Suspension button constructs restore posterior knee laxity in solid tibial avulsion of the posterior cruciate ligament.

PURPOSE: Dislocated tibial avulsions of the posterior cruciate ligament (PCL) require surgical intervention. Several arthroscopic strategies are options to fix the fragment and restore posterior laxity, including two types of suspension button devices: adjustable (self-locking) and rigid knotted systems. Our hypothesis was that a rigid knotted button construct has superior biomechanical properties regarding laxity restoration compared with an adjustable system. Both techniques were compared with standard screw fixation and the native PCL. METHODS: Sixty porcine knees were dissected. The const

Results after spica cast immobilization following hip reconstruction in 95 cases: is there a need for alternative techniques?

INTRODUCTION: Developmental dysplasia of the hip (DDH), neurogenic dysplasia of the hip (NDH), and Perthes disease often require surgical treatment. Spica casting is a common postoperative immobilization. The purpose of this study was to evaluate the complications related to the immobilization. MATERIALS AND METHODS: In a retrospective analysis, we included 83 patients (95 hips), who underwent hip reconstructive surgery between 2008 and 2018. We had 43 female and 40 male patients. Age reached from 3 months to 19 years. All patients were treated with a spica cast postoperatively for a 6-week p

The relationship of the femoral physis and the medial patellofemoral ligament in children: a cadaveric study.

BACKGROUND: Patellar dislocations are common in skeletally immature athletes, and the medial patellofemoral ligament (MPFL) is an important primary restraint to lateral patellar translation. The relationship between the MPFL femoral origin footprint and femoral physis is unclear. The purpose of this study was to evaluate the MPFL femoral origin footprint and its relationship to the femoral physis in skeletally immature anatomic specimens. METHODS: Six skeletally immature cadaver knee specimens were examined through gross dissection (group A: 1, 11, and 11 mo; and group B: 8, 10, and 11 y). Me

Femoral head sphericity in untreated developmental dislocation of the hip.

BACKGROUND: Although many studies have discussed acetabular changes in walking aged children with developmental dysplasia of the hip (DDH), almost no data exists in the literature regarding dysplasia of the femoral head. The purpose of our study was to quantify the sphericity of the femoral head in a large, consecutive series of walking aged children with DDH. METHODS: We conducted a prospective study on 37 consecutive hips (30 patients) that were undergoing surgical procedures for DDH. After induction with general anesthesia (before the planned procedure), hip arthrograms were conducted. An

Magnetic resonance imaging characteristics of the medial patellofemoral ligament lesion in acute lateral patellar dislocations considering trochlear dysplasia, patella alta, and tibial tuberosity-trochlear groove distance.

PURPOSE: The objective of this study was to analyze the injury patterns of the medial patellofemoral ligament (MPFL) in acute lateral patellar dislocations (LPDs) considering the anatomically relevant factors of patellar instability. METHODS: Knee magnetic resonance images were collected from 73 patients within 7 weeks after LPD, and the injury patterns of the MPFL were evaluated for trochlear dysplasia, for patellar height, and for the tibial tuberosity-trochlear groove (TT-TG) distance. RESULTS: Injury to the MPFL was found in 98.6% of the patients (72 of 73) after the acute LPD, with a co

Anteromedialisation tibial tubercle osteotomy for recurrent patellar instability in young active patients: A retrospective case series.

INTRODUCTION: Recurrent patellar instability can be a source of continued pain and functional limitation in the young, active patient population. Instability in the setting of an elevated tibial tubercle-trochlear groove (TT-TG) distance can be effectively managed with a tibial tubercle osteotomy. At the present time, clinical outcome data are limited with respect to this surgical approach to patellar instability. METHODS: A retrospective chart review was performed to identify all cases of tibial tubercle osteotomy for the management of patellar instability performed at our institution with a

Medial and lateral segond fractures in a skeletally immature patient: a radiographic marker for the multiply injured knee.

