Thirty-eight athletes with unilateral patellar tendinopathy (17 with a tendinopathy of the main body of the tendon, and 21 with an insertional tendinopathy) underwent ultrasound-guided multiple percutaneous longitudinal tenotomy under local anaesthetic infiltration after failure of conservative management. Thirty-four patients were reviewed at least 24 months after the operation. Sixteen patients were rated excellent, nine good, eight fair, and five poor. Nine of the 13 patients with a fair or poor result had an insertional tendinopathy, and eight of them underwent a formal exploration of the

BACKGROUND: Patellofemoral instability (PFI) is common in adolescents and may require surgical treatment. Patient-reported outcome scores (PROs) are frequently used to evaluate outcomes after treatment. Commonly used PROs for PFI include disease-specific Banff Patellofemoral Instability Instrument 2.0 (BPII 2.0) and region-specific measures such as the Kujala Anterior Knee Pain Scale (Kujala), Pediatric International Knee Documentation Committee Subjective Knee Form (Pedi-IKDC), and Knee injury and Osteoarthritis Outcome Score (KOOS).

BACKGROUND: Chondral pathology of the knee-specifically involving the femoral condyles and trochlea-is commonly observed during knee arthroscopies. This condition may necessitate supplementary interventions, such as a concomitant tibial tuberosity osteotomy (TTO). Approaching this patient population with a straightforward, less invasive arthroscopic technique may offer an advanced solution.
INDICATIONS: Arthro matrix-induced autologous chondrocyte implantation (MACI; autologous cultured chondrocytes on a porcine collagen membrane) targets symptomatic chondral lesions in the knee that are cons

BACKGROUND: Anterior cruciate ligament (ACL) repairs, once widely abandoned due to historically high failure rates, have recently regained interest with the development of the bridge-enhanced ACL repair (BEAR) implant, a novel arthroscopic technique that uses a resorbable protein-based implant combined with autologous blood to primarily repair a midsubstance ACL tear. This technical note presents a step-by-step surgical method for performing an isolated midsubstance ACL repair using the BEAR implant.
INDICATIONS: The BEAR implant is indicated for skeletally mature patients at least 14 years o

BACKGROUND: Bone-patellar tendon-bone (BTB) autografts have been considered the gold standard for anterior cruciate ligament reconstruction, although the popularity of quadriceps tendon (QT) autografts is increasing.
HYPOTHESIS/PURPOSE: The purpose of this study was to compare the ultrasound-derived cross-sectional area (CSA) and biomechanical strength of BTB and QT grafts harvested with different techniques. The authors hypothesized that ultrasound CSA measurements would be reliable and that QT grafts would be larger and stronger than BTB grafts, without differences between QT harvest method

BACKGROUND: Patient-reported outcome measures (PROMs) are essential for assessing knee function; however, their complexity can hinder efficiency. The subjective knee value (SKV) offers a simple alternative, but lacks distinction between daily activities and sports.
PURPOSE: We propose the SKV for daily activity (SKV-d) and sport (SKV-s). This study aimed to validate the SKV-d and SKV-s as simple and accurate tools for assessing knee function.
STUDY DESIGN: Cohort Study (Diagnosis); Level of evidence, 3.
METHODS: A total of 79 consecutive athletic patients (mean age, 39 years; 49% women) wit

BACKGROUND: Tibial deflexion osteotomy (TDO) is sometimes indicated for revision anterior cruciate ligament (ACL) reconstruction in knees with posterior tibial slope (PTS) ≥12° and aims to decrease PTS to around 5°. When planning TDO, measuring the anterior tibial metaphyseal height (aHt) could help ascertain whether the available metaphyseal bone would be sufficient to create the wedge and leave adequate residual bone.
PURPOSE: To (1) determine whether, compared with knees with normal native PTS (<12°), aHt is greater in knees with excessive native PTS (≥12°), and (2) verify if, aiming to de

BACKGROUND: Preclinical studies suggest that for complete midsubstance anterior cruciate ligament (ACL) injuries, a suture repair of the ACL augmented with a protein implant placed in the gap between the torn ends (bridge-enhanced ACL repair [BEAR]) may be a viable alternative to ACL reconstruction (ACLR).
HYPOTHESIS: We hypothesized that patients treated with BEAR would have a noninferior patient-reported outcomes (International Knee Documentation Committee [IKDC] Subjective Score; prespecified noninferiority margin, -11.5 points) and instrumented anteroposterior (AP) knee laxity (prespecifi

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) rates are rising, and quadriceps tendon (QT) autografts have gained popularity in recent years. Although QT autografts show objective and subjective outcomes comparable to hamstring tendon (HT) and bone-patellar tendon-bone (BPTB) autografts, nonrevision reoperations across graft types and their impact on patient-reported outcome measure (PROM) scores remain poorly studied.
PURPOSE: To compare nonrevision reoperation rates and PROM scores between primary ACLR using HT, BPTB, and QT autografts at 2 years' follow-up.

BACKGROUND: Tibiofemoral rotation is often increased in patients with recurrent patellar dislocations (RPDs), and excessive tibiofemoral rotation may contribute to a higher failure rate of medial patellofemoral ligament (MPFL) reconstruction. However, there is currently limited research on the optimal treatment strategy for RPDs with excessive tibiofemoral rotation.
PURPOSE: To report the clinical outcomes of derotational femoral osteotomy (DFO) combined with MPFL reconstruction for managing RPDs with excessive tibiofemoral rotation and to compare them with those of nonderotational surgical t

BACKGROUND: The literature suggests lateral unicompartmental knee arthroplasties are associated with low revision rates. However, there are fewer reports describing techniques for lateral unicompartmental arthroplasty and whether technique influences ROM and function compared to reports for medial unicompartmental arthroplasty.
QUESTIONS/PURPOSES: We report our indications for lateral unicompartmental arthroplasty, how we perform this procedure, and the subsequent Knee Society scores, ROM, and revision and reoperation rates.

Context • Osteoarthritis (OA) is one of the most prevalent chronic diseases of the musculoskeleton, causing functional disability among older adults. Management of OA includes conventional pharmacological treatments consisting primarily of nonsteroidal anti-inflammatory drugs (NSAIDs), acetaminophen, physiotherapy, and surgical procedures. The medications are not ideal therapeutic agents; NSAIDs in particular can cause serious side effects. Objective • The study was conducted to investigate the effects of Balsamodendron mukul (BDM) gum resin extract on cartilage damage and microstructural chan

BACKGROUND: In recent years, the quadriceps tendon has gained popularity for primary anterior cruciate ligament (ACL) reconstruction surgery due to its robustness, easy access, and ability to be tailor-made to each patient. However, concerns remain regarding potential complications associated with quadriceps tendon harvest, including impaired healing and damage to the extensor mechanism.
INDICATIONS: Preserving the quadriceps paratenon, which is a source of blood supply, nutrients, and therapeutic cells during ACL reconstruction surgery, may improve postoperative outcomes by mitigating potent
