-bone (BPTB) in randomized controlled trials, with a lesser incidence of complications. Up until now, only 2 studies have compared QT to hamstring tendon (HT) autograft. HYPOTHESIS: The functional outcomes of the QT technique are at least as good as those of the HT technique, with the same morbidity. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Ninety-five patients underwent isolate

Long-term clinical and radiographic results after delayed anterior cruciate ligament reconstruction in adolescents.

BACKGROUND: The risk of further intra-articular damage associated with nonoperative or delayed anterior cruciate ligament (ACL) reconstruction must be considered against the risk of growth disturbance with early reconstruction and transphyseal drilling. Long-term follow-ups after the surgical treatment of ACL injuries in adolescents are rare. PURPOSE: To evaluate results 10 to 20 years after ACL reconstruction in terms of the radiographic presence of osteoarthritis, clinical assessments, and health-related quality of life in patients who were adolescents at the time of surgery. STUDY DESIGN:

Mid-term outcomes after isolated bone-patellar tendon-bone ACL reconstruction versus hamstring ACL reconstruction combined with lateral extra-articular tenodesis in competitive soccer players: A prospective comparative study.

INTRODUCTION: Lateral extra-articular tenodesis (LET) has gained popularity as an adjunct to anterior cruciate ligament reconstruction (ACLR) in high-risk athletes, particularly when combined with hamstring tendon grafts. However, its added value compared with established graft options such as bone-patellar tendon-bone (BPTB) remains unclear. The purpose of this study was to compare the mid-term clinical, functional, and radiographic outcomes between isolated BPTB ACL reconstruction and hamstring tendon ACL reconstruction combined with LET in competitive soccer players. HYPOTHESIS: We hypothe

The Clinical Utility of Strength Measures in Predicting Patient Progression Following ACLR.

BACKGROUND: Prognostic indicators of lack of strength progression or poor self-reported function may alter rehabilitation decision-making following anterior cruciate ligament reconstruction (ACLR). The torque-velocity relationship is a noninvasive measure of muscle function that is altered following ACLR, but its prognostic value has not been explored. PURPOSE: To compare the torque-velocity relationship of knee extensors (quadriceps) and flexors (hamstrings) between the ACLR and uninvolved limbs and to determine whether the torque-velocity relationship was prognostic of subsequent achievemen

Associations Between Graft Choice and Return to Sports After Primary Anterior Cruciate Ligament (ACL) Reconstruction in Athletes.

BACKGROUND: Anterior cruciate ligament (ACL) injuries commonly occur in athletes, and ACL reconstruction (ACLR) after ACL injury can predictably allow athletes to return to sports (RTS). However, the time to RTS can be variable and dependent on a variety of patient and surgical factors. PURPOSE: To evaluate the associations between hamstring tendon (HT), quadriceps tendon (QT), and bone-patellar tendon-bone (BPTB) autograft choice and RTS data for athletes after primary ACLR. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Recreational and competitive athletes aged between 13 and

Clinical and Radiological Outcomes of Revision Anterior Cruciate Ligament Reconstruction Using a Quadriceps Tendon Autograft with a Bone Block: A Single-Center Case Series.

Background and Objectives: Revision anterior cruciate ligament reconstruction (ACLR) is demanding and yields inferior outcomes compared with primary procedures. The quadriceps tendon (QT) autograft with bone block has biomechanical and biological advantages though clinical evidence in revision remains limited. This study evaluated the clinical and radiological outcomes of revision ACLR using bone-block QT autograft in young, active patients. Materials and Methods: A case series with a level of evidence of 4. Thirty-four patients (28 men, 6 women; mean age, 27.2 ± 5.8 years) who underwent revis

Reflexes in prior polio and their relation to weakness and anterior horn cell loss.

The aim of this study was to evaluate the reflex pattern in patients with prior polio and to relate these findings to the degree of anterior horn cell (AHC) involvement and loss of muscle force. Twenty-five prior polio subjects were investigated with electromyography (EMG), force testing and reflex studies, which included the patellar and Achilles reflex, H-reflex, T-response and interlimb reflex (ILR). The clinical reflexes, H/M-ratio and T-response amplitude at rest were positively correlated with force and negatively correlated with the degree of AHC loss. The H/M-ratio was decreased compar

Surgical reconstruction of severe patellofemoral maltracking.

Dysfunction of the patellofemoral mechanism presents in many ways. Results from different realignment procedures show great variability in patient outcome. A surgical technique is presented that attempts to correct all the abnormalities of patellofemoral maltracking. The procedure consists of a lateral release, a vastus medialis (obliquus) tendon advancement, and a tibial tubercle transfer. Along with being moved medially, the tubercle also is moved distally to correct patella alta and elevated anteriorly to reduce patellofemoral joint reaction forces. One hundred seven knees in 84 patients we