Marginal fractures of the medial tibial plateau have been reported in the literature as a secondary type of Segond fracture. Some reports described this entity in the setting of combined injuries such as root avulsions of the medial meniscus, complete disruption of the posterior cruciate ligament (PCL), partial tear of the anterior cruciate ligament (ACL), and tears of the medial meniscus and medial collateral ligament. It has been postulated that medial marginal fractures are secondary to compression of the medial aspect of the femoral condyle and tibial plateau with a corresponding posterola

Gait analysis in adults with severe hip dysplasia before and after total hip arthroplasty.

Anatomical reconstruction of high riding hips by total hip arthroplasty (THA) and subtrochanteric shortening osteotomy aims to normalise gait pattern and improve functional hip scores. We present the medium-term clinical results of a group of patients with high riding dislocated hips in whom a cementless THA and subtrochanteric shortening osteotomy had been performed. We compared them with their preoperative status, with patients who had undergone a cementless THA for primary osteoarthritis, and also with a group of healthy gender and age-matched controls. Prospective computerized, three-dimen

Late results of hip and knee surgery in severely handicapped cerebral palsy patients.

Seventy-three involved cerebral palsied patients were reinvestigated 10 or more years after hip and/or knee surgery. The average age was 6.3 years at operation and 18.1 years at retest. The results were evaluated as to motor progress and hip position. While preoperatively none of the patients was independently ambulatory, this was true in 18 cases after surgery. Usually free gait however was insecure (household walking). All patients that had been unable to sit independently before surgery remained strictly wheelchair bound. The results of hip realignment differed according to degree of handic

Fractures and dislocations encountered by the general surgeon: general principles.

The definitive treatment selected for the care of a fracture is dependent upon what will offer the best cosmesis and function for the individual patient. Closed reductions and immobilization which require the joints to be in abnormal positions may offer an anatomic reduction but after the fracture is healed, joint function may be poor. Consequently, immobilization in the functional position with early motion and acceptance of some deformity is often better in terms of the ultimate use of the extremity. Traction treatment which requires prolonged recumbency is dangerous in the elderly because o

3D Knee Trochlear Morphology Assessment by Magnetic Resonance Imaging in Patients With Normal and Dysplastic Trochleae.

BACKGROUND: Trochlear dysplasia is recognized as a significant risk factor for patellar instability, but current imaging modalities fail to allow full visualization and classification of the complex 3-dimensional (3D) anatomy of the trochlea. The purpose of this study was to elucidate primary differences in trochlear morphology between patients with and without patellar instability by utilizing 3D magnetic resonance imaging (MRI) reconstructions of the trochlea. METHODS: An institutional review board-approved retrospective review included 24 patients with a diagnosis of patellar instability a

Report of Galeazzi fracture resulting from a ballistic injury.

BACKGROUND: Galeazzi fractures are a rare injury to occur in adults, which are typically associated with high energy trauma. To our knowledge, the reported cases in the literature have only been associated with motor vehicle accidents, falls from a height, or athletic endeavors. The application of the force in the setting of a Galeazzi fracture has been disputed, but it is believed to occur with the wrist in hyperextension and pronation. CASE REPORT: We describe a 27-year-old male who presented with multiple gunshot wounds that included his right forearm. The injury sustained to his right for

Acute compartment syndrome in tibial plateau fractures--beware!

Acute compartment syndrome can complicate fractures of the tibial plateau and must be recognized early so that timely fasciotomy can be performed. The presence of the open fasciotomy wound complicates management of the underlying proximal tibial fracture, which ideally needs early anatomic reconstruction of the articular surface and stable metaphyseal fixation. The purpose of this review is to highlight the importance of this problem, understand treatment strategies, and review outcomes.

Comparative Epidemiology of Revision Arthroplasty: Failed THA Poses Greater Clinical and Economic Burdens Than Failed TKA.

BACKGROUND: Revision THA and TKA are growing and important clinical and economic challenges. Healthcare systems tend to combine revision joint replacement procedures into a single service line, and differences between revision THA and revision TKA remain incompletely characterized. These differences carry implications for guiding care and resource allocation. We therefore evaluated epidemiologic trends associated with revision THAs and TKAs. QUESTIONS/PURPOSES: We sought to determine differences in (1) the number of patients undergoing revision TKA and THA and respective demographic trends; (
